      摘要:ObjectiveTo observe the effect of Huanglian Jiedutang on the inflammatory injury in the mouse model of acute gouty arthritis (AGA) and to explore the mechanism of Huanglian Jiedutang in treating AGA.MethodForty male C57BL/6J mice were randomized into blank, model, colchicine (0.83 mg·kg-1), and Huanglian Jiedutang (5 g·kg-1) groups. The mouse model of AGA was established by injecting monosodium urate (MSU) crystals into the ankle joint. The swelling degree of the right ankle joint of each mouse was measured every day for 7 days, and the pathological changes of the ankle joint were detected by hematoxylin-eosin (HE) staining after 7 days. The other 40 C57BL/6J mice were grouped as above. After 18 hours of modeling, the right ankle joint was collected, and real-time polymerase chain reaction was employed to measure the mRNA levels of interleukin (IL)-1β, tumor necrosis factor (TNF)-α, IL-6, NOD-like receptor protein 3 (NLRP3), apoptosis-associated speck-like protein containing a CARD (ASC), and Caspase-1. The expression levels of IL-1β, TNF-α, and IL-6 were measured by the enzyme-linked immunosorbent assay. Western blot was employed to determine the protein levels of NLRP3 inflammasome, Toll-like receptor 4 (TLR4), and nuclear factor-κB (NF-κB).ResultCompared with the blank group, the model group showed swelling right ankle joint (P<0.01), obvious foreign body granuloma in the ankle joint with inflammatory cell infiltration. After the treatment with Huanglian Jiedutang, the ankle joint swelling was relieved (P<0.05, P<0.01), and the size of foreign body granuloma was reduced. Compared with the blank group, the model group showed up-regulated mRNA levels of IL-1β, TNF-α, and IL-6 in the ankle joint tissue (P<0.01), up-regulated mRNA levels of NLRP3 and Caspase-1 in the NLRP3 inflammasome (P<0.05, P<0.01), and up-regulated protein levels of NLRP3, Caspase-1, TLR4, and NF-κB (P<0.05, P<0.01). Huanglian Jiedutang down-regulated the mRNA levels of IL-1β, TNF-α, IL-6, NLRP3, and Caspase-1 (P<0.05, P<0.01) and the protein levels of IL-1β, TNF-α, IL-6, NLRP3, Caspase-1, TLR4, and NF-κB (P<0.05 or P<0.01).ConclusionInjecting MSU crystal resulted in local inflammatory injury of the joints in the mouse model of AGA. The treatment with Huanglian Jiedutang may alleviate the inflammatory injury by regulating the NLRP3 inflammasome and TLR4/NF-κB signaling pathway.

      摘要:ObjectiveTo explore the mechanism of Shaoyaotang (SYT) in the treatment of ulcerative colitis (UC) based on network pharmacology and experimental verification.MethodThe core components, target genes, and main pathways of SYT were predicted based on network pharmacology, and UC-related components, target genes, and pathways were screened. Dextran sodium sulfate (DSS) was used to induce the UC model in mice, and the effect of SYT on UC mice was observed, followed by mechanism verification.ResultNetwork pharmacology indicated that 174 active components and corresponding 159 target genes of SYT were screened, and the related pathways were those mediated by 5-hydroxytryptamine (5-HT) degredation and 5-HT receptor 3. The results of animal experiments showed that compared with the model group, the SYT group showed increased body weight and colon length(P<0.01), reduced disease activity index (DAI) score (P<0.01), improved histopathological manifestations, reduced concentrations of 5-HT in the colonic tissues and serum (P<0.05, P<0.01), and increased mRNA expression of monoamine oxidase A (MAOA), monoamine oxidase B (MAOB), sodium-dependent serotonin transporter (SLC6A4), and 5-HT receptor 3A (5-HTR3A) related to 5-HT metabolism in the colon (P<0.01).ConclusionSYT can alleviate the local inflammatory response of the intestinal tract in UC by regulating 5-HT degredation pathways.

      摘要:ObjectiveTo explore the therapeutic effect and mechanism of Yiqi Huoxue Tongbian prescription on slow transit constipation (STC) in rats.MethodThe rat model of STC was established by gavage of loperamide hydrochloride. Rats were assigned into control, model, mosapride, low-, medium-, and high-dose (3.51, 7.02, and 14.04 g·kg-1, respectively) Yiqi Huoxue Tongbian prescription groups. The changes of general signs, fecal moisture content, and intestinal propulsion rate were measured after model establishment and drug administration. The colonic mucosal changes were observed by hematoxylin eosin staining. Enzyme-linked immunosorbent assay was employed to determine the content of substance P (SP) and vasoactive intestinal peptide (VIP) in the colon of rats in each group. The gray values of aquaporin (AQP) 3, AQP4, AQP8, and c-Kit in rat colon tissue were measured by immunohistochemistry and Western blot, and the changes of intestinal flora were detected by 16S rRNA high-throughput sequencing.ResultCompared with the model group, 10 days of treatment with Yiqi Huoxue Tongbian prescription increased the fecal moisture content and intestinal propulsion rate (P<0.01). The medium- and high-dose Yiqi Huoxue Tongbian prescription groups and the mosapride group showed no obvious mucosal inflammation and neat arrangement of goblet cells with a large number in the colon tissue. Moreover, the three groups showed increased SP content (P<0.01) and decreased VIP content (P<0.01) in the serum. The medium- and high-dose Yiqi Huoxue Tongbian prescription groups showed down-regulated protein levels of AQP3, AQP4, and AQP8 (P<0.01) and up-regulated protein level of c-Kit (P<0.01). The drug administration groups presented slightly increased observed species, Chao1, ACE, and Shannon, Simpson, and PD whole tree. The principal component analysis showed that the control group had a short distance with the high- and medium-dose Yiqi Huoxue Tongbian prescription groups, indicating that high- and medium-dose Yiqi Huoxue Tongbian prescription can recover the intestinal flora to that in the control group.ConclusionYiqi Huoxue Tongbian prescription can alleviate the defecation status of rats with slow transit constipation by down-regulating the expression of AQP3, AQP4, and AQP8 to reduce the absorption of water in the intestine, up-regulating the expression of c-Kit to increase the number and distribution of Cajal interstitial cells, and regulating the balance of flora in the colon tissue.

      摘要:ObjectiveTo investigate the inhibitory effects and mechanism of the compound Phyllanthus urinaria Ⅱ (CPU Ⅱ)on the growth of transplanted hepatocellular carcinoma Hep3B2.1-7 (Short for Hep3R) cells in nude mice.MethodAfter the establishment of a xenograft model of hepatocellular carcinoma Hep3B cells in mice, the model mice were randomly divided into a model group, a high-dose CPU Ⅱ group (57.5 g·kg-1), a low-dose CPU Ⅱ group (28.75 g·kg-1), and a 5-fluorouracil (5-FU) group (0.025 g·kg-1), with eight mice in each group. The mice in the high- and low-dose CPU Ⅱ groups were treated with drugs by gavage, once per day, and those in the model group were treated with the same volume of normal saline. The mice in the 5-FU group were treated by 5-FU by intraperitoneal injection, once every other day. After 28 days of administration, mice were sacrificed, and transplanted tumors were collected. Immunohistochemistry (IHC) was used to detect the expression of proliferating cell nuclear antigen (PCNA) of tumor tissues. Terminal-deoxynucleotidyl transferase-mediated nick end labeling (TUNEL) was used to detect cell apoptosis of tumor tissues. The mRNA expression of miR-122 and insulin-like growth factor 1 receptor (IGF-1R) in tumor tissues was detected by Real-time quantitative PCR (Real-time PCR). The protein expression of CCAAT/enhancer-binding protein α (C/EBPα), hepatocyte nuclear factor-4α (HNF-4α), and IGF-1R in tumor tissues was detected by Western blot.ResultThe tumor suppression rates of the high- and low-dose CPU Ⅱ groups and the 5-FU group were 74.90%, 63.62%, and 64.15%, respectively. Compared with the model group, the CPU Ⅱ groups and the 5-FU group showed reduced weight (P<0.01) and volume of tumors (P<0.01), decreased PCNA positive cells, shallow staining, increased apoptosis cells of transplanted tumor tissues (P<0.05, P<0.01), increased expression of mRNA expression of miR-122 (P<0.01), down-regulated mRNA expression of IGF-1R (P<0.01), and up-regulated protein expression of C/EBPα and HNF-4α in nude mouse transplanted tumor tissues (P<0.01). The expression of IGF-1R protein in the high-dose CPU Ⅱ group was down-regulated (P<0.05). Compared with the low-dose CPU Ⅱ group, the high-dose CPU Ⅱ group showed increased apoptotic cells (P<0.01), up-regulated mRNA expression of miR-122 (P<0.01), and increased expression of C/EBPα and HNF-4α proteins (P<0.01).ConclusionCPU Ⅱ has an obvious inhibitory effect on the growth of transplanted hepatocellular carcinoma Hep3B cells in nude mice. The mechanism of action is related to enhancing the expression of transcription factors HNF-4α and C/EBPα, thereby promoting the expression of miR-122 and inhibiting the expression of its target gene IGF-1R.

      摘要:ObjectiveTo investigate the mechanism of Yiqi Huoxue Tongluo prescription (YHTP) in the treatment of diabetic neuropathic pain (DNP).MethodNinety SPF-grade SD male rats were randomized into blank, model, low- (2.25 g·kg-1), medium- (4.5 g·kg-1), and high-dose (9 g·kg-1) YHTP, and mecobalamin (0.175 mg·kg-1) groups. Except those in the blank group, the rats in the remaining 5 groups were fed with a high-fat and high-glucose diet and subjected to intraperitoneal injection of low-dose (35 mg·kg-1) streptozotocin (STZ) to establish the model of DNP. The sciatic nerve conduction velocity in DNP rats was measured by the neurophysiological method, and the levels of interleukin-6 (IL-6), IL-1β, and tumor necrosis factor-α (TNF-α) were measured by enzyme-linked immunosorbent assay (ELISA). Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was employed to measure the mRNA levels of glial fibrillary acidic protein (GFAP) and extracellular signal-regulated kinase (ERK) in the spinal cord. Western blot was employed to measure the protein levels of GFAP and phosphorylated ERK (p-ERK), and immunofluorescence staining to measure the fluorescence intensity of GFAP and p-ERK in the spinal cord. In the cell experiments, 100 mmol·L-1 high glucose was used to induce the activation of astrocytes (CTX-TNA2) for the modeling of nerve cell injury. The cells were randomized into the normal, model, drug-containing serum (10% YQHT), inhibitor [10 mol·L-1 corynoxeine (COR)], drug-containing serum + inhibitor (10% YHTP + 10 mol·L-1 COR) groups. The levels of pro-inflammatory factors (TNF-α and IL-1β) and the anti-inflammatory factor IL-10 in CTX-TNA2 cells were determined by ELISA, and the protein levels of GFAP and p-ERK in CTX-TNA2 cells by Western blot.ResultThe animal experiments showed that compared with the blank group, the model group presented reduced mechanical withdrawal threshold (MWT), thermal work limit (TWL), and nerve conduction velocity, elevated levels of fasting blood glucose, IL-1β, TNF-α, and IL-6, and up-regulated protein levels of GFAP and p-ERK, and mRNA levels of ERK1, ERK2, GFAP (P<0.01). Compared with model group, YHTP increased the MWT, TWL, and sciatic nerve conduction velocity (P<0.01), lowered the levels of IL-1β, TNF-α, and IL-6 (P<0.01), and down-regulated the protein levels of GFAP and p-ERK, and mRNA levels of ERK1, ERK2, GFAP in the spinal cord (P<0.05, P<0.01). The cell experiments showed that compared with the blank group, the model group had decreased survival rate, elevated levels of pro-inflammatory factors, and up-regulated protein levels of ERK and GFAP (P<0.01). Compared with the model group, the YHTP-containing serum lowered the levels of IL-1β and TNF-α (P<0.05, P<0.01), elevated the level of IL-10 (P<0.01), and down-regulated the protein levels of ERK and GFAP (P<0.01).ConclusionYHTP may inhibit the activation of astrocytes by inhibiting the ERK signaling pathway to reduce inflammation and thus relieve DNP.

      摘要:ObjectiveTo establish and evaluate a chronic obstructive pulmonary disease (COPD) model with lung-spleen qi deficiency.MethodA rat model mimicking COPD with lung-spleen qi deficiency was established by the combination of cigarette smoking and intratracheal instillation of lipopolysaccharide (LPS) along with gavage of Sennae Folium infusion. Forty male SPF-grade SD rats were randomly assigned to blank, model, and low- (L-FXY), medium- (M-FXY), and high-dose (H-FXY) Sennae Folium infusion groups. Other groups except the blank group were exposed to daily cigarette smoke, with LPS administrated via intratracheal instillation on the 1st and 14th days. On the 28th day of modeling, the L-FXY, M-FXY, and H-FXY groups were administrated with Sennae Folium infusion at 5, 10, and 20 g·kg-1, respectively, and at 4 ℃ for three weeks. The modeling lasted for 49 days. The general conditions (body mass, food intake, fecal water content, and anal temperature) and behaviors (grip strength test and tail suspension test) of rats in different groups were examined. The lung function, lung histopathology, D-xylose, amylase, and gastrin levels in the serum, interleukin(IL)-1β and IL-6 levels in the alveolar lavage fluid, levels of T-lymphocyte subsets (CD4+, CD8+, and CD4+/CD8+) in the peripheral blood, and thymus and spleen indices were measured.ResultTwo rats died in the H-FXY group. Compared with the blank group, both the M-FXY and H-FXY groups exhibited reduced body mass and food intake (P<0.01) and increased fecal water content (P<0.01). The anal temperature in the H-FXY group was lower than that in the blank group (P<0.01). The grip strength decreased in the modeling groups compared with the blank group (P<0.01), and the duration of immobility in the tail suspension test increased in the M-FXY and H-FXY groups (P<0.05, P<0.01). Compared with the blank group, the modeling groups showed reduced 0.3 second forced expiratory volume (FEV0.3), FEV0.3/forced vital capacity (FVC)(P<0.01), thickening of bronchial walls, proliferation of goblet cells, and the presence of emphysematous changes. In terms of gastrointestinal function, the M-FXY and H-FXY groups had lower levels of D-xylose, gastrin, and α-amylase than the blank group (P<0.01). Regarding the immune and inflammatory indices, the M-FXY and H-FXY groups showed lower thymus and spleen indices than the blank group (P<0.01). Compared with the blank group, the modeling groups presented lowered CD4+ level (P<0.01) and CD4+/CD8+ ratio (P<0.05, P<0.01) in the peripheral blood and elevated levels of IL-1β and IL-6 in the alveolar lavage fluid (P<0.01) than the blank group.ConclusionA model of COPD with lung-spleen Qi deficiency was established through the combination of daily cigarette smoke, intratracheal instillation with LPS, and gavage of Sennae Folium infusion. The comprehensive evaluation results suggested medium-dose (10 g·kg-1) Sennae Folium infusion for gavage during the modeling of COPD with lung-spleen Qi deficiency.

      摘要:ObjectiveTo investigate the mechanism of Jiedu Huoxue prescription in promoting the reendothelialization of injured vessels by regulating the nuclear factor (NF)-κB/NOD-like receptor protein 3 (NLRP3)/cysteine-aspartic acid protease (Caspase)-1-mediated pyroptosis.MethodA rat model of injured thoracic aorta was established by balloon injury, and 36 rats were assigned into shame surgery, model, low-, medium-, and high-dose Jiedu Huoxue prescription, and atorvastatin calcium tablet groups. The injured aortic segment was collected 28 days after surgery. Hematoxylin-eosin (HE) staining and Evans blue staining were conducted to reveal the changes of vascular structural morphology and the reendothelialization of blood vessels, respectively. The enzyme-linked immunosorbent assay (ELISA) was employed to determine the levels of tumor necrosis factor-α (TNF-α), intercellular adhesion molecule-1 (ICAM-1), interleukin (IL)-1β, and nitric oxide (NO) in the serum. Western blotting was employed to determine the expression of endothelial nitric oxide synthase (eNOS), NF-κB p65, phospho-NF-κB p65 (p-NF-κB p65), NLRP3, and Caspase-1 in the vascular tissue.ResultThe model group showed thickened endovascular membrane, proliferation and disarrangement of smooth muscle cells of the artery wall, obvious inflammatory cell infiltration, and narrowed luminal area. Jiedu Huoxue prescription and atorvastatin calcium tablets mitigated the pathological changes of the thoracic aorta in different degrees. After balloon injury, the endothelial coverage rate of the model group decreased significantly, while Jiedu Huoxue prescription and atorvastatin calcium tablets increased the reendothelialization rate (P<0.05). Compared with the shame surgery group, the model group showed elevated levels of TNF-α, ICAM-1, and IL-1β (P<0.01) and lowered NO level (P<0.01) in the serum. In addition, the model group presented down-regulated protein level of eNOS (P<0.01) and up-regulated phosphorylation of pyroptosis-associated proteins NLPR3, Caspase-1, and NF-κB p65 in the vascular tissue (P<0.05, P<0.01). Compared with the model group, Jiedu Huoxue prescription and atorvastatin calcium tablets lowered TNF-α, ICAM-1, and IL-1β levels (P<0.05, P<0.01) and elevated the NO level in the serum (P<0.05, P<0.01). Moreover, the drugs up-regulated the expression of eNOS (P<0.01) and down-regulated the expression of NLRP3, Caspase-1, and NF-κB p65 (P<0.05, P<0.01) in the vascular tissue.ConclusionJiedu Huoxue prescription can promote the reendothelialization and inhibit the intimal hyperplasia of vessels after balloon injury by regulating the NF-κB/NLRP3/Caspase-1 pathway to inhibit pyroptosis and reduce endothelial inflammatory injury.

      摘要:ObjectiveTo observe the effects of Dendrobium polysaccharides on the secretion of inflammatory cytokines and Toll-like receptor 4 (TLR4)/nuclear factor (NF)-κB pathway in 16HBE cells exposed to cigarette smoke extract (CSE).MethodThe 16HBE cells were classified into the control, CSE, and CSE+ Dendrobium polysaccharides (100, 200, 400 mg·L-1) groups. The cell-counting kit-8 (CCK-8) assay was employed to measure the cell viability, and a microscope was used to observe the cell morphology. The enzyme-linked immunosorbent assay was employed to measure the levels of interleukin (IL)-8, IL-1β, IL-4, IL-13, and transforming growth factor (TGF)-β in cell culture supernatants. Real-time PCR was carried out to determine the mRNA levels of Toll-like receptor 4 (TLR4), nuclear factor-κB (NF-κB), and IL-4. Western blot was employed to determine the protein levels of interleukin-4 receptor (IL-4R), TLR4, myeloid differentiation primary response protein 88 (MyD88), NF-κB, phosphorylated nuclear factor-κB (p-NF-κB), and nucleoproteins nuclear factor-κB (NEs-NF-κB). The immunofluorescence assay was employed to measure the nuclear translocation of NF-κB.ResultCompared with the control group, the CSE group showed elevated levels of IL-8, IL-1β, IL-4, IL-13, and TGF-β in the cell culture supernatants (P<0.05, P<0.01), up-regulated expression levels of TLR4, MyD88, NF-κB, p-NF-κB, NEs-NF-κB, and IL-4 (P<0.01), and significant nuclear translocation of NF-κB. Compared with the CSE group, Dendrobium polysaccharides increased the cell survival rate, recovered the cell activity, lowered the levels of IL-8, IL-1β, IL-4, IL-13, and TGF-β, down-regulated the expression of TLR4, MyD88, NF-κB, p-NF-κB, NEs-NF-κB, and IL-4 (P<0.05, P<0.01), and reduced the nuclear translocation of NF-κB.ConclusionDendrobium polysaccharides showed significant protective effects on the 16HBE cells exposed to CSE by inhibiting the TLR4/NF-κB signaling pathway.

      摘要:ObjectiveTo explore the anti-gout effect and mechanism of Derris eriocarpa extract by network pharmacological analysis combined with in vivo and in vitro experimental verification.MethodThe chemical components and candidate targets of D. eriocarpa were obtained from the database. The key targets and potential active components of D. eriocarpa in the treatment of gout were screened by the protein-protein interaction analysis, and then the Gene Ontology (GO) functional annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment were performed for the key targets. A mouse model of hyperuricemia was established by intraperitoneal injection of hypoxanthine to observe the effect of D. eriocarpa alcohol extract on hyperuricemia. A rat model of gouty inflammation induced by the injection of microcrystalline sodium urate crystals into the foot and plantar was used to observe the effect of D. eriocarpa alcohol extract on gouty inflammation. A xylene-induced acute inflammation model was established to observe the anti-inflammatory effect of D. eriocarpa alcohol extract. The hot plate test and twisting test were performed to observe the pain-relieving effect of D. eriocarpa. The lipopolysaccharide (LPS)-induced RAW264.7 cells were used to study the anti-gout effect and mechanism of D. eriocarpa alcohol extract.ResultA total of 12 key targets and 15 potential active components were obtained from the D. eriocarpa-component-gout target network. The emodin, betulinic acid, and medicarpin endowed D. eriocarpa with anti-hyperuricemia, anti-inflammatory, and pain-relieving effects by acting on Toll-like receptor 4 (TLR4), NOD-like reception protein 3 (NLRP3), and nuclear factor (NF)-κB. Compared with the control group, the model groups showed elevated serum uric acid level in mice (P<0.01), increased swelling degree of rats (P<0.05, P<0.01), alleviated the auricular swelling of mice (P<0.05), reduced the twisting times of mice (P<0.05, P<0.01), and increased the hot plate pain threshold (P<0.05). Moreover, the model group showed up-regulated mRNA level of TLR4 and protein levels of TLR4, NF-κB, and NLRP3 in cells (P<0.01), and elevated levels of TLR4 and NF-κB in the cell supernatant (P<0.05, P<0.01). Compared with the model group, the alcoholic extracts (20, 10, 5 g·kg-1) of D. eriocarpa lowered the serum uric acid level in hyperuricemic mice (P<0.01), inhibited foot and plantar swelling in rats (P<0.05, P<0.01), down-regulated the mRNA level of TLR4 and the protein levels of TLR4, NF-κB, NLRP3 in cells, and lowered the levels of TLR4, TNF-α, NF-κB, and IL-6 in cell supernatants (P<0.05, P<0.01).ConclusionD. eriocarpa alcohol extract may exert the anti-gout, anti-inflammatory, and pain-relieving effects by regulating the TLR4/NF-κB/NLRP3 signaling pathway.

      摘要:ObjectiveTo investigate the mechanism of Gegen Qinliantang(GQT) on the intestinal flora of antibiotic-associated diarrhea(AAD) by 16S rRNA sequencing and network pharmacology.MethodSixty SD rats were randomly divided into six groups(n=10), including blank group, model group, GQT high-, medium- and low-dose groups(10.08, 5.04, 2.52 g·kg-1) as well as Lizhu Changle group(0.15 g·kg-1), except for the blank group, each group was given clindamycin(250 mg·kg-1) by gavage once a day for 7 consecutive days. After successful modeling, the blank group and the model group were given equal volumes of normal saline by gavage. The other groups were given corresponding doses of drugs by gavage for 14 days. Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP) was used to screen the active components and targets of GQT, GeneCards, Online Mendelian Inheritance in Man(OMIM) database, Pharmacogenetics and Pharmacogenomics Knowledge Base(PharmGKB), DrugBank and DisGeNET were used to search for AAD disease targets. The drug-disease common targets were obtained by R software. STRING was applied to analyze the target protein-protein interaction, and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analysis was performed. Then hematoxylin-eosin(HE) staining was used to observe the pathological changes of the colon, and 16S rRNA sequencing of AAD colon content flora structure further verified the results of network pharmacology.ResultThrough network pharmacology, it was found that 238 active components were screened from GQT and acted on 276 component targets, among which quercetin, puerarin, wogonin and apigenin were the main core components of GQT, 1 097 AAD disease targets and 127 drug-disease intersection targets. The protein-protein interaction network mainly included core targets such as protein kinase B1(Akt1), interleukin(IL)-6 and IL-1β, which were mainly enriched in the IL-17 signaling pathway. It was verified through animal experiments that compared with the blank group, the colon structure of the model group was seriously abnormal, the intestinal epithelial columnar cells were damaged, the goblet cells were reduced, and a large number of inflammatory cells were infiltrated. Compared with the model group, the colon structure of the GQT high-dose group improved, but there were still abnormalities, the colon structure of GQT medium- and low- dose groups and Lizhu Changle group improved significantly and reached the normal level. GQT could improve the structural diversity of AAD intestinal flora. At the phylum level, the abundance of Firmicutes was increased and the abundance of Bacteroidetes was decreased. At the genus level, the abundance of Lactobacillus was increased, and the abundances of Prevotella and Bacteroides were decreased. Among them, Lactococcus could be used as a biomarker for AAD treatment with GQT, and the prediction of functional metabolism of intestinal flora revealed that GQT could promote acetate and lactate metabolic pathways in the intestine.ConclusionGQT may activate IL-17 signaling pathway by acting on the targets of Akt1 and IL-6 through key components such as quercetin and wogonin, and improve the abundance of Lactococcus in the intestinal tract as well as acetate and lactate metabolic pathways, so as to play a role in repairing the intestinal barrier for the treatment of AAD.

      摘要:ObjectiveTo evaluate the effect of Shengmaisan granules on myocardial fibrosis in chronic heart failure patients with Qi-Yin deficiency syndrome by cardiac magnetic resonance (CMR) imaging and serological indicators.MethodSixty-six chronic heart failure patients with Qi-Yin deficiency syndrome who visited the Affiliated Hospital of Integrated Traditional Chinese and Western Medicine, Nanjing University of Chinese Medicine from October 2021 to January 2023 were selected. The patients were assigned into a control group (33 cases) and an observation group (33 cases) by the minimization random method. Both groups received standardized Western medicine treatment for heart failure. In addition, the control group was treated with placebo granules, and the observation group with Shengmaisan granules for a course of 6 months. The baseline data, clinical efficacy, TCM symptom scores, serological indicators [high-sensitivity C-reactive protein (hs-CRP), soluble growth stimulation expressed gene 2 protein (sST2), pro-collagen Ⅲ N-terminal peptide (PⅢNP), interleukin (IL)-6, IL-11, transforming growth factor-β1 (TGF-β1)], echocardiography [Left atrial diameter (LAD), left ventricular end systolic diameter (LVEDs), left ventricular end diastolic diameter (LVEDd)] and CMR indicators [left ventricular ejection fraction (LVEF), myocardial extracellular volume fraction (ECV), and longitudinal relaxation time (T1)] were compared between the two groups.ResultFinally, 31 patients in the control group and 30 patients in the observation group were included. There was no significant difference in baseline data or indicators between the two groups before treatment. Compared with those before treatment, the scores of TCM symptoms (shortness of breath, fatigue, palpitations, spontaneous or night sweats, thirst/dry throat, feverish feeling in palms and soles, and edema in lower limbs), total score of TCM symptoms, ECV, T1, inflammation/fibrosis indicators (hs-CRP, sST2, PⅢNP, IL-6, IL-11, and TGF-β1) in observation group decreased (P<0.05, P<0.01), and the scores of TCM symptoms (except feverish feeling in palms and soles), T1, and inflammation/fibrosis indicators in the control group decreased (P<0.05, P<0.01). After treatment, the observation group had lower scores of TCM symptoms (except feverish feeling in palms and soles and edema in lower limbs), ECV, T1, and inflammation/fibrosis indicators than the control group (P<0.05, P<0.01). After treatment, the total response rate in the observation group was 93.33% (28/30), which was higher than that (80.65%, 25/31) in the control group (Z=2.976, P<0.01). There was no significant difference in adverse reactions between the two groups during treatment.ConclusionFor patients with chronic heart failure with Qi-Yin deficiency syndrome, Shengmaisan Granules can alleviate the TCM symptoms, reduce inflammation, and inhibit myocardial fibrosis by regulating the TGF-β1/IL-11 signaling axis.

      摘要:ObjectiveTo explore the clinical efficacy and safety of Qifu Yixin prescription in treating chronic heart failure in the patients with the syndrome of heart Qi deficiency, so as to provide clinical evidence for the treatment of chronic heart failure with this prescription and promote the clinical application and transformation of this prescription.MethodA total of 106 chronic heart failure patients with the syndrome of heart Qi deficiency who met the criteria in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine (TCM) from September 2022 to May 2023 were selected and randomized into an observation group (53 cases) and a control group (53 cases). Both groups received routine Western medicine treatment. In addition, the observation group received Qifu Yixin prescription, while the control group received placebo. The treatment course for both groups was 12 weeks. The New York heart association (NYHA) cardiac function grading, N-terminal pro-B-type natriuretic peptide (NT-proBNP), soluble growth-stimulating expression gene 2 (sST2), left ventricular ejection fraction (LVEF), the ratio of early diastolic maximum mitral flow velocity (E) to early diastolic mitral annular motion velocity (e') (E/e'), left ventricular end diastolic diameter (LVEDD), TCM syndrome scores, 6-minute walking test (6MWT), Minnesota Living with Heart Failure Questionnaire (MLHFQ), and Kansas City Cardiomyopathy Questionnaire (KCCQ) were determined before and after treatment.ResultFinally, 102 patients were included for analysis, including 51 patients in the observation group and 51 patients in the control group. After treatment, 42 patients in the observation group showed improved cardiac function grading, with a total response rate of 82.35%, and 32 patients in the control group showed improved cardiac function grading, with a total response rate of 62.75%. The total response rate regarding the cardiac function in the observation group was higher than that in the control group (χ2=4.923, P<0.05). The observation group outperformed the control group in lowering the NT-proBNP level, elevating LVEF, decreasing the E/e' ratio (P<0.05), reducing LVEDD and sST2 levels, and recovering TCM syndrome score, 6MWT score, MLHFQ score, and KCCQ score (P<0.05). None of the safety indexes in the two groups showed abnormal values before and after treatment, and no serious adverse reaction was observed.ConclusionQifu Yixin prescription can improve the heart function, exercise tolerance, and quality of life and alleviate the TCM syndrome of the chronic heart failure patients with the syndrome of heart Qi deficiency by inhibiting myocardial fibrosis.

      摘要:ObjectiveTo observe the therapeutic effect of Chaishao Longmu decoction on insomnia in the patients with the syndrome of live depression and spleen deficiency and explore the correlation between infrared thermal imaging and insomnia with liver depression and spleen deficiency.MethodA total of 72 insomnia patients treated in the outpatient department of Shenzhen Hospital of Guangzhou University of Chinese Medicine (Futian) from May to December in 2022 were selected and randomized into a treatment group and a control group, with 36 patients in each group. The patients in the treatment group were treated with Chaishao Longmu decoction and those in the control group with eszopiclone for 4 weeks. The clinical efficacy, Pittsburgh sleep quality index (PSQI) score, TCM syndrome score and infrared thermal imaging characteristics [temperature and temperature changes of frontal sleep line, frontal region, anterior trunk, Zhongwan (CV12), conception vessel (CV), left hypochondrium, right hypochondrium, dorsal trunk, and governor vessel (GV)] of two groups were determined before and after treatment.ResultAfter treatment, the clinical response rate in treatment group was 91.67% (33/36), which was higher than that (66.67%, 24/36) in the control group (Z=-2.617, P<0.01). The treatment in both groups decreased the PSQI score and TCM syndrome score (P<0.01), and the decreases were more significant in treatment group than in the control group (P<0.01). After treatment, the total response rate of sleep line improvement in the treatment group was 86.11% (31/36), which was higher than that (66.67%, 24/36) in the control group (Z=-2.591, P<0.05). The frontal temperature of the two groups decreased (P<0.01) after treatment. Compared with those before treatment, the temperatures of anterior trunk, CV12, CV, left hypochondrium, right hypochondrium, dorsal trunk, and GV rose after treatment (P<0.01). After treatment, the treatment group had lower frontal temperature and higher temperatures of anterior trunk, CV12, CV, left hypochondrium, right hypochondrium, dorsal trunk, and GV than the control group (P<0.01). The treatment in the treatment group reduced the ∆T values of GV (P<0.01) and increased that of the CV12, CV, left hypochondrium, and right hypochondrium (P<0.05,P<0.01). The treatment in the control group increased the ∆T value of CV12 (P<0.01). After treatment, the treatment group had lower ∆T values of GV (P<0.01) and higher ∆T value of CV12, CV, left hypochondrium, and right hypochondrium (P<0.05, P<0.01) than the control group. Compared with that before treatment, the temperature difference between GV and CV in the two groups increased after treatment (P<0.01). According to the infrared thermal image characteristics, the normal temperature difference between GV and CV was within the range of 0.5-1. The median value after treatment in the treatment group was 0.69 (0.52, 0.88), which was within the normal range, indicating that the treatment group outperformed the control group.ConclusionChaishao Longmu decoction can alleviate short-term insomnia by soothing liver, invigorating spleen, harmonizing the middle energizer, and regulating GV and CV. With definite clinical effect, this decoction deserves promotion. Furthermore, the frontal temperature, sleep line, CV12, CV ∆T, and temperature difference between GV and CV revealed by the infrared thermal images could be used for the diagnosis and of insomnia with liver depression and spleen deficiency.

      摘要:ObjectiveTo observe the clinical effect of modified Baqia Baweitang on diabetic foot due to dampness-heat toxicity.MethodOne hundred and four patients were randomized into control and observation groups (52 cases). The control group was treated with epalrestat combined with Compound Huangbai Liquid, and the observation group with modified Baqia Baweitang on the basis of the therapy in the control group. The resting pain, intermittent claudication, foot cold feeling, traditional Chinese medicine (TCM) symptoms, ulcer area, ulcer surface pH, ulcer depth, and percutaneous oxygen partial pressure were measured before and after treatment. The blood glucose and lipid levels [fasting blood glucose (FPG), 2 h postprandial blood glucose (2 h PG), triglyceride (TG), and total cholesterol (TC)], hemorheological indexes (peak blood flow, blood flow, vascular diameter, and pulse index), motor and sensory nerve conduction velocity, and inflammatory cytokines [tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-13, and C-reactive protein (CRP)] in the serum and skin lesions were determined. The clinical efficacy and safety were compared between the two groups.ResultThe total response rate in the observation group was 96.2% (50/52), which was higher than that (80.8%, 42/52) in the control group (χ2=6.029, P<0.05). The treatment in both groups decreased the resting pain, intermittent claudication, foot cold feeling, TCM symptom scores, ulcer area, ulcer surface pH, and ulcer depth (P<0.05) and increased the percutaneous oxygen partial pressure (P<0.05). Furthermore, the treatment lowered the levels of FPG, 2 h PG, TG, TC, blood flow peak, TNF-α, IL-6, IL-18, and CRP (P<0.05) and increased the blood flow, vascular diameter, pulse index, and the motor and sensory nerve conduction velocity (P<0.05). Moreover, the changes in the above indexes were more significant in the observation group than in the control group (P<0.05).The incidence of adverse reactions was 21.2% (11/52) in the control group and 23.1% (12/52) in the observation group, with no statistical significance.ConclusionModified Baqia Baweitang can promote wound healing, improve the dorsal artery hemorheology of feet, accelerate nerve conduction, and reduce inflammation in the patients with diabetic foot due to dampness-heat toxicity.

      摘要:ObjectiveBased on serum pharmacochemistry and ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS) the transitional components in the serum of rats after intragastric administration of water extract of Alismatis Rhizoma(AR)and salt-processed Alismatis Rhizoma(SAR) were compared.MethodSD rats were randomly divided into blank group, AR group(10 g·kg-1) and SAR group(10 g·kg-1), 3 rats in each group, the administration groups were given AR and SAR aqueous extracts by gavage, respectively, and the blank group was given an equal volume of drinking water by gavage once in the morning and once in the evening, for 3 consecutive days. Sixty minutes after the last administration, blood was collected from the eye orbits, and the serum samples were prepared. The serum samples were prepared on an ACQUITY UPLC BEH C18 column(2.1 mm×50 mm, 1.7 μm) with the mobile phase of acetonitrile(A)-0.1% formic acid aqueous solution(B) in a gradient elution(0-10 min, 10%-50% A; 10-27 min, 50%-95%A; 27-27.1 min, 95%-10% A; 27.1-30 min, 10%A), the data were collected at a flow rate of 0.3 mL·min-1 in positive ion mode with a scanning range of m/z 100-1 200. Based on the self-constructed chemical composition library of AR, the total ion flow diagrams and secondary MS fragmentation information of the aqueous extracts of AR and SAR, as well as the administered serum and the blank serum, were compared with each other by UNIFI 1.9.2, so as to deduce the possible blood-migrating constituents and their cleavage patterns in the aqueous extracts, and the response intensity ratios of each chemical component were calculated before and after processing.ResultA total of 20 components, including 5 prototypical components and 15 metabolites, were analyzed and deduced from the serum of rats given aqueous extract of AR. And 14 components, including 5 prototypical components and 9 metabolites, were analyzed and deduced from the serum of rats given aqueous extract of SAR. Of these, 13 components were common to both of them, including 5 prototypical components and 8 metabolites. The 5 prototypical components were 16-oxoalisol A, alisol A 24-acetate, alisol A, alisol B and alisol C. The metabolites were mainly involved in phase Ⅰ metabolism(oxidation) and phase Ⅱ metabolism(glucuronidation). There was a big change in the intensity of response of the common components before and after salt-processing, and the response intensities of the prototypical components, 16-oxoalisol A, alisol B and alisol C, were elevated, while the type and response intensity of metabolites were generally decreased, and it was hypothesized that the metabolic rate of terpenoids might be slowed down after salt-processing of AR, so that the blood-migrating constituents could participate in the metabolism of the body more in the form of prototypes.ConclusionSalt-processing of AR may promote the absorption of prototypical components into the blood by slowing down the metabolic rate of terpenoids, which can provide support for the research on material basis of AR and SAR.

      摘要:ObjectiveTo investigate the effects of spleen-Yin deficiency on gastrointestinal absorption, water metabolism and intestinal flora in rats with spleen-Yin deficiency syndrome.MethodA rat model of spleen-Yin deficiency syndrome was established by using the composite factors, including irregular meat and vegetable diet, weight-bearing fatigue swimming and gavage with warm-heat injury-Yin drugs. The changes of body weight, food intake, water intake and duration of swimming in the blank and model groups were observed. Hematoxylin-eosin(HE) staining was used to observe the histopathological damage of the stomach and colon. Urinary excretion rate of D-xylose was determined by phloroglucinol method. The content of gastrin(GAS) in serum was determined by enzyme-linked immunosorbent assay(ELISA). The relative expression levels of vasoactive intestinal peptide(VIP), aquaporin 3(AQP3) and AQP4 in gastric tissues were detected by Western blot. The relative mRNA expression levels of VIP, AQP3 and AQP4 in gastric tissues were detected by Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), and the changes of intestinal flora were analyzed by 16S rDNA sequencing.ResultCompared with the blank group, the results of general physical signs showed that the body weight and food intake of rats in the model group were significantly decreased, the water intake was significantly increased(P<0.05, P<0.01), and the duration of swimming was significantly decreased(P<0.01). Pathological examination results showed that in the mucosa of gastric tissues of rats in the model group appeared to be misaligned, the mucosa of colonic tissues could be seen to be obviously thinned or mutilated, and the epithelial cells appeared to be necrotic or even exfoliated. Compared with the blank group, the urinary D-xylose excretion rate of rats in the model group was significantly decreased(P<0.01), and the serum GAS content was significantly decreased(P<0.05). Compared with the blank group, Western blot results showed that the relative expression level of VIP protein in gastric tissues of rats in the model group was significantly decreased, while the relative expression levels of AQP4 and AQP3 proteins were significantly increased(P<0.01). Compared with the blank group, Real-time PCR results showed that the relative expression level of VIP mRNA in gastric tissues of rats in the model group was significantly decreased(P<0.01), and the relative mRNA expression levels of AQP3 and AQP4 were significantly increased(P<0.05, P<0.01). Compared with the blank group, the results of intestinal flora analysis showed that the number of operational taxonomic units(OTUs) and α-diversity increased and β-diversity decreased significantly in the model group, the abundance of Porphyromonadaceae was increased significantly, and the abundance of Oscillibacter_ruminantium was decreased significantly(P<0.05). Spearman correlation analysis showed that Porphyromonadaceae was significantly positively correlated with AQP4 protein level, while Oscillibacter_ruminantium was significantly positively correlated with VIP protein level, and negatively correlated with AQP3 and AQP4 protein levels(P<0.05). Linear discriminant analysis effect size(LEfSe) analysis results showed that there were significant differences in a variety of intestinal bacteria between groups, and the intestinal bacteria of the model group were significantly enriched in the phylum/order/family/genus of Elusimicrobia, Betaproteobacteria, Burkholderiales, Sutterellaceae and Parasutterella(P<0.05).ConclusionSpleen-Yin deficiency syndrome can weaken the digestion and absorption capacity of gastrointestinal tract, and cause the disturbance of water metabolism and intestinal flora. AQP4, AQP3 and VIP protein levels of gastric mucosa are closely related to Porphyromonadaceae and Oscillibacter_ruminantium. And AQP4, AQP3 and VIP may be involved in the regulation of intestinal flora in order to affect the physiological function of spleen governing transportation and transformation.

      摘要:ObjectiveTo explore the material basis of bile-processed Coptidis Rhizoma clearing excessive fire of liver-gallbladder based on ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF/MS) metabolomics and molecular docking.MethodUPLC-Q-TOF/MS metabolomics was used to analyze the chemical constituents of Coptidis Rhizoma, water-processed Coptidis Rhizoma and bile-processed Coptidis Rhizoma. Chromatographic separation was achieved with 0.1% formic acid aqueous solution(A)-acetonitrile(B) as the mobile phase in gradient elution(0-2 min, 5%B; 2-20 min, 5%-65%B; 20-40 min, 65%-10%B; 40-45 min, 10%B; 45-46 min, 10%-95%B; 46-49 min, 95%B), and electrospray ionization(ESI) was applied and operated in positive and negative ion modes, the acquisition range was m/z 80-1 200. Based on this, partial least squares-discriminant analysis(PLS-DA) and variance analysis were used to screen the differential compounds among the three products of Coptidis Rhizoma. Network pharmacology and molecular docking were used to verify the degree of association between differential compounds and excessive fire of liver-gallbladder syndrome.ResultA total of 33 chemical constituents were identified, including 2 phenolic acids, 5 binding bile acids and 26 alkaloids. And 16 differential compounds were identified by multivariate statistical analysis, including 11 alkaloids and 5 binding bile acids. Pathway enrichment analysis in the Kyoto Encyclopedia of Genes and Genomes(KEGG) database yielded 8 pathways related to excessive fire of liver-gallbladder, and the key protein phosphatidylinositol 4,5-bisphosphate 3-kinase catalytic subunit alpha isoform(PIK3CA) was obtained according to the "component-target-pathway" network analysis. Molecular docking results showed that 11 alkaloids had good binding ability with PIK3CA.ConclusionPorcine bile is unique in the processing of bile-processed Coptidis Rhizoma, which can promote the production and dissolution of 11 alkaloids, including berberine and dihydrochelerythrine. Based on the results of molecular docking and reported pharmacological experiments, it can be concluded that 16 different compounds such as berberine, dihydrochelerythrine and taurohyodeoxycholic acid are the material basis of bile-processed Coptidis Rhizoma.

      摘要:ObjectiveTo investigate the identification of kidney Yang deficiency syndrome of patients with osteoporosis(OP), and to form the clinical syndrome identification rules of traditional Chinese medicine(TCM).MethodBasic information, etiology, clinical symptoms and other characteristics of 982 OP patients were included, and statistical tests were used to screen the variables associated with kidney Yang deficiency syndrome. Taking the decision tree as the base model, bootstrap aggregation algorithm(Bagging algorithm) was utilized to establish the classification model of kidney Yang deficiency syndrome in OP, generating numerous rules and removing redundancy. Combining least absolute shrinkage and selection operator(LASSO) regression to screen key rules and integrate them to construct an identification model, achieving the identification of kidney Yang deficiency syndrome in OP patients.ResultEighteen key identification rules were screened out, and of these, where 11 rules with regression coefficients>0 correlated positively with the kidney Yang deficiency syndrome, the rule with the highest coefficient was chilliness(present)&feverish sensation over the palm and sole(absent). The other 7 rules with regression coefficients<0 correlated negatively with the syndrome, the rule with the lowest coefficient was reddish tongue(present)&diarrhea(absent)&deficiency of endowment(absent). According to the regression coefficients of each key rule, variables with importance>0.2 were ranked as chilliness, reddish tongue, feverish sensation over the palm and sole, cold limbs, clear urine, diarrhea, deficiency of endowment, prolonged illness. The results of the partial dependence analysis of the identification model showed that compared to OP patients without chilliness, those with chilliness(present) had a 0.266 8 higher probability of being identified as having kidney Yang deficiency syndrome, indicating that this variable had the highest impact on identification of the syndrome. Similarly, compared to OP patients without reddish tongue, those with reddish tongue had a 0.141 9 lower probability of being identified as having kidney Yang deficiency syndrome, indicating that this variable had the highest impact on identifying non-kidney Yang deficiency syndrome. The accuracy, sensitivity, specificity and area under receiver operating characteristic curve(AUC) of the established kidney Yang deficiency syndrome identification model in the test set were 0.865 9, 0.853 7, 0.872 0 and 0.931 5, respectively.ConclusionA precise identification model of OP kidney Yang deficiency syndrome is conducted basing on the rule ensemble method of Bagging combining LASSO regression, and the screened key rules can explain the identification process of kidney Yang deficiency syndrome. In this research, according to the regression coefficients of rules, the importance and partial dependence of variables, combined with the thinking of TCM, the influence of patient characteristics on the identification of syndromes is described, so as to reveal the primary and secondary syndromes of identification and assist the clinical identification of kidney Yang deficiency syndrome.

      摘要:ObjectiveTo study the characteristics of animal models of acute lung injury caused by non-physical factors, so as to provide a reference for the standardization of the preparation of such animal models and lay a foundation for the research on the pathogenesis and the diagnosis and treatment of acute lung injury.MethodThe articles about the animal experiments of acute lung injury published in the last decade were retrieved from China National Knowledge Infrastructure (CNKI), Wanfang, SinoMed, VIP, and PubMed with the theme terms of "acute lung injury" and "animal model". The animal species, drugs used in modeling, modeling period, methods used in molding, model standards, and model evaluation indicators were summarized, and Excel was used for the frequency analysis.ResultA total of 338 articles were included in this study. The results of the frequency analysis showed that SD rats/C57BL/6 mice were mainly used to establish the animal models of acute lung injury. Male mice were mostly used for modeling, and the commonly used modeling agent was lipopolysaccharides (LPS). In most cases, the modeling lasted for 6 h after drug administration. Hematoxylin-eosin staining was mainly used for the observation of histological changes in the lungs, which were taken as the criteria for modeling. The established models were mainly evaluated based on lung dry/wet weight ratio, lung index, morphological changes in the lung tissue, myeloperoxidase (MPO), superoxide dismutase (SOD), and levels of inflammatory cytokines in the serum and bronchoalveolar lavage fluid (BALF).ConclusionThe models of acute lung injury were mostly prepared by intraperitoneal injection of LPS (5 mg·kg-1) in SD rats and tracheal instillation of LPS (5 mg·kg-1) in C57BL/6 mice, which were praised for the simple operation, high success rate, and consistent with the pathogenesis of acute lung injury. This study provides a reference for the basic research on acute lung injury by animal experiments.

      摘要:As an important complementary form of decoction pieces of traditional Chinese medicine(TCM), TCM dispensing granules has the advantages of being free of decoction, easy to take, easy to carry and easy to be dispensed, which greatly improves the capacity of emergency services of TCM and is more in line with the needs of modern society. With the end of the pilot project of TCM dispensing granules, the market has been fully liberalized, the competition has been intensified, and it is in the transition period of switching between the new standard and the old one, and there are some problems such as the shortage of varieties, the change of specifications and the difference of quality, and the production enterprises are facing new opportunities and challenges. Based on this, the authors intend to systematically sort out the policies and regulations, enterprise layout and standard formulation since the pilot of TCM dispensing granules. In view of the problems in the post-pilot stage and from the perspective of survival and development of enterprises, it is suggested that enterprises should establish a quality control system for the whole industry chain of TCM dispensing granules to reduce process costs and increase enterprise competitiveness, further increase the investment in scientific research, overcome the key technical problems of difficult varieties, actively and orderly promote the research of national standards, in order to ensure the integrity of clinical formula varieties, and establish and improve the efficacy evaluation mechanism of TCM dispensing granules, build a consistency evaluation system between TCM dispensing granules and decoction pieces. Government departments should strengthen the guidance, fully mobilize the enthusiasm of scientific research institutions, enterprises and hospitals, and explore the establishment of "government-industry-study-research-application" mode to promote the development of TCM dispensing granule industry.

      摘要:The aberrant activation of the nucleotide-binding oligomerization domain-like receptor family pyrin domain containing 3 (NLRP3) inflammasome as an essential component of the innate system is implicated in the pathogenesis of several human inflammatory diseases. Studies have confirmed its association with digestive system diseases such as ulcerative colitis, Crohn's disease, and acute pancreatitis, suggesting that the NLRP3 inflammasome plays a role in the initiation and progression of these diseases. Based on the mechanism of NLRP3 inflammasome activation and the pathways that mediate the inflammatory response, this article introduced the relationship between the NLRP3 inflammasome and the pathogenesis of multiple digestive system diseases and the Chinese and western medical therapies. Traditional Chinese medicine (TCM) has demonstrated definite effects on the NLRP3 inflammasome-mediated digestive system diseases. Some single Chinese medicines or TCM prescriptions can treat digestive system diseases by activating or inhibiting NLRP3 inflammasome activation. NLRP3 inflammasome can receive a variety of endogenous and exogenous stimulatory signals, which can initiate, activate, and mediate inflammatory responses. The inflammasome formation and downstream inflammatory cytokines are involved in not only the inflammatory responses but also the development and progression of multiple digestive system diseases. Therefore, the NLRP3 inflammasome can serve as an ideal target for disease treatment. The future rediscovery and in-depth studies of multiple inflammasomes will shed new light on the treatment of multiple digestive system diseases.

      摘要:Chronic obstructive pulmonary disease (COPD) is a chronic heterogeneous airway disease characterized by persistent and progressive airflow restriction, which can be divided into stable COPD and acute exacerbations of COPD (AECOPD). Its morbidity and mortality remain high, posing a serious threat to human health. Traditional Chinese Medicine (TCM) believes that COPD belongs to the categories of "cough", "dyspnea syndrome", "lung distension", etc. And its basic pathogenesis is intermingled phlegm and stasis with deficiency in origin and excess in superficiality. Qianjin Weijingtang, derived from the Records of Proved Prescriptions, Ancient and Modern (古今录验方), consists of Phragmitis Caulis, Persicae Semen, Coicis Semen, and Benincasae Semen, with remarkable functions in clearing the lung, resolving phlegm and eliminating blood stasis, and has definite clinical efficacy in treating COPD and its syndromes. At present, in clinical studies, Qianjin Weijingtang has been used to treat COPD with modifications. It can be used alone or in combination with other prescriptions/western medicines to treat stable COPD, AECOPD, COPD complications, and other TCM syndromes of COPD such as phlegm-heat-stagnation obstructing the lung syndrome. It can significantly improve clinical symptoms, lung function, and blood gas indexes, and inhibit inflammatory response. Animal experiments mainly explored the mechanism of COPD from the level of pathological changes. Specifically, the underlying mechanism may be related to regulating T helper 17 (Th17)/regulatory T cells (Treg) balance, up-regulating single immunoglobulin IL-1-related receptor (SIGIRR) for resisting inflammation, up-regulating hyperplasia suppressor gene (HSG) and inhibiting Wnt signaling pathway activation to inhibit airway remodeling. It was found that there were many problems, such as low quality of clinical research, failure in sharing research standards, and the lack of mechanism research. This article systematically reviewed clinical studies of Qianjin Weijingtang in the treatment of COPD and its mechanism based on animal experiments in recent years, and put forward thoughts and suggestions according to the existing problems to provide references for the clinical application and further research on Qianjin Weijingtang.

      摘要:Diabetic nephropathy (DN), a major cause of chronic kidney disease (CKD), aggravates the prevalence of end-stage renal disease (ESRD) and threatens human health. The pathogenesis of DN is complex, in which inflammation is a key pathological link in the cascade injury. Therefore, the treatment targeting inflammation helps to delay the progression of DN. NOD-like receptor protein 3 (NLRP3), a classical proteasome, acts as an inducer of innate immune responses. The activated NLRP3 inflammasomes produce and release inflammatory mediators to trigger pyroptosis and uncontrolled autophagy and mediate the stress signals promoting renal fibrosis, thus participating in the development and progression of DN. The NLRP3 inflammasome as a core site inducing inflammation is widely involved in DN progression and may be a novel target. The active components and compound prescriptions of Chinese medicines are increasingly applied in the prevention and treatment of DN. The latest studies have discovered that Chinese medicines can treat DN by regulating the activation of NLRP3 inflammasomes. Although studies have been conducted to explore the mechanism of Chinese medicines in the treatment of DN via NLRP3 inflammasome, the systematic review remains to be carried out. This paper reviews the relevant publications in recent years and introduces the research progress from the assembly and activation of NLRP3 inflammasomes, the mechanism of NLRP3 inflammasomes in the treatment of DN, and the regulation of NLRP3 inflammasomes by Chinese medicines for the prevention and treatment of DN, aiming to lay a foundation for the relevant studies and provide new targets and strategies for the prevention and treatment of DN.

      摘要:Colorectal cancer (CRC), a malignant tumor of the digestive system, originates from the colorectal mucosa epithelium and is usually asymptomatic until it progresses to an advanced stage. With high incidence around the globe and the increasingly younger patients, this disease poses a serious threat to the health and lives of the patients. Although the pathogenesis of this disease is not fully understood, it is generally believed that it is associated with autophagy, apoptosis, and inflammation. Autophagy and apoptosis as two types of programmed cell death are subject to complex interactive regulation, and the imbalance between them is closely related to the occurrence, development, and prognosis of a variety of diseases. Studies have shown that autophagy-apoptosis balance plays a key role in CRC. On the one hand, autophagy and apoptosis coordinate with each other to inhibit CRC cell growth. On the other hand, autophagy can antagonize apoptosis to promote CRC cell growth. In clinical practice, surgery is often combined with radiotherapy and chemotherapy to treat CRC, which can control the progression of CRC to a certain extent but has serious adverse effects and poor long-term results. In recent years, traditional Chinese medicine (TCM) has been proved to be effective in the treatment of CRC. Studies have shown that numerous herbal active components can promote CRC cell death by regulating the autophagy-apoptosis balance, thereby blocking the progression of this disease. The process of autophagy-apoptosis balance in regulating cell activities has similar theoretical connotations with the Yin and Yang theory of TCM. Applying TCM in regulating autophagy-apoptosis balance at various stages of CRC has become a frontier, while the comprehensive elaboration remains to be conducted. By reviewing the relevant studies in recent years, this paper introduces the correlation between the Yin and Yang theory and the autophagy-apoptosis balance, the role of autophagy-apoptosis balance in CRC, and the research progress in the application of 27 Chinese herbal active components such as flavonoids, terpenoids, glycosides, and phenols capable of regulating autophagy-apoptosis balance in the treatment of CRC. The active components in Chinese medicines can recover the autophagy-apoptosis balance in CRC by acting on microtuble-associated protein 1 light chain 3(LC3), Beclin-1, and B-cell lymphoma-2(Bcl-2)to regulate multiple signaling pathways such as protein kinase B(Akt)/mammalian target of rapamycin(mTOR)and reavtive oxygen species(ROS)/ c-Jun N-terminal kinase(JNK), thus balancing Yin and Yang. This review aims to provide a reference for the treatment of CRC and the development of new drugs.

      摘要:Non-small cell lung cancer (NSCLC) is a malignant tumor of the respiratory system with a high incidence. The early symptoms are not typical, and most patients are diagnosed at an advanced stage, which seriously threatens the lives and health of people. Surgery, chemotherapy, and targeted therapy are the main means of treatment at present, but the consequent drug resistance and adverse reactions restrict these treatment methods with certain limitations. In recent years, with the development of traditional Chinese medicine (TCM) in tumor resistance, TCM has attracted extensive attention for its obvious therapeutic effect and fewer adverse reactions. Numerous signaling pathways are involved in the formation and development of NSCLC, where phosphatidylinositol 3 kinase (PI3K)/protein kinase B (Akt)/mammalian target of rapamycin (mTOR) signaling pathway is one of the key regulatory pathways. The PI3K/Akt/mTOR signaling pathway affects the proliferation, invasion, and metastasis of NSCLC cells by affecting the cycle, inhibiting the apoptosis, inhibiting the autophagy of tumor cells, and promoting tumor neovascularization. As revealed by the recent literature, Chinese medicine plays an indispensable role in NSCLC cell autophagy, cell cycle, apoptosis, invasion and metastasis, neovascularization, and reversal of drug resistance by regulating the PI3K/Akt/mTOR signaling pathway. Although some Chinese medicinal extracts or compounds have made great breakthroughs in some mechanisms of action in the treatment of NSCLC, these studies only remain at the level of in vitro cell experiments and animal studies in vivo. Researchers are faced with the great challenge of "transforming the research results of Chinese medicines into clinical applications". Based on the current research status in China and abroad, this paper reviewed Chinese medicine in the intervention in NSCLC through the regulation of PI3K/Akt/mTOR signaling pathway in recent years, in order to open up new ideas for NSCLC drug therapy research and also provide a useful reference for further mechanism research.

      摘要:Bone marrow mesenchymal stem cells (BMSCs) are derived from stem cells isolated from bone marrow and have the potential for multidirectional differentiation and self-renewal. Under certain conditions, BMSCs can be induced to differentiate into osteoblast (OB), chondrocyte, adipocyte, fibroblast, etc. BMSCs play an important role in maintaining the stability of bone structure and balancing bone metabolism. Promoting the proliferation of BMSCs and inducing their differentiation into OB of great significance for the clinical prevention and treatment of osteoporosis, bone defects, fracture healing, and other diseases. Because the proliferation and osteogenic differentiation of BMSCs are complex processes controlled by multiple genes and regulated by multiple signal transduction pathways, traditional Chinese medicine (TCM) happens to have the advantages of multi-bioactive component, multi-target, and multi-pathway synergism, which can affect the proliferation and differentiation of BMSCs through multiple channels and induce the proliferation of BMSCs. The transcription and expression of genes related to osteogenesis can be enhanced to promote the differentiation of BMSCs into OB, so as to achieve the purpose of preventing and treating osteoporosis, bone defects, and other bone diseases. Based on the literature on the intervention of TCM monomers and compounds in the proliferation and osteogenic differentiation of BMSCs, this study reviewed TCM monomers and compounds in promoting the proliferation and osteogenic differentiation of BMSCs by regulating secreted glycoprotein (Wnt), neurogenic locus notch homolog protein (Notch), mitogen-activated protein kinase (MAPK), phosphatidylinositol-3 kinase (PI3K) /protein kinase B (Akt), bone morphogenetic protein (BMP)/Smad, Janus kinase (JAK)/signal transducer and activator of transcription protein (STAT), osteoprotegerin (OPG)/receptor activator of nuclear factor-kappa B (RANK)/RANK ligand (RANKL), and other signaling pathways to provide new ideas for the research and clinical application of Chinese medicine in the prevention and treatment of orthopedic diseases.

      摘要:Hepatic fibrosis characterized by various chronic liver injuries can lead to abnormal activation of hepatic stellate cells, unbalanced production and degradation of extracellular matrix proteins, and excessive deposition that destroys the normal structure of the liver. The aggravated liver fibrosis can cause irreversible cirrhosis and hepatocellular carcinoma, becoming a great challenge to the global health. Ferroptosis is a new form of iron-dependent cell death discovered in recent years, which mainly involves abnormal iron metabolism, lipid peroxide accumulation, and weakening of the antioxidant defense system. A number of studies have reported that inducing ferroptosis in hepatic stellate cells or alleviating ferroptosis in the liver can ameliorate liver fibrosis and reduce liver injury. Chinese medicine widely applied in the treatment of chronic liver diseases has demonstrated good safety, wide therapeutic effects, and easy access compared with Western medicine. Therefore, The intervention of hepatic stellate cells or hepatic ferroptosis by Chinese medicine may be a new direction for the prevention and treatment of liver fibrosis in the future. This paper summarized the various regulatory mechanisms of ferroptosis and expounded how ferroptosis affected the progression of liver fibrosis, providing theoretical support for the prevention and treatment of liver fibrosis with Chinese medicine in the future.

      摘要:Ulcerative colitis (UC), a chronic inflammatory bowel disease with the accumulation of colorectal mucosa and submucosa, has a risk of developing into cancer. In recent years, the incidence of UC has been on the rise worldwide. However, its pathogenesis has not been fully elucidated by modern medicine, and even the remission rate of the latest drugs is lower than 50%, which seriously affects the patients' work and quality of life. Mitochondria, as the "power station" of eukaryotic cells, are involved in a variety of physiological processes such as the production of reactive oxygen species and the production of adenosine triphosphate by oxidative phosphorylation, and their dysfunction can lead to a series of diseases. Mitochondrial quality control (MQC) is an important way to maintain the stability of mitochondrial form, quantity, and quality. Studies have shown that MQC disorders characterized by low mitochondrial biogenesis, excessive mitochondrial oxidative stress, mitochondrial autophagy defects, mitochondrial dynamics disorders, and calcium regulation abnormalities are closely related to the occurrence and development of UC. Although progress has been achieved in the treatment of UC by traditional Chinese medicine (TCM) which can regulated MQC in a multi-pathway and multi-target manner in recent years, a review on the treatment of UC by TCM via the intervention in MQC remains to be carried out. Therefore, this paper summarized the TCM treatment of UC by regulating MQC, aiming to provide new ideas for the clinical treatment of UC by TCM.

      摘要:Osteoporosis (OP), a common systemic skeletal disease in the elderly, is characterised by bone loss and bone microstructural degeneration. Its clinical manifestations include increased bone fragility and bone pain. Furthermore, OP increases the risk of fracture due to the high bone fragility, which leads to lifelong disability or death, imposing a heavy economic and psychological burden on the patients and their families. The pathogenesis of OP is extremely complex and associated with a variety of factors such as proliferation and differentiation of osteoblasts, impairment of osteoclast activity and function, and abnormalities in autophagy activation. Recent studies have found that mammalian target of rapamycin (mTOR) signaing pathway is involved in the regulation of bone homeostasis, which can promote bone formation and improve bone metabolism and bone microstructure by regulating osteoblast proliferation and differentiation and osteoclast function and activating cellular autophagy, thus playing a crucial role in the prevention and treatment of OP. The prevention and treatment of OP with Chinese medicine has a long history, clear efficacy, multiple targets of action, low adverse effects, and wide medicine sources. Therefore, this paper briefly describes the role of mTOR signaling pathway in the development of OP by reviewing the latest research reports and summarizes in detail the latest research results on the treatment of OP with Chinese medicine extracts and prescriptions via the mTOR signaling pathway. This review aims to provide a basis for the in-depth research on the relationship between mTOR signaling pathway and OP and the clinical application of traditional Chinese medicine in the prevention and treatment of OP.

      摘要:Ulcerative colitis (UC) is a chronic recurrent inflammatory bowel disease which primarily affects the colonic mucosa. The UC patients mainly present diarrhea, abdominal pain, tenesmus, and mucous bloody stools, and even malnutrition and systemic symptoms in severe cases, with rising incidence, which has a significant impact on the health and quality of life of patients. The pathogenesis of UC is not clear, and the Western medical therapies include sulfasalazine, glucocorticoids, and immunosuppressants, which, however, have side effects and unsatisfactory effects. Chinese medicine with high safety, mild adverse reactions, and a multi-component, multi-target, and multi-pathway treatment manner has garnering increasing attention. Therefore, finding the Chinese medicine to treat UC has become a hot spot. Glycyrrhizae Radix et Rhizoma is one of the commonly used Chinese herbal medicines, with the effects of tonifying spleen and reinforcing qi, clearing heat and detoxifying, dispelling phlegm and relieving cough, relieving pain, and harmonizing medicines. Glycyrrhizae Radix et Rhizoma mainly contains glycyrrhizic acid, glycyrrhetinic acid, diammonium glycyrrhizinate and other active ingredients. Modern pharmacological studies have shown that Glycyrrhizae Radix et Rhizoma has anti-inflammatory, antioxidant, antimicrobial, and immunomodulatory activities. According to statistics, Glycyrrhizae Radix et Rhizoma is among the top three Chinese herbal medicines for the treatment of UC. The recent years have witnessed progress in the treatment of UC with Glycyrrhizae Radix et Rhizoma and the related prescriptions. The present study summarized the mechanisms underlying the anti-inflammatory, immunomodulatory, intestinal flora-regulating, cell apoptosis-inducing, and oxidative stress-reducing effects of the key chemical constituents (glycyrrhetinic acid, diammonium glycyrrhizinate, polysaccharide, glycyrrhetinic acid, and isoglycyrrhizin) and compound prescriptions of Glycyrrhizae Radix et Rhizoma. The findings provide a solid foundation for further development and clinical application of Glycyrrhizae Radix et Rhizoma.

      摘要:Hepatic fibrosis is a common complication of chronic liver disease, seriously affecting patients' quality of life and leading to severe consequences such as cirrhosis and liver cancer. Modern medicine has made progress in the treatment of hepatic fibrosis, while it still faces certain challenges and limitations. Therefore, seeking new therapeutic strategies is of great clinical significance. The nuclear factor-κB (NF-κB) signaling pathway plays a role in regulating inflammation and immune responses. Recent studies have shown that the NF-κB signaling pathway plays a key role in the occurrence and development of hepatic fibrosis. The abnormal activation of the NF-κB signaling pathway leads to the overexpression of genes related to liver inflammation and fibrosis, thereby promoting the development of hepatic fibrosis. Traditional Chinese medicine (TCM) is a traditional treatment method with unique advantages and potential. In recent years, increasing studies have proved that TCM can treat hepatic fibrosis by regulating the NF-κB signaling pathway. The active ingredients in Chinese herbal medicines can intervene in the activation of the NF-κB signaling pathway to inhibit inflammatory responses, thereby reducing the severity of hepatic fibrosis. This article reviews the mechanisms of TCM in treating hepatic fibrosis via the NF-κB signaling pathway and evaluates the efficacy and discusses the clinical application prospects of relevant Chinese herbs and formulae, aiming to provide references for further research and clinical practice.
