目的

基于Janus激酶2(JAK2)/信号转导子和转录激活子3(STAT3)信号通路,从血管新生的角度探讨桃红四物汤对视网膜血管炎的保护作用及机制。
方法

利用SPF级C57BL/6J小鼠,建立实验性自身免疫性葡萄膜炎(EAU)诱导的视网膜血管炎(RV)小鼠模型,研究桃红四物汤对视网膜血管炎的保护作用。50只小鼠随机分为空白组,模型组,桃红四物汤低、中、高剂量组(3.315、6.63、13.26 g·kg
-1
),每组10只,造模后连续灌胃给药20 d。小动物视网膜成像系统与荧光素钠血管造影观察小鼠视网膜组织及血管病理的改变;苏木素-伊红(HE)染色法观察小鼠视网膜组织病理变化;免疫组化评估小鼠CD31阳性表达情况;蛋白免疫印迹法(Western blot)检测小鼠视网膜组织中JAK2、磷酸化(p)-JAK2、STAT3、p-STAT3、内皮生长因子受体2(VEGFR2)蛋白表达水平;实时荧光定量聚合酶链式反应(Real-time PCR)检测小鼠视网膜组织中血管VEGFR2 mRNA的相对表达水平。
结果

与空白组比较,模型组小鼠视盘相对肿胀,视网膜静脉血管周围可见多处炎症细胞浸润并遮挡部分血管,血管增粗且形态发生改变,且小鼠视网膜出现组织厚度不均匀,内层和外层均出现褶皱和弯曲,部分血管扩张,细胞排列紊乱等病理改变;与模型组比较,桃红四物汤干预组小鼠CD31阳性表达明显减少,视网膜血管周围炎性浸润、血管迂曲扩张、血管新生、血管阻塞及出血程度均得到有效改善;Western blot结果显示,与模型组比较,桃红四物汤干预组小鼠VEGFR2的蛋白表达明显减少,JAK2、STAT3磷酸化水平显著降低(

0.01),Real-time PCR结果显示,与模型组比较,桃红四物汤干预组小鼠VEGFR2 mRNA的表达明显减少(

结论

桃红四物汤可有效改善视网膜血管炎血管新生程度,并可通过JAK2/STAT3信号通路,减少血管新生,改善视网膜病理损伤,对视网膜血管具有保护作用。
Objective

Based on the Janus kinase 2 (JAK2)/signal transducer and activator of transcription 3 (STAT3) signaling pathway, this study explores the protective effect and mechanism of Taohong Siwutang against retinal vasculitis (RV) from the perspective of angiogenesis.
Methods

SPF-grade C57BL/6J mice were used to establish a RV model induced by experimental autoimmune uveitis (EAU), and the protective ef
fect of Taohong Siwutang on RV was investigated. Fifty mice were randomly assigned to a blank group, model group, and low-, medium-, and high-dose Taohong Siwutang groups (3.315、6.63、13.26 g·kg
-1
,10 mice in each group). After modeling, gavage administration was performed for 20 consecutive days. A small-animal retinal imaging system and fluorescein sodium angiography were used to observe pathological changes in the retinal tissue and vessels. Hematoxylin-eosin (HE) staining was used to assess retinal histopathological changes. Immunohistochemistry was performed to evaluate CD31-positive expression. Western blot was used to detect the protein expression levels of JAK2, phosphorylated (p)-JAK2, STAT3, p-STAT3, and vascular endothelial growth factor receptor 2 (VEGFR2) in retinal tissue. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to determine the relative expression level of VEGFR2 mRNA in retinal vessels.

Compared with the blank group, the model group showed relative optic disc swelling, multiple areas of inflammatory cell infiltration around retinal veins with partial vascular occlusion, vessel thickening and morphological alterations, uneven retinal thickness, wrinkling and bending of inner and outer layers, vascular dilation, and disordered cellular arrangement. Compared with the model group, the Taohong Siwutang groups showed markedly reduced CD31-positive expression and effectively improved perivascular inflammatory infiltration, vascular tortuous dilation, angiogenesis, vascular occlusion, and hemorrhage. Western blot results showed that compared with the model group, the expression of VEGFR2 and the phosphorylation levels of JAK2 and STAT3 were significantly decreased in the Taohong Siwutang groups (

0.01). Real-time PCR results indicated that VEGFR2 mRNA expression was significantly decreased in the Taohong Siwutang groups compared with the model group (

0.05).
Conclusion

Taohong Siwutang can effectively alleviate angiogenesis in RV and, through the JAK2/STAT3 signaling pathway, reduce angiogenesis and improve retinal pathological injury, thereby exerting a protective effect on retinal vessels.
