摘要

目的：探讨柴芍承气汤联合低分子肝素对高脂血症性胰腺炎（hyperlipidemic pancreatitis，HLP）腑实热结证患者炎症介质、血脂水平及凝血功能的影响。方法： 2014年3月至2016年3月郑州大学第二附属医院共收治167例HLP患者，以该批患者为研究对象，随机数字表法分为治疗组（89例），对照组（88例）。对照组在西医常规疗法的基础上采用低分子肝素钙注射给药，治疗组加服柴芍承气汤联合治疗，每日1剂，清水煎煮至200 mL，连续用药7 d后对两组患者疗效、炎症介质、血脂水平及凝血功能进行评价。结果：治疗组患者总有效率为93.25%，对照组为80.68%，治疗组高于对照组（P<0.05）；经治疗，治疗组HLP患者在腹痛、腹胀缓解时间及胃肠道功能、血尿淀粉酶（amylase，AMY）恢复正常时间方面明显短于对照组（P<0.05）；与本组治疗前比较，治疗后两组患者血清中肿瘤坏死因子（tumor necrosis factor，TNF）-α，白细胞介素-6（interleukin-6，IL-6），C-反应蛋白（C-reactionprotein，CRP）与血清甘油三酯（triglyceride，TG）水平均明显降低，且治疗组明显低于对照组（P<0.05）；通过治疗两组患者凝血酶原时间（prothrombin time，PT），活化部分凝血激酶时间（activated partial thromboplastin time，APTT）水平均明显升高，且治疗组明显高于对照组（P<0.05）。结论：柴芍承气汤可以缓解HLP患者体征及临床症状，明显缩短病患病程，下调血清炎症介质的表达，降低血脂TG水平，改善患者的凝血功能，因此对HLP具有积极的治疗效果。

Abstract

Objective: To study the clinical efficacy of Chaishao Chengqitang combined with low molecular heparin in treating hyperlipidemic pancreatitis (HLP) and the effect on inflammatory mediator

lipid and coagulation function. Method: Totally 167 cases of HLP patients treated in the Second Affiliated Hospital of Zhengzhou University from March 2014 to March 2016 were selected and divided into two groups according to the random number table

with 89 cases in clinical group and 88 in control group. Patients in both groups were treated with the routine therapy. In addition
