摘要

目的 :探讨清胰颗粒对重症急性胰腺炎(SAP)肺损伤的防治作用及其机制。方法 :SAP患者58例分为中西医结合治疗组(n=32)和常规西医治疗组(n=26)。分别观察两组患者腹痛缓解时间、肠鸣恢复时间、低氧血症纠正时间

治疗前及治疗后72 h后血中内毒素(ET)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)的水平变化

并再次行上腹部螺旋CT扫描评估胰腺炎症进展情况。结果: 中西医结合治疗组与常规西医治疗组低氧血症的发生率分别为43.75％

76.92% (P<0.05);ARDS发生率分别为9.38％

34.62% (P<0.05);治疗72 h后中西医结合治疗组血比常规西医治疗组血中ET

TNF-α

IL-6水平明显下降(P<0.05);中西医结合治疗组腹痛缓解时间、肠鸣恢复时间、低氧血症纠正时间、CT扫描显示胰腺炎程度进展的比例明显低于常规西医治疗组(P<0.05)。结论: 清胰颗粒通过保护肠屏障

减少了细菌移位

降低血清中内毒素、TNF-α

IL-6等炎症介质水平

纠正机体致炎和抗炎系统失衡等多方面机制而对肺脏起到全面保护作用。

Abstract

Objective: To explore the preventive and therapeutic effects of Qingyi granule on the lung injury induced by severe acute pancreatitis(SAP). Method: SAP consists of 58 patients with combination therapy group (n=32) and conventional western medicine treatment group (n= 26).The two groups were observed in patients with abdominal pain and ease the time
