摘要:ObjectiveTo observe the effect of modified Gegen Qinliantang on the expression levels of proteins related to the farnesoid X receptor/small heterodimer partner/peroxisome proliferator-activated receptor α (FXR/SHP/PPARα) signaling pathway in the liver tissue of db/db model mice with type 2 diabetes mellitus (T2DM) and explore the underlying mechanism of action of modified Gegen Qinliantang.MethodThirty db/db mice were randomly divided into model group, metformin group (0.2 g·kg-1), and high-, medium-, and low-dose modified Gegen Qinliantang groups (31.9, 19.1, 6.4 g·kg-1), with 6 mice in each group. An additional six m/m mice were assigned to the blank group. Respective drugs were administered via oral gavage for 12 weeks. Mouse body weight, fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) levels were measured. Oil red O staining was used to observe hepatic lipid accumulation and periodic acid-schiff (PAS) staining was used to assess hepatic glycogen deposition. Ammonium ferric sulfate staining was used to observe cholesterol deposition in intestinal tissues. Western blot was employed to detect the expression of FXR, cholesterol 7α-hydroxylase (CYP7A1), SHP, and PPARα proteins in liver tissues, and enzyme-linked immunosorbent assay (ELISA) was used to measure serum free fatty acid (FFA) levels.ResultAt the end of the treatment, compared with the blank group, the model group exhibited significant increases in mouse body weight, FBG, FFA, TC, TG, and LDL-C levels (P<0.01), along with significant hepatic lipid droplets, reduced hepatic glycogen, noticeable cholesterol accumulation in intestinal tissues, significantly decreased expression of FXR, SHP, PPARα proteins, and significantly increased expression of CYP7A1 protein in liver tissues (P<0.01). Compared with the model group, the metformin group and the high- and medium-dose modified Gegen Qinliantang groups demonstrated significant reductions in mouse body weight, FBG, FFA, TC, TG, LDL-C levels (P<0.05, P<0.01), significant increases in HDL-C levels (P<0.05, P<0.01), decreased hepatic lipid accumulation, increased hepatic glycogen, reduced intestinal cholesterol accumulation, significantly increased expression of FXR, SHP, PPARα proteins, and significantly decreased expression of CYP7A1 protein in liver tissues (P<0.01).ConclusionModified Gegen Qinliantang may regulate the FXR/SHP/PPARα signaling pathway to suppress FFA levels and improve lipid metabolism in T2DM mice.

摘要:ZHANG Zhongjing's Zhenwutang is a classic formula for warming Yang and excreting water. It is composed of Aconiti Lateralis Radix Praeparata, Poria, Atractylodis Macrocephalae Rhizoma, Paeoniae Radix Alba, and Zingiberis Rhizoma Recens. Physicians of later generations have inherited and developed this formula by detailed recording and application. This paper adopted bibliometrics method to analyze Zhenwutang in terms of history, indications, dosage, drug processing, usage, and modification. The results showed that Zhenwutang was most widely used in Ming and Qing dynasties. Many physicians have inherited ZHANG Zhongjing's theory regarding the application of Zhenwutang in disease treatment, and a few physicians have used it to treat diphtheria and water-related diseases. Some physicians modified this formula to treat maculae, intermittent dysentery, jaundice and so on. Zhenwutang was mainly used to treat diseases of the circulatory system, respiratory system and urinary system in modern clinical practice. The processing of herbal medicines in this formula was clear. Specifically, the raw material of Aconiti Lateralis Radix Praeparata needed to be processed and peeled, while those of Poria, Atractylodis Macrocephalae Rhizoma, Paeoniae Radix Alba, and Zingiberis Rhizoma Recens can be used directly. Although being different, most of the dosages were consistent with those in Treatise on Febrile Diseases. According to the textual research, it is suggested that the reference dosage of this prescription in clinical practice is 41.25 g for Poria, Paeoniae Radix Alba, and Zingiberis Rhizoma Recens, respectively, 27.5 g for Atractylodes macrocephala, and 15 g for Aconiti Lateralis Radix Praeparata. The medicinal materials should be decocted in 1 600 mL water to reach a volume of 600 mL. After removal of the residues, the decoction should be taken warm with 140 mL each time, three times a day. The textual research of Zhenwutang is expected to provide a theoretical reference for the clinical application and formulation of Zhenwutang.

摘要:ObjectiveTo explore the material basis and molecular mechanism of Linggui Qihua prescription (LGQH) against myocardial fibrosis in heart failure with preserved ejection fraction (HFpEF).MethodLiquid chromatography-mass spectrometry (LC-MS) was used to qualitatively analyze the active components of LGQH. AutoDock software was employed for molecular docking between the active components of LGQH and target proteins including α-smooth muscle actin (α-SMA), type Ⅰ collagen (ColⅠ), type Ⅲ collagen (ColⅢ), matrix metalloproteinase-9 (MMP-9), and tissue inhibitor of metalloproteinase-1 (TIMP-1). In vivo experiments were conducted on 40 spontaneously hypertensive rats (SHRs) aged 4 weeks, which were divided into an HFpEF group, an Entresto group (0.018 g·kg-1), and low- and high-dose LGQH groups (3.87, 7.74 g·kg-1). A high-fat, high-salt, and high-sugar diet was administered for 16 weeks along with intraperitoneal injection of streptozotocin solution for 8 weeks to establish an HFpEF model in rats. The blank group consisted of 10 Wistar Kyoto (WKY) rats and 10 SHRs. After successful modeling, the WKY, SHR, and HFpEF groups were given equal volumes of normal saline, while the other three groups received predetermined interventions. Daily oral gavage was performed for 6 weeks. After intervention, echocardiography was conducted to measure left ventricular (LV) anterior wall thickness (LVAWd), LV posterior wall thickness (LVPWd), LV internal diameter at end-diastole (LVIDd), LV ejection fraction (LVEF), isovolumic relaxation time (IVRT), early diastolic peak velocity of mitral valve inflow (E), and early diastolic mitral annular velocity (e'). The E/e' ratio was calculated. Enzyme-linked immunosorbent assay (ELISA) was used to detect serum atrial natriuretic peptide (ANP), B-type natriuretic peptide (BNP), and galectin-3 (Gal-3). Myocardial fibrosis was observed through Masson staining of pathological sections, and collagen volume fraction (CVF) and perivascular fibrosis ratio (PFR) were calculated. Real-time polymerase chain reaction (PCR) and Western blot were employed to detect LV myocardial mRNA and protein expression of α-SMA, ColⅠ, ColⅢ, MMP-9, and TIMP-1.ResultLC-MS identified 13 active components in LGQH. Molecular docking indicated stable binding of the 13 compounds with five target proteins. In vivo experiments showed that compared with the blank group, the HFpEF group had significantly increased LVAWd, LVPWd, LVIDd, IVRT, E/e', ANP, BNP, Gal-3, CVF, and PFR. LV myocardial α-SMA, ColⅠ, and ColⅢ mRNA and protein expression was significantly upregulated, while MMP-9/TIMP-1 mRNA and protein ratios were significantly downregulated (P<0.05, P<0.01). Compared with the HFpEF group, LGQH might dose-dependently reduce LVAWd, LVPWd, LVIDd, IVRT, E/e', ANP, BNP, Gal-3, CVF, and PFR, downregulated myocardial α-SMA, ColⅠ, ColⅢ mRNA expression, α-SMA, and ColⅠ protein expression, and upregulated MMP-9/TIMP-1 mRNA and protein expression (P<0.05, P<0.01).ConclusionLGQH contains multiple active components and may inhibit myocardial fibrosis in HFpEF rats. It may further alleviate LV hypertrophy, dilation, and diastolic dysfunction, making it an effective Chinese medicinal prescription for treating HFpEF.
摘要:ObjectiveTo investigate the therapeutic effect of the water extract of Zanthoxylum bungeanum aqueous extract(ZBAE)on rheumatoid arthritis.MethodThe sixty SD rats were divided into normal group, model group [complete Freund's adjuvant (CFA), 10 mg·kg-1], methotrexate(MTX) group (0.25 mg·kg-1), low -, medium -, and high-dose ZBAE groups (90, 180, 360 mg·kg-1). The rats in MTX group were given intraperitoneal injection for two weeks, three times a week, and the rats in ZBAE group were administrated for 14 days. The swelling of the ankle joint and body weight were observed, and arthritis scores were also performed. Computed tomography (CT), hematoxylin-eosin (HE) staining and safranine-O and fast green staining were used to observe the effect of ZBAE on synovial hyperplasia and bone protection. Cell counting kit-8(CCK-8)method was used to detect the proliferation of the RA-FLSs cells treated with ZBAE. According to the results of CCK-8 experiment, the optimal concentration and time of administration were determined, blank group, low -, medium -, and high-dose ZBAE groups (0.08,0.10,0.12 g·L-1) were set up. The cell cycle distribution and apoptosis rate were analyzed by flow cytometry,the migration ability of RA-FLSs cells was examined by scratch test. Western blot was used to detect the effect of ZBAE on phosphatidylinositol 3-kinase (PI3K), protein kinase B (Akt), cyclin-dependent kinase 2 (CDK2), Cyclin A and phosphorylated PI3K, Akt (p-PI3K,p-Akt) protein expression in RA-FLSs cells.ResultCompared with the normal group,joint swelling index and arthritis score were increased in the model group (P<0.05),the bone of the ankle was seriously damaged, and there was obvious synovial hyperplasia. Compared with the model group, the ZBAE group could significantly reduce the joint swelling index (P<0.05), inhibit synovial hyperplasia and cartilage destruction. In vitro study showed that compared with the blank group, ZBAE could inhibit the migration of RA-FLSs (P<0.05), promoted cell apoptosis (P<0.05), and acted on RA-FLSs cells in S phase to inhibit cell proliferation. Moreover, the result of Western blot showed that compared with the blank group, the expression of p-PI3K, p-Akt, CDK2 and Cyclin A proteins were significantly decreased in the high dose group of ZBAE (P<0.05).ConclusionThese results suggest that ZBAE has a therapeutic effect on rheumatoid arthritis by inhibiting synovial hyperplasia, promoting synovial apoptosis and inhibiting its migration.

摘要:ObjectiveTo explore the mechanism of plumbagin as a novel ferroptosis inducer in bladder cancer inhibition.MethodBladder cancer T24 cells were used in this study. The effect of different concentrations of plumbagin (0.1, 1, 2, 3, 6, 12, 24, 48 μmol·L-1) on the viability of T24 cells was detected by cell counting kit-8 (CCK-8). The effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the apoptosis of T24 cells was detected by annexin V-fluorescein isothiocyanate (Annexin V FITC)/PI apoptosis kit. Different inhibitors (ferroptosis inhibitor Fer-1, apoptosis inhibitor VAD, and necroptosis inhibitor Nec-1) were used in combination with plumbagin (6 μmol·L-1). Reactive oxygen species (ROS) fluorescent probe (DCFH-DA), malonaldehyde (MDA), and glutathione (GSH) kits were used to detect the effects of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the level of ROS and the content of MDA and GSH in T24 cells, respectively. The effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on peroxide levels in T24 cells was detected by C11-BODIPY fluorescent probe. Western blot was used to detect the effect of different concentrations of plumbagin (1.5, 3, 6 μmol·L-1) on the protein expression of solute carrier family 7 member 11 (SLC7A11), glutathione peroxidase 4 (GPX4), nuclear factor E2-related factor-2 (Nrf-2), and Kelch-like ECH-associated protein 1 (Keap1).ResultCompared with the blank group, plumbagin could inhibit the activity of T24 cells (P<0.05) with IC50 of 3.52 μmol·L-1. At the concentrations of 1.5, 3, 6 μmol·L-1, plumbagin significantly promoted the apoptosis of T24 cells (P<0.05) as compared with the blank group. Compared with the plumbagin group at 6 μmol·L-1, the ferroptosis inhibitor and apoptosis inhibitor groups could reverse the inhibitory effect of 6 μmol·L-1 plumbagin on the proliferation of T24 cells (P<0.05). Compared with the blank group, the plumbagin groups at 1.5, 3, 6 μmol·L-1 showed increased content of ROS, MDA, and lipid peroxides in T24 cells, decreased GSH level, and reduced SLC7A11, GPX4, and Nrf-2/Keap1 (P<0.05).Conclusionplumbagin can induce ferroptosis, and its mechanism is related to the Nrf-2/Keap1 signaling pathway.

摘要:ObjectiveTo investigate the effect of different oxygen concentration on the proliferation and autophagy of colon cancer cells and to explore the effect of Yangyin Huayu Jiedu Preseription (YHJP) on autophagy and apoptosis of colon cancer cells under hypoxia based on phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway.MethodHCT-116 cells were divided into normoxia group, 1% O2 group, and 5% O2 group. Cell viability was detected by cell proliferation assay (MTS), and autophagy was observed based on monodansylcadaverine (MDC) staining. HCT-116 cells were treated with YHJP in 5% O2 microenvironment. The cells were divided into normal group, blank serum group, and low-, medium-, high-dose YHJP groups (5%, 15%, 25% serum containing YHJP). Cell inhibition rate in each group was calculated by MTS, and changes in the rate of autophagy were detected based on MDC staining. Annexin V-fluorescein isothiocyanate (FITC)/propidium iodide (PI) was employed to detect the apoptosis rate of each group. Western blotting was applied to measure the expression of autophagy proteins microtubule-associated protein 1 light chain 3 (LC3Ⅱ/Ⅰ), yeast Atg6 homolog (Beclin-1), ubiquitin-binding scaffold protein p62 (p62), apoptosis-related proteins B-cell lymphoma-2 (Bcl-2), Bcl-2/adenovirus E1B interacting protein 3 (BNIP-3), and Bcl-2 associated X protein (Bax), cleaved cysteine-aspartic acid protease-3 (Caspase-3), hypoxia-inducible factor-1α (HIF-1α) and pathway proteins PI3K, phosphorylated (p)-PI3K, Akt, and p-Akt.ResultCell survival rates of the 1% O2 and 5% O2 groups were increased compared with that in the normoxia group, particularly the 5% O2 group (P<0.01). The fluorescence intensity for autophagy in 1% O2 and 5% O2 groups was significantly increased compared with that in the normoxia group, especially the 5% O2 group. In the presence of 5% O2, compared with the blank serum group, medium-dose and high-dose YHJP groups showed high cell inhibition rate, low autophagy rate, high apoptosis rate (P<0.01), and low expression of Beclin-1 protein (P<0.05). Compared with low-dose YHJP group, high-dose YHJP group demonstrated low expression of Beclin-1 protein (P<0.05). Compared with the blank serum group, the three YHJP groups had low expression of LC3Ⅱ/Ⅰ protein (P<0.05, P<0.01). Compared with the blank serum group, medium-dose and high-dose YHJP groups showed high expression of p62 protein (P<0.01). Compared with low-dose YHJP group, high-dose YHJP group showed high expression of p62 protein (P<0.05). Compared with the blank serum group, high-dose YHJP increased the expression of BNIP-3 and Bax and decreased the expression of Bcl-2 (P<0.01). The expression of Bax protein in the high-dose YHJP group was increased compared with that in the low-dose YHJP group (P<0.05). The expression of HIF-1α in the medium-dose and high-dose YHJP groups was decreased (P<0.01) and the expression of p-PI3K/PI3K and p-Akt/Akt in the high-dose YHJP group was increased (P<0.05, P<0.01) compared with that in the blank serum group. The expression of p-Akt/Akt was higher in the high-dose YHJP group than in the medium-dose YHJP (P<0.05).ConclusionHypoxic microenvironment can significantly promote autophagy and proliferation of colon cancer cells. YHJP can significantly inhibit autophagy and proliferation and promote apoptosis of colon cancer cells in 5% O2 environment by up-regulating the PI3K/Akt signaling pathway.
摘要:ObjectiveTo investigate the effect and mechanism of osthole on the proliferation and apoptosis in human intrahepatic cholangiocarcinoma HuCCT1 cells.MethodThe effect of 10, 20, 40, 80, and 120 μmol·L-1 osthole on the proliferation of HuCCT1 cells was detected by the cell counting kit-8 (CCK-8). A blank group, and low-, medium-, and high-dose osthole groups (16, 32, and 64 μmol·L-1) were set up. The effect of osthole on cell clone formation rate was detected by colony formation assay. The effect of osthole on cell cycle and apoptosis was detected by flow cytometry. The effect of osthole on cell apoptotic morphology was detected by Hoechst 33342 fluorescent staining. The effect of osthole on cell cycle protein cyclin B1, proliferating cell nuclear antigen (PCNA), cysteine-aspartic acid protease (Caspase)-9, Caspase-3, cleaved Caspase-9, cleaved Caspase-3, cleaved poly(ADP-ribose) polymerase (cleaved PARP), B-cell lymphoma-2 (Bcl-2), phosphorylated protein kinase B (p-Akt), phosphorylated mammalian target of rapamycin (p-mTOR), and phosphorylated ribosomal protein S6 (p-RPS6) was detected by Western blot.ResultThe cell viability in the osthole group(40,80,120 μmol·L-1) decreased (P<0.05,P<0.01), with the half maximal inhibitory concentration (IC50) of 63.8 μmol·L-1 as compared with that in the blank group. Compared with the blank group, the osthole groups(32,64 μmol·L-1)showed reduced clone formation rate (P<0.01), increased number of cells in the G2 phase (P<0.05,P<0.01), decreased number of cells, increased pyknosis and fragmentation, increased apoptosis rate (P<0.05,P<0.01), down-regulated expression of cyclin B1, PCNA, Bcl-2, Caspase-3, Caspase-9, p-Akt, p-mTOR, and p-RPS6 (P<0.05,P<0.01), and up-regulated expression of cleaved Caspase-3, cleaved Caspase-9, and cleaved PARP (P<0.05,P<0.01).ConclusionOsthole can inhibit the proliferation and promote the apoptosis of HuCCT1 cells, and its mechanism may be related to the Akt/mTOR signaling pathway.

摘要:ObjectiveTo reveal the intervention effect of Dahuang Mudantang on pancreatic injury in rats with acute pancreatitis (AP) of dampness-heat in large intestine syndrome and explore its possible mechanism based on network pharmacology.MethodNinety-six SPF-grade Wistar rats were randomly divided into the following six groups: a blank group, a model group, low-, medium-, and high-dose Dahuang Mudantang groups (3.5, 7, and 14 g·kg-1), and a Qingyi Lidan granules group (3 g·kg-1), with 16 rats in each group. The AP model of dampness-heat in large intestine syndrome was induced in rats except for those in the blank group by "high-temperature and high-humidity environment + high-sugar and high-fat diet + retrograde injection of 5% sodium taurocholate into the pancreaticobiliary duct". The blank and model groups received equal volumes of distilled water by gavage, while the treatment groups were administered Dahuang Mudantang or Qingyi Lidan granules 1 hour before modeling, and 12 and 24 hours after modeling. Samples were collected 1 hour after the last administration. The general conditions of the rats were observed. The AP model of dampness-heat in large intestine syndrome was evaluated. Serum amylase (AMS) and C-reactive protein (CRP) levels were determined using biochemical methods. Pancreatic tissue morphology was observed using hematoxylin-eosin (HE) staining. Network pharmacology was employed to predict potential targets of Dahuang Mudantang in the intervention in AP, and molecular biology technique was used to verify relevant targets.ResultCompared with the blank group, the model group exhibited lethargy, unkempt fur, loose and foul-smelling stools, elevated anal temperature with arching and twisting reactions, significantly increased serum levels of AMS and CRP (P<0.05), abnormal pancreatic ductules, disordered interlobular spaces, and inflammatory cell infiltration in histopathological examination, as well as pathological changes including pancreatic acinar cell swelling, congestion, and necrosis. Compared with the model group, the treatment groups showed varying degrees of improvement in general survival conditions, reduced twisting reactions, visibly improved stool characteristics, reduced pancreatic tissue edema and necrosis, decreased serum AMS and CRP levels (P<0.05), with the high-dose Dahuang Mudantang group showing the most pronounced effects (P<0.05). Network pharmacology prediction indicated that hederagenin, β-sitosterol, and quercetin were the most widely connected active compounds with disease targets. Protein-protein interaction (PPI) network analysis revealed that protein kinase B (Akt), tumor protein P53 (TP53), tumor necrosis factor (TNF), interleukin-6 (IL-6), transcription factor (JUN), vascular endothelial growth factor α (VEGFα), interleukin-1β (IL-1β), and vascular cell adhesion molecule-1 (VCAM1) were key targets in the "drug-disease" interaction. KEGG enrichment analysis suggested that the response of the mitogen activated protein kinase (MAPK) signaling pathway might be a core mechanism for DHMDT in the intervention in AP. Molecular biology analysis showed that compared with the blank group, the model group had significantly increased levels of TNF-α, IL-6, and VCAM-1 in pancreatic tissue (P<0.05), as well as significantly elevated expression levels of p38 mitogen-activated protein kinase (p38 MAPK), mitogen-activated protein kinase-activated protein kinase 2 (MK2), and human antigen R (HUR) genes and proteins (P<0.05). Compared with the model group, the treatment groups exhibited decreased levels of TNF-α, IL-6, and VCAM-1 in pancreatic tissue (P<0.05), reduced expression levels of p38 MAPK, MK2, and HUR genes and proteins, with the high-dose Dahuang Mudantang group showing the most pronounced effects (P<0.05).ConclusionDahuang Mudantang activates and regulates the p38 MAPK/MK2/HUR signaling pathway to suppress the release of inflammatory factors, thereby improving pancreatic injury.

摘要:ObjectiveTo establish and validate a clinical prediction model for 1-year major adverse cardiovascular events(MACEs)risk after percutaneous coronary intervention (PCI) in coronary heart disease (CHD) patients with blood stasis syndrome.MethodThe consecutive CHD patients diagnosed with blood stasis syndrome in the Department of Integrative Cardiology at China-Japan Friendship Hospital from September 1, 2019 to March 31, 2021 were selected for a retrospective study, and basic clinical features and relevant indicators were collected. Eligible patients were classified into a derivation set and a validation set at a ratio of 7∶3, and each set was further divided into a MACEs group and a non-MACEs group. The factors affecting the outcomes were screened out by least absolute shrinkage and selection operator (Lasso) and used to establish a logistic regression model and identify independent prediction variables. The goodness-of-fit of the model was evaluated by the Hosmer-Lemeshow test, and the area under curve (AUC) of the receiver operating characteristic (ROC) curve, calibration curve, decision curve analysis (DCA), and clinical impact curve (CIC) were employed to evaluate the discrimination, calibration, and clinical impact of the model.ResultA total of 731 consecutive patients were assessed and 404 eligible patients were enrolled, including 283 patients in the derivation set and 121 patients in the validation set. Lasso identified ten variables influencing outcomes, which included age, sex, fasting plasma glucose (FPG), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), homocysteine (Hcy), brachial-ankle pulse wave velocity (baPWV), flow-mediated dilatation (FMD), left ventricular ejection fraction (LVEF), and Gensini score. The multivariate Logistic regression preliminarily identified age, FPG, TG, Hcy, LDL-C, LVEF, and Gensini score as the independent variables that influenced the outcomes. Of these variables, male, high FMD and high LVEF were protective factors, and the rest were risk factors. The prediction model for 1-year MACEs risk after PCI in CHD patients with blood stasis syndrome showed χ2=12.371 (P=0.14) in Hosmer-Lemeshow test and the AUC of 0.90. With the threshold probability > 10%, the model showed better prediction performance for 1-year MACEs risk after PCI in CHD patients with blood stasis syndrome than for that in all the patients. With the threshold probability > 60%, the estimated value was much closer to the real number of patients.ConclusionThe established clinical prediction model facilitates the early prediction of 1-year MACEs risk after PCI in CHD patients with blood stasis syndrome, which can provide ideas for the precise treatment of CHD patients after PCI and has guiding significance for improving the prognosis of the patients. Meanwhile, multi-center studies with larger sample sizes are expected to further validate, improve, and update the model.
摘要:ObjectiveTo explore the clinical efficacy of Huanglian Jiedutang as an adjunctive treatment for acute cerebral infarction complicated with gastric motility disorder.MethodSixty patients with acute cerebral infarction complicated with gastric motility disorder with fire toxin syndrome were randomly divided into a western medicine control group (control group) and a traditional Chinese medicine (TCM) combined treatment group (observation group), with 30 cases in each group. The control group received basic treatment for cerebral infarction and relevant western medical symptomatic treatment based on the patients' gastrointestinal symptoms. The observation group received Huanglian Jiedutang in addition to the treatment provided to the control group. The treatment course was 7 days. Neurological deficit scores and gastrointestinal dysfunction scores were assessed in both groups before treatment and on the 4th and 7th days of treatment. Gastrointestinal electrographic parameters, serum citrulline (CIT), and motilin (MTL) levels were measured in both groups before treatment and on the 7th day of treatment. Clinical efficacy was compared between the two groups.ResultCompared with the baseline in both groups, the neurological deficit scores and gastrointestinal dysfunction scores were significantly reduced on the 4th and 7th days of treatment (P<0.05). The reductions in these scores were more significant on the 7th day compared with those on the 4th day of treatment (P<0.05). On the 4th and 7th days of treatment, the observation group showed a significantly greater reduction in neurological deficit scores and gastrointestinal dysfunction scores compared with the control group (P<0.05). On the 7th day of treatment, compared with the baseline, both groups showed a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. On the 7th day of treatment, compared with the control group, the observation group had a significant increase in gastric antral and gastric body electric wave amplitudes as well as serum CIT and MTL levels (P<0.05), and there were no statistically significant differences in the frequency of gastric antral and gastric body electric waves. After 7 days of treatment, the total effective rate in the observation group was 90.00% (27/30), higher than 76.67% (23/30) in the control group, but the difference was not statistically significant.ConclusionAdjunctive treatment with Huanglian Jiedutang can effectively improve the symptoms of neurological function impairment and gastrointestinal dysfunction in patients with acute cerebral infarction complicated with gastric motility disorder, increase gastric antral and gastric body electric wave amplitudes, improve gastric motility disorder, and increase serum CIT and MTL levels, thereby improving the imbalanced secretion function of the gastrointestinal tract.
摘要:ObjectiveTo explore the predictive factors for the efficacy of Yiqi Yangxue prescription combined with western medicine in treating aplastic anemia (AA) in non-elderly adults, so as to provide a reference for predicting the prognosis of this therapy.MethodA retrospective study was conducted with the clinical data of non-elderly adult AA patients who visited 19 hospitals including Xiyuan Hospital of the China Academy of Chinese Medical Sciences from September 2018 to March 2021 and were treated with Yiqi Yangxue Prescription combined with western medicine. According to the efficacy evaluation results at the 6th month of treatment, the patients were assigned into effective and ineffective groups. The two groups were compared in terms of the gender, age, disease classification [non-severe aplastic anemia (NSAA)/severe aplastic anemia (SAA)], course of disease, family history, complications, history of drug allergy, baseline blood routine examination [hemoglobin (HGB), white blood cell (WBC), neutrophil (ANC), platelet (PLT), and reticulocyte (Ret)], T lymphocyte subsets, degree of proliferation of nucleated cells in bone marrow, and expression of T-bet and GATA-3.ResultA total of 101 non-elderly adult AA patients were enrolled in this study, including 81 in the effective group and 20 in the ineffective group. The effective group had a higher proportion of the patients without a history of drug allergy than the ineffective group (P<0.05). The body height, body weight, gender, age, disease classification, course of disease, family history, and complications showed no significant differences between two groups. The effective group had higher levels of ANC and PLT before treatment (P<0.05) and higher proportion of patients with ANC≥1.6×109/L and PLT≥25×109/L (P<0.05, P<0.01) than the ineffective group. The baseline levels of WBC, HGB, and Ret showed no significant statistical differences between two groups. The levels of CD3+HLA-DR+T cells in the effective group before treatment was higher than that in the ineffective group (P<0.05). The levels of CD3+CD19-T cells, CD4+T cells, CD8+T cells, Th1 cells, Th2 cells, and CD3+CD25+T cells showed no significant statistical differences between two groups before treatment. The proportion of patients with active bone marrow nucleated cells proliferation in the effective group before treatment were significantly higher than that in the ineffective group, while the proportion of patients with reduced or extremely reduced proliferation were significantly lower than that in the ineffective group (P<0.05). The expression levels of T-bet and GATA-3 genes had no significant differences between two groups before treatment. The multivariate binary logistic regression analysis showed that the ANC level before treatment and history of drug allergy were independent influencing factors for efficacy (P<0.05, P<0.01), while other indicators were not influencing factors for efficacy. The receiver operating characteristic (ROC) curve was applied to analyze the predictive value of the ANC level before treatment in the treatment of AA in non-elderly adults with Yiqi Yangxue prescription combined with western medicine. The area under the curve was 0.679 (P<0.05), with the critical value of 1.595×109/L, the sensitivity of 0.42, and the specificity of 0.95.ConclusionThe history of drug allergy, pre-treatment ANC, PLT, CD3+HLA-DR+ T cell levels, and proliferation of nucleated cells in bone marrow before treatment are predictive factors for the efficacy of Yiqi Yangxue prescription combined with western medicine in treating AA in non-elderly adults. This therapy tends to be more effective for the patients with no history of drug allergy, higher ANC and PLT levels before treatment, especially those with ANC≥1.6×109/L, PLT≥25×109/L, and higher CD3+ HLA-DR+T cell levels and the more active proliferation of nucleated cells in bone marrow before treatment.

摘要:ObjectiveTo review the information and clinical studies of oral Chinese patent medicines (CPMs) for chronic kidney disease (CKD).MethodThe CPMs for treating CKD were retrieved from the Pharmacopoeia of the People's Republic of China, National Essential Drugs List, and Medicine List for National Basic Medical Insurance, Employment Injury Insurance and Maternity Insurance. China National Knowledge Infrastructure(CNKI), VIP, Wanfang Data, SinoMed, PubMed, Embase, Cochrane, and Web of Science were searched for the clinical trials of the treatment of CKD by CPMs from their inception dates to September 25, 2022. A database was established with the collected CPMs, and then the general conditions of the clinical trials were analyzed and presented visually.ResultA total of 16 CPMs for CKD were included in this study, including 5 classical traditional Chinese medicine (TCM) prescriptions involving Rehmanniae Radix and 11 new CPMs. The indications of the TCM prescriptions did not mention the corresponding western disease names, and those of the new CPMs mainly included chronic renal insufficiency, chronic renal failure, and chronic nephritis. Four CPMs were prepared with single Chinese medicine or active components. Specifically, Bailing Preparation and Jinshuibao Preparation were mainly prepared with the powder of Cordyceps, and the main components of Haikun Shenxi capsules and Huangkui capsules were fucoidan sulfate and the flower extract of Abelmoschi Corolla, respectively. The CPMs mainly exerted tonifying and eliminating effects on the lung, spleen, and kidney. A total of 892 clinical trials were screened out, covering all the areas in China and presented an increasing trend. Bailing Preparation was the most studied, followed by Niaoduqing Preparation. Among the 892 studies, 475 focused on single CPMs without combination with other CPMs or therapies. These studies mainly compared between conventional intervention and conventional intervention + CPM, which accounted for 75.58%. The 475 studies covered different kidney diseases, such as chronic kidney disease, chronic renal failure, nephrotic syndrome, diabetic kidney disease, IgA nephropathy, and membranous nephropathy, and involved a variety of populations including the elderly and children. Thirty-six studies evaluated TCM syndromes, reflecting the characteristics and advantages of TCM treatment.ConclusionThere are abundant oral CPMs for CKD, with varied efficacy and characteristics for different kidney diseases. However, the instruction manuals of the oral CPMs are not detailed or standard. According to the clinical research evidence in this field, the research on oral CPMs for CKD is characterized by a wide scope, rich study types, and wide disease coverage, while the sample size and quality remain to be improved.

摘要:ObjectiveIn this study, based on ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS/MS) and high-throughput transcriptome sequencing technology(RNA-seq), we investigated the mechanism of Yishen Huashi granules in regulating serum metabolites and renal messenger ribonucleic acid(mRNA) expression to improve diabetic kidney disease(DKD).MethodSD rats were randomly divided into normal group , model group and Yishen Huashi granules group, with 8 rats in each group. The rat model of DKD was established by intraperitoneal injection of streptozotocin. Yishen Huashi granules group was given 5.54 g·kg-1·d-1 of Yishen Huashi granules by gavage, and the normal group and the model group were given the same amount of normal saline for 6 weeks. During the experiment, the body weight and blood glucose of rats were monitored, and the rats were anesthetized 24 hours after the last administration, blood was collected from the inferior vena cava, serum was separated, and renal function, blood lipid, and inflammatory indicators were detected. Kidney tissue of rats was fixed in neutral paraformaldehyde, and stained with hematoxylin-eosin(HE), Masson and periodic acid-Schiff(PAS) to observe the renal pathological changes. UHPLC-MS/MS and RNA-seq were used to identify the changes of serum metabolism and the differences of renal mRNA expression, and real time fluorescence quantitative polymerase chain reaction(Real-time PCR) and Western blot were used to detect the differential mRNA and protein expression in renal tissue to explore the common expression mechanism.ResultCompared with the normal group, rats in the model group showed a decrease in body weight, a significant increase in blood glucose, urinary microalbumin to urinary creatinine ratio(UACR), blood urea nitrogen(BUN), cystatin-C(Cys-C), β2-microglobulin(β2-MG), interleukin-6(IL-6), triglyceride(TG) and total cholesterol(TC), and a significant decrease in total superoxide dismutase(T-SOD)(P<0.01). After the intervention of Yishen Huashi granules, all the indexes were improved to different degrees in rats(P<0.05, P<0.01). Compared with the normal group, the model group showed renal mesangial stromal hyperplasia, fibrous tissue hyperplasia and tubular vacuolar degeneration. Compared with the model group, the renal pathology of rats in Yishen Huashi granules group was improved to a certain extent. A total of 14 target metabolites and 96 target mRNAs were identified, the target metabolites were mainly enriched in 20 metabolic pathways, including sphingolipid metabolism, glycerophospholipid metabolism, and the biosynthesis of phenylalanine, tyrosine and tryptophan. The target mRNAs were enriched to obtain a total of 21 differential mRNAs involved in the TOP20 pathways closely related to glycolipid metabolism. A total of 6 pathways, glycerophospholipid metabolism, arachidonic acid metabolism, purine metabolism, primary bile acid biosynthesis, ascorbic acid and uronic acid metabolism, and galactose metabolism, were enriched by serum differential metabolites and renal differential mRNAs, among them, there were 7 differential metabolites such as phosphatidylethanolamine(PE) and 7 differential mRNAs such as recombinant adenylate cyclase 3(ADCY3). Seven differential metabolites had high predictive accuracy as verified by receiver operating characteristic(ROC) curve, and the results of Real-time PCR and Western blot were highly consistent with the sequencing results.ConclusionYishen Huashi granules can reduce UACR, BUN and other biochemical indexes, correct the disorder of glucose and lipid metabolism, and improve renal function of DKD rats. And its mechanism may be related to the regulation of the level of PE and other blood metabolites, and expression of Phospho1 and other mRNAs in the kidney, of which six pathways, including glycerophospholipid metabolism, may play an important role.
摘要:ObjectiveTo investigate the changes of endogenous metabolites in serum of ovariectomized rats and the effect of Erxiantang on them based on liquid chromatography-mass spectrometry(LC-MS).MethodTwenty-four healthy female SD rats were randomly divided into sham-operated group, model group and Erxiantang group(7.5 g·kg-1), with 8 rats in each group. Bilateral ovarian tissues were excised in the model and Erxiantang groups, and small pieces of adipose tissues were excised in the abdominal cavity of the sham-operated group bilaterally, and gastric administration was started 2 weeks after surgery, and equal volumes of distilled water were gavaged in the sham-operated and model groups. After 12 weeks of administration, blood was collected from abdominal aorta, and non-targeted metabonomics was performed on rat serum by LC-MS, and orthogonal partial least squares-discriminant analysis(OPLS-DA) was used to screen differential metabolites. Metabolic pathway analysis was performed based on Kyoto Encyclopedia of Genes and Genomes(KEGG), and the levels of key enzymes of metabolic pathways were verified by enzyme-linked immunosorbent assay(ELISA).ResultThe results of metabonomics showed that 82 differential metabolites between the model group and the sham-operated group were glycerophospholipids, fatty acyls, steroids and steroid derivatives, of which the most significant difference was glycerophospholipids. At the same time, Erxiantang could call back 65 out of 82 differential metabolites, of which 11 were statistically significant, mainly phosphatidylcholine(PC) and lysophosphatidylcholine(LysoPC) in glycerophospholipids, followed by corticosterone and 11-deoxycortisol in steroids and steroid derivatives. Metabolic pathway analysis showed that the pathways of glycerophospholipid metabolism and steroid hormone biosynthesis in model group were changed, and were recovered after the administration of Erxiantang. ELISA results showed that compared with the sham-operated group, serum levels of cholinephosphate cytidylytransferase(CCT), secretory phospholipase A2(sPLA2) and lysophosphatidylcholine acyltransferase(LPCAT), which were the key metabolic enzymes of glycerophospholipid metabolite PC and LysoPC, were significantly decreased in the model group(P<0.05, P<0.01), and choline phosphotransferase 1(CPT1) levels decreased but the difference was not statistically significant, compared with the model group, the levels of CCT, sPLA2 and CPT1 were significantly increased in Erxiantang group(P<0.01). In addition, compared with the sham-operated group, the levels of cholesterol(TC), triglyceride(TG) and low density lipoprotein cholesterol(LDL-C) were significantly increased in the model group(P<0.01), the high density lipoprotein cholesterol(HDL-C) level was decreased(P<0.05), compared with the model group, the levels of TC, TG and LDL-C were significantly decreased and the level of HDL-C was significantly increased in Erxiantang group(P<0.01).ConclusionEndogenous metabolites and related metabolic pathways in ovariectomized rats were altered, and Erxiantang can reverse some of the different metabolites and related pathways, such as regulating glycerophospholipid metabolism by regulating metabolic enzymes CCT, sPLA2 and CPT1 to increase the levels of PC and LysoPC, and then improve the pathological changes such as lipid metabolism disorder in ovariectomized rats.
摘要:ObjectiveUltra-high performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UHPLC-Q-Orbitrap HRMS) was used to identify the metabolites of limonin in rats, and to explore the gender differences in the distribution of prototype components and metabolites in rats after single dose intragastric administration of limonin, as well as to speculate the metabolic pathways.MethodThe separation was performed on a Thermo Scientific Accucore™ C18 column(3 mm×100 mm, 2.6 μm) with 0.1% formic acid aqueous solution(A)-0.1% formic acid acetonitrile solution(B) as mobile phase in a gradient elution mode(0-1 min, 5%B; 1-6 min, 5%-20%B; 6-18 min, 20%-50%B; 18-23 min, 50%-80%B; 23-25 min, 80%-95%B; 25-30 min, 95%B) at a flow rate of 0.3 mL·min-1 and a column temperature of 30 ℃. MS data of biological samples were collected under the positive ion mode of electrospray ionization(ESI) and in the scanning range of m/z 100-1 500. Plasma, tissues(heart, liver, spleen, lung, kidney, stomach and small intestine), urine and fecal samples were collected and prepared after intragastric administration, and the prototype component and metabolites of limonin were identified.ResultThe prototype component of limonin were detected in the feces, stomach, small intestine of female and male rats, and in the heart, liver, spleen, lung and kidney tissues of female rats. A total of 23 metabolites related to limonin were detected in rats, of which 2, 1, 5, 4, 23 metabolites were detected in liver, stomach, small intestine, urine and feces, respectively, and the main metabolic pathways were hydrolysis, reduction, hydroxylation and methylation, etc. The distribution of some metabolites differed between male and female rats.ConclusionThe prototype component of limonin are mainly distributed in the stomach and small intestine in rats, and the distribution of prototype component and some metabolites are different by gender. Limonin is mainly excreted through feces with phase Ⅰ metabolites as the main ones. The results of this study can provide a reference for further elucidation of the effect of gender differences on the metabolism of limonin in vivo and its mechanism of action.

摘要:ObjectiveTo investigate the effect of microemulsion on the distribution of index components in different phases of Zexietang extract based on high performance liquid chromatography(HPLC) and phase separation process.MethodParticle size meter and transmission electron microscope were used to characterize the colloidal particles in blank microemulsion, aqueous extract of Zexietang and microemulsion extract of Zexietang. The phase separation process was established by high-speed centrifugation and dialysis, and based on this process, the aqueous extract and microemulsion extract of Zexietang were separated into the true solution phase, the colloidal phase and the precipitation phase, respectively. The contents of six components, including atractylenolide Ⅲ, atractylenolide Ⅱ, 23-acetyl alisol C, alisol A, alisol B and alisol B 23-acetate, were determined by HPLC with the mobile phase of water(A)-acetonitrile(B) for gradient elution(0-5 min, 40%-43%B; 5-20 min, 43%-45%B; 20-45 min. 45%-60%B; 45-75 min, 60%-80%B). The solubility of the index components in water and microemulsion was determined by saturation solubility method.ResultThe colloidal particles in the aqueous extract, microemulsion extract and blank microemulsion were all spherical, and the particle size, polydispersity index(PDI) and Zeta potential of the colloidal particles were in the order of aqueous extract >microemulsion extract >blank microemulsion. The results of phase separation showed that the colloidal phase and the true solution phase could be completely separated by dialysis for 2.5 h, and the phase separation process was tested to be stable and feasible. Compared with the aqueous extract of Zexietang, the use of microemulsion as an extraction solvent could increase the contents of atractylenolide Ⅲ, 23-acetyl alisol C, atractylenolide Ⅱ , alisol A, alisol B and alisol B 23-acetate by 3.75, 6.82, 35.47, 10.66, 35.41, 27.75-fold, and could increase the extraction efficiencies of the latter five constituents by 2.03, 1.15, 1.70, 6.43, 5.53 times. The solubility test showed that the microemulsion could significantly improve the solubility of atractylenolide Ⅱ, alisol A, alisol B and alisol B 23-acetate, but it had less effect on the solubility of atractylenolide Ⅲ and 23-acetyl alisol C.ConclusionMicroemulsion can improve the extraction efficiency and increase the distribution of the index components in the colloidal phase state of Zexietang to different degrees, providing a reference for the feasibility of microemulsion as an extraction solvent for traditional Chinese medicine.
摘要:ObjectiveTo establish the characteristic sugar spectrum of polysaccharides, oligosaccharides and monosaccharides of wild-simulated and transplanted Astragali Radix, and find out the difference of the sugar spectrum between the two, so as to provide a basis for quality evaluation of Astragali Radix.MethodThe relative molecular weight distribution of polysaccharides from 18 batches of wild-simulated Astragali Radix and 12 batches of transplanted Astragali Radix were characterized by high performance liquid chromatography-evaporative light scattering detection(HPLC-ELSD) to establish the characteristic chromatograms of two kinds of polysaccharides. The difference in the peak area ratio of APS-Ⅱ, a polysaccharide component with a relative molecular weight of 10 kDa, in two kinds of Astragali Radix was analyzed, and the critical value of peak area ratio of APS-Ⅱ was determined by receiver operating characteristic(ROC) curve. At the same time, APS-Ⅱ was partially acid-hydrolyzed by trifluoroacetic acid(TFA) to establish characteristic spectra of two kinds of oligosaccharides from Astragali Radix based on HPLC-ELSD, and the characteristics of differential oligosaccharides were found by principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA). Two kinds of APS-Ⅱ were completely acid-hydrolyzed by TFA and derivatized to establish characteristic spectra of two kinds of monosaccharides from Astragali Radix based on HPLC, PCA and OPLS-DA were performed on the peak area ratio of two kinds of monosaccharides to explore the differences in the composition of two kinds of APS-Ⅱ monosaccharides.ResultThe characteristic sugar spectrum of polysaccharides from Astragali Radix showed that the peak area ratio of APS-Ⅱ was the main difference, and the peak area of APS-Ⅱ of wild-simulated and transplanted Astragali Radix were 89.17%-97.17% and 80.14%-91.96%, respectively. The ROC curve determined the critical value of 92.28% for the difference of APS-Ⅱ peak area ratio of the two kinds of Astragali Radix. The multivariate analysis of APS-Ⅱ oligosaccharides revealed that the peak area ratio of oligosaccharides with polymerization degree≥10 was the main difference, which ranged from 11.835%-19.092% for wild-simulated products and 2.778%-7.017% for transplanted products. The results of monosaccharide characteristic sugar spectrum analysis showed that both Astragali Radix species consisted of six monosaccharides, and glucose and arabinose were the differential monosaccharide fractions. The peak area ratios of glucose and arabinose in wild-simulated products were 85%-93.9% and 2.7%-5.8%, respectively, while those of transplanted products were 74.3%-87.3% and 5.3%-10.7%, suggesting that the structures of the two polysaccharide fractions APS-Ⅱ of Astragali Radix may be different.ConclusionThe difference of sugar spectrum between two kinds of Astragali Radix may be related to the content and structure of APS-Ⅱ, and this study may provide a reference for the study of carbohydrates in Astragali Radix and the quality evaluation of medicinal materials.
摘要:ObjectiveTo identify potential distribution areas for wild Panax ginseng cultivated under forest in Liaoning province, China, and analyze the ecological factors of spatial stratified heterogeneity affecting its ecological suitability and quality suitability.MethodWild Panax ginseng samples cultivated under forest were collected from 33 cultivation bases in Liaoning province. The Maxent maximum entropy model and ArcGIS were used to delineate the ecological suitability zones. Correlation analysis was performed on seven indicators and 110 ecological factors. Variables with significant correlation (P<0.05) were used to build partial least squares regression analysis models. A comprehensive quality zoning was conducted using the analytic hierarchy process (AHP). The geographic detector was employed to analyze the interactions among dominant ecological factors of spatial stratified heterogeneity affecting habitat suitability, quality suitability, and the ecological driving factors.ResultVegetation type was the most influential ecological factor for delineating the ecological suitability zones for wild Panax ginseng in Liaoning province. The main ecological suitability areas for wild Panax ginseng cultivated under forest were located in the northeast, east, and southeast regions along the line from Xifeng County to Gaizhou City. The comprehensive quality suitability of wild Panax ginseng cultivated under forest was highest in Kuandian County and Huanren County and gradually decreased to the northwest and southwest. Within the delineated regions, the suitability conditions and comprehensive quality of wild Panax ginseng cultivated under forest were primarily influenced by the interactions between radiation and precipitation factors. The content of the measured samples was significantly higher than the standards in the 2020 edition of the Chinese Pharmacopoeia, indicating the high overall quality of wild Panax ginseng in Liaoning Province.ConclusionAccording to the zoning and prediction results, areas in Fengcheng City, Xiuyan County, Zhuanghe City, Liaoyang County, Tieling County, Xifeng County, Gaizhou City, Haicheng City, and Dashiqiao City showed large potential distribution areas with high quality, making them highly promising for wild Panax ginseng cultivation. However, further experimental verification is required. The zoning results can provide insights for research on habitat suitability and comprehensive quality accumulation of wild Panax ginseng cultivated under forest, as well as guidance for the search for potential cultivation areas and industrial development of wild Panax ginseng in Liaoning Province.

摘要:ObjectiveTo explore the macroscopic medication pattern of traditional Chinese medicine (TCM) in treating esophageal cancer (EC) and provide medication references for the clinical application of TCM in EC treatment.MethodRelevant literature on TCM treatment of EC was retrieved from three major Chinese databases: China National Knowledge Infrastructure (CNKI), Wanfang Data, and VIP. Information about Chinese herbal medicines was entered into Excel to establish a prescription database for EC. The data were standardized, summarized, and subjected to frequency analysis, association rules, and cluster analysis of medication in the prescriptions. Based on the TCM classification of EC syndromes, clinical indications corresponding to each syndrome were identified, and high-frequency drugs and drug pairs were analyzed correspondingly with syndromes.ResultA total of 136 prescriptions containing 240 Chinese herbal medicines were screened, with a cumulative frequency of 1 853 times. The top 5 frequently used Chinese herbal medicines were Glycyrrhizae Radix et Rhizoma, Poria, Atractylodis Macrocephalae Rhizoma, Astragali Radix, and Pinelliae Rhizoma. In terms of functions, the Chinese herbal medicines were mainly deficiency-tonifying, urination-promoting and dampness-draining, deficiency-tonifying, deficiency-tonifying, and phlegm-resolving and cough and dyspnea-relieving ones. The statistical analysis of flavor, property, and meridian tropism showed that Chinese herbal medicines were mainly bitter and sweet, warm, cold, and neutral, and acted on the spleen, lung, and stomach meridians. Association rule analysis yielded nine potential drug combinations, and cluster analysis of high-frequency drugs resulted in four combination categories. The four TCM syndromes for EC corresponded to respective clinical indications, treatment drugs, and drug pairs.ConclusionTonifying deficiency, reinforcing healthy Qi, descending adverse Qi, resolving phlegm, activating blood, and resolving stasis are the basic principles of TCM treatment for EC, which are supplemented by clearing heat and dissipating mass while focusing on regulating and smoothing the qi movement. The drug combinations obtained from high-frequency drug and association rule analysis provide references for different TCM syndrome treatments of EC, offering valuable insights for clinical medication.

摘要:In contemporary clinical practice of traditional Chinese medicine (TCM), there is a strong emphasis on treating chronic difficult diseases and preventing diseases before they occur ("pre-disease" treatment). However, the treatment of critical care medicine has not received as much attention and is even facing a trend of diminishing influence in the field of emergency medicine and critical care. Critical care medicine refers to a class of diseases that are characterized by rapid onset, severity, quick progression, and high mortality rates in clinical practice. The growth and development of renowned TCM experts throughout history have invariably been honed through practical experiences in managing critical care medicine. Treatise on Cold Damage (《伤寒论》) is a dedicated work for treating critical care medicine, and the classic prescriptions in Treatise on Cold Damage are particularly central to the treatment of these diseases. In the current clinical context, it is important to break through the prevailing mindset that the strengths of TCM lie solely in treating chronic diseases and maintenance-type illnesses, identify the entry point of TCM in the clinical advantages in the field of critical care medicine, explore the interdisciplinary relationship between Treatise on Cold Damage and critical care medicine, enhance the ability to treat critical care medicine using classic prescriptions, establish a disciplinary system for prescription-based treatment of critical care medicine, reveal clinical evidence for prescription-based treatment of these diseases, and elevate the contribution of TCM in the domain of critical care medicine. These have become a strong call and demand in contemporary TCM clinical practice. Regarding key scientific issues, the system of using classic prescriptions to treat critical care medicine encompasses traditional core theories such as the nature of cold damage, the essence of the six meridians, the implications of specific clauses, and the dosages of classic prescriptions. In the context of future academic inheritance and system construction, it is recommended to focus on clinical challenges, break away from the current limitations of traditional classical disciplinary systems, such as their singular patterns and weak innovative and translational capabilities, suggest defining diseases in critical care medicine responding specifically to TCM, advocate a research model based on combining disease mechanisms with pathology and combining medicinal properties with pharmacology, establish a comprehensive multidisciplinary thinking mode, and encourage collaborative innovative research among medical, educational, research, and manufacturing institutions. This will help chart the path and address key aspects of constructing a comprehensive system for prescription-based treatment of critical care medicine.

摘要:Gastric cancer is one of the most common malignant tumors of the digestive system, with a high incidence, a low overall survival rate, and poor prognosis after treatment. It has become a major public health problem that threatens the lives and health of people. Since the pathogenesis of gastric cancer is still not fully unraveled, it is difficult to implement primary prevention. Therefore, the research on secondary prevention of gastric cancer, i.e., precancerous lesions of gastric cancer, is extremely important and has become the focus of many researchers. Precancerous lesions of gastric cancer are the key pathological links in the transformation of inflammatory lesions of gastric mucosa into gastric cancer, including chronic atrophic gastritis, intestinal metaplasia, and dysplasia. Relevant studies have confirmed that with the aggravation of gastric mucosal lesions, the incidence and risk of gastric cancer also increase. Therefore, early diagnosis and effective intervention in the pathological link of gastric precancerous lesions is a key measure to prevent and reduce the incidence of gastric cancer, with great significance. More studies have shown that traditional Chinese medicine (TCM) can truncate and reverse the pathological grading of gastric mucosa, and can also effectively improve the clinical symptoms and quality of life of patients, with few adverse reactions and low recurrence rate, which has unique advantages and characteristics. The theory of ''pathogen invading nutrient and blood aspects'' was first proposed by WU Youxing, a febrile disease doctor, in Treatise on Pestilence (《温疫论》). It was originally used for the treatment of syndrome changes in the late stage of febrile epidemics, but after being enriched and developed by different doctors, it is now mostly used to guide the treatment of various chronic diseases. Precancerous lesions of gastric cancer are a common and difficult disease in clinical practice. Its evolution process is characterized by asthenia in origin and sthenia in superficiality and deficiency-excess in complexity, which is consistent with the core pathogenesis of ''pathogen invading nutrient and blood aspects'', namely, positive deficiency and intruding pathogen, and intruding pathogen cementation in the blood vessels. They are also interlinked in terms of treatment principles. Therefore, with the theory of ''pathogen invading nutrient and blood aspects'' as the breakthrough point, this article expounded the intervention effect of TCM on precancerous lesions of gastric cancer from the perspective of this theory, reflecting its important guidance and application value and providing new ideas for clinical treatment of diseases.

摘要:ObjectiveTo explore the formulation rules for the treatment of heart pain in the Medical Heart Enlightenment (《医学心悟》) from the perspective of the "Tangye Jingfa Tu", thereby providing a way of thinking about the treatment of heart pain in traditional Chinese medicine (TCM) and the study of the Medical Heart Enlightenment.MethodThe "Tangye Jingfa Tu" contained in the Secrets for Auxiliary Cultivation Life: The Essential Method of Using Herbal Medicine for the Differential Treatment of the Five Zang Organs (《辅行诀五脏用药法要》) was used to analyze the nine prescriptions for heart pain in Volume Ⅲ of the Medical Heart Enlightenment by CHENG Guopeng in the Qing dynasty. A "table for analyzing the formulation of the nine prescriptions for heart pain" was developed. The analysis diagrams for decoction and meridian rules in nine prescriptions for heart pain were plotted and the compatible structure of the medicinal flavors was analyzed.ResultThe composition of Chenxiang Jiangqisan for the treatment of Qi-induced heart pain is "five pungent, four bitter, and two sweet drugs", the composition of Shouniansan for the treatment of blood-induced heart pain is "five bitter, four pungent, three sweet, and one salty drugs", the composition of Qingzhongtang for the treatment of heat-induced heart pain is "six bitter, three pungent, and two sweet drugs”, the composition of Jiangfutang with Cinnamomi Cortex for cold-induced heart pain is "three pungent and two sweet drugs", the composition of Xiaobanxia modified with Fuling Tang for treating fluid retention-induced heart pain is "two pungent and two sweet drugs", the composition of Baohetang for treating heart pain due to dietary stagnation is "five sweet, four pungent, four bitter, and one sour drugs", the composition of Guipitang for the treatment of deficiency-induced heart pain is "eight sweet, four bitter, three pungent, and one sour drugs", the combination of Huachongwan for the treatment of worm-induced heart pain is "seven bitter, six pungent, and four sweet drugs", the composition of Shenzhusan, Congbaijiu, and Shengjiangtang for the treatment of resistance-induced heart pain is "eight pungent, four bitter, and two sweet drugs".ConclusionFrom the perspective of the "Tangye Jingfa Tu", the Medical Heart Enlightenment is based on the compatibility principle of "pungent-bitter-sweet drugs" in the treatment of heart pain, with heart deficiency treated with salty drugs for tonifying, or bitter-sweet-salty drugs, and heart excess treated with bitter drugs for purging, or sweet-pungent-bitter drugs, mostly applying the transformation rules of five medicinal flavors, i.e., "sweet-pungent-bitter drugs" and promoting the action of the pungent and sweet drugs acting on the spleen into the heart to relieve and purge the heart. In most cases, the treatment focuses on harmonizing the heart, liver, spleen, and kidneys, with an emphasis on the mother-child relationship and the application of the principles of the five elements generating and controlling each other. If the progression of the disease involves both the mother and child organs, the formulation should adhere to the compatibility rule of "the child organ makes the mother organ excess and the mother organ makes the child organ deficient".
摘要:ObjectiveTo effectively organize the interdisciplinary knowledge of traditional Chinese medicine (TCM) and evidence-based medicine contained in the clinical trial literature of TCM and facilitate the processing and mapping of multi-source data, this paper organized the knowledge of clinical trial literature of TCM by ontology modeling.MethodThe seven-step method and skeleton method were used to develop the ontology. After the structure and language characteristics of TCM clinical trial literature were analyzed, the ontological and non-ontological resources such as top-level framework of Scientific Evidence and Provenance Information Ontology (SEPIO) and TCM language system (TCMLS) were reused to determine the domain concepts and attribute relationship. Finally, the core concepts and attribute relationship such as disease, syndrome, symptom, grouping, intervention measures, outcome indicators and literature quality information were determined.ResultThe information contained in the clinical trial literature of TCM was divided into five categories. According to the Cochrane risk of bias assessment tool and CONSORT 2010 statement, the literature quality evaluation information was mapped to the ontology, and a total of 68 categories, 8 object attributes, and 38 data attributes were established, which basically realized the structured expression of clinical trial literature.ConclusionThe ontology of TCM clinical trial literature constructed in this study can well organize, utilize, and present the construction and association of internal knowledge system in TCM clinical trial literature, underpinning the reasoning of strength of evidence and information of diagnosis and treatment in the future.

摘要:As the overweight and obese population is growing, the incidence of obstructive sleep apnea is rising, and most of the cases are complicated with coronary heart disease and other cardiovascular diseases. The two diseases affect each other and seriously endanger the patients' health, becoming a major public health problem of global concern. It is of great clinical importance to explore the combination of Chinese and Western medicine in the prevention and treatment of coronary heart disease complicated with obstructive sleep apnea syndrome. Researchers have explored the relationship between the two based on traditional Chinese medicine(TCM) theory and found that the two diseases belong to the TCM disease categories of chest impediment and snoring, respectively, and their co-morbidity is associated with the abnormal physiological functions of the heart and lungs. The failure of the heart to govern blood leads to the generation of blood stasis, and that of the lung to govern Qi movement leads to the generation of phlegm. The accumulation of phlegm and blood stasis in the chest causes chest impediment and snoring due to obstruction of the airway. This paper discusses the internal linkage between the pathogenesis of coronary heart disease and obstructive sleep apnea syndrome in Chinese and Western medicine from the TCM theory of heart-lung correlation. Furthermore, this paper proposes the treatment principles of simultaneously treating the heart and lung and activating blood and resolving phlegm, aiming to provide a theoretical basis for the clinical prevention and treatment of coronary heart disease complicated with obstructive sleep apnea.

摘要:Steroid-induced necrosis of the femoral head (SNOFH) is a common orthopedic disease,which is difficult to cure and has poor clinical prognosis. The number of SNOFH patients in China is still increasing year by year,which seriously threatens human health. Long-term non-standard or short-term extensive use of hormone (GC) is an important reason for the occurrence of this disease. At present,SNOFH is mostly treated by surgical methods such as hip replacement,which has limitations of great harm to patients and high cost. In recent years,with the continuous deepening and innovation of traditional Chinese medicine(TCM) research,the use of TCM to treat SNOFH has been widely used in clinical practice. The main TCM pathogenesis of SNOFH is kidney deficiency and blood stasis. Therefore,TCM monomer and compound compound of tonifying kidney and promoting blood circulation are used to treat SNOFH. And TCM has obvious therapeutic effect,small side effects,less cost and other advantages. Glycoprotein/beta chain protein secretion (Wnt/beta- catenin) signaling pathway as a classic signaling pathway is closely related to the bone,between its by promoting bone marrow mesenchymal stem cell update,enhance the activity of osteoblast and suppress the apoptosis,which adjust the metabolic balance of bone tissue,increase bone density,will play an important role in the process of bone formation. In recent years,the use of TCM monomers and compounds to regulate Wnt/β-catenin signaling pathway to accelerate bone marrow mesenchymal stem cells,promote their transformation into osteoblasts,and maintain bone metabolic balance mechanism to treat SNOFH has become a new research hotspot. This article reviews the research progress of TCM in the prevention and treatment of SNOFH by regulating Wnt/β-catenin signaling pathway,in order to provide reference for the application of TCM in the treatment of SNOFH.

摘要:Total knee arthroplasty, as a common treatment option for advanced knee osteoarthritis, can alleviate the clinical symptoms of patients. Deep vein thrombosis of the lower extremity is the most common complication of total knee arthroplasty. Previous studies have shown that the occurrence of deep vein thrombosis after total knee arthroplasty is mostly related to the overexpression of inflammatory factors in vivo. Nuclear transcription factor-κB, Toll-like receptor 4, phosphatidylinositol 3-kinase/protein kinase B, tumor necrosis factor, and nuclear transcription factor E2-related factor 2 are typical signaling pathways related to inflammation. Regulating the expression of the signaling pathways can intervene the formation of inflammatory factors. Inhibiting the formation of inflammatory factors can help suppress the activation of platelets, thereby blocking thrombosis. According to previous research, Chinese medicine monomers, Chinese medicine extract, and compound Chinese medicine prescriptions all directly or indirectly inhibit the expression of inflammatory factors by regulating the above signaling pathways, thereby suppressing the occurrence of deep vein thrombosis after total knee arthroplasty. Therefore, Chinese medicine can reduce postoperative complications and promote postoperative recovery of patients at low cost with small side effects. This article summarizes the research on Chinese medicine intervention on deep vein thrombosis-related signaling pathways after total knee arthroplasty, which is expected to lay a basis for the in-depth study and clinical application of Chinese medicine in deep vein thrombosis.
摘要:In recent years, with the changes of population structure and the aggravation of aging, the prevalence of osteoporosis is increasing year by year. Osteoporosis poses great impacts on the body and family life of the patients and increases the burden on the society. Therefore, the research on osteoporosis is urgent and significant. The imbalance between osteoblasts, osteocytes, and osteoclasts causes abnormal bone metabolism, which destroys the fine structure of bone and increases bone fragility, thus increasing the risk of fracture. Although the pathogenesis of osteoporosis is complex, researchers have confirmed that the imbalance of the endocrine system directly or indirectly promotes the occurrence and development of osteoporosis. Traditional Chinese medicine (TCM) is a treasure of Chinese traditional culture and plays a key role in safeguarding the public health. With unique therapeutic effects and advantages, TCM has been widely accepted. Chinese medicines, moxibustion, acupuncture and other TCM therapies have play a unique role in the treatment of osteoporosis. Particularly, TCM prevention and treatment of osteoporosis by regulating the endocrine system has received extensive attention. By reviewing relevant literature, this paper introduces the research progress in the TCM modulation of bone metabolism and alleviation of bone loss by regulating estrogen, calcitonin, and parathyroid hormone in the endocrine system and affecting the hypothalamus capable of regulating these hormones, aiming to provide ideas for the TCM treatment of osteoporosis.

摘要:As one of the classic prescriptions of traditional Chinese medicine, Didangtang is an effective prescription for breaking and expelling blood stasis. It was considered that Didangtang damage the healthy qi and contained toxic Chinese medicinal materials such as leeches and gadflies, and thus it was rarely used. However, as the attention to classic prescriptions increases, Didangtang has been widely used in clinical practice and demonstrated definite efficacy in treating diabetes mellitus and its complications, malignant tumors, Alzheimer's disease, stroke, renal diseases, cardiovascular diseases, peripheral vascular diseases, gynecological disease, and male diseases according to the disease location, pathogenesis, symptoms, and pharmacological effects. Didangtang has the effects of mitigating insulin resistance, improving microvascular and peripheral vascular circulation, delaying diabetic macrovascular lesions, preventing vascular fibrosis, improving immunity, inhibiting tumor growth, protecting the brain tissue, nerve cells, vascular endothelial function, and kidney, reducing inflammation, and delaying aging. This paper summarizes the clinical application of Didangtang and initially explores the underlying mechanism.
摘要:Gegen Qinliantang composed of Puerariae Lobatae Radix, Scutellariae Radix, Coptidis Rhizoma, and Glycyrrhizae Radix et Rhizoma exhibits exterior-releasing and interior-clearing effects and can treat diarrhea caused by fever invasion. Therefore, it is highly praised by physicians for treating damp-heat dysentery. This article systematically reviews the clinical applications and basic research advances of Gegen Qinliantang in treating ulcerative colitis. Modern clinical research has demonstrated that modified Gegen Qinliantang has also been used to treat ulcerative colitis, and it is often used in combination with Western drugs (mesalazine, olsalazine, sulfasalazine, etc.), other Chinese medicine prescriptions (Tongxie Yaofang, Baitouweng Tang, Shaoyaotang, Xianglianwan, etc.), and acupuncture. Such therapties can significantly alleviate symptoms, such as abdominal pain, diarrhea, and mucopurulent bloody stools, lower traditional Chinese medicine symptom scores and Baron score, improve the quality of life and sleep, reduce adverse reactions, and decrease the relapse rate within one year. Pharmacological studies have demonstrated that Gegen Qinliantang can inhibit the secretion of pro-inflammatory cytokines, such as tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6), and enhance the release of the anti-inflammatory cytokine interleukin-10 (IL-10) to alleviate intestinal inflammation. Additionally, it can modulate cellular signaling pathways and interfere with upstream and downstream connections in the pathways to inhibit oxidative stress response and prevent cell apoptosis. By regulating the balance of T-helper 17 (Th17) and regulatory T (Treg) cells and controlling the normal expression of immunoglobulins, Gegen Qinliantang can restore immune function. Moreover, this prescription can enrich the beneficial microorganisms in the intestine and stimulate intestinal epithelial cell regeneration to protect and repair the intestinal mucosal barrier. The active components in Gegen Qinliantang, such as puerarin, berberine, baicalin, and glycyrrhizin, can exert antibacterial, anti-inflammatory, and metabolic regulatory effects. This review provides a basis for treating ulcerative colitis with Gegen Qinliantang.
摘要:Atherosclerosis is a chronic inflammatory disease caused by lipid accumulation and vascular endothelial dysfunction. The Toll-like receptor (TLR)/nuclear transcription factor-κB (NF-κB) pathway and the NOD-like receptor protein 3 (NLRP3) inflammasome pathway play a proinflammatory role, while the transient receptor potential vanilloid subtype 1 (TRPV1) and transient receptor potential ankyrin 1 (TRPA1) play a protective role in the occurrence of atherosclerosis. We reviewed the relevant studies published in the last 10 years. The results showed that activation of TRPV1/TRPA1 could activate endothelial-type nitric oxide synthase (eNOS) and inhibit the generation of reactive oxygen species (ROS) and cholesterol crystal (CC) to modulate the TLR/NF-κB and NLRP3 inflammasome pathways, thereby inhibiting TLR/NLRP3-mediated inflammatory response. A variety of compound prescriptions and active components of Chinese medicinal materials can activate TRPV1/TRPA1 or its downstream pathway to regulate the TLR/NLRP3 pathway in atherosclerosis. This paper introduces the mechanisms of compound prescriptions and active components of Chinese medicinal materials in regulating the TLR/NLRP3 pathway via TRPV1/TRPA1 in atherosclerosis. This review provides new ideas for the research on the interactions between Chinese medicines in the treatment of atherosclerosis and provides a new strategy for the clinical treatment of atherosclerosis with traditional Chinese medicine.
摘要:Alzheimer's disease (AD) is a neurodegenerative disorder characterized by mechanisms including excessive deposition of β amyloid (Aβ), neuroinflammatory responses, and hyperphosphorylation of microtubule-associated protein (Tau protein). Currently, there are no effective clinical treatments available for AD. Traditional Chinese herbal drugs have gained attention for their potential to exert anti-AD effects through multi-component, multi-target approaches. As a traditional Chinese herbal drug with over 600 years of clinical use, Geranii Herba has substantial medicinal potential and wide application prospects. Geranium medicinal plants contain chemical components such as tannins, flavonoids, organic acids, and volatile oils. Research has indicated that various tannin compounds found in Geranii Herba possess pharmacological activities like enhancing learning and memory abilities, and improving cognitive function. These effects are linked to mechanisms involving anti-Aβ effects, Tau protein regulation, antioxidation, anti-inflammation, and inhibition of acetylcholinesterase activity. These compounds can act through multiple pathways and targets to inhibit neurodegenerative changes in neurons, thus effectively preventing and treating neurodegenerative diseases like AD. Based on relevant literature, this study focused on reviewing various tannin components in Geranium plants and their role in preventing and treating AD and identified potential drug components for treating neurodegenerative diseases such as AD by exploring the tannin components and their mechanisms in Geranium plants, thereby providing a theoretical foundation and research direction for further development and clinical application.
摘要:Cordyceps, a unique precious Chinese herbal medicine in China, has the effects of tonifying the kidney and lungs, stopping bleeding, and resolving phlegm. It has definite clinical effects on diabetes, respiratory diseases (such as chronic obstructive pulmonary disease), chronic kidney diseases, slow arrhythmia, and hyperlipidemia. We searched China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP, SpringerLink (Biomedical Sciences), Embase, Proquest (Life Sciences), PubMed, Cochrane Library, and SinoMed for the research on the arsenic content in Cordyceps. According to statistical analysis, arsenic in Cordyceps mainly exists in the non-toxic or low-toxic form. The arsenic-containing compounds in Cordyceps may be an arsenic-sugar compound with anticancer effect. The inorganic arsenic content in Cordyceps is lower than the arsenic standard stipulated by the State Food and Drug Administration. It remains uncertain whether different measurement methods can affect the results. The total arsenic content varies in Cordyceps from different producing areas, and the general rule of total arsenic content in Cordyceps produced in different provinces, districts, and counties cannot be obtained from the results of the existing studies. The arsenic content in Cordyceps is mainly concentrated in the insect part, which is significantly different from that in stroma, and no arsenic is detected in the mycelia. The soil arsenic content in the growing areas is generally high, which may be a main reason for the high arsenic content in Cordyceps. As a valuable medicinal material for tonifying and nourishing, Cordyceps still plays an irreplaceable role in disease prevention, health care, chronic disease management, and rehabilitation.
摘要:Direct acid hydrolysis of Dioscorea zingiberensis rhizomes for preparation of diosgenin is wildly used in the traditional industry, which uses a large amount of inorganic acid catalysts, with high wastewater discharge and serious environmental pollution. Therefore, exploring clean and efficient preparation methods and processes has become an inevitable choice to realize the sustainable development of industrial production of diosgenin. Herein, the author reviewed and analyzed the research progress and problems of enzymatic hydrolysis, microbial transformation and modified acid hydrolysis in the preparation of diosgenin from D. zingiberensis rhizomes during the last ten years, and their application prospects are analyzed. Enzymatic hydrolysis has mild reaction conditions, but the yield of diosgenin is low, the economic cost is high, and the purification process of active enzyme is complicated. Microorganism shows specific activity to the substrate and high efficiency for diosgenin production, and microbial transformation is clean and environmentally friendly, but microbial transformation is time-consuming and the metabolic intermediates are complicated. For the modified acid hydrolysis, two-phase acid hydrolysis can reduce the amount of acid catalyst, and sulfonic acid-functionalized ionic liquid displays good recyclable performance by replacing the traditional inorganic acid, however, the wastewater discharge should still be considered. Solid acid catalysts are non-corrosive and easy to be recycled, but the need to use ethanol as the reaction solvent has certain safety hazards, and the catalyst preparation process is cumbersome. In conclusion, exploring clean and efficient conversion methods is an important research trend for preparation of diosgenin from D. zingiberensis rhizomes. For the enzymatic hydrolysis, the key glycoside hydrolases in the bioconversion process should be explored in depth, the conversion pathway of enzymatic saponins and enzyme specificity should be fully elucidated, and efforts should be made to improve the efficiency of enzymatic hydrolysis. For the microbial transformation, we should accelerate its industrial application process based on selecting and breeding efficient transformation strains, and optimizing stable transformation systems and processes, and in-depth investigation of the mechanism of microbial transformation, fully elucidating the specific key hydrolases and its catalytic properties, and striving to improve the efficiency of microbial transformation. For the modified acid hydrolysis, novel acid catalytic system with simple structure, stable performance and good biodegradability should be explored and applied, which can effectively solve the problems of environmental pollution and production safety.
