目的

观察中药三七粉治疗血脂异常的有效性和安全性及其对磷脂酰肌醇3-激酶/蛋白激酶B(PI3K/Akt)信号通路的调节作用。
方法

选取河南中医药大学第三附属医院2021年5月至2022年6月60例痰瘀互结型血脂异常患者,按照随机、双盲对照原则分为对照组和试验组各30例,对照组给予血脂康胶囊+三七粉安慰剂,试验组给予三七粉+血脂康胶囊安慰剂,连续用药6周。分别于0、3、6周评估临床疗效、中医证候积分及肝肾功能等安全性指标;采用酶联免疫吸附测定法(ELISA)检测外周血清血管内皮生长因子(VEGF)、激酶插入区受体(KDR)、表皮生长因子(EGF)、表皮生长因子受体(EGFR)表达,采用实时荧光定量聚合酶链式反应(Real-time PCR)检测外周血单个核细胞中KDR、EGFR和PI3K/Akt的mRNA表达。
结果

试验组总有效率为83.33%,对照组总有效率为89.66%,两组患者疗效相当,差异无统计学意义,且未见明显不良反应发生。与本组治疗前比较,试验组患者在3、6周时中医证候积分、血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)表达均明显下降(

0.05),高密度脂蛋白胆固醇(HDL-C)表达呈升高趋势,但差异无统计学意义;与对照组治疗后比较,两组患者差异无统计学意义。与本组治疗前比较,试验组外周血单个核细胞中PI3K、Akt和EGFR的mRNA表达及血清中EGF、VEGF、KDR表达随时间变化呈现下降趋势,其中,PI3K、Akt的mRNA及VEGF、KDR差异具有统计学意义(

0.05),KDR mRNA和血清EGFR表达水平呈现先增高后降低趋势;与对照组治疗后比较,两组患者在同一时间点PI3K、Akt、EGFR和KDR的mRNA表达及血清中EGF、EGFR、VEGF和KDR水平差异无统计学意义。
结论

三七粉在纠正血脂异常方面安全有效,其有效机制可能是通过下调VEGF/KDR表达抑制PI3K/Akt信号通路活性达到降脂的目的。
Objective

To evaluate the clinical efficacy and safety of Notoginseng Radix et Rhizoma powder in treating dyslipidemia by regulating the phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway.
Method

Sixty patients with dyslipidemia (syndrome of combined phlegm and stasis) treated in the Third Affiliated Hospital of Henan University of Chinese Medicine from May 2021 to June 2022 were selected in this study and randomized into two groups according to the randomized, double-blind control principle. The control group was treated with Xuezhikang capsules + Notoginseng Radix et Rhizoma powder placebo and the observation group with Notoginseng Radix et Rhizoma powder + Xuezhikang capsules placebo for 6 weeks. The clinical efficacy, traditional Chinese Medicine (TCM) syndrome scores, and liver and kidney function indicators were evaluated at weeks 0, 3, and 6. Enzyme-linked immunosorbent assay (ELISA) was employed to determine the expression of vascular endothelial growth factor (VEGF), kinase insert domain receptor (KDR), epidermal growth factor (EGF), and epidermal growth factor receptor (EGFR) in the peripheral serum. Quantitative Real-time PCR was employed to measure the mRNA levels of KDR, EGFR, PI3K, and Akt in the mononuclear cells of the peripheral blood.

The observation group (83.33%) showed the total effective rate comparable to that of the control group (89.66%) and no adverse reactions. Compared with before treatment, the patients in the observation group showed decreased TCM syndrome score and serum levels total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) and after being treated for 3 and 6 weeks (

0.05), the level of high-density lipoprotein cholesterol (HDL-C) showed an upward trend
but the difference was not statistically significant. After treatment, the two groups showed no significant differences. Compared with that before treatment
the mRNA expression of PI3K
Akt and EGFR in peripheral blood mononuclear cell and the expression of EGF
VEGF and KDR in serum of the observation group showed a downward trend with time
in which the mRNA expression of PI3K

Conclusion

Notoginseng Radix et Rhizoma powder is safe and effective in correcting dyslipidemia. It may inhibit the PI3K/Akt signaling pathway by down-regulating the expression of VEGF/KDR to lower the blood lipid level.
