摘要

目的:探讨健脑灵片联合复方银杏通脉口服液治疗血管性认知障碍(VCI)的临床疗效及对炎症反应和氧化损伤的影响。方法:将90例VCI患者随机按数字表法分为对照组和观察组各45例。对照组服用尼莫地平片

20 mg/次

1次/d;和盐酸多奈哌齐片

5 mg/次

1次/d。观察组在对照组治疗的基础上加服健脑灵片

4片/次

3次/d;和复方银杏通脉口服液

10 mL/次

3次/d。两组疗程均为12周。认知功能评定采用简易精神状态(MMSE)和蒙特利尔认知评估量表(MoCA)评定

生活质量采用日常生活功能(ADL)量表

中医证候采用血管性痴呆辨证量表(DSVD)

治疗前后各评价1次;检测治疗前后超氧化物歧化酶(SOD)

丙二醛(MDA)

C-反应蛋白(CRP)

肿瘤坏死因子α(TNF-α)和同型半胱氨酸(Hcy)水平。结果:经Ridit分析

治疗后观察组认知能力疗效优于对照组(P<0.05);治疗后观察组MoCA

MMSE和ADL评分均高于对照组

DSVD评分比对照组低(P<0.01);治疗后观察组MMSE量表定向力、注意和计算力和语言能力评分均高于对照组(P<0.01);治疗后观察组CRP

Hcy

TNF-α和MDA水平低于对照组

SOD高于对照组(P<0.01)。结论:在西医常规治疗的基础上

健脑灵片联合复方银杏通脉口服液治疗能改善血管性认知障碍患者的认识功能

减轻临床症状

提高生活能力

其作用机制可能与减轻炎症和氧自由基损伤有关。

Abstract

Objective: To explore the clinical efficacy of Jiannaoling tablets combined with compound Yinxing Tongmai oral liquid in treatment of vascular cognitive impairment(VCI) and its impact on inflammation response and oxidative damage. Method: Ninety patients with VCI were randomly divided into control group (45 cases) and observation group (45 cases) by number table method. Patients in control group orally took Nimodipine tablets

20 mg/time

1 time/day;donepezil hydrochloride tablets

5 mg/time

1 time/day. The patients in observation group also took Jiannaoling tablets

4 pieces/time

3 times/day and Yinxing Tongmai oral liquid

10 mL/time

3 times/day based on the treatment of control group. Treatment course was 12 weeks in both groups. Minimum mental state examination(MMSE) and Montreal cognitive assessment scale (MoCA) were taken to evaluate cognitive function.The activity of daily living (ADL) scale was used to test the quality of life

and dialectical scale of vascular dementia (DSVD) was taken once both before and after treatment to evaluate the traditional Chinese medicine syndrome. Superoxide dismutase (SOD)

tumor necrosis facor α (TNF-α) and homocysteine (Hcy) were detected both before and after therapy. Result: By Ridit analysis

the cognitive effect in observation group was superior to that in control group after treatment (P<0.05). MoCA

while the DSVD score was lower than that in control group (P<0.01). The scores in directional force

attention and calculation force and language ability in MMSE scale in observation group were higher than the data in control group (P<0.01). CRP

wile SOD was higher than that in control group (P<0.01). Conclusion: Based on the conventional western medicine treatment

Jiannaoling tablets combined with compound Yinxing Tongmai oral liquid can improve the cognitive function in patients with vascular cognitive impairment

reduce clinical symptoms and enhance the activities of daily living. Its mechanism may be related to reducing inflammation and oxygen free radical damages.
