L'asthme et l'asthme variant par la toux (CVA) sont des maladies chroniques hétérogènes dont la base pathologique centrale est la perturbation de l'homéostasie du microenvironnement des voies aériennes. Ces dernières années, les microARN (miRNA), en tant que molécules clés de la régulation post-transcriptionnelle, régulent finement des voies de signalisation clés telles que Janus kinase/activateur de la transcription et du transducteur de signal (JAK/STAT), le facteur nucléaire de transcription-κB (NF-κB), le facteur de croissance transformant-β/Smad (TGF-β/Smad), et phosphatidylinositol 3-kinase/protéine kinase B (PI3K/Akt), ainsi que de multiples processus pathologiques tels que la réparation de la barrière épithéliale des voies aériennes, l'équilibre immunitaire des cellules T auxiliaires 1 (Th1)/Th2, la polarisation des macrophages M1/M2, la fonction des cellules musculaires lisses des voies aériennes et l'hyperréactivité des voies aériennes, jouant un rôle central dans le maintien ou la perturbation de l'homéostasie du microenvironnement des voies aériennes. Cette étude, basée sur les recherches nationales et internationales récentes, construit un cadre logique autour du thème « perturbation de l'homéostasie du microenvironnement des voies aériennes - régulation par miRNA - réparation du microenvironnement », visant à faire une revue systématique du rôle thérapeutique des miRNA dans l'asthme et le CVA du point de vue du microenvironnement des voies aériennes, à clarifier les mécanismes de régulation en cascade des miRNA tout au long de la maladie, à identifier les miRNA clés, ainsi qu'à discuter des progrès de la recherche sur les stratégies d'intervention de la médecine traditionnelle chinoise. L'étude analyse les recherches cliniques actuelles sur la thérapie par ARN et la médecine traditionnelle chinoise visant un traitement « multipoints et multipathways » via la régulation des miRNA, résume la valeur des miRNA comme biomarqueurs pour le diagnostic, la classification et l'évaluation pronostique, ainsi que le potentiel et les perspectives d'application des mimétiques/antagonistes de miRNA pour une thérapie ciblée, afin de promouvoir le développement de traitements précis pour l'asthme et le CVA.
