El objetivo es explorar el efecto de la fórmula Wumei Wan en el proceso de metástasis hepática y pulmonar del cáncer colorrectal metastásico, así como sus posibles mecanismos de acción. Métodos: los ratones se dividieron aleatoriamente en grupo control, grupos de baja y alta dosis de Wumei Wan (20, 40 g·kg⁻¹) y grupo de paclitaxel (0.01 g·kg⁻¹), luego se estableció un modelo de metástasis hepática y pulmonar de cáncer colorrectal en ratones. La intervención con medicamentos duró 4 semanas, registrando el peso corporal, peso del hígado y pulmón, y la supervivencia. Se usó tinción con hematoxilina y eosina (HE) para evaluar el efecto de Wumei Wan en las metástasis. La PCR en tiempo real evaluó los niveles de ARNm de marcadores de macrófagos M1 y M2 en tejidos hepáticos y pulmonares. La citometría de flujo midió los marcadores de macrófagos M1 CD80 y CD86. Resultados: en el modelo, comparado con el control, los grupos de Wumei Wan y paclitaxel mostraron disminución significativa en peso corporal y peso hepático/pulmonar (P<0.01) y aumento significativo en la supervivencia (P<0.01). La histopatología mostró inhibición de las metástasis. La PCR mostró aumento significativo de marcadores M1 (IL-1β, IL-6, TNF-α, iNOS, PTGS2) y disminución de marcadores M2 (Arg1, CD163, CD206) (P<0.01). La citometría mostró incremento en CD86 y CD80 (P<0.01). La inmunohistoquímica indicó aumento en expresión de CD86 e inhibición de CD206. Conclusión: Wumei Wan inhibe las metástasis hepáticas y pulmonares del cáncer colorrectal al promover la polarización de macrófagos M1 asociados al tumor.
