摘要

目的：探讨解郁化浊汤对2型糖尿病前期（气滞痰阻证）的临床疗效。方法：将111例患者随机按数字法分为对照组55例和观察组56例。两组均采用基础干预，包括控制饮食、健康教育、运动等一般治疗，观察组加服解郁化浊汤，1剂/d，疗程均为12周。检测治疗前后空腹血糖（FBG）、餐后2 h血糖（2 hPBG）、空腹胰岛素（FINS）、餐后2 h胰岛素（2 hINS）糖化血红蛋白（HbA1c）、血脂（TC总胆固醇、TG甘油三酯、HDL-C高密度脂蛋白、LDL-C低密度脂蛋白），测量患者体重、腰围和臀围及记录中医证候积分，并随访患者的转归。结果：治疗后观察组15例（26.78%）糖代谢正常，优于对照组的6例（10.91%）（P<0.05），观察组为7例（12.5%）转为T2 DM，少于对照组的17例（30.91%）（P<0.05）；对照组治疗后FBG，2 hPBG，HbA1c，FINS，2 hINS均呈上升趋势，观察组以上指标则明显下降（P<0.05，P<0.01），观察组FBG，2 hPBG，HbA1c、FINS，2 h INS均低于对照组（P<0.05，P<0.01）；观察组BMI、体重、腰围及WHR改善均明显优于对照组（P<0.05）；观察组TG，HDL-C均较治疗前下降（P<0.05）；观察组形体肥胖、脘腹胀闷、心烦、大便干结及口苦等症状评分均低于对照组（P<0.01）。结论：在基础干预的基础上，解郁化浊汤能改善胰岛素抵抗，调节糖脂代谢，改善患者临床症状、体征，对防止糖尿病前期患者转化为T2 DM有一定的疗效。

Abstract

Objective: To discuss the clinical efficacy of Jieyu Huazhuo decoction on treating type 2 prediabetes (qi stagnation and phlegm obstruction type). Method: One hundred and eleven patients with chronic pyelonephritis were randomly divided into observation group (n=56) and control group (n=55). Patients in the two groups were both treated with basis interventions

including diet

health

education and sports. Jieyu Huazhuo decoction was used in the observation group (one dose per day
