摘要

目的：探讨乌头赤石脂汤对老年缺血性心肌病伴心力衰竭患者血清肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α），B型钠尿肽（B-type natriuretic peptide，BNP）及同型半胱氨酸（homocysteine，Hcy）水平的影响。方法：取郑州市中医院治疗的缺血性心肌病心力衰竭老年患者94例，按随机数字表法分为治疗组和对照组，对照组（47例）予以基础治疗，治疗组（47例）在基础治疗上联用乌头赤石脂汤治疗，观察并记录心室重构及心功能、血清指标、生活质量等指标，同时观察临床疗效及不良反应。结果：治疗后对照组治疗有效率（74.47%）低于治疗组（91.49%）（P<0.05）；与治疗前比较，两组治疗后心室重构及心功能、血清指标、生活质量均发生变化，治疗组治疗后左心室舒张末内径（left ventricular end diastolic diameter，LVEDD），左室收缩末期内径（left ventricular end systolic diameter，LVESD）指标低于对照组（P<0.05），左心室射血分数（left ventricular ejection fraction，LVEF），每分钟输出量（output per minute，CO）指标高于对照组（P<0.05），治疗组治疗后血清BNP，肌钙蛋白I（TroponinI，cTnI），高敏C反应蛋白（high-sensitivity C-reactive protein，hs-CRP），TNF-α，Hcy水平低于对照组（P<0.05），治疗组治疗后生活质量改善程度优于对照组（P<0.05）；两组治疗期间无严重不良反应发生。结论：采用乌头赤石脂汤对老年缺血性心肌病伴心力衰竭疗效确切，能降低血清TNF-α，BNP及Hcy水平，提高生活质量。

Abstract

Objective: To investigate the effect of Wutou Chishizhi Tang on tumor necrosis factor-α (TNF-α)

B-type natriuretic peptide (BNP) and homocysteine (Hcy) levels in serum of patients with Senile Ischemic Cardiomyopathy and heart failure.Method: The 94 patients with senile ischemic cardiomyopathy and heart failure were selected randomly divided into 2 groups. 47 cases in control group were treated with basic treatment

and 47 cases in treatment group were treated with Wutou Chishizhi Tang based on the treatment in control group. Ventricular remodeling and cardiac function

as it can reduce the levels of serum TNF-α

BNP and Hcy levels

and improve the quality of life.
