摘要

目的：探讨抗敏煎内服及穴位敷贴治疗儿童过敏性鼻炎（AR）的疗效及对血清总免疫球蛋白E（IgE）水平和白细胞介素-4，-10（IL-4，IL-10）水平的影响。方法：将116例AR患者随机按数字表法分为对照组和联合观察组各58例，门诊体检的健康儿童15例作为健康对照组。对照组口服盐酸左西替利嗪片，5～10 mg/次，1次/d，疗程4周；布地奈德鼻喷雾剂，根据不同情况喷鼻，疗程10 d。联合观察组在对照组治疗的基础上采用抗敏煎内服，1剂/d及中药穴位敷贴，1次/周。两组疗程均为4周。进行治疗前后鼻炎的症状评分；记录治疗前1周及治疗后1周AR发作次数；进行治疗前后生活质量评价；检测治疗前后IgE，IL-4，IL-10水平。结果：联合观察组临床疗效总有效率100%，对照组86.2%，联合观察组优于对照组（P<0.05）；治疗后联合观察组鼻炎的症状评分、AR发作次数和生活质量评分均低于对照组（P<0.01）；两组AR患者治疗前后IgE和IL-4水平均高于健康对照组，IL-10水平低于健康对照组（P<0.01）；治疗后两组IgE，IL-4水平均比治疗前下降，IL-10水平升高（P<0.01）；治疗后联合观察组 IgE，IL-4水平低于对照组，IL-10水平高于对照组（P<0.01）。结论：在西医常规治疗AR的基础上，抗敏煎内服和中药穴位敷贴治疗AR近期疗效显著，其作用机制可能与下调IgE水平和调节Thl/Th2平衡有关。

Abstract

Objective: To investigate the effect of oral Kangmin decoction and acu-point application of traditional Chinese medicine on children with allergic rhinitis (AR)

and the influence on the levels of total immunoglobulinE (IgE)

interleukin-4 (IL-4)

interleukin-10 and (IL-10). Method: One hundred and sixteen patients with AR were divided into control group and observation group of 58 patients each

and 25 healthy subjects were assigned to a healthy control group. Patients in control group took oral 5-10 mg levocetirizine dihydrochloride tablets once daily for 4 weeks

and took budesonide nasal spray for 10 days when needed. Based on the treatment of control group

patients in observation group received oral Kangmin decoction one dose daily plus acu-point application of traditional Chinese medicine once weekly for 4 weeks.Rhinitis symptoms score was conducted before and after treatment

and attack rate of AR were recorded before and one week after treatment. Quality of life were assessed and the levels of total IgE

IL-4

IL-10 were detected before and after treatment. Result: The total clinical effective rate of 100% in observation group was better than 86.2% in control group (P<0.05). Rhinitis symptom score

AR attack rate and quality of life score after treatment in observation group were lower than those in control group (P<0.01). Blood levels of total IgE and IL-4 in patients with AR in two groups were higher

and IL-10 level was lower than those in healthy control group before and after treatment (P<0.01). The levels of total IgE

IL-10 increased after treatment

as compared with that before treatment (P<0.01). While levels of total IgE and IL-4 were lower

IL-10 level was higher in observation group than those in control group (P<0.01). Conclusion: Based on the conventional western medicine therapy

oral Kangmin decoction and acu-point application of traditional Chinese medicine have significant short-term curative effect in treating AR

which may be achieved by reducing level of IgE and regulating balance of Thl/Th2.
