摘要

目的

探索补肾健脾方改善再生障碍性贫血（AA）患者细胞自噬水平，以及对磷脂酰肌醇3-激酶/蛋白激酶B/哺乳动物雷帕霉素靶蛋白（PI3K/Akt/mTOR）信号通路的影响。

方法

收集2020年9月至2021年8月期间就诊于西苑医院的AA患者30例及304医院预备行外周血造血干细胞移植的健康供者6例，分为AA组和对照组。AA组予以3个月的补肾健脾方联合环孢素A（CsA）、雄激素治疗，收集AA组（治疗前后）骨髓及对照组外周血的单个核细胞，采用透射电子显微镜检测自噬小体、实时荧光定量聚合酶链式反应（Real-time PCR）检测微管相关蛋白1轻链3（LC3）、mTOR、Akt、PI3K mRNA水平及蛋白免疫印迹法（Western blot）检测LC3Ⅰ、LC3Ⅱ、mTOR、磷酸化（p）-mTOR、Akt、p-Akt、PI3K、p-PI3K蛋白水平。

结果

AA组实际完成29例，总有效率为51.72%（15/29）。①与对照组比较，AA患者治疗前外周血中白细胞（WBC）、血红蛋白（HGB）、血小板（PLT）、中性粒细胞绝对计数（ANC）均显著降低（

0.01），自噬小体偏少，LC3 mRNA水平显著降低（

0.01），mTOR、Akt和PI3K

mRNA水平显著升高（

0.01），LC3Ⅰ和LC3Ⅱ蛋白水平均显著降低（

0.01），Akt、p-Akt、PI3K、p-PI3K、mTOR和p-mTOR蛋白水平均显著升高（

0.01）；②与本组治疗前比较，AA患者治疗后，血红蛋白（HGB）和血小板计数（PLT）水平均明显升高（

0.05），自噬小体增多，LC3 mRNA水平显著升高（

0.01），mTOR、Akt和PI3K

mRNA水平均显著降低（

0.01），LC3Ⅰ和LC3Ⅱ蛋白水平显著增高（

0.01），Akt、p-PI3K蛋白水平显著降低（

0.01），p-Akt、PI3K、mTOR、p-mTOR蛋白水平明显降低（

0.05）。

结论

AA患者细胞自噬水平降低，补肾健脾方能有效改善AA患者低水平细胞自噬状态，降低PI3K/Akt/mTOR信号通路的表达。

Abstract

Objective

To explore the effects of Bushen Jianpi prescription on the autophagy and phosphatidylinositol-3-kinase/protein kinase B/mammalian target of rapamycin （PI3K/Akt/mTOR） signaling pathway in the patients with aplastic anemia （AA）.

Method

A total of 30 AA patients admitted to Xiyuan Hospital and 6 healthy donors who were prepared to undergo peripheral blood hematopoietic stem cell transplantation in 304 Hospital from September 2020 to August 2021 were enrolled and assigned into an AA group and a control group. The AA group was treated with Bushen Jianpi prescription combined with cyclosporin A （CsA） and androgen for 3 months. The mononuclear cells from bone marrow in the AA group before and after treatment and the peripheral blood of the control group were collected. Transmission electron microscopy was then employed to detect autophagosomes. Western blotting was employed to determine the protein levels of microtuble-associated protein 1 light chain 3 （LC3）Ⅰ， LC3Ⅱ， mTOR， phosphorylated （p）-mTOR， Akt， p-Akt， PI3K， and p-PI3K， and real-time polymerase chain reaction （PCR） to determine the mRNA levels of LC3， mTOR， Akt， and PI3K.

Result

In the AA group， the treatment was completed in 29 patients， and the total response rate was 51.72% （15/29）. ① The AA group showed lower levels of white blood cell （WBC）， hemoglobin （HGB）， platelet （PLT）， and absolute neutrophil count （ANC） in the peripheral blood （

0.01） and lower number of intracellular autophagosomes than the control group before treatment. Moreover， the AA group showed lower mRNA level of LC3 （

0.01） and protein levels of LC3Ⅰ and LC3Ⅱ （

0.01） and higher mRNA levels of mTOR， Akt， and PI3K

（

0.01） and protein levels of Akt， p-Akt， PI3K， p-PI3K， mTOR， and p-mTOR （

0.01） than the control group. ② In AA group， the levels of HGB and PLT elevated （

0.05） and the number of intracellular autophagosomes increased after treatment compared with those before treatment. Moreover， the mRNA level of LC3 and the protein levels of LC3Ⅰ and LC3Ⅱ were up-regulated （

0.01）， the mRNA levels of mTOR， Akt， and PI3Kα （

0.01） and the protein levels of Akt， p-PI3K （

0.01）， p-Akt， PI3K， mTOR， p-mTOR （

0.05） were down-regulated after treatment.

Conclusion

AA patients show lower autophagy levels， while Bushen Jianpi prescription can effectively improve the autophagy level and down-regulated the expression of PI3K/Akt/mTOR signaling pathway in AA patients.
