El cáncer de pulmón, especialmente el cáncer de pulmón no microcítico, es el tumor maligno con la incidencia y mortalidad más altas en China y en todo el mundo; en 2022, el número de muertes globales alcanzó 1,8 millones, representando el 18,7 % del total de muertes por cáncer, lo que representa una grave amenaza para la salud y la vida humanas, y una situación difícil en cuanto a prevención y tratamiento. Aunque las estrategias de tratamiento del cáncer de pulmón se han enriquecido en los últimos años y se han logrado avances en las terapias dirigidas e inmunitarias, los beneficios a largo plazo para los pacientes siguen siendo limitados debido a la resistencia primaria o adquirida, la baja respuesta inmunitaria y la toxicidad de la quimioterapia, lo que requiere explorar estrategias auxiliares seguras y efectivas. La medicina tradicional china, gracias a su regulación integral y diagnóstico individualizado, está ganando terreno en el tratamiento integral de tumores. La medicina tradicional china considera que "la insuficiencia de yang conduce a la acumulación" es la clave en la fisiopatología tumoral; basado en la teoría de que "el yang transforma el qi, el yin forma la materia", la deficiencia de yang causa fallo en el calentamiento, acumulación interna de frío, relacionada estrechamente con el desequilibrio del microambiente inmunológico, las características del "tumor frío" y la disfunción del sistema neuroendocrino en la medicina moderna. Por lo tanto, la estrategia de "calentar yang y fortalecer la defensa para combatir el cáncer" recibe cada vez más atención. En los últimos años, hierbas calientes para yang comúnmente usadas como Aconitum, jengibre seco, canela, epimedium, psoraleae y sus componentes activos han logrado avances significativos en la investigación anti-cáncer de pulmón mediante la regulación de múltiples vías de señalización, inducción de apoptosis, inhibición de la metástasis y reversión de la resistencia. Además, fórmulas para calentar yang como Sini Tang y Yanghe Tang han mostrado buenos prospectos para aliviar la supresión de la médula ósea, mejorar la fatiga relacionada con el cáncer, el derrame pleural maligno y el dolor por cáncer, con efectos de reducir la toxicidad y aumentar la eficacia, mejorando significativamente la calidad de vida y beneficios de supervivencia de los pacientes. Para un resumen sistemático sobre el papel y los mecanismos de las hierbas calientes para yang y sus fórmulas en el cáncer de pulmón, este artículo realiza por primera vez una revisión profunda de las investigaciones recientes con el objetivo de proporcionar ideas para la práctica clínica de la medicina tradicional china en la prevención y tratamiento del cáncer de pulmón.
