      摘要:Da Qinjiaotang is a common classical prescription for the treatment of stroke. It originates from Collection of Writings on the Mechanism of Disease, Suitability of Qi, and the Safeguarding of Life as Discussed in the Basic Questions (《素问病机气宜保命集》) by physician LIU Wansu, and is composed of Gentianae Macrophyllae Radix, Glycyrrhizae Radix et Rhizoma, Chuanxiong Rhizoma, Angelicae Sinensis Radix, Paeoniae Radix Alba, Asari Radix et Rhizoma, Notopterygii Rhizoma et Radix, Saposhnikoviae Radix, Scutellariae Radix, Gypsum Fibrosum, Angelicae Dahuricae Radix, Atractylodis Macrocephalae Rhizoma, Rehmanniae Radix, Rehmanniae Radix Praeparata, Poria, and Angelicae Pubescentis Radix. Doctors of all dynasties have disputed the composition principle of the prescription and argued whether its treatment of stroke belongs to the theory of "internal wind" or "external wind". Through collating and analyzing ancient and modern literature related to the indications of Da Qinjiaotang, this paper was dedicated to the origin of syndrome differentiation and treatment of Da Qinjiaotang. According to LIU Wansu's original works, Da Qinjiaotang is a prescription for the treatment of "internal wind", and in the prescription, wind medicinal herbs such as Gentianae Macrophyllae Radix, Notopterygii Rhizoma et Radix and Angelicae Pubescentis Radix removes stagnation, clears sweat pore, and makes qi and blood channels flow smoothly. However, later generations, affected by the idea of "external wind", believe that this prescription is used for the treatment of "external wind". Ancient physicians gradually supplemented the symptoms of stroke, such as wry eye and mouth, hemibody pain and limb numbness, which were treated by Da Qinjiaotang, and Da Qinjiaotang was also applied to the treatment of other diseases, such as tendon dryness, convulsion and arthralgia. Modern doctors still explain the disease pathogenesis from the theory of "external wind" as deficiency in channels and collaterals and the entry of pathogenic wind, and the prescription has the effect of dispersing wind, clearing heat and nourishing and activating blood. In clinical practice, Da Qinjiaotang is mainly used to treat cerebrovascular diseases and peripheral facial paralysis in nervous system diseases, gouty arthritis and rheumatic arthritis in the rheumatic immune system and skin diseases. The above findings facilitate the research and development of Da Qinjiaotang.

      摘要:Danggui Liuhuangtang is the 47th of the 100 famous classical formulas published by the National Administration of Traditional Chinese Medicine, and is known as the holy medicine for night sweat. By bibliometrics, the authors collected the ancient books on Danggui Liuhuangtang and screened out 269 valid data, involving 156 ancient books of traditional Chinese medicine. The analysis on the historical origin, disease syndromes, pathogenesis, composition, dosage, preparation, usage, and processing of Danggui Liuhuangtang found that this famous classical formula originated from Secret Book of the Orchid Chamber (《兰室秘藏》) written by LI Dongyuan, and is composed of Angelicae Sinensis Radix, Rehmanniae Radix, Rehmanniae Radix Praeparata, Phellodendri Chinensis Cortex, Scutellariae Radix, Coptidis Rhizoma and Astragali Radix. It has the functions of nourishing Yin, reducing fire, consolidating exterior and stopping sweating, and mainly treats night sweat due to Yin deficiency and fire exuberance. In the later generations, disease syndromes are mostly treated based on LI Dongyuan's theory, and have expanded to more than 30 kinds (339 in total), among which night sweat (208) was the most, accounting for 61.36% of the total disease syndromes, followed by spontaneous sweating (38), accounting for 11.21%. Additionally, it was found that Danggui Liuhuangtang was widely used in modern clinical practice for various disease syndromes. Among them, endocrine disease (77, 28.21%) was predominant, followed by gynecological disease (48, 17.58%), and pediatric disease (24, 8.79%). Although Danggui Liuhuangtang treats many disease syndromes, their pathogenesis was always yin deficiency and fire exuberance. Through the systematic excavation of the ancient books on Danggui Liuhuangtang and the analysis of its modern clinical application, this paper probed into the historical evolution and confirmed the key information of the formula, providing detailed literature basis for the research and development application of famous classical formulas.

      摘要:ObjectiveTo explore the mechanism of Danggui Niantongtang (DGNTT) against adjuvant-induced arthritis (AA) in rats with wind-dampness-heat arthralgia (FSR) based on the variation of intestinal flora.MethodA total of 60 SD rats were randomized into normal (control) group, FSR group, low-, medium-, and high-dose DGNTT (5.67, 11.34, 22.68 g·kg-1) groups, and methotrexate (MTX) group (1.35 mg·kg-1), with 10 rats in each group. The rats, except the control group, were injected with Mtb adjuvant and then exposed to artificial climatic chamber (hot and humid with wind) for 64 h for modeling. The rats were treated with water, DGNTT or MTX for 28 days from the day of injection. Arthritis index (AI) of rats was measured and paw volume was determined with a volume meter. The morphology of synovial tissues of the knees was observed based on hematoxylin-eosin (HE) staining and the changes of intestinal flora were analyzed based on 16S rRNA sequencing.ResultDGNTT can alleviate the hyperplasia of synovial tissue and inflammation of AA rats with FSR and inhibit the formation of pannus. The results of 16S rRNA sequencing showed that the relative abundance of Firmicutes, Lactobacillus, Prevotella 9, and Alloprevotella decreased (P<0.05, P<0.01) and the relative abundance of Bacteroidetes and Bacteroides increased (P<0.01) in FSR group compared those in the control group. Compared with the FSR group, all DGNTT groups and MTX group had high relative abundance of Lactobacillus (P<0.05, P<0.01) and low relative abundance of Bacteroidetes (P<0.01) and medium-dose and high-dose DGNTT groups and MTX group showed high abundance of Firmicutes, Prevotella 9, and Alloprevotella and low abundance of Bacteroides (P<0.05, P<0.01). Spearman's correlation analysis suggested that the abundance of Bacteroides and Helicobacter was in positive correlation with AI (P<0.05), while the abundance of Prevotella 9 and Candidatus Saccharimonas was in negative correlation with AI (P<0.01, P<0.05). There was a negative correlation between the abundance of Prevotella 9 and paw volume (P<0.01), and the abundance of Ruminococcaceae NK4A214 group, Christensenellaceae R-7 group, and Bacteroides was in negative correlation with spleen index (P<0.05). The abundance of Prevotella 9 was in negative correlation with spleen index (P<0.01).ConclusionDGNTT is effective for arthritis with FSR, as it can regulate the composition of intestinal flora in AA rats by increasing the abundance of probiotics and inhibiting the growth of pathogenic bacteria. The mechanism is the likelihood that it improves intestinal immune metabolism to ensure intestinal homeostasis.

      摘要:ObjectiveTo evaluate the effect of Astragali Radix (AR)-Angelicae Sinensis Radix (ASR) drug pair on supplementing Qi and activating blood circulation in rats with Qi deficiency and blood stasis and provide a theoretical basis for clinical rational medication and identification and quality control of compound pharmacodynamic substances from the three aspects of characteristic map, identification of pharmacodynamic substances, and comparison of blood components.MethodHigh-performance liquid chromatography (HPLC) was employed to establish the fingerprint of AR∶ASR (3∶1), and ultra-high performance liquid chromatography-Q-Exactive Orbitrap-mass spectrometry (UPLC-Q-Exactive Orbitrap-MS) was employed to analyze the ingredients of the decoction. Adult male Wistar rats with SPF grades were selected and randomly divided into a blank group, a model group, a 3∶1 group, and a 5∶1 group. The rat model of Qi deficiency and blood stasis syndrome was prepared by controlling food intake and swimming in cold water every day. In parallel, each group was given medicine (or water) once a day. The dose of drug groups was 10.2 g∙kg-1, and the model group and blank group were given the same amount of distilled water for 15 d. Animal behavior, body weight, whole blood and plasma viscosity, thymus index, spleen index, the levels of adenosine triphosphate (ATP), adenosine diphosphate(ADP), von willebrand factor (vWF), and ATP/ADP value in serum of rats were recorded. The morphology of vascular endothelium was observed by hematoxylin-eosin (HE) staining and scanning electron microscopy. UPLC-Q-Exactive Orbitrap-MS was used to analyze prototype and metabolic components in serum.ResultThe fingerprint of AR-ASR drug pair (AR-ASR 3∶1) was established. UPLC-Q-Exactive Orbitrap MS identified 49 chemical components in vitro and preliminarily identified 11 prototype components absorbed into blood in vivo. As compared with the blank group, the body mass decreased significantly (P<0.01), the whole blood (high shear, middle shear, and low shear) viscosity and plasma viscosity were significantly increased (P<0.05, P<0.01), the thymus index and spleen index decreased significantly (P<0.05, P<0.01), serum ATP content decreased significantly (P<0.01), ADP content increased significantly (P<0.01), ATP/ADP value decreased significantly (P<0.01), and vWF content increased significantly (P<0.01). The results of HE staining and scanning electron microscopy showed that the vessels were partially damaged, showing the structural disorder of the intima, the bulge, defect, and roughness of the endothelium, and the obvious cell adhesion and migration in the model group. As compared with the model group, the body mass also increased significantly (P<0.01). The results of whole blood and plasma viscosity showed that the whole blood low shear viscosity was significantly decreased in the 3∶1 group (P<0.05). The results of thymus index and spleen index showed that 5∶1 group significantly increased the thymus index of rats (P<0.05). The results of serum ATP and ADP levels showed that the 5∶1 group had more significant effects on ATP and ADP levels (P<0.05), and both groups significantly reduced ATP/ADP values (P<0.01). The results of serum vWF level showed that the vWF content in the 3∶1 group decreased significantly (P<0.05). The results of HE staining and scanning electronic microscopy showed that the damage of vascular endothelium was improved in the treatment group and the structure of intima was neat.ConclusionAR-ASR drug pair can improve the macro and micro indexes of rats with qi deficiency and blood stasis in the 3∶1 and 5∶1 groups. Overall, the 5∶1 ratio has a better effect on supplementing Qi but 3∶1 ratio has a better effect on promoting blood circulation.

      摘要:Shaoyaotang is composed of Cptidis Rhizoma, Scutellariae Radix, Rhei Radix et Rhizoma, Paeoniae Radix Alba, Angelicae Sinensis Radix, Aucklandiae Radix, Arecae Semen, Cinnamomi Cortex and Glycyrrhizae Radix et Rhizoma, with the functions of clearing away heat, eliminating dampness, regulating Qi and activating blood. Thus, it is proposed as the main formula for the treatment of dampness-heat dysentery by later generations of doctors. In modern clinical application, in addition to original Shaoyaotang, its modified formulas are also used for the treatment of ulcerative colitis, and can be used in combination with other prescriptions (such as Tongxie Yaofang, Pulsatilla Soup, Shenling Baizhu San), western medicine (such as mesalazine, sulfasalazine, Infliximab), traditional Chinese medicine (TCM) acupuncture or moxibustion and other characteristic therapies. Clinical efficacy results indicate that Shaoyaotang and its modified formulas can significantly lower Mayo Endoscopic Score (MES), Baron score, TCM syndrome score and other disease scores, and improve patients’ intestinal symptoms, with few side effects. Experimental pharmacological studies reveal that Shaoyaotang can inhibit tumor necrosis factor-α (TNF-α), nuclear transcription factor-κB (NF-κB), interleukin-1β (IL-1β) and other pro-inflammatory factors to up regulate the expression of anti-inflammatory factors such as interleukin-10 (IL-10), thereby reducing the inflammatory response. The formula could also reduce apoptosis by regulating inflammatory signaling pathway and blocking the chain reaction, and repair abnormal immune barrier by balancing immune axis and regulating immune proteins. Additionally, it could adjust the balance of intestinal flora, promote intestinal epithelial cell regeneration and improve mucosal permeability, so as to restore the balance of intestinal environment and thus treat ulcerative colitis. Its monomers baicalin, paeoniflorin, and berberine have anti-inflammatory, antibacterial, metabolism-regulating and other effects. This paper systematically reviewed the clinical and basic research progress of Shaoyaotang in the treatment of ulcerative colitis.

      摘要:ObjectiveTo explore the role of interleukin-6 (IL-6)/signal transducer and activator of transcription 3 (STAT3) pathway in the balance of T helper 17 (Th17)/regulatory T (Treg) cells in ulcerative colitis (UC) with internal dampness-heat accumulation syndrome and the intervention mechanism of Shaoyaotang.MethodA total of 60 SD rats were randomized into blank group (equivalent volume of normal saline), model group (equivalent volume of normal saline), western medicine control group (0.42 g·kg-1 mesalazine), and low-dose (11.1 g·kg-1), medium-dose (22.2 g·kg-1), and high-dose (44.4 g·kg-1) Shaoyaotang groups. UC with internal dampness-heat accumulation syndrome was induced in rats with the compound method except for the blank group. The administration lasted 14 days for each group. At 24 h after the last administration, rats were killed and the spleen and colon tissues were separated. The histopathological changes of colon were observed based on hematoxylin and eosin (HE) staining and the levels of interleukin-17 (IL-17) and transforming growth factor-β1 (TGF-β1) in colon tissue were detected by immunohistochemistry (IHC). Flow cytometry was employed to determine the levels of Th17/Treg cells in the spleen, and Western blot to measure the levels of IL-6 and STAT3 proteins in colon tissue.ResultCompared with the blank group, the model group had lesions such as congestion and erosion, low percentage of spleen Treg cells (P<0.01), high percentage of Th17 cells (P<0.01), and high levels of IL-6 and STAT3 proteins in colon tissue (P<0.01). Compared with the model group, the administration groups showed alleviation of colon injury, high percentage of spleen Treg cells (P<0.05, P<0.01), low percentage of Th17 cells (P<0.01), and low levels of IL-6 and STAT3 proteins in colon tissue (P<0.01).ConclusionShaoyaotang regulates the balance of Th17/Treg by inhibiting the IL-6/STAT3 pathway, thereby relieving the pathological damage of UC rats with internal dampness-heat accumulation syndrome and affecting their immune function.

      摘要:ObjectiveTo investigate the role of cyclic adenosine monophosphate (cAMP)/protein kinase A (PKA)/cAMP-response element binding protein (CREB) signaling pathway in water metabolism and intestinal epithelial permeability in ulcerative colitis (UC) and the intervention mechanism of Shaoyaotang based on the theory of large intestine governing fluids.MethodSixty male SD rats were divided into blank group, model group, mesalazine group (0.42 g·kg-1), Shaoyaotang low-dose group (11.1 g·kg-1), Shaoyaotang medium-dose group (22.2 g·kg-1) and Shaoyaotang high-dose group (44.4 g·kg-1), with 10 in each group. The UC rat model of internal retention of dampness-heat was established by compound factors. The blank group and the model group were given normal saline (ig). The mesalazine group was given mesalazine (ig), and Shaoyaotang low-, medium- and high-dose groups were administrated with corresponding doses of Shaoyaotang (ig). The treatment lasted for 14 days. The diarrhea score and fecal moisture content of rats in each group were observed. The contents of diamine oxidase (DAO) and D-lactic acid in plasma were detected by enzyme-linked immunosorbent assay (ELISA). The protein expressions of aquaporin (AQP)8, AQP4, ZO-1 and Occludin in colon tissues were detected by immunohistochemistry, while those of cAMP, PKA and CREB in colon tissues were determined by Western blot.ResultCompared with the normal group, the model group had elevated diarrhea score and fecal moisten content (P<0.01), increased contents of DAO and D-lactic acid in plasma (P<0.01) and decreased protein expressions of ZO-1, Occludin, AQP8, AQP4, cAMP, PKA and CREB in colon (P<0.01). Compared with the conditions in the model group, the contents of DAO and D-lactic acid in plasma in each administration groups were lower (P<0.01), while the protein expressions of ZO-1, Occludin, AQP8, AQP4, cAMP, PKA and CREB in colon were higher (P<0.01).ConclusionShaoyaotang alleviates the diarrhea in UC, probably through activating cAMP/PKA/CREB signaling pathway, up-regulating expressions of AQPs, enhancing tight junctions in intestinal epithelium and thus improving the water metabolism in colon and the intestinal mucosal permeability.

      摘要:ObjectiveTo investigate the effect of Shaoyaotang on fecal metabolites in rats with ulcerative colitis (UC) induced by 2,4,6-trinitrobenzenesulfonic acid (TNBS) based on liquid chromatography-mass spectrometry (LC-MS).MethodMale SPF SD rats were randomly divided into normal group, model group and Shaoyaotang group (11.1 g·kg-1). Except for normal group, UC rat model was induced by TNBS, and each group was given normal saline except Shaoyaotang group. All groups were treated for 7 days, and the general condition and disease activity index (DAI) were observed. Hematoxylin-eosin (HE) staining was used to observe the histopathological changes of colon, and the protein expressions of interleukin-8 (IL-8) and interleukin-22 (IL-22) in colon tissue were detected by immunohistochemistry (IHC). Rat fecal samples were detected by LC-MS, and the data were analyzed by principal component analysis (PCA) and partial least squares discriminant analysis (PLS-DA). Human Metabolome Database (HMDB) and Kyoto Encyclopedia of Genes and Genomes (KEGG) were searched to screen differential metabolites in combination with literature reference. Then, pathway enrichment analysis was conducted using Metabo Analyst 5.0.ResultShaoyaotang (ig) decreased the DAI of UC rats. Compared with the normal group, the model group had damaged colonic mucosa structure, submucosal inflammatory cell infiltration, increased protein expressions of IL-8 (P<0.01) and IL-22 (P<0.05) in colon tissue. Compared with the conditions in the model group, the colonic damage was alleviated in the Shaoyaotang group, and the protein expressions of IL-8 and IL-22 in colon tissue were decreased (P<0.01). After screening, 15 differential metabolites were identified from the Shaoyaotang group, and the involved pathways mainly included biosynthesis of unsaturated fatty acids, linoleic acid metabolism, terpenoid backbone biosynthesis, porphyrin and chlorophyll metabolism, amino sugar and nucleotide sugar metabolism, pyrimidine metabolism and steroid hormone biosynthesis.ConclusionShaoyaotang has a therapeutic effect on UC, and its anti-inflammatory effect may be related to improving lipid metabolism and regulating the metabolism of cofactors and vitamins as well as the abnormal carbohydrate metabolism.

      摘要:ObjectiveThis study aims to investigate the therapeutic effect of Tangbikang granules(TBK) on type 2 diabetes mellitus (T2DM) complicated with non-alcoholic fatty liver disease (NAFLD) and to elucidate the underlying mechanism.MethodT2DM and NAFLD were induced in ZDF rats, which were then respectively treated (ig) with low-dose (0.625 g·kg-1), medium-dose (1.25 g·kg-1), and high-dose (2.5 g·kg-1) TBK for 12 weeks. Fasting blood glucose (FBG) and body mass were recorded every 4 weeks during the treatment. One week before sampling, the feed intake of rats was detected, and after 12 h night fasting, oral glucose tolerance test (OGTT) was performed. The area under the curve (AUC) was used to evaluate glucose tolerance, and the homeostatic model assessment for insulin resistance (HOMA-IR) was calculated. Blood in abdominal aorta and liver were collected for determination of blood glucose and lipid metabolism indexes: Fasting serum insulin (FINS), serum total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), and nonesterified fatty acids (NEFA). The liver was weighed to calculate the liver index, and the liver tissue morphology was observed and analyzed based on hematoxylin-eosin (HE) staining and periodic acid-Schiff (PAS) staining. The protein levels of insulin receptor substrate (IRS), phosphatidylinositol 3-kinase (PI3K), protein kinase B (Akt) and phosphorylated IRS and Akt were detected by Western blotting. All data were analyzed by SPSS 20.0.ResultThe feed intake of the model group was higher than that in the normal group (P<0.01), and the feed intake the administration groups was lower than that in the model group (P<0.05, P<0.01). At the 8th and 12th week, the body mass in the model group was lower than that in the normal group (P<0.01). Compared with the model group, TBK reduced FBG in a concentration-dependent manner. The blood glucose level in OGTT and AUC in the model group were higher/larger than those in the normal group (P<0.01). The blood glucose value in OGTT was decreased in TBK groups and the metformin group compared with that in the model group, and AUC in the administration groups was significantly different from that in the model group (P<0.01). The serum level of FINS and HOMA-IR in the model group were higher than those in the normal group (P<0.01), and they were lower in the TBK groups than in the model group (P<0.01). Serum levels of TG, TC, HDL-C, NEFA (P<0.05, P<0.01), and LDL-C were higher in the model group than in the normal group. Serum levels of TG, TC, LDL-C, and NEFA in the TBK groups were lower than those in the model group, and the levels of TG, LDL-C, and NEFA in TBK groups were concentration-dependent (lowest levels in high-dose TBK group). Compared with the model group, high-dose TBK significantly increased the level of HDL-C (P<0.05). Liver index of the model group was higher than that in the normal group (P<0.01). The liver index of the administration groups showed a decreasing trend with no significant difference from that in the model group. As for the HE staining result of liver, the model group had unclear structure of liver lobule, enlarged cells of different sizes, and obvious steatosis of hepatocytes. TBK of all doses alleviated liver injury, particularly the high dose. For the PAS staining, compared with the normal group, the model group demonstrated significant fat vacuoles and significant reduction in purplish red glycogen granules in the cytoplasm. The staining results of high- and medium-dose groups of TBK were more similar to the normal group. Western blot was used to detect the protein expression of liver tissue. The expression of PI3K protein, p-IRS1/IRS1, and p-Akt/Akt in the model group were lower than those in the normal group (P<0.01), and they were higher in the high-dose TBK group than in the model group (P<0.01).ConclusionTBK exerts therapeutic effect on T2DM combined with NAFLD in ZDF rats by activating the typical PI3K signaling pathway.

      摘要:ObjectiveTo explore the mechanism of Tangbikang granules (TBK) against diabetic peripheral neuropathy (DPN) based on network pharmacology and in-vivo experiment.MethodThe active components in medicinals of TBK and their target genes were searched from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP). The active components of the medicinals which are not included in TCMSP were searched from previous research. After the analysis of drug-likeness by SwissADME, the target genes of them were predicted with SwissTargetPrediction. DPN-related target genes were retrieved from GeneCards. The common targets of the disease and the prescription were the hub genes of TBK against DPN, which were uploaded to Metascape for Gene Ontology (GO) term enrichment and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis. High-sugar and high-fat diet and low-dose streptozotocin (STZ, ip) were employed to induce diabetes in rats, and then the model rats were respectively treated with low-dose (0.625 g·kg-1), medium-dose (1.25 g·kg-1), and high-dose (2.5 g·kg-1) TBK for 12 weeks. Sensory nerve conduction velocity (SNCV) was evaluated. After hematoxylin and eosin (HE) staining, the sciatic nerve was observed under light microscope to examine the nerve damage. Real-time PCR was performed to detect the gene expression of adenosine monophosphate-activated protein kinase (AMPK) pathway-related targets in rat sciatic nerve, and Western blot to measure the protein expression of AMPK and phosphorylated (p)-AMPK in rat sciatic nerve.ResultThe main active components of TBK, such as quercetin, kaempferol, β-sitosterol, leech pteridine A, stigmasterol, and baicalein were screened out, mainly acting on interleukin-6 (IL-6), tumor necrosis factor (TNF), protein kinase B (Akt), JUN, and HSP90AA1 and signaling pathways such as AMPK, nuclear factor-κB (NF-κB), and Janus kinase/signal transducer and activator of transcription (JAK/STAT). Molecular docking results showed that β-sitosterol and stigmasterol had high binding affinity with IL-6, TNF, JUN, and HSP90AA1. As for the animal experiment, compared with the normal group, model group had low SNCV of sciatic nerve (P<0.01), disordered and loose myelinated nerve fibers with axonotmesis and demyelinization, low mRNA expression of AMPKα, AMPKβ, peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α), Sirtuin 3 (SirT3), mitochondrial transcription factor A (TFAM), and low p-AMPK/AMPK ratio in sciatic nerve (P<0.05, P<0.01). Compared with the model group, TBK of the three doses raised the SNCV (P<0.01), restored nerve morphology and nerve compactness, and increased the mRNA expression of AMPKα, AMPKβ, PGC-1α, SirT3, and TFAM (P<0.05, P<0.01). The ratio of p-AMPK/AMPK in the high-dose and medium-dose TBK groups was higher than that in the model group (P<0.01), while the protein expression in the low-dose TBK group was insignificantly different from that in the model group.ConclusionTBK exerts therapeutic effect on DPN through multiple pathways and targets. The mechanism is that it activates and regulates AMPK/PGC-1α/SirT3 signaling, which lays a basis for further study of TBK in the treatment of DPN.

      摘要:ObjectiveTo explore the effect of Tangbikang granules (TBK) on sciatic nerve inflammation in diabetic rats through modulation of adenosine monophosphate-activated protein kinase (AMPK)/nuclear factor (NF)-κB pathway.MethodSD rats were fed with high-fat and high-sugar diet for 8 weeks and then treated with streptozotocin (STZ, ip) at 35 mg·kg-1 for modeling. Then the rats were randomized into diabetes group, low-dose (0.625 g·kg-1), medium-dose (1.25 g·kg-1), and high-dose (2.5 g·kg-1) TBK groups, and lipoic acid group (0.026 8 g·kg-1) according to body weight and blood glucose level, and a normal group was designed. After modeling, administration began and lasted 12 weeks. The body mass, blood glucose level, and thermal withdrawal latency (TWL) of the rats were detected before treatment and at the 4th, 8th, and 12th week of administration. At the 12th week, the sciatic nerve was collected for hematoxylin-eosin (HE) and Luxol fast blue (LFB) staining, and the structural changes of sciatic nerve were observed under scanning electron microscope. The levels of interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) in sciatic nerve were measured by enzyme-linked immunosorbent assay (ELISA), and the levels of AMPK, phosphorylated (p)-AMPK, and NF-κB proteins in the sciatic nerve were measured by Western blot.ResultThe blood glucose concentration and TWL in the model group were higher than those in the normal group at each time point (P<0.01). The levels of IL-1β, TNF-α, and NF-κB protein in sciatic nerve in the model group were higher than those in the normal group (P<0.01), and the p-AMPK/AMPK ratio was smaller than that in the normal group (P<0.01). Compared with the model group, TBK of the three doses lowered the TWL (P<0.05, P<0.01) and the levels of IL-1β, TNF-α, and NF-κB protein in sciatic nerve of rats (P<0.05, P<0.01), and high-dose and medium-dose TBK raised p-AMPK/AMPK (P<0.05, P<0.01). The sciatic nerve fibers were orderly and compact with alleviation of demyelination in rats treated with TBK compared with those in the model group.ConclusionTBK improves the function of sciatic nerve and alleviates neuroinflammation in diabetic rats. The mechanism is the likelihood that it up-regulates the expression of AMPK in the AMPK/NF-κB pathway and inhibits the expression of downstream NF-κB, thereby alleviating the neuroinflammation caused by high levels of inflammatory factors such as IL-1β and TNF-α due to NF-κB activation.

      摘要:Diabetic peripheral neuropathy (DPN) is a symptom and/or sign of peripheral nerve dysfunction that occurs in patients with diabetes mellitus when other causes are excluded. DPN, one of the most common complications of diabetes mellitus, can lead to disability, foot ulcers, and amputation at a later stage. Its pathogenesis is closely related to high glucose-induced inflammatory damage, oxidative stress, mitochondrial disorders, and apoptosis in neural tissues. The p38 mitogen-activated protein kinase (p38 MAPK) signaling pathway is a key mechanism mediating the expression of inflammatory factors, oxidative factors, and apoptotic factors of neural tissues in DPN. The inflammatory response, oxidative stress damage, and apoptosis, induced by the activation of p38 MAPK phosphorylation by factors such as high glucose, can cause cell lipid peroxidation, protein modification, and nucleic acid damage, which results in axonal degeneration and demyelination changes. The current treatment of DPN with western medicine has obvious shortcomings such as adverse effects and addictive tendencies. In recent years, the research on traditional Chinese medicine (TCM) in the prevention and treatment of DPN has gradually increased, and the exploration of Chinese medicine intervention in the p38 MAPK pathway transduction to improve DPN has advanced. The present study reviewed the relations of the p38 MAPK pathway with insulin resistance and peripheral neuropathy and summarized the molecular biological mechanisms involved in the pathological process of DPN, such as inflammation regulation, oxidative stress, polyol pathway regulation, and Schwann cell apoptosis in the past 10 years. In addition, the literature on Chinese medicine monomers, Chinese patent medicines, and Chinese medicine compounds in inhibiting inflammatory reactions, oxidative injury, and apoptosis of DPN peripheral nerves based on the p38 MAPK pathway, resisting axonal degeneration and demyelination changes, improving sensory and motor abnormalities, relieving peripheral pain sensitization, and facilitating nerve conduction mechanism to provide references for the development of new drugs for clinical prevention and treatment of DPN.

      摘要:ObjectiveTo observe the effect of modified Erchentang on the expression of key molecules in the Jagged1/Notch1/Hes1 signaling pathway in lung tissues of rats with chronic obstructive pulmonary disease (COPD) and explore its anti-inflammatory effect and molecular mechanism on COPD through the Jagged1/Notch1/Hes1 signaling pathway.MethodSixty SD rats were randomly divided into normal group, model group, low-, medium-, and high-dose modified Erchentang groups (5, 10, 20 g·kg-1), and γ-secretase inhibitor DAPT group (0.02 g·kg-1), with 10 rats in each group. The COPD model was induced in rats by cigarette smoking combined with intratracheal instillation of lipopolysaccharide (LPS). Rats were treated with corresponding drugs by gavage, while those in the normal group and the model group were treated with the same amount of normal saline by gavage. The serum levels of Notch1, soluble intercellular adhesion molecule-1 (sICAM-1), activated leukocyte cell adhesion molecule (ALCAM), and soluble vascular adhesion molecule-1 (sVCAM-1) were detected by enzyme-linked immunosorbent assay (ELISA). The mRNA expression of Jagged1, Notch1, and Hes1 was detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). The protein expression of Jagged1, Notch1, Notch1 intracellular domain (NICD1), and Hes1 in lung tissues of rats was detected by immunohistochemistry (IHC).ResultCompared with the normal group, the model group showed increased serum content of Notch1, sICAM-1, ALCAM, and sVCAM-1 (P<0.01), increased mRNA expression of Jagged1, Notch1, and Hes1 in lung tissues (P<0.01), and increased protein expression of Jagged1, Notch1, NICD1, and Hes1 (P<0.01). Compared with the model group, the medium- and high-dose modified Erchentang groups and the DAPT group showed decreased serum content of Notch1, sICAM-1, ALCAM, and sVCAM-1 (P<0.05, P<0.05), down-regulated mRNA expression of Jagged1, Notch1, and Hes1 (P<0.05, P<0.01), and reduced protein expression of Jagged1, Notch1, NICD1, and Hes1(P<0.05, P<0.01).ConclusionModified Erchentang may inhibit the inflammatory response in the lung of COPD rats, and its mechanism may be related to the resistance of inflammatory injury in the lung by decreasing the mRNA expression of Jagged1, Notch1, and Hes1 and inhibiting the release of Notch1, sICAM-1, ALCAM, and sVCAM-1.

      摘要:ObjectiveTo investigate the effect of different doses of Jiedu Tongluo Shengjin prescription (JTSP) on serum interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and forkhead box P3 (FoxP3) in submandibular gland of NOD/Ltj mice with Sjögren's syndrome, and to explore the mechanism of JTSP on immune regulation in NOD/Ltj mice.MethodThirty NOD/Ltj mice (eight weeks old) were randomly divided into model group, JTSP low-dose group, JTSP medium-dose group, JTSP high-dose group and hydroxychloroquine group, and were administrated with normal saline, JTSP 9, 18, and 36 g·kg-1, and hydroxychloroquine 60 mg·kg-1 daily, respectively from the age of 12 weeks. Six ICR mice were given an equal amount of normal saline by gavage as the control group. During the experiment, daily water consumption and saliva secretion of mice at the age of 9, 12, 16 weeks were recorded. After 4 weeks of administration, submandibular gland and spleen tissues were dissected to calculate corresponding indexes. The pathological morphology of submandibular gland was observed by hematoxylin-eosin (HE) staining. Meso Scale Discovery (MSD) and immunohistochemistry were employed to detect the serum levels of IL-6, TNF-α and IL-10, and the expression and distribution of FoxP3 in submandibular gland, respectively. The protein expression of FoxP3 in mouse submandibular gland was determined by Western blot, and the mRNA expressions of FoxP3 and TNF-α were determined by real-time polymerase chain reaction (Real-time PCR).ResultCompared with the control group, the model group presented increased daily water consumption, decreased saliva secretion, lowered submandibular gland index, elevated pathological score of submandibular gland, up-regulated serum IL-6 and TNF-α and mRNA expression of TNF-α while down-regulated serum IL-10 and protein and mRNA expressions of FoxP3 in submandibular gland (P<0.05). Compared with the conditions in model group, daily water consumption in JTSP groups was reduced while saliva secretion was increased, especially in medium-dose and high-dose groups (P<0.05), and there was an increase in the submandibular gland index of JTSP medium-dose group (P<0.05) while a decrease in the spleen index of JTSP high-dose group (P<0.05). Additionally, JTSP groups had lower pathological score of submandibular gland than the model group (P<0.05), especially high-dose group, as well as lower serum IL-6 and TNF-α and mRNA expression of TNF-α while higher serum IL-10 (P<0.05). JTSP at medium and high doses up-regulated the protein and mRNA expressions of FoxP3 in submandibular gland (P<0.05).ConclusionJTSP may inhibit the secretion of inflammatory cytokines by regulating the stability of FoxP3+ regulatory T (Treg) cells, thus alleviating the systemic immune inflammation in Sjögren's syndrome.

      摘要:ObjectiveTo explore the effect and underlying mechanism of alcohol extract of Phyllanthi Fructus on silicosis mice induced by silicon dioxide (SiO2).MethodThirty-six male Kunming mice of SPF grade were randomly divided into a blank group,a model group,high-, medium, and low-dose Phyllanthi Fructus groups (800, 400, 200 mg·kg-1),and a tetrandrine group (0.039 mg·kg-1),with six mice in each group. The silicosis model was induced by static SiO2 exposure in mice except for those in the blank group. After 28 days of administration by gavage,the lung tissues were collected and the organ coefficient was calculated. Hematoxylin-eosin(HE)staining and Masson staining were used to detect the morphology of lung tissues. The content of hydroxyproline (HYP),superoxide dismutase (SOD),malondialdehyde (MDA), and catalase (CAT) in serum was detected by enzyme-linked immunosorbent assay (ELISA). Western blot and Real-time polymerase chain reaction(Real-time PCR) were used to detect the protein and mRNA expression of nuclear factor E2-related factor 2 (Nrf2),heme oxygenase-1 (HO-1),NAD(P)H:quinone oxidoreductase 1 (NQO1),and Kelch-like ECH-associated protein 1 (Keap1), respectively.ResultCompared with the blank group,the model group showed seriously damaged morphological structure of lung tissues with inflammatory cell infiltration and fibrous tissue proliferation, reduced serum content of SOD and CAT(P<0.01),increased content of HYP and MDA(P<0.01), down-regulated protein and mRNA expression of Nrf2,HO-1, and NQO1(P<0.01),and up-regulated protein and mRNA expression of Keap1 (P<0.05,P<0.01). Compared with the model group,the high- and medium-dose Phyllanthi Fructus groups showed significantly restored morphological structure of lung tissues with reduced collagen deposition, increased serum content of SOD and CAT(P<0.05,P<0.01),decreased content of HYP and MDA(P<0.01), up-regulated protein and mRNA expression of Nrf2,HO-1, and NQO1 (P<0.05,P<0.01),and down-regulated protein and mRNA expression of Keap1(P<0.05,P<0.01).ConclusionThe alcohol extract of Phyllanthi Fructus can inhibit pulmonary fibrosis in silicosis mice,and the underlying mechanism may be related to the regulation of the Nrf2/antioxidant response element (ARE) signaling pathway.

      摘要:ObjectiveTo investigate the mechanism of Xiaonang Tiaojing decoction(XNTJD)in improving polycystic ovary syndrome with insulin resistance(PCOS-IR)model rats based on anti-Müllerian hormone(AMH)/AMH type Ⅱ receptor(AMHRⅡ)signaling pathway.MethodForty-eight adult female SD rats were randomly divided into the blank group, model group, XNTJD group(11.4 g·kg-1·d-1) and Diane-35 group(0.21 g·kg-1·d-1), PCOS-IR model was established by high-fat and high-sugar diet combined with letrozole in rats of all groups except the blank group, rats in the administration groups were given the corresponding dose of drugs by gavage for 15 days with an interval of 1 d every 4 d, normal saline of the same volume was given to the blank group and the model group. Estrous cycle was recorded daily during treatment. At the end of treatment, body weight and Lee's index were recorded, AMH, luteinizing hormone(LH), LH/follicle stimulating hormone(FSH), testosterone(T)and estradiol(E2)levels were measured by enzyme-linked immunosorbent assay(ELISA), fasting plasma glucose(FPG)was measured by glucometer, fasting insulin(FINS) level was measured by radioimmunoassay(RIA), and the insulin resistance index(HOMA-IR) and insulin sensitivity index(QUICKI)were calculated, triglyceride(TG)and total cholesterol(TC)levels were measured by automatic biochemical analyzer, hematoxylin-eosin(HE)staining was used to observe the morphological changes of the ovary, the levels of AMHRⅡ, bone morphogenetic protein-15(BMP-15)and Smad5 in ovarian tissues were detected by immunohistochemistry(IHC),Western blot was used to analyze the protein expression levels of AMHRⅡ, BMP-15 and Smad5.ResultCompared with the blank group, a large number of leukocytes were observed in the vaginal exfoliated cells of rats in the model group, mainly in diestrus, the levels of body weight, Lee's index, glucose-lipid metabolism indexes(FPG, FINS, HOMA-IR, TG, TC), AMH and sex hormones(LH, LH/FSH, T)were significantly increased(P<0.01), and QUICKI and E2 levels were significantly decreased(P<0.01), there were more cystic bulges on the ovarian surface, more wet weight, more atretic and cystic dilated follicles in the ovarian tissues, and the thickness of granulosa cell layer was reduced without oocytes, the expression level of AMHRⅡ protein in ovarian tissues was significantly increased(P<0.01), and the expression levels of BMP-15 and Smad5 proteins were significantly decreased(P<0.01). Compared with the model group, the exfoliated cells in the vagina of rats treated with XNTJD group showed keratinocytes from the 5th to 6th day of treatment, and a stable estrous cycle gradually appeared, body weight, Lee's index, glucose-lipid metabolism indexes(FPG, FINS, HOMA-IR, TG, TC), AMH and sex hormones(LH, LH/FSH, T)levels were significantly decreased(P<0.05, P<0.01), QUICKI and E2 levels were significantly increased(P<0.01), ovarian surface was smoother and lighter in wet weight, oocytes and mature follicles were observed in ovarian tissues, the thickness of granulosa cell layer increased and the morphology was intact, the expression levels of BMP-15 and Smad5 proteins were significantly increased(P<0.01)and the expression level of AMHRⅡ protein was significantly decreased(P<0.01)in ovarian tissues.ConclusionXNTJD may mediate the up-regulation of BMP-15 and Smad5 in ovarian tissues of PCOS-IR rats by down-regulating AMH/AMHRⅡ, thereby improving ovarian function, sex hormones and glucose-lipid metabolism levels in PCOS-IR rats.

      摘要:ObjectiveTo study the effect of Bazi Bushen capsules on delaying the aging process of naturally aging mice and its mechanism.MethodThe mice were randomly divided into four groups according to their body weight, namely, aging group, low-dose Bazi Bushen capsules group (1 g·kg-1), high-dose Bazi Bushen capsules group (2 g·kg-1), and rapamycin group (0.002 g·kg-1). The debilitating signs were detected by behavioral tests and the weakness index was measured. The percentages of spleen T and B lymphocytes, effector T cells (TE), memory T cells (TM), naive T cells (TN), helper T cells (Th), cytotoxic T cells (Tc) ,Th1 cells, Th2 cells, and regulatory T cells (Treg) were determined by flow cytometry. Cell proliferation and the cell counting kit-8 (CCK-8) assay was used to detect the proliferation of lymphocytes in mice. The hematoxylin-eosin (HE) staining was used to observe the histopathological changes in the mouse spleen. The expression of cyclin-dependent kinase inhibitor 2A (p16) and cyclin-dependent kinase inhibitor 1A (p21) was detected by immunohistochemistry (IHC). The mRNA expression of senescence-related proteins p16 and p21 was determined by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), and the inflammatory cytokines, including tumor necrosis factor-α (TNF-α), interferon-γ (IFN-γ), interleukin (IL)-1β, IL-2, IL-4, IL-10, and IL-12 p70, in peripheral blood of mice were detected by Luminex.ResultAs compared with the aging group, mice in the Bazi Bushen capsules and rapamycin groups showed significantly improved debilitating signs and reduced weakness index scores (P<0.05, P<0.01), increased proportions of T cells, TN cells, Tc cells, Th2 cells, and Treg cells in the spleen, decreased proportions of TE cells, TM cells, Th cells, Th1 cells (P<0.05, P<0.01), and increased proliferation of splenic lymphocytes (P<0.05, P<0.01). In the Bazi Bushen capsules and rapamycin groups, clear structure of the red and white marrow marginal zone was observed in the spleen of mice, the area of the white marrow was increased, and the area of the red marrow was correspondingly decreased. The protein and mRNA expression of aging-related proteins p16 and p21 in the spleen was decreased (P<0.01), the levels of serum pro-inflammatory cytokines IL-1β, IL-2, IL-12 p70, IFN-γ, and TNF-α levels were decreased, and the levels of IL-4 and IL-10 were increased (P<0.05, P<0.01) in the Bazi Bushen capsules and rapamycin groups as compared with the aging group.ConclusionBazi Bushen capsules have the effect of regulating the debilitating signs of natural aging mice, regulating the immune homeostasis and inflammation level of the body, and reducing cell aging.

      摘要:ObjectiveTo investigate the intervention effect of Qufeng Gutong Babu ointment (QFGT) on rats with osteoarthritis (OA) with cold-dampness obstruction, and preliminarily clarify its mechanism.MethodSD male rats were divided into 6 groups, namely, the blank group, model group, positive control drug Huoxue Zhitong ointment (HXZTG) group (1.26 cm2·d-1), and low, medium, and high-dose QFGT group (75, 150, 300 mg·d-1). OA model was prepared by joint cavity injection of papain and L-cysteine. On the second day of modeling, climate factors were applied to establish an animal model of combination of disease and syndrome of OA rats with cold-dampness obstruction. Standard VonFrey fiber was used to evaluate the threshold of mechanical pain. Weight bearing difference score and joint function score of both hind limbs were recorded. Hematoxylin-eosin (HE) staining and safranine fixation green staining were used to observe the pathological changes and cartilage degeneration of rat knee joint. Immunohistochemistry (IHC) was used to detect the expression of interleukin-1β (IL-1β), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α), matrix metalloproteinase-9 (MMP-9), and cathepsin K (CTSK). Western blot was used to detect the protein expression of kinase B (Akt), phosphorylated protein kinase B (p-Akt), phosphatidylinositol 3-kinase (PI3K), nuclear factor 1 (NFATc1), MMP-9, and CTSK in T cells.ResultCompared with the normal group, the model group showed significant mechanical pain sensitivity reaction after modeling (P<0.01), and the weight bearing difference of both hind limbs and joint function score were significantly increased (P<0.05, P<0.01). Compared with the model group, both the high-dose QFGT group and the HXZTG group significantly reduced the mechanical pain sensitivity, weight difference, and joint function score of rats (P<0.05, P<0.01), and the medium-dose QFGT group also improved the joint function to a certain extent, and the degeneration of the knee joint cartilage of rats was significantly reduced (P<0.05, P<0.01). QFGT and HXZTG both inhibited the protein expression of IL-1β, IL-8, TNF-α, MMP-9, CTAK, PI3K, p-Akt, Akt, and other related proteins in articular cartilage of rats with OA to a certain extent (P<0.05, P<0.01).ConclusionQFGT can inhibit the release of inflammatory factors and matrix metalloproteinases by inhibiting the PI3K/Akt signal pathway in articular articular cartilage of rats with OA with cold-dampness obstruction, thus ultimately weakening local cartilage degeneration and improving joint function.

      摘要:ObjectiveTo investigate the changes of differential metabolites in the serum of mice at different stages of bleomycin sulfate(BLM)-induced pulmonary fibrosis modeling and administration, and the mechanism of Wenfei Huaxian granules(WHG)against idiopathic pulmonary fibrosis.MethodMice were randomly divided into control group, control group of 14 days, model group, model group of 14 days, low-dose WHG group and high-dose WHG group. BLM(0.04 U per mouse)was injected into the trachea of mice in the model group, model group of 14 days, low-dose WHG group and high-dose WHG group, and sterile normal saline was injected into the trachea of mice in the control group and control group of 14 days. Mice of low-dose WHG group and high-dose WHG group were given different doses of WHG by gavage every day after injection of BLM, and mice of control group, control group of 14 days, model group and model group of 14 days were given sterile water by gavage every day. The peripheral blood of mice in the control group of 14 days and model group of 14 days were taken to prepare serum after injection of BLM for 14 days, and the peripheral blood and other materials of mice in the other groups were taken after continuous administration for 28 days. The bronchoalveolar lavage fluid(BALF)was collected for leucocyte differential count, the pathological examination and hydroxyproline(HYP)content determination of lung tissues of mice were performed, and the small molecule metabolites in serum samples of mice in each group were determined by ultra-high performance liquid chromatography-mass spectrometry(UHPLC-MS). Principal component analysis(PCA)and orthogonal partial least squares-discriminant analysis(OPLS-DA)were conducted to screen differential metabolites and their biological functions were analyzed.ResultCompared with the control group, a large number of continuous fibrotic foci appeared in the lung tissue of mice in the model group, the alveolitis score, fibrosis degree score and HYP content increased significantly(P<0.01), and the total number of leukocytes, macrophages and lymphocytes in BALF increased significantly(P<0.05). A total of 33 differential metabolites were screened between the control group of 14 days and model group of 14 days, mainly lipid metabolites, which were mainly involved in oxidative damage and inflammatory process. A total of 34 differential metabolites, mainly amino acid metabolites, were screened between the control group and model group, mainly involving nucleic acid damage and inflammatory process. Compared with the model group, the HYP content, fibrosis score and alveolitis score in the lung tissue of mice from high-dose WHG group decreased significantly(P<0.05, P<0.01), and the total number of lymphocytes in BALF decreased significantly(P<0.05). Compared with the model group, 27, 40 differential metabolites were identified in the serum of mice from the low-dose WHG group and high-dose WHG group separately. There were totally 9 common differential metabolites between the model group and low-dose WHG group/high-dose WHG group, which mainly involved in the metabolic pathways of inflammation related lipids metabolism, arginine and tryptophan metabolism, and the change trends in low-dose WHG group and high-dose WHG group were significantly back-regulated compared with the model group.ConclusionWHG can alleviate BLM-induced pulmonary fibrosis, collagen deposition and inflammatory reaction in mice, and its anti-fibrotic effect may be related to the adjusting of inflammatory factors, nitric oxide and oxidative stress related metabolic pathways.

      摘要:ObjectiveTo study the metabolism of chemical components from Citri Reticulatae Pericarpium(CRP)in different parts of rats by sequential metabolism and ultra performance liquid chromatography-high resolution mass spectrometry(UPLC-HRMS).MethodSD male rats were employed as experimental subjects, and blood samples of intestinal metabolism and hepatic metabolism were prepared after administration of CRP ethanol extract by in situ intestinal perfusion, and comprehensive metabolic samples were collected after intragastric administration. UPLC-HRMS was used to analyze the samples with acetonitrile(A)-0.1% formic acid aqueous solution(B)as the mobile phase for gradient elution(0-10 min, 10%-30%A; 10-30 min, 30%-95%A; 30-31 min, 95%-10%A; 31-35 min, 10%A)at a flow rate of 0.35 mL·min-1, using a heated electrospray ionization with positive and negative ion mode scanning in the range of m/z 100-1 500. Under these conditions, the differences in the profiles of CRP ethanol extract, blank plasma and drug-containing plasma under different treatment groups were compared, and the chemical components of each sample were analyzed and identified based on the retention time, accurate relative molecular mass, primary and secondary ion fragments, and the information of reference substances.ResultA total of 44 chemical components were identified in the CRP ethanol extract, including flavone-O-glycosides, flavone-C-glycosides and polymethoxyflavonoids, etc. The results of sequential metabolism showed that 22 chemical components in CRP were detected in the intestinal metabolic sample, 18 chemical components were detected in the hepatic metabolic sample, and 9 identical chemical components(narirutin, hesperidin, meranzin, 5,7,8,3ʹ,4ʹ,5ʹ-hexamethoxy-flavone, isosinensetin, sinensetin, 3,5,6,7,8,3ʹ,4ʹ-heptamethoxyflavone, nobiletin and tangeretin)could be detected in all three metabolic samples, with a total of 22 compounds entering the blood in prototype form.ConclusionThe identified 21 components with well-defined structures entering the blood as prototypes may be potential active components of CRP, and differences in the components at different metabolic parts can provide an experimental basis for elucidating the in vivo biotransformation process of the metabolic components of CRP.

      摘要:ObjectiveTo analyze the long-term survival of non-small cell lung cancer (NSCLC) patients treated with Fuzheng Gushe therapy in the real world.MethodA retrospective cohort study was carried out with the NSCLC patients treated with Fuzheng Gushe therapy from January 2014 to December 2018 in Dongzhimen Hospital, Beijing University of Chinese Medicine as the integrated traditional Chinese medicine (TCM) and western medicine cohort. The NSCLC data from January 2011 to December 2015 in the Surveillance, Epidemiology, and End Results (SEER) database were downloaded as the western medicine cohort. The propensity score matching (PSM) was employed to balance the confounding factors between the groups, and comparable samples were obtained from the two groups. The Log-rank test was conducted to compare the overall survival between the groups and the survival curves were established.ResultThe integrated TCM and western medicine cohort and the western medicine cohort included 511 and 5 022 NSCLC patients, respectively. The age, sex, and chemotherapy had no significant differences between the two cohorts, while pathological type, lymph node metastasis (N), distant metastasis (M), surgery, and radiotherapy were different between the two cohorts (P<0.05). After PSM, 122 comparable samples were obtained. The overall survival analysis showed that the total survival of the integrated TCM and western medicine cohort was better than that of the western medicine cohort (P<0.05). The stratified survival analysis showed that the integrated TCM and western medicine cohort had survival advantages in females, males, the young, and adenocarcinoma, N+, and M+ patients (P<0.05).ConclusionFuzheng Gushe therapy can significantly improve the overall survival of NSCLC patients, especially in the females, the males, the young, and the patients with adenocarcinoma, lymph node metastasis, and distant metastasis.

      摘要:ObjectiveTo investigate the clinical value of Longbeisan application at Shenque (CV 8) combined with oral administration of Chinese medicine in the treatment of premature ejaculation (PE).MethodA total of 98 PE patients treated in the andrology department of the First Affiliated Hospital of Henan University of Chinese Medicine at the same time period were randomly assigned into an observation group and a control group, with 49 patients in each group. The observation group received Longbeisan application at Shenque (CV 8) combined with oral treatment of Chinese medicine according to syndrome differentiation, and the control group was treated with dapoxetine hydrochloride tablets. The treatment in both groups lasted for 8 weeks. The intravaginal ejaculatory latency time (IELT), Chinese index of premature ejaculation-5 (CIPE-5) score, patient's sexual life satisfaction, spouse's sexual life satisfaction, effective rate, and adverse reaction incidence were compared between the two groups.ResultAfter treatment, the observation group had higher total effective rate than the control group [(85.71% (42/49) vs. 67.35% (33/49), χ2=6.262, P<0.05]. The IELT, CIPE-5 score, and patient's and spouse's satisfaction scores after treatment increased compared with those before treatment (P<0.01), and the increases were more significant in the observation group (P<0.05, P<0.01). The clinical effect of the observation group was better than that of the control group. During the treatment, 7 (7/49,14.29%) patients in the control group and 2 ,2/49,4.08%) patients in the observation group showed adverse reactions, which indicated the safety of the observation group was better than that of the control group (χ2=9.000, P<0.05). In the follow-up period, 11 (11/49,22.45%) patients in the control group and 3 (3/49,6.12%) patients in the observation group showed aggravation of symptoms, which meant that the observation group had better lasting effect (χ2=0.317, P<0.05).ConclusionLongbeisan application at Shenque (CV 8) combined with oral administration of Chinese medicine has better clinical effect, stronger safety, and longer effect than dapoxetine hydrochloride in the treatment of PE.

      摘要:ObjectiveTo develop a quality control method for the simultaneous determination of multiple active components in Nymphaeae Flos aiming at the problems of the single index for quality control and the relatively low overall quality control level.MethodUltra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS)was used to identify and select the index components for quality control with the mobile phase of 0.1% formic acid aqueous solution(A)-acetonitrile(B)for gradient elution (0-2 min, 3%-8%B; 2-4 min, 8%-10%B; 4-13 min, 10%-15%B; 13-19 min, 15%-20%B; 19-26 min, 20%-45%B) at a flow rate of 0.4 mL·min-1, detection wavelength of 350 nm, electrospray ionization(ESI), negative ion scanning mode, ion source temperature of 120 ℃, scanning range of m/z 100-1 200, transmit collision energy of 6 eV for low-energy scanning and 25-50 eV for high-energy scanning. High performance liquid chromatography(HPLC)was used to establish the quality control method for the simultaneous determination of multi-index components with the mobile phase of 0.2% phosphoric acid aqueous solution(A)-acetonitrile(B) for gradient elution(0-30 min, 12%-15%B; 30-60 min, 15%-22%B; 60-90 min, 22%-40%B)and detection wavelength of 350 nm. The preparation method of the test solution for content determination was refluxing extraction for 60 min with 80 times the amount of 70% methanol.ResultBy comparing the retention time, ultraviolet absorption characteristics, MS and MS/MS spectrometric signals in the samples with the reference substances, 8 active components with high contents, including brevifolincarboxylic acid, ellagic acid, rutin, nicotiflorin, astragalin, quercetin, quercetin-3-methylether and kaempferol, were identified qualitatively from Nymphaeae Flos, which were selected as the index components for quality control. Under the established HPLC conditions, the above 8 components could be well separated(resolution>1.5), and showed good linearity(r=0.999 9)between the concentration ranges of 1.99-99.6, 1.76-176, 1.52-75.8, 3.60-180, 0.964-96.4, 1.18-118, 1.94-96.8, 1.04-104 mg·L-1 and the peak areas, respectively. The detection limits of them were 10-49 μg·L-1, and the limits of quantitation were 34-164 μg·L-1. The average recoveries were 97.12%-103.1% with the relative standard deviations (RSDs) were 1.1%-2.2%.ConclusionA quality control method for simultaneous determination of the multiple active components in Nymphaeae Flos have been developed, which is simple, accurate and reproducible, and it can provide a scientific basis for the formulation of quality standard of this herb and lay a research foundation for the transformation of Uygur hospital preparations containing Nymphaeae Flos into new drugs.

      摘要:ObjectiveBy comparing the differences in composition and content of volatile components between Atractylodis Macrocephalae Rhizoma(AMR)and bleaching AMR, bran-fried AMR and bran-fried bleaching AMR, the effect of processing with rice-washed water on the volatile components in AMR and bran-fried AMR were investigated.MethodHeadspace gas chromatography-mass spectrometry(HS-GC-MS)was used to determine the volatile components in raw products, bran-fried products and their processed products with rice-washed water. GC conditions were programmed temperature(starting temperature of 50 ℃, rising to 140 ℃ at 10 ℃·min-1, maintained for 5 min, then rising to 210 ℃ at 4 ℃·min-1), splitting ratio of 10∶1, high purity helium as the carrier gas and a solvent delay time of 3 min. MS conditions were an electron bombardment ion source(EI) with an electron collision energy of 70 eV, ion source temperature of 230 ℃, and the detection range of m/z 20-650. The relative contents of the components were determined by the peak area normalization method, the obtained sample data were subjected to principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA) by SIMCA 14.1 software, and the differential components of AMR and bleaching AMR, and bran-fried AMR and bran-fried bleaching AMR were screened according to variable importance in the projection(VIP) value>1 and P<0.05.ResultA total of 71 volatile components were identified, including 53 in AMR, 50 in bleaching AMR, 51 in bran-fried AMR, and 44 in bran-fried bleaching AMR. OPLS-DA results showed that there were significant differences between AMR and bleaching AMR, bran-fried AMR and bran-fried bleaching AMR, but not between AMR samples from different origins. The compound composition of AMR and bleaching AMR, bran-fried AMR and bran-fried bleaching AMR did not change, but the contents of monoterpenes and sesquiterpenes changed significantly.ConclusionSignificant changes in the contents of volatile components were observed in AMR and bleaching AMR, bran-fried AMR and bran-fried bleaching AMR, among them, 1,2-dimethyl-4-methylidenecyclopentene, 9,10-dehydro-isolongifolene, γ-elemene, zingiberene, atractylone, silphinene, modhephene and (1S,4S,4aS)-1-isopropyl-4,7-dimethyl-1,2,3,4,4a,5-hexahydronaphthalene can be used as candidate differential markers of volatile components of AMR before and after processing with rice-washed water.

      摘要:ObjectiveTo analyze the fingerprint of six pungent herbs based on the molecular connectivity index(MCI)and the matching frequency total statistical moment method, and to study the division and integration of the "imprinting template" of their volatile components, so as to find the common "imprinting template" characteristics of the pungent herbs.MethodThe volatile components of six pungent herbs were extracted by steam distillation, and their fingerprints were established by gas chromatography-mass spectrometry(GC-MS) with a programmed temperature increase(80 ℃ for 5 min, 5 ℃·min-1 to 200 ℃ for 5 min, 2 ℃·min-1 to 230 ℃ for 10 min), a splitting ratio of 20∶1, an electron bombardment ion source(EI) and the detection range of m/z 35-650, and the average MCI and total statistical moment parameters of the fingerprints were calculated. Then the matching frequency method was used to classify, integrate and confirm the chromatographic peaks of the fingerprints of six pungent herbs.ResultThe average zero order, first-order and second-order MCI values of the volatile components of Pogostemonis Herba, Artemisiae Argyi Folium, Atractylodis Rhizoma, Asari Radix et Rhizoma, Magnoliae Flos and Schizonepetae Herba were 9.02, 5.28 and 5.05, respectively. The average values of peak number, total zero-order moment, total first-order moment and total second-order moment were 60, 169×107, 22.49 min and 36.82 min2, respectively. The 20 integrated imprinting templates were obtained by the matching frequency method for the six pungent herbs, among which three were common imprinting templates with the retention times of (25.97±0.21),(26.90±0.20),(31.64±1.24) min, respectively, and the representative components were valencene,β-elemene, caryophyllin, etc.ConclusionMCI combined the matching frequency total statistical moment can divide and integrate the characteristics of imprinting templates of six pungent herbs, and find their common chromatographic imprinting characteristics, which can provide a reference for the determination of effective substances of pungent herbs.

      摘要:ObjectiveTo study the modeling characteristics of the animal model of premature ovarian insufficiency and provide references for the standardization of the animal model of premature ovarian insufficiency, thus offering a better research basis to the pathogenesis, diagnosis, and treatment of the disease.MethodThe animal experimental literature of premature ovarian insufficiency in the past decade was obtained by searching China National Knowledge Infrastructure (CNKI), Wanfang, VIP Chinese Technology Periodical Database, China Biology Medicine (CBM), and PubMed. The types of experimental animals, modeling methods, dosage, administration scheme, modeling standards, and detection indicators were summarized. The frequency analysis was performed with Excel, the association rule analysis was performed with SPSS Modeler 18.0, and the results were visually upgraded with Cytascape 3.6.1.ResultA total of 281 articles were included, and most animal experiments on premature ovarian insufficiency were performed on SD rats or BALB/c mice. Most modeling methods were iatrogenic induction, and the main modeling drug was cyclophosphamide. Erestrous cycle disorder was selected as the modeling standard. Ovarian histomorphology, estradiol, follicle stimulating hormone, and estrous cycle were selected as the detection indicators to comprehensively evaluate the model from multiple aspects.ConclusionSD rats are mostly used to induce modeling in animal experiments on premature ovarian insufficiency through first intraperitoneal injection of cyclophosphamide 50 mg·kg-1 and 8 mg·kg-1·d-1 from the next day for 14 d, which has the advantages of easy operation, high modeling rate, and consistency with the disease pathogenesis. This paper can provide references for basic animal experimental research of premature ovarian insufficiency.

      摘要:Chronic fatigue syndrome (CFS) is a heterogeneous disease with dysfunction in multiple systems and multiple organs. Its etiology and pathogenesis have not been fully clarified, and its treatment also lacks specificity. The key to studying CFS is developing animal models that reflect the underlying mechanisms and etiology of CFS. The existing CFS modeling methods are complicated and not unified. By sorting out relevant literature,the present study evaluated the modeling methods,modeling standards,mechanisms, and clinical coincidence of the immune model,the stress model, and the disease-syndrome combination model in traditional Chinese medicine (TCM). The immune model is mainly constructed from the perspective of pathophysiology, with easy operation and wide investigation, which can simulate the pathological characteristics of CFS to ensure pathogenesis research,but the experimental repeatability is general. Stress modeling is a common method for a variety of neuropsychiatric diseases,including CFS. Many different stressors can be employed to investigate the etiology of CFS, but their effects are unpredictable. Compared with the two western medicine models mentioned above,the TCM disease-syndrome combination model integrates modern medicine with TCM theory,with high clinical coincidence and great practical value. However,the TCM disease-syndrome combination model of CFS is still in the exploratory stage with a few types of models,which needs to be further improved, aiming to establish scientific,reasonable,simple, and efficient animal models to provide support for exploring the etiology,pathogenesis, and new treatment ideas of CFS.

      摘要:Irritable bowel syndrome (IBS) is a functional gastrointestinal disease, but it often causes extreme gastrointestinal discomfort and prolonged illness, which seriously affects the quality of life of patients. The global incidence rate is increasing year by year. Clinically, western medicine mainly uses oral antispasmodics, secretagogues, and antidepressants, but there are many disadvantages such as adverse reactions and poor long-term efficacy. Therefore, finding an efficient and safe treatment method is an urgent problem to be solved. A large number of studies have shown that traditional Chinese medicine has definite curative and long-lasting effects on the treatment of IBS, which has become a hot research direction in recent years. By searching Chinese and foreign literature, it is found that electroacupuncture, moxibustion, Chinese medicine monomers, and compound decoctions are the main methods in the mechanism research of traditional Chinese medicine in the treatment of IBS-related pathways, and their signaling pathways involve nuclear transcription factor kappa B (NF-κB), transient receptor potential vanillin subfamily 1 (TRPV1), 5-hydroxytryptamine (5-HT), mitogen-activated protein kinase (MAPK), and so on. Traditional Chinese medicine can repair intestinal inflammation, reduce visceral sensitivity, enhance intestinal mucosal barrier, and regulate intestinal motility by regulating this series of signaling pathways, thereby playing an important role in the treatment of IBS with multi-level, multi-link and multi-target characteristics. Based on the cell signaling pathways, this paper reviewed the research progress on the mechanism of traditional Chinese medicine in the treatment of IBS, hoping to provide theoretical support and diagnosis and treatment ideas for the clinical treatment of IBS with traditional Chinese medicine.

      摘要:In recent years, coronary microvascular disease (CMVD), a main type of ischemic heart disease with high incidence and low diagnosis rate, has become a new research hotspot and received much clinical attention. The etiology of CMVD is complex and the symptoms are various. Traditional Chinese and Western medicine have different opinions on its pathogenesis and treatment plan. Western medicine believes that CMVD is related to structural abnormalities (such as microvascular remodeling, vascular invasion, lumen obstruction, sparse vascular vessel and perivascular fibrosis) and functional abnormalities (such as endothelial dysfunction, smooth muscle cell dysfunction, microvascular constriction, microvascular spasm, inflammation and autonomic nervous dysfunction) of coronary microvascular vessels as well as the extravascular factors (such as heart rate and blood pressure). In clinics, conventional western medicines are usually used for empirical treatment, but with undesirable effects. Traditional Chinese medicine (TCM) believes that CMVD belongs to the category of "chest impediment", "heart pain" and "collateral disease", and the common syndromes include Qi deficiency and blood stasis, Qi stagnation and blood stasis, Qi and Yin deficiency, congealing cold in heart vessel, heart and spleen deficiency, blood stasis obstructing collaterals, combined phlegm and blood stasis, and liver and kidney deficiency, with a variety of treatment methods. Specifically, Chinese patent medicines, self-designed prescriptions, modified classical prescriptions and TCM characteristic therapies have achieved certain effects. This review discussed the risk factors, pathological mechanism, TCM etiology and pathogenesis and traditional Chinese and Western medicine treatment of CMVD, to provide reference for the study and treatment of CMVD.

      摘要:Diabetic peripheral neuropathy (DPN) is one of the common complications of diabetes. The disease has a long course with nerve pain and other symptoms, seriously affecting the quality of life of patients. DPN is related to high glucose in vivo, inflammation, oxidative stress, apoptosis, and autophagy, involving phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), Janus kinase (JAK)/signal transducer and activator of transcription (STAT), nuclear factor-κB (NF-κB), mitogen-activated protein kinase (MAPK), and other signaling pathways. At present, the treatment of DPN mainly focuses on symptomatic treatments such as blood glucose control and neurotrophic therapy, but the effect is not ideal. Therefore, it is particularly important to select a reasonable and effective drug to prevent and treat DPN. In recent years, Chinese medicine has played an important role in the treatment of DPN. Many studies have explored the mechanism of Chinese medicine in the treatment of DPN, and it has been found that some Chinese medicine monomers and compounds can regulate signaling pathways to prevent and treat DPN. This paper reviewed the research results of signaling pathways involved in DPN and the regulation of related pathways by Chinese medicine, aiming to provide references for the clinical treatment of DPN.

      摘要:This article reviews the clinical studies about the treatment of allergic rhinitis (AR) with traditional Chinese medicine (TCM) in recent years. AR is a common and frequently occurring disease in the department of otolaryngology. The common manifestations of AR include nasal congestion, sneezing, runny nose, and nasal itching. AR, belonging to the category of rhinitis, is a dominant disease in the TCM treatment of otorhinolaryngologic diseases and head and neck diseases. The ancient literature has laid a solid theoretical basis on the etiology and pathogenesis of AR. According to the ancient literature, the theoretical basis, and their own experience, modern doctors classified the causes of AR into the disorders in Zang-fu organs and six meridians. Most of the explanations focus on the disorders in Zang-fu organs, especially the lung, spleen, and kidney. The clinical studies in this field mainly involve TCM treatment alone, integrated TCM and Western medicine treatment, and TCM external treatment. These therapies have good efficacy in the clinical treatment of AR. Among them, TCM treatment alone has significant advantages in alleviating the symptoms and nasal signs, declining the scores of related scales, and reducing the eosinophil count (EOS) of nasal secretions, with definite long-term efficacy. Integrated TCM and Western medicine treatment can complement with each other. The TCM external treatment methods include acupuncture, moxibustion, acupoint catgut embedding, acupoint application, and nasal irrigation with TCM, which are safe, simple, acceptable by patients and have good therapeutic effect. Finally, the clinical research status of TCM treatment of AR was summarized, and suggestions were put forward from three aspects: standardizing the clinical research protocol of TCM, encouraging pure TCM research, and exploring the mechanism of TCM treatment on the basis of frontier research achievements. This review aims to provide higher-level evidence for subsequent clinical research and promote the research on dominant AR diseases.

      摘要:Ulcerative colitis (UC) mainly occurs in the colon and rectum, with complex pathological mechanism. The occurrence of ulcerative colitis is associated with the uncontrollable inflammatory response of the intestine. The Western medicine therapy of UC mainly uses glucocorticoids and immunosuppressants to reduce intestinal inflammation. While blocking the progress of UC to a certain extent, it causes severe adverse reactions. More and more studies have confirmed that traditional Chinese medicine (TCM) has obvious advantages in the prevention and treatment of UC and can significantly reduce the recurrence of the disease. Pyroptosis, a novel form of cell death, can destroy cell structure, release intracellular pro-inflammatory substances, and mediate intestinal immune response in UC. TCM can promote pyroptosis (removing excess) or inhibit pyroptosis (replenishing deficiency), which is consistent with the regulation of Yin and Yang. TCM plays a role in the treatment of UC mainly by inhibiting pyroptosis (replenishing deficiency) and reducing intestinal immune response. In recent years, a large number of studies have been carried out to decipher the mechanism of TCM in the treatment of UC via NOD-like receptor protein domain 3 (NLRP3)-mediated pyroptosis pathway. The results have demonstrated that NLRP3 pathway is the key target of TCM in the treatment of UC. However, a comprehensive summary remains to be carried out on the inhibition of NLRP3-mediated pyroptosis pathway by TCM in the treatment of UC. Therefore, we retrieved the articles in this field in recent years with the keywords "pyroptosis", "NLRP3", "ulcerative colitis", and "Chinese medicine". The Chinese medicines regulating NLRP3 pathway mainly have the functions of clearing heat and drying dampness, harmonizing Qi and blood, moving Qi and dredging fu-organs, and invigorating spleen and removing dampness. The findings can help researchers to fully understand the mechanism of TCM in the treatment of UC via the NLRP3 pathway and provide a theoretical basis for the treatment of UC and further drug development.
