摘要

目的：观察三才封髓丹加导赤散加减治疗复发性口腔溃疡（RAU）的临床疗效及对T细胞亚群及炎性因子的影响。方法：将105例患者随机按数字法分为对照组52例和观察组53例。对照组采用甘草锌颗粒，5 g/次，3次/d，口服；维生素B2片，5 mg/次，3次/d，口服，疗程8周；维生素C片，0.2 g/次，3次/d，疗程4周；冰硼散适量涂敷溃疡面，3~6次/d，连续使用5 d。观察组在对照组治疗的基础上加用三才封髓丹加导赤散加减内服，1剂/d，常规水煎服，疗程8周。近期疗效以溃疡面积、充血程度及疼痛为主要观察指标；观察3，6个月复发率；检测治疗前后外周血T淋巴细胞亚群（CD3+，CD4+，CD8+，CD4+/CD8+）；检测治疗前后白细胞介素-2（IL-2）、白细胞介素-10（IL-10）和肿瘤坏死因子-α（TNF-α）水平。结果：经Ridit分析，观察组疗效优于对照组（P<0.05）；治疗后第5天，观察组主要症状、体征总积分低于对照组，溃疡面积和充血面积小于对照组（P<0.01）；治疗后3个月两组复发率比较差异无统计学意义；治疗后6个月对照组复发率达55.8%，高于观察组的26.4%（P<0.01）；治疗后观察组CD3+，CD4+和CD4+/CD8+高于对照组，CD8+低于对照组（P<0.05或P<0.01）；观察组血清IL-2高于对照组（P<0.01）；观察组IL-10及TNF-α低于对照组（P<0.01）。结论：在西医常规治疗的基础上，三才封髓丹加导赤散加减内治疗复发性口腔溃疡快速减轻临床症状，降低复发率，近期和远期疗效显著，其作用机制可能与调节T淋巴细胞亚群及相关炎性因子水平，改善失衡的免疫功能有关。

Abstract

Objective: The purpose is to observe the clinical effects of Sancai Fengsui dan coordinated with Daochi san to treat recurrent oral ulceration(RAU) and influence of T cell subset

inflammatory factor. Method: One hundred and five cases of patients with were randomly divided into control group (52 cases) and observation group (53 cases) by digital method.For patients in control group

they took licorzine granules

5 g/time

3 times/day

taken orally.Treatment course were 8 weeks.Besides

they took Vitamin B2 piece

5 mg/time

3 times/day

taken orally

continuing 8 weeks.Vitamin C tablets

0.2 g/time

3 times/day

continuing 4 weeks.Coat ulcer surface with Bingpeng san

3-6 times/day

continuing 5 days.Based on control group

patients in observation group took Sancai Fengsui Dan and Daochi san by dialectical add and subtract treatment

it showed no statistically significant of the recurrence rate among the two groups

After 6 mouths of the treatment

the recurrence rate of the control group 55.8% was higher than 26.4% of the observation group(P<0.01)

CD3+

CD4+ and CD4+/CD8+ of the observation group were higher than the control group.The observation group was lower than the CD8+ of control group(P<0.05 or P<0.01)

Serum interleukin-2 of the observation group was higher than the control group(P<0.01)

IL-10 and TNF-α of the observation group were lower than the control group(P<0.01). Conclusion: On the base of western medical routine therapy

it can relief the clinical symptoms quickly and reduce the recurrence rate when Sancai Fengsui dan which combined with Daochi san was use for curing recurrent aphthous stomatitis

besides

there was a obvious curative effect in short-term and long-term

the mechanism of action may be related to regulating the T lymphocyte subpopulation and the level of relative inflammatory factors

and improving the unbalanced immune function.
