摘要

目的：探讨三七脂肝丸治疗肝功能异常非酒精性脂肪肝（NAFLD）的临床疗效及作用机制。方法：将116例患者随机按数字法分为对照组57例和观察组59例。两组均参照“非酒精性脂肪性肝病诊疗指南”进行非药物干预，对照组服用多烯磷脂酰胆碱胶囊，2粒/次，3次/d，口服，还原型谷胱甘肽片200 mg/次，3次/d，口服。观察组服用三七脂肝丸，5 g/次，3次/d，口服。两组疗程均为12周。监测丙氨酸转氨酶（ALT）、天冬氨酸转氨酶（AST）、甘油三酯（TG）、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）及低密度脂蛋白胆固醇（LDL-C）；治疗前后肝脾B超检查；测定血清超氧化物歧化酶（SOD）、肿瘤坏死因子-α（TNF-α）和白细胞介素-18（IL-18）水平。结果：治疗后疗效观察组总有效率93.22%优于对照组的77.19%（P<0.05）；治疗后两组ALT，AST较治疗前明显好转（P<0.01），两组ALT，AST复常率相当；观察组TC，TG，LDL-C和HDL-C复常率优于对照组；两组治疗后B超检查脂肪肝程度均有好转，观察组的好转程度优于对照组（P<0.05）；治疗后两组患者血清SOD均较治疗前升高，治疗后观察组高于对照组（P<0.05）；治疗后两组血清TNF-α和IL-18水平均治疗前下降，观察组低于对照组（P<0.05）。结论：三七脂肝丸对肝功能异常NAFLD患者的肝功能有保护作用，能促使患者血脂好转，其作用机制可能与升高患者血清SOD水平，降低TNF-α和IL-18水平与有关。

Abstract

Objective: The discuss clinical effects and mode of Sanqi Zhigan pill on non-alcoholic fatty liver disease(NAFLD) of 59 cases abnormal liver function patients. Method: One hundred and sixteen cases of patients were randomly divided into control group (57 cases) and observation group (59 cases) by digital method. The both groups were meddled with non-drug referred to clinical practice guideline of non-alcoholic fatty liver disease.The patients in control group took polyene phosphatidyl choline capsule

orally.Reduced glutathione tablets

3 times/day

orally.For patients in observation group

they took Sanqi Zhigan pill

5 g/time

3 times/day

orally.The treatment course was 12 weeks for both groups.Monitor alanine aminotransferase (ALT)

glutamic oxalacetic transaminase(AST)

triglyceride(TG)

total cholesterol(TC)

high density lipoprotein cholesterol(HDL-C) and low density lipoprotein cholesterin (LDL-C).Examine liver and spleen B ultrasound before and after treatment.Measure the level of serum superoxide dismutase (SOD)
