摘要

目的：探讨利湿通络方治疗急性痛风性关节炎（AGA）及高尿酸血症的临床疗效及对白细胞介素-1β（IL-1β）和白细胞介素-8（IL-8）的影响。方法：将92例患者随机按数字法分为对照组和观察组各46例，对照组给予塞来昔布胶囊，0.2 g/次，1次/d，口服，苯溴马隆片，50 mg/次，1次/d，早餐后服用；观察组在对照组基础上加用利湿通络方，1剂/d，常规水煎分2次服用。两组疗程均为4周。记录治疗前后关节疼痛、压痛、关节肿胀及关节活动受限评分；记录急性期症状缓解所用的时间及随访3个内复发率；检测治疗前后血尿酸（UA），血沉（ESR），IL-1β和IL-8水平。结果：观察组总有效率93.47%，优于对照组的76.08%（P<0.05）；治疗后观察组关节疼痛、压痛、关节肿胀及关节活动受限方面评分均低于对照组（P<0.01）；观察组急性期症状缓解时间低于对照组（P<0.05）；随访3个月，观察组复发率为8.69%，低于对照组的28.26%（P<0.05）；观察组ESR，UA水平低于对照组（P<0.01）；观察组血清IL-1β和IL-8水平低于对照组（P<0.01）。结论：利湿通络方能快速减轻急性痛风性关节炎（AGA）患者的临床症状，提高临床疗效，并能降低复发率，还能促进患者尿酸的排泄、减轻炎症损伤，值得临床推广使用。

Abstract

Objective: To discuss the effect on interleukin-1β(IL-1β) and interleukin-8(IL-8) and the clinical efficacy of Lishi Tongluo decoction on treating acute gouty arthritis(AGA) and hyperuricaemia. Method: Ninety two patients were randomly divided into observation group (n=46) and control group (n=46). The control group were given celecoxib capsules

one times/d. Lishi Tongluo decoction was given to the patients in observation group

tenderness and score of joint swelling and joint mobility were recorded before and after treatment. Times of symptom relief in acute phase and recurrence rate in three months were also recorded. Serum uric acid (UA)
