La necrosis avascular de la cabeza femoral inducida por hormonas (SANFH) es una enfermedad osteoarticular causada por el uso prolongado y en grandes cantidades de hormonas. Sus características patológicas típicas incluyen un trastorno progresivo en la circulación sanguínea del sistema de suministro de la cabeza femoral, lo que conduce a la apoptosis de osteocitos y la necrosis de la estructura ósea. A medida que avanza la enfermedad, se produce el colapso y necrosis estructural de la cabeza femoral, afectando gravemente la función de las extremidades y la calidad de vida diaria del paciente. Los glucocorticoides medían el daño patológico a través de un mecanismo dual: por un lado, inhiben la diferenciación de osteoblastos y activan la formación de osteoclastos, rompiendo el equilibrio dinámico entre la formación y absorción ósea; por otro lado, inducen trastornos del metabolismo lipídico, inhiben la angiogénesis y dañan la función de las células endoteliales, provocando trastornos en la microcirculación. La medicina tradicional china, especialmente la Epimedium y sus componentes activos, muestra efectos reguladores multidimensionales en la prevención y tratamiento de SANFH. La revisión de la literatura revela que contiene múltiples componentes activos, principalmente flavonoides totales, icariina, icariside, kaempferol e icariside II, que actúan a través de múltiples vías de señalización clave como Wnt/β-catenina, TGF-β/BMP/Smad, MAPK, PI3K/Akt, OPG/RANKL/RANK, desempeñando funciones reguladoras en la homeostasis del metabolismo óseo, la angiogénesis, la corrección de trastornos del metabolismo lipídico y la regulación de la autofagia celular. Este artículo explica sistemáticamente el mecanismo de intervención de los componentes activos de la Epimedium en las etapas patológicas clave de SANFH a través de estas vías, y analiza en profundidad la regulación de los nodos clave entre diferentes vías señalizadoras, con el fin de proporcionar una referencia para futuros tratamientos clínicos e investigaciones experimentales.
