升降散是一种常用的临床疗法，其疗效确切。笔者发现该方出处众说纷纭，且尚无学者考证关键信息。该文采用文献计量法对升降散的出处、组成等关键信息进行全面考证。经考证，升降散总共有“内府仙方”“加味僵黄丸”“赔赈散”“太极丸”4个同方异名，升降散首以“内府仙方”为名见于明代龚廷贤所著《万病回春》，经清代杨栗山传承和发扬广为流传至今。剂型分为丸剂和散剂2种，当代以散剂流行。据明清度量衡剂量换算建议该方入丸剂则单次制备需僵蚕74.6 g，姜黄、蝉蜕各9.325 g，大黄149.2 g，单次服用剂量为僵蚕1.15 g，姜黄、蝉蜕各0.14 g，大黄2.3 g，小儿用量减半。散剂病情轻者单次剂量为僵蚕1.84 g、蝉蜕0.92 g、姜黄0.28 g、大黄3.68 g；病情重者单次剂量为僵蚕2.48 g、蝉蜕1.23 g、姜黄0.37 g、大黄4.91 g，病情最重者僵蚕3.68 g、蝉蜕1.84 g、姜黄0.55 g、大黄7.36 g。4味药物研成均匀细末，病轻者，分4次服，每服6.71 g，用黄酒200 mL，蜂蜜18.65 g，搅拌均匀冷服，症状好转即可停药。病重者，分3次服，每服8.95 g，黄酒300 mL，蜂蜜27.98 g，搅拌均匀冷服。最重者，分2次服，每服13.43 g，黄酒400 mL，蜂蜜37.3 g，搅拌均匀冷服。全方升清降浊，散风清热，主治温病表里三焦大热，现代临床运用广泛。该研究厘清升降散流传脉络，考证其关键信息表，为学者研究提供参考并为其推向市场转化提供理论依据。
