Le but de cette étude est d'établir provisoirement des critères de diagnostic de l'insuffisance rénale vésicale ischémique pour fournir une base à la normalisation du diagnostic et du traitement de l'accident vasculaire cérébral ischémique. Les méthodes utilisées dans cette étude ont consisté en la recherche de la littérature pertinente sur les critères de diagnostic de l'insuffisance rénale vésicale ischémique, l'exploitation et l'extraction des données pour former un pool d'éléments. En fonction de ce pool d'éléments, un sondage a été effectué auprès d'experts sélectionnés par la méthode Delphi, comprenant deux tours de questionnaires pour procéder au premier filtrage des éléments et déterminer l'importance des symptômes. En utilisant une méthode d'enquête clinique prospective, des informations cliniques ont été collectées auprès des patients pour déterminer les principaux et les seconds diagnostics en combinant des méthodes d'analyse statistique et des techniques d'extraction de données. Avec les diagnostics principaux et secondaires déjà définis dans cette étude, un groupe d'experts a ensuite discuté et établi provisoirement des critères de diagnostic de l'insuffisance rénale vésicale ischémique. Un total de 25 critères de diagnostic pertinents ont été inclus, ils ont été ensuite fractionnés, normalisés et sélectionnés pour établir un pool d'éléments comprenant 48 éléments. La première enquête par questionnaire a été réalisée auprès de 30 experts. Le coefficient de concordance de Kendall était de 0,359, tous les experts ont atteint un taux de satisfaction de 100 % et le taux d'efficacité du questionnaire était de 100 %. Sur la base des critères de filtrage des éléments, cette enquête a déterminé que 26 éléments seront conservés pour l'étude. Un total de 176 cas cliniques ont été collectés lors de l'enquête d'informations cliniques, dont 94 cas d'insuffisance rénale vésicale et 82 cas non insuffisance rénale veineuse. Les résultats statistiques étaient les suivants : ① statistiques descriptives, les symptômes principaux avec une fréquence ≥ 30 % : fatigue, faiblesse, lombalgie, etc. ; les symptômes secondaires avec 10 % ≤ fréquence ≤ 30 % : fatigue, discours paresseux, essoufflement, etc. ② Analyse de régression logistique bivariée, les diagnostics principaux avec un rapport de cotes (OR) ≥ 3 : fatigue, faiblesse, lombalgie, etc. ; les diagnostics secondaires avec 1 ≤ OR ≤ 3 : fatigue, discours paresseux, essoufflement, etc. ③ Réseau de neurones artificiels, les diagnostics principaux avec 0,5 ≤ valeur de poids (W ij) < 1: fatigue, discours paresseux, etc., les diagnostics secondaires avec 0,3 ≤ Wij < 0,5 : essoufflement, parésie, etc. La deuxième enquête par questionnaire a été réalisée auprès de 37 experts. Le coefficient de concordance de Kendall était de 0,237, tous les experts ont atteint un taux de satisfaction de 100 % et le taux d'efficacité du questionnaire était de 100 %. Selon les critères de jugement des diagnostics principaux et secondaires par la méthode Delphi, cette enquête a inclus les diagnostics principaux suivants : fatigue, faiblesse, lombalgie, etc. ; et les diagnostics secondaires suivants : essoufflement, vertiges, acouphènes, perte d'appétit ou ballonnements après les repas, pâleur du visage, nycturie, pâleur de la langue, enduit blanchâtre et lisse de la langue. En conclusion, les critères de diagnostic provisoires de l'insuffisance rénale vésicale ischémique de cette étude fournissent une base normalisée et objective pour guider le diagnostic et le traitement clinique de l'accident vasculaire cérébral ischémique.
