摘要

目的：

探讨培元通脑胶囊对缺血性中风恢复期神经功能缺损的康复效果，并从改善微循环、抗炎和神经保护方面探讨了其作用机制。

方法：

将131例患者随机按数字表法分为对照组66例和观察组65例。对照组进行康复治疗，口服阿司匹林肠溶片，0.1 g·d

；和辛伐他汀片，10 mg·d

－1

；并采用经皮神经电刺激仪，30 min/次，1次/d，5次/周，共治疗30次。观察组西医治疗及康复理疗措施同对照组，并加服培元通脑胶囊，3粒/次，3次/d。两组疗程均为连续治疗12周。于治疗前、治疗后4，8，12周进行美国国立卫生院神经功能缺损(NIHSS)评分；进行治疗前后日常生活活动能力量表(ADL)，平衡能力量表(BBA)，运动能力量表(FMA)和中医证候评分；检测治疗前后脑源性神经营养因子(BDNF)，神经元特异性烯醇化酶(NSE)，白细胞介素-6(IL-6)，肿瘤坏死因子-

(TNF-

)，同型半胱氨酸(Hcy)和血清胱抑素-C(Cys-C)水平；检测治疗前后血液流学指标[全血黏度、高切还原黏度、低切还原黏度、血浆黏度、血小板聚集率、纤维蛋白原(FIB)和血液沉降率(ESR)]。

结果：

观察组在治疗后8，12周，NIHSS评分均低于对照组(

0.01)；治疗后观察组NIHSS评分下降幅度多于对照组(

0.01)；观察组ADL，BBA，FMA评分均高于对照组(

0.01)，中医证候评分低于对照组(

0.01)；观察组BDNF水平高于对照组(

0.01)，NSE水平低于对照组(

0.01)；观察组IL-6，TNF-

，Hcy和Cys-C水平均低于对照组(

0.01)；观察组血液流变学指标(血黏度、高切还原黏度、低切还原黏度、血浆黏度、血小板聚集率，FIB和ESR)均低于对照组(

0.01)；经秩和检验，观察组临床疗效优于对照组(

0.05)。

结论：

培元通脑胶囊可促进缺血性中风恢复期(肝肾不足，风痰瘀阻证)神经功能缺损的康复，具有调节炎症因子、改善血液流变和神经功能保护作用，可增加常规西医康复治疗的效果。

Abstract

Objective:

To discuss the effect of Peiyuan Tongnao capsules on neurological impairment of patients with ischemic stroke at convalescence

and discuss the mechanism of action in microcirculation improvement

anti-inflammation and neuroprotection.

Method:

One hundred and thirty one patients were randomly divided into control group (66 cases) and observation group (65 cases) by random number table. Patients in control group were given aspirin enteric-coated tablets

0.1 g/day

simvastatin tablets

10 mg/days

and treated by percutaneous nerve electrical stimulator for 30 times

30 min/time

1 time/day

5 times/week. In addition of the therapy in control group

patients in observation group were also given Peiyuan Tongnao capsules

3 grains/time

3 times/days. A course of treatment was 12 weeks. Before treatment

fugl-meyer scale (FMA) and traditional Chinese medicine (TCM) syndromes were scored. And levels of brain-derived neurotrophic factor (BDNF)

homocysteine (Hcy) and serum cystatin C (Cys-C) and hemorheological indicators whole blood viscosity

Result:

At the 8

and 12

week after treatment

score of NIHSS in observation group was lower than that in control group (

and NIHSS in observation group declined more than that in control group (

0.01). Scores of ADL

BBA and FMA and levels of BDNF were higher than those in control group (

0.01)

while scores of TCM syndromes and levels of NSE

TNF-

Hcy

Cys-C and hemorheological indices were all lower than those in control group (

0.01). And by rank sum test

the clinical effect in observation group was superior to that in control group (

0.05).

Conclusion:

Peiyuan Tongnao capsules can promote the recovery of neurological impairment at recovery period of ischemic stroke

regulate inflammatory factors

improve blood rheology

protect neural function

and improve the effect of routine western medicine rehabilitation therapy.
