Астма и вариабельная астма с кашлем (CVA) являются хроническими гетерогенными заболеваниями, основой которых является нарушение гомеостаза микросреды дыхательных путей. В последние годы микроРНК (miRNA), как ключевые молекулы посттранскрипционного регулирования, посредством точной регуляции таких сигнальных путей, как Janus-киназа/фактор транксрипции и сигнализации (JAK/STAT), ядерный транскрипционный фактор-κВ (NF-κB), трансформирующий фактор роста-β/Smad (TGF-β/Smad), а также фосфатидилинозитол-3-киназа/протеинкиназа B (PI3K/Akt), а также множества патологических процессов, включая восстановление эпителиального барьера дыхательных путей, баланс иммунитета Т-хелперов 1 (Th1)/Th2, поляризацию макрофагов М1/М2, функции гладкомышечных клеток дыхательных путей и гиперреактивность дыхательных путей, играют ключевую роль в поддержании или нарушении гомеостаза микросреды дыхательных путей. Данное исследование основано на зарубежной и отечественной литературе последних лет и строит логическую схему вокруг концепции «нарушение гомеостаза микросреды дыхательных путей — регуляция miRNA — восстановление микросреды». Цель — систематический обзор терапевтических эффектов miRNA при астме и CVA с точки зрения микросреды дыхательных путей, разъяснение каскадных механизмов регуляции miRNA на протяжении всего заболевания, выявление ключевых miRNA, а также обсуждение прогресса исследований стратегий вмешательства традиционной китайской медицины. Анализируются клинические исследования, в которых RNA-терапия и китайская медицина достигают комплексного лечения «многотаргетно и многопутево» путем регулирования miRNA, а также суммируется ценность miRNA как биомаркеров для диагностики, типирования и прогноза, потенциал точной терапии с помощью миметиков/антагонистов miRNA и перспективы их применения, с целью продвижения прецизионной медицины при астме и CVA.
