يهدف هذا البحث إلى تحليل دور مرهم YXDW في تحسين وظيفة القلب للفئران بعد الإصابة بنوبة قلبية وآليته. تم تقسيم الفئران إلى مجموعة جراحة وهمية ومجموعة نموذج ومجموعة أقراص الإنالابريل ومجموعة YXDW بجرعات منخفضة ومتوسطة وعالية. استخدمت عملية خياطة الفرع الأمامي الأيسر لتصنيع نموذج القلب الناجم عن نوبة قلبية للفئران. تم تغذية مجموعة YXDW بجرعات 0.27 و 0.54 و 1.08 غ/كغ/يوم، بينما تم تغذية مجموعة الإنالابريل بجرعة 3.60 مغ/كغ/يوم. تم التعامل مع مجموعة الجراحة الوهمية ومجموعة النموذج بتغذية بنسبة 0.5% من محلول كربوكسي ميثيل سيليلوز الصوديوم بالجرعة المعادلة. تم متابعة التأثيرات الناتجة عن YXDW على علامات الفئران ومؤشرات القلب وتغيرات وظيفة القلب المرئي باستخدام الصورة بالموجات فوق الصوتية، وتقييم التغيرات في شكل القلب باستخدام الفحص النسيجي، وتقدير المؤشرات الالتهابية في الخلايا بواسطة مقايسة امتصاص الإنزيم المرتبط باللواءات (ELISA) للإنترلوكين-1β (IL-1β)، والإنترلوكين-6 (IL-6)، والبروناتيوريتين (NT-proBNP)، وعامل نخر الورم-α (TNF-α)، بالإضافة إلى استخدام تقنية البوليميراز سلسلة الرنا في الوقت الحقيقي (Real-time PCR) وتقنية تحليل البروتين بالإنتقال العلمي (Western blot) للاختبار لتعبير البروتينات الناتجة عن قناة الأيون الانتقائي الكاثودي القائمة على الجهد 1 (VDAC1)، وبروتين 3 ذو النطاق الساخن المتشابه بالبروتين (NLRP3)، وجينات تخصص ناتجة عن تفكك الميتوكو.الناتجة عن مرض الخلية B-2 (Bcl-2)، وبروتين الدم كذاب الخلية B-2 ذو الصلة (Bax). وجاءت نتائج هذا البحث بتحسين حالة البقاء لدى جميع مجموعات الفئران المعالجة. كما أظهرت نتائج الصورة بالموجات فوق الصوتية أن نسبة الانقباض الانبعاثي الأيسر في قلب الفئران في مجموعة النموذج قد انخفضت بشكل ملحوظ مقارنة بمجموعة الجراحة الوهمية (LVEF)، ونسبة انقباض القلب الأيسر القطري (LVFS) (P <0.01)؛ كما أظهرت نتائج المقارنة أن نسبة LVEF و LVFS لدي جميع مجموعات العلاج قد زادت بشكل ملحوظ مقارنة بمجموعة النموذج (P <0.01). وأظهرت النتائج النسيجية أن الخلايا العضلية القلبية للفئران في مجموعات العلاج كانت أقل تغيراً وكان الالتهاب فيها أقل، وكانت الألياف العضلية أكثر اكتمالًا، وكان الشكل أكثر انتظامًا، وكانت ألياف العضل القلبية أقل بشكل ملحوظ. أما النتائج الخلوية فأظهرت أن خلية العضل القلبية في جميع مجموعات YXDW كانت توجد بها ميتوكوندريا أكثر وضوحًا، وكانت الغشاء أكثر اكتمالًا، وكانت الأُوتان أكثر كثافةً، وكان المصقول أكثر وضوحًا، وكانت ألياف العضل القلبية و القطع الانتقالي أكثر ترتيباً. وأظهرت نتائج الإنزيم المرتبط باللواءات أن حاويات البروتين في مجموعة النموذج قد زادت بشكل ملحوظ مقارنة مع مجموعة الجراحة الوهمية (P <0.05)؛ وأظهرت النتائج أن حاويات البروتين لدي جميع مجموعات العلاج قد انخفضت بشكل ملحوظ مقارنة مع مجموعة النموذج (P <0.05). وبناء على هذه النتائج، فإن مرهم YXDW قادر على تحسين وظيفة القلب للفئران بعد الإصابة بنوبة قلبية بشكل واضح، وقد يكون آلية عمله عن طريق تحسين بنية وظيفة الميتوكوندريا من خلال مسار VDAC1/NLRP3/Bax/Bcl-2، وتثبيط الاستجابة الالتهابية، مما يؤدي إلى تحسين وظيفة القلب.
