Le but de cette étude était d'observer l'effet du sérum médicamenteux Tongluo Baoshen (TLBS) sur les cellules podocytaires de rats induites par lipopolysaccharide (LPS) et d'explorer son mécanisme d'action potentiel. Méthode : Préparation de sérums blancs SD de rats vides, de sérums de valsartan de potassium et de sérums médicamenteux TLBS à doses faible, moyenne et élevée ; culture in vitro de cellules podocytaires de rats, détection de la prolifération et de l'activité cellulaire (méthode CKK-8) pour évaluer l'effet des sérums médicamenteux sur l'activité cellulaire des podocytes et déterminer le meilleur taux d'intervention du sérum médicamenteux et la meilleure concentration de LPS pour induire des lésions podocytaires ; les cellules ont été réparties en un groupe normal (groupe NC, 10 % de sérum blanc), un groupe modèle (groupe MC, 20,00 mg·L-1 de LPS+10 % de sérum blanc), un groupe valsartan (groupe LP, 20,00 mg·L-1 de LPS+10 % de sérum de valsartan de potassium), un groupe à faible dose de TLBS (groupe TLBS-L, 20,00 mg·L-1 de LPS+10 % de sérum à faible dose de TLBS), un groupe à dose moyenne de TLBS (groupe TLBS-M, 20,00 mg·L-1 de LPS+10 % de sérum à dose moyenne de TLBS) et un groupe à dose élevée de TLBS (groupe TLBS-H, 20,00 mg·L-1 de LPS+10 % de sérum à dose élevée de TLBS) pour une intervention de 48 h. Observation de l'ultrastructure des podocytes au microscope électronique ; détection de l'apoptose des podocytes avec le kit de réactifs TUNEL ; détection de l'expression fluorescente du domaine N-terminal de Gasdermin D (GSDMD-NT) des podocytes par immunofluorescence ; détection de l'expression des ARNm et des protéines des récepteurs couplés aux protéines G de type 5B (GPRC5B), du facteur de transcription nucléaire-κB (NF-κB) p50, NF-κB p52, NF-κB p65, Rel B, c-Rel, du récepteur nucléotide-oligomérisation, leucine riche contenant le motif pyrin domaine 3 (NLRP3), de la caspase-1, de l'expression fluorescente de GSDMD-NT, de l' interleukine (IL)-1β, de l'IL-18, du néphrine, de l'intégrine α3, de l'intégrine β1, des ARNm et de l'expression protéique par PCR quantitative en temps réel et par immunobuvardage. Résultat : Comparé au groupe NC, le nombre de podocytes dans le groupe MC présentait une diminution significative, avec des segments membranaires manquants, moins d'organites, davantage de mitochondries et d'œdèmes, une membrane nucléaire irrégulière et une chromatine pâle ; le nombre de noyaux de podocytes positifs à la coloration TUNEL augmentait de manière significative (P<0,01) ; l'expression fluorescente de GSDMD-NT augmentait de manière significative ; les ARNm et l'expression protéique de GPRC5B, NF-κB p50, NF-κB p52, NF-κB p65, Rel B, c-Rel, NLRP3, Caspase-1, GSDMD-NT, IL-1β, IL-18 augmentaient de manière significative (P<0,01), alors que l'expression des ARNm et des protéines de néphrine, d'intégrine α3, d'intégrine β1 diminuait de manière significative (P<0,05, P<0,01). Comparé au groupe MC, les groupes LP, TLBS-M et TLBS-H montraient une amélioration de l'ultrastructure des podocytes dans différentes mesures, une augmentation des segments membranaires, une membrane cellulaire intacte, moins d'organites, un œdème mitochondrial réduit ; le nombre de noyaux de podocytes positifs à la coloration TUNEL diminuait de manière significative (P<0,01) ; la fluorescence GSDMD-NT diminuait dans différentes mesures ; l'expression des ARNm et des protéines de GPRC5B, NF-κB p50, NF-κB p52, NF-κB p65, Rel B, c-Rel, NLRP3, Caspase-1, GSDMD-NT, IL-1β, IL-18 diminuait de manière significative (P<0,01), tandis que l'expression des ARNm et des protéines de néphrine, d'intégrine α3, d'intégrine β1 augmentait de manière significative (P<0,05, P<0,01), et ces indicateurs présentaient les changements les plus significatifs dans le groupe TLBS-H. Conclusion : Le sérum médicamenteux TLBS peut améliorer les lésions podocytaires induites par le LPS, et son mécanisme d'action pourrait être lié à la régulation de la voie GPRC5B/NF-κB/NLRP3 pour inhiber l'apoptose cellulaire.
