Objetivo: explorar las diferencias en las características clínicas y el desenlace entre pacientes con la enfermedad de Wilson tipo hepático con síndrome de obstrucción mutua de flema y estasis sanguínea y síndrome de humedad-calor interno. Métodos: este estudio retrospectivo de cohorte incluyó de manera consecutiva a pacientes con enfermedad de Wilson tipo hepático del Centro de Enfermedades Cerebrales del Primer Hospital Afiliado a la Universidad de Medicina China de Anhui, desde enero de 2022 hasta agosto de 2025. Se dividieron en grupo con síndrome de obstrucción mutua de flema y estasis sanguínea y grupo con síndrome de humedad-calor interno según el diagnóstico de medicina tradicional china. Todos los pacientes recibieron tratamiento estándar, y se registraron datos basal, indicadores de laboratorio, complicaciones, además de las puntuaciones del modelo MELD, Child-Turcotte-Pugh (CTP) y evaluación CLIF-SOFA. Se utilizaron pruebas t, U y regresión logística multivariada para comparar las características clínicas y el desenlace entre los grupos. Resultados: se incluyeron 141 pacientes con enfermedad de Wilson tipo hepático, 68 en el grupo de síndrome de obstrucción mutua de flema y estasis sanguínea, con edad media de (28.22±10.47) años, 43 hombres y 25 mujeres; y 73 en el grupo de síndrome de humedad-calor interno, con edad media de (30.22±8.79) años, 44 hombres y 29 mujeres. El análisis univariado mostró que no hubo diferencias estadísticamente significativas en edad y sexo entre ambos grupos. Comparado con el grupo de síndrome de humedad-calor interno, el grupo de síndrome de obstrucción mutua de flema y estasis sanguínea mostró niveles significativamente más bajos de plaquetas (PLT), alanina aminotransferasa (ALT), aspartato aminotransferasa (AST), creatinina (CRE), colesterol total (TC) y triglicéridos (TG) (P<0.05, P<0.01), y niveles significativamente más altos de bilirrubina total (TBIL) y tiempo de protrombina (PT) (P<0.05). No hubo diferencias estadísticamente significativas en la incidencia de encefalopatía hepática, infecciones, peritonitis bacteriana espontánea, ascitis ni hemorragia gastrointestinal entre los grupos. En comparación con el grupo de síndrome de humedad-calor interno, el grupo de síndrome de obstrucción mutua de flema y estasis sanguínea mostró una incidencia significativamente mayor de esplenomegalia (P<0.05), puntuaciones MELD significativamente más altas (P<0.01), y una tendencia al aumento de las puntuaciones CTP y CLIF-SOFA sin significancia estadística. Dentro de los grupos, 11 pacientes del síndrome de obstrucción mutua de flema y estasis sanguínea y 9 del síndrome de humedad-calor interno presentaron insuficiencia hepática. Los resultados de la regresión logística multivariada indicaron que PT [odds ratio (OR) = 1.794, intervalo de confianza 95% (95%CI) 1.249~2.576], TBIL (OR=1.111, 95%CI 1.026~1.203), ALT (OR=1.053, 95%CI 1.004~1.105) y tipo de síndrome de medicina tradicional china (OR=5.420, 95%CI 1.384~21.227) son factores de riesgo independientes para insuficiencia hepática concurrente en enfermedad de Wilson tipo hepático. Conclusión: en comparación con el síndrome de humedad-calor interno, los pacientes con enfermedad de Wilson tipo hepático y síndrome de obstrucción mutua de flema y estasis sanguínea presentaron daño hepático más grave y enfermedad más severa, y el tipo de síndrome de medicina tradicional china es un factor predictor independiente de insuficiencia hepática en estos pacientes.
