Objectif Observer l'effet thérapeutique des comprimés de serpent de saison sur le virus de la varicelle-zona (VZV) et la douleur nerveuse post-herpétique (PHN) induite par celui-ci chez le cobaye, afin de fournir des bases expérimentales pour l'application clinique et le développement de ce médicament. Méthodes: Cinquante-six cobayes ont été répartis aléatoirement en 7 groupes selon leur poids corporel, à savoir un groupe normal, un groupe de modèles, un groupe d'aciclovir, et des groupes à doses élevées, moyennes et faibles de comprimés de serpent de saison (1,92, 0,96, 0,48 g.j -1), et un groupe de traitement par voie orale de comprimés de serpent de saison + de médicaments par voie topique (0,96 g.j -1+1,2 g.kg -1.j -1). Après le déplumage de la peau dorsale des cobayes à l'exception du groupe normal, 200 μL de liquide contenant le virus VZV ont été gouttés sur la zone lésée pendant 2 jours consécutifs. La médication a débuté 2 heures après l'infection, et a été administrée pendant 7 jours consécutifs. À partir du deuxième jour après l'infection, l'évolution quotidienne des lésions dorsales de chaque groupe de cobayes a été observée. Au 7ème jour, les cobayes ont été sacrifiés par anesthésie au CO2, et des échantillons de peau localement infectée ont été collectés. La quantité d'acide désoxyribonucléique (ADN) viral a été mesurée par réaction en chaîne par polymérase quantitative en temps réel (PCR en temps réel), et des examens histologiques ont été effectués après coloration à l'hématoxyline-éosine (HE). Soixante-dix rats ont été répartis aléatoirement en 7 groupes selon leur poids corporel, à savoir un groupe normal, un groupe de modèles, un groupe d'aciclovir, et des groupes à doses élevées, moyennes et faibles de comprimés de serpent de saison (1,08, 0,54, 0,27 g.j -1), et un groupe de traitement par voie orale de comprimés de serpent de saison + de médicaments par voie topique (0,54 g.j -1+1,2 g.kg -1.j -1). Tous les groupes de rats (à l'exception du groupe normal) ont été inoculés par voie sous-cutanée avec 50 μL de suspension virale de VZV entre les premier et deuxième doigts du membre antérieur gauche. La peau dorsale des rats a été dépoilée, la médication a commencé 2 heures après l'infection, et a été administrée pendant 10 jours consécutifs. Les seuils de douleur mécanique des rats ont été mesurés avec des fibres de Von Frey à différentes périodes avant et après l'inoculation, et les périodes latentes de réflexe de contraction thermique des pattes ont été mesurées avec une plaque chauffante et froide. Après des examens comportementaux effectués 10 jours après l'inoculation, les rats ont été anesthésiés, la ganglion de la racine dorsale de la moelle épinière et les segments médullaires ont été prélevés, et le contenu en substance P (SP), en neurokinine (NK), en facteur de nécrose tumorale-α (TNF-α), et en interleukine-1β (IL-1β) a été mesuré par dosage immunoenzymatique (ELISA). Résultats Comparativement au groupe normal, les lésions cutanées des cobayes du groupe de modèles étaient significatives (P<0,01) ; la charge virale était élevée (P<0,01) ; la structure cellulaire sous-cutanée était perturbée, et l'infiltration des cellules inflammatoires ainsi que la nécrose cellulaire étaient significatives (P<0,01) ; les seuils de douleur mécanique et les périodes latentes de réflexe de contraction thermique des pattes des rats étaient nettement réduits (P<0,05) ; et la teneur en NK, SP, TNF-α, et IL-1β était fortement élevée (P<0,01). Comparativement au groupe de modèles, les groupes à doses élevées et moyennes de médicament par voie topique de comprimés de serpent de saison, ainsi que le groupe de traitement par voie orale de comprimé de serpent de saison + de médicaments par voie topique pouvaient nettement améliorer les rougeurs, les lésions cutanées des cobayes provoquées par l'infection à VZV (P<0,01) ; réduire la charge virale dans les tissus cutanés (P<0,01) ; diminuer le degré d'infiltration des cellules inflammatoires ainsi que la nécrose cellulaire dans les tissus cutanés (P<0,05) ; nettement augmenter les seuils de douleur mécanique et les périodes latentes de réflexe de contraction thermique des pattes (P<0,05) ; et réduire la teneur en NK, SP, TNF-α, et IL-1β dans la ganglion de la racine dorsale de la moelle épinière et les segments médullaires des rats (P<0,01). Conclusion Les comprimés de serpent de saison ont un effet thérapeutique significatif sur l'infection cutanée causée par VZV et la PHN. Cette étude fournit des médicaments candidates efficaces pour le traitement de l'infection par VZV.
