摘要

目的：观察降糖保心丸对2型糖尿病（T2DM）合并不稳定型心绞痛（UAP）患者炎症因子及血浆内皮素的影响。方法：选择78例2型T2DM合并UAP患者随机分为两组：对照组40例给予常规治疗；观察组38例在对照组治疗的基础上加用降糖保心丸，6 g/次，3次/d。两组患者均持续治疗6个月。同时以30 例健康体检人员作为健康组。检测治疗前后血脂、炎症、血管内皮功能指标，观察两组患者治疗6个月后的临床疗效，随访6个月内心血管事件。结果：治疗6个月后，观察组的总有效率显著高于对照组（P<0.05）；观察组患者的白细胞介素-6（IL-6）、肿瘤坏死因子（TNF-α）、基质金属蛋白酶-9（MMP-9）、超敏C反应蛋白（hs-CRP）水平均显著低于对照组（均P<0.01）；观察组患者的低密度脂蛋白胆固醇（LDL-C）、甘油三酯（TG）、一氧化氮（NO）、内皮素-1（ET-1）的改善均显著优于对照组（均P<0.01）；6个月内观察组患者的心血管事件发生率显著低于对照组（P<0.05），6个月内观察组中未发现与降糖保心丸有关的不良反应。结论：降糖保心丸治疗T2DM合并UAP安全有效，拮抗炎症反应，改善血管内皮功能可能为其作用机制之一。

Abstract

Objective: This study aimed to observe the effect of Jiangtang Baoxin pill on the inflammatory factors and endothelial function in patients with type 2 diabetes(T2DM)and unstable angina pectoris(UAP). Method: Seventy-eight patients with T2DM and UAP were randomly divided into two groups:the control group (40 cases) was given conventional therapy;the observation group(38 cases) was given Jiangtang Baoxin pill(6 grams once

three times daily)in addition to conventional treatment. These patients of two groups were treated for 6 months. At the same time

30 healthy people were selected as the healthy control group. Lipids

inflammation and endothelial function indicators were detected before and after treatment

clinical efficacy in patients of two groups were observed before and after 6 months of treatment

cardiovascular events were followed up for 6 months. Result: After 6 months of treatment

the total efficacy of the observation group was significantly higher than that of the control group(P<0.05);the levels ofinterleukin-6(IL-6)

tumor necrosis factor-α(TNF-α)

matrix metalloproteinase 9(MMP-9)

high-sensitivity C-reactive protein(hs-CRP)of patients in the observation group were significantly lower than those in the control group(all P<0.01);the improvement of low-density lipoprotein-cholesterol(LDL-C)

triacylglycerol(TG)

nitric oxide(NO)

endothelin-1(ET-1) in the observation group were significantly better than those of the control group(all P<0.01);Incidence of cardiovascular events of the observation group was significantly lower than that of the control group within 6 months(P<0.05). There was no adverse reaction associated with Jiangtang Baoxin pill in the observation group within 6 months. Conclusion: It was safe and effective that Jiangtang Baoxin pill can be used in the treatment of patients with T2DM and UAP

in which the antagonism of inflammatory reaction and improvement of endothelial function may be one of the mechanisms.
