痛みはがん患者に最も一般的な症状の一つであり、患者の生活の質に深刻な影響を及ぼします。現在臨床で使用されている三段階の鎮痛法では、がん患者の痛みを完全に緩和できず、多くの問題を伴います。本論文は中医学の厥陰病の視点から、がん痛の核心病機は「正虚」と「寒熱錯雑」であるとし、烏梅丸を主方とし、証に応じて加減し、上を清め下を温め、寒熱を調和させ、身体の「虚・寒・瘀・毒・熱」の病理環境を調整し、身体の「陽化気」「陰成形」の生理機能を回復させ、がん痛を予防、緩和、あるいは消除し、良好な臨床効果を得ました。これはがん患者の痛みを和らげるだけでなく、「腫瘍をコントロール」し「消失」させることも可能にし、痛みの緩和と腫瘍の制御という二重の効果を実現し、中医学の証に基づく治療の特色と利点を発揮し、張仲景の厥陰病脈証並治の範囲を拡大し、虚実混在、寒熱錯雑の観点からがん性疼痛の診断・治療に臨床上の示唆を提供します。
