摘要

目的

采用急性脑缺血后引起心肌损伤模拟“脑损及心”病理过程，以Toll样受体（TLR）2/TLR4为切入点，探讨脑缺血状态下脑心通胶囊干预“脑损及心”的作用机制。

方法

将C57BL/6雄性小鼠随机分为假手术组、模型组、脑心通胶囊组和金纳多组。采用大脑中动脉闭塞法（MCAO）建立小鼠脑缺血模型。通过对小鼠神经行为学评分、脑梗死面积、神经元损伤、细胞凋亡、离子钙结合适配器分子1（Iba-1）阳性小胶质细胞比率及血清中氨基端前脑钠素（NT-proBNP）、肌酸激酶同工酶MB（CK-MB）及乳酸脱氢酶（LDH）进行分析，评价脑心通胶囊对小鼠脑缺血后心脑损伤的药效作用。通过蛋白免疫印迹法（Western blot）检测小鼠大脑和心脏中TLR2/TLR4蛋白的表达水平。

结果

与假手术组比较，模型组小鼠脑梗死面积与神经行为学评分，细胞凋亡率、Iba-1阳性小胶质细胞比率、血清中NT-proBNP、CK-MB及LDH含量显著上升（

0.01）。与模型组比较，脑心通胶囊能够显著降低小鼠脑梗死面积与神经行为学评分，细胞凋亡率、Iba-1阳性小胶质细胞比率（

0.01）及血清中NT-proBNP与CK-MB含量（

0.05）。Western blot结果表明，在脑缺血状态下，脑心通能够明显降低小鼠大脑组织中TLR2（

0.05）及心脏组织中TLR2与TLR4的表达水平（

0.05，

0.01）。

结论

脑缺血后会引起心肌损伤，反映“脑损及心”的病理过程，脑心通胶囊可显著改善脑缺血后脑损伤和心肌损伤，实现“脑心同治”，而TLR2/4是脑心通胶囊干预“脑损及心”的作用途径之一。

Abstract

Objective

After the brain and heart injuries were simulated by myocardial injury caused by acute cerebral ischemia， this study explored the mechanism of Naoxintong capsules in treating brain and heart injuries under cerebral ischemia state with Toll-like receptor （TLR） 2/TLR4 as the breakthrough point.

Method

C57BL/6 male mice were randomly assigned into the sham operation， model， Naoxintong， and Ginaton groups. The middle cerebral artery occlusion （MCAO） method was used to establish a mouse model of cerebral ischemia. The neuroethological score， cerebral infarction area， cell apoptosis， ionized calcium-binding adaptor molecule 1 （IBA-1）-positive microglia proportion， and serum levels of N-terminal pro-brain natriuretic peptide （NT-proBNP）， creatine kinase-MB （CK-MB）， and lactic dehydrogenase （LDH） were determined to evaluate the pharmacodynamic effects of Naoxintong capsules on heart and brain injuries after cerebral ischemia in mice. Western blotting was employed to determine the expression of TLR2/TLR4 protein in the brain and heart of mice.

Result

Compared with the sham operation group， the model group showed increased cerebral infarction area， neuroethological score， apoptosis rate， IBA-1-positive microglia proportion， and serum levels of NT-proBNP， CK-MB， and LDH （

0.01）. Naoxintong capsules reduced the cerebral infarction area， neuroethological score， apoptosis rate， IBA-1-positive microglia proportion （

0.01）， and serum NT-proBNP and CK-MB levels （

0.05） in mice compared with the model group. Western blotting results showed that Naoxintong Capsules down-regulated the expression levels of TLR2 （

0.05） in the brain and TLR2 （

0.01） and TLR4 （

0.05） in the heart.

Conclusion

Cerebral ischemia can cause myocardial damage， reflecting the pathological process of cardiac injury after cerebral ischemia. Naoxintong capsules can mitigate brain and heart injuries after cerebral ischemia and achieve the simultaneous treatment of the brain and the heart， in which TLR2/TLR4 plays a role.
