摘要

目的

观察参苓白术散治疗脾虚型小儿泄泻对胃电图的影响，明确该病患儿胃电节律紊乱的发生情况，并研究参苓白术散是否具有改善小儿泄泻（脾虚型）胃动力异常的作用。

方法

选取2019年10月至2021年3月于上海市儿童医院门诊就诊的脾虚型泄泻患儿共125例，随机分为两组，其中观察组65名，对照组60名。对照组患儿给予常规的蒙脱石散联合益生菌治疗，观察组患儿在对照组治疗基础上加用参苓白术散口服，两组疗程均为1周。将两组患儿治疗后的临床疗效及治疗前后主要中医症状积分进行比较；并将两组患儿治疗前后胃电图主要参数的变化进行比较。

结果

研究结果显示治疗后观察组总有效率为90.77％（59/65），明显高于对照组的76.67％（46/60），差异有统计学意义（

0.05）。治疗后，与本组治疗前比较，两组的粪便形态、排便频率、精神状态、食欲状态等症状积分均明显降低（

0.05）；与对照组治疗后比较，观察组积分均明显降低（

0.05）。与本组治疗前比较，治疗后两组患儿的胃电主频、正常慢波比例和胃电正常节律比例明显升高（

0.05）；与对照组治疗后比较，观察组胃电主频、正常慢波比例和胃电正常节律比例明显升高（

0.05）。与本组治疗前比较，胃动过缓患儿所占的比例明显降低（

0.05），与对照组治疗后比较，观察组胃动过缓患儿所占比例明显降低（

0.05）。

结论

参苓白术散治疗小儿泄泻（脾虚型）可明显缓解患儿腹泻症状，并可显著改善患儿胃电节律紊乱情况，有助于恢复小儿胃动力，其临床疗效良好。

Abstract

Objective

To study the effect of Shenling Baizhusan on electrogastrogram in children with spleen deficiency diarrhea. To clarify the occurrence of gastric electrical rhythm disorder in children with this disease， and to study whether Shenling Baizhusan can improve the abnormal gastric motility in children with diarrhea （spleen deficiency）

Method

A total of 125 children with spleen deficiency diarrhea in the outpatient department of Children's Hospital of Shanghai from October 2019 to March 2021 were selected as the research objects， and they were randomly divided into a control group （60 cases） and an observation group （65 cases）. The children in the control group were treated with Montmorillonite powder combined with probiotics treatment， and the children in the observation group were additionally treated with Shenling Baizhusan. The course of treatment for both groups was 1 week. The clinical efficacy of the two groups of children after treatment and the scores of main traditional Chinese medcine（TCM） symptoms before and after treatment were compared， and the changes in the main parameters of electrogastrogram in children before and after treatment were compared.

Result

After treatment， the total effective rate of observation group （90.77%， 59/65） was higher than that of control group （76.67%，46/60）（

0.05）. After treatment， scores of fecal morphology， frequency of defecation， fatigue， inappetence， and other symptoms in both groups were lower than that before treatment （

0.05）， and the observation group was lower than the control group （

0.05）. As compared with before treatment， the main frequency， the percentage of normal slow wave， and the percentage of normal gastric electrical rhythm in the two groups increased after treatment （

0.05）， and the control group was lower than the observation group （

0.05）. The proportion of children with slow gastric rhythm decreased （

0.05） as compared with before treatment， and the control group was higher than the observation group （

0.05）.

Conclusion

Shenling Baizhusan can significantly relieve the diarrhea symptoms in children with spleen deficiency diarrhea and improve gastric motility with good clinical effects.
