摘要

目的：观察加味独活寄生合剂治疗膝骨性关节炎的临床疗效及对关节液中白细胞介素-1（interleukin-1，IL-1），白细胞介素-6（interleukin-1，IL-6），肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α）及一氧化氮（NO）的影响。方法：将90例膝关节患者随机分为治疗组和对照组，各45例，治疗组予加味独活寄生合剂治疗，对照组予硫酸氨基葡萄糖胶囊治疗，8周后观察两组患者疗效，治疗前后均行关节穿刺取关节液，检测关节液中IL-1，IL-6，TNF-α及NO的含量，对两组患者进行分析比较。结果：两组患者治疗后视觉模拟评分（visual analogue scales，VAS）均明显降低（P<0.05）；治疗后两组患者VAS评分比较，治疗组低于对照组（P<0.05）；治疗组治疗后膝关节功能Western Ontario and McMaster Univesrsities（WOMAC）评分优于对照组（P<0.05）。与治疗前比较，治疗后两组患者IL-1，IL-6，TNF-α及NO的含量均明显降低（P<0.05），两组患者组间比较无明显差异。结论：加味独活寄生合剂可减轻膝骨性关节炎患者疼痛，改善膝关节功能；加味独活寄生合剂能减少关节液中IL-1，IL-6，TNF-α及NO的含量，抑制炎症反应，从而改善临床症状，这可能是加味独活寄生合剂治疗膝骨关节炎的作用机制。

Abstract

Objective: To observe the clinical efficacy of Jiawei Duhuo Jisheng mixture in knee osteoarthritis and observe its effect on the levels of interleukin-1(IL-1)

tumor necrosis factor-α (TNF-α)and NO in the synovial fluid. Method: The 90 patients with knee osteoarthritis were randomly divided into treatment group and control group. The 45 cases in treatment group were treated with Jiawei Duhuo Jisheng mixture and 45 cases in control group were treated with glucosamine hydrochloride capsule for eight weeks. Then their curative effect was observed in both groups. Arthrocentesis was conducted before and after treatment to take synovial fluid

and then the levels of IL-1

IL-6

TNF-α and NO were detected and compared. Result: Knee pain Visual Analogue Scales(VAS) scores were decreased after treatment in both groups (P<0.05). VAS score in treatment group was lower than that in control group after treatment (P<0.05). Knee joint function Western Ontario and McMaster Univesrsities (WOMAC) score in treatment group was superior to that in control group after treatment (P<0.05). The levels of IL-6

but there was no significant difference between two groups. Conclusion: Jiawei Duhuo Jisheng mixture could reduce the levels of IL-6
