  • Houshihei San Repairs Skeletal Muscle Injury After Ischaemic Stroke by Regulating Ferroptosis Pathway Enhanced Publication AI Introduction

    Abstract:ObjectiveTo investigate the pharmacodynamic effects of Houshihei San (HSHS) recorded with the effects of treating wind and limb heaviness on muscle tissue injury after middle cerebral artery occlusion (MCAO) in rats through the ferroptosis pathway.MethodsThirty SD male rats were selected and randomly grouped as follows: sham, MCAO, deferoxamine mesylate, high-dose HSHS (HSHS-H, 0.54 g·kg-1), and low-dose HSHS (HSHS-L, 0.27 g·kg-1), with 6 rats in each group. A laser scattering system was used to evaluate the stability of the MCAO model, and rats were administrated with corresponding agents by gavage for 7 days. During the administration period, behavioral, imaging and other methods were used to systematically evaluate the skeletal muscle tissue injury after MCAO and the therapeutic effect in each administration group. Hematoxylin-eosin staining was employed to evaluate the cross-section of muscle cells. Subsequently, immunohistochemistry was used to detect tumor suppressor p53 and glutathione peroxidase 4 (GPX4) in the soleus tissue. Western blot was employed to determine the protein levels of p53, GPX4, myogenic differentiation 1 (MyoD1), nuclear factor E2-related factor 2 (Nrf2), Myostatin, solute carrier family 7 member 11 (SLC7A11), muscle ring-finger protein-1 (MuRF1), and muscle atrophy F-box protein (MAFbx) to verify the therapeutic effect in each group.ResultsCompared with the MCAO group, HSHS enhanced the locomotor ability and promoted muscle regeneration, which suggested that the pharmacological effects of HSHS were related to the inhibition of muscle tissue ferroptosis to reduce the expression of muscle atrophy factors. Behavioral and imaging results suggested that compared with the MCAO group, HSHS ameliorated neurological impairments in rats on day 7 (P<0.01), enhanced 5-min locomotor distance and postural control (P<0.01), strengthened grasping power and promoted muscle growth (P<0.01), stabilized skeletal muscle length and weight (P<0.01), and increased the cross-section of muscle cells (P<0.01). Compared with the MCAO group, HSHS promoted the increases in glutathione and superoxide dismutase content and inhibited the increase in malondialdehyde content (P<0.05,P<0.01). Ferroptosis pathway-related assays suggested that HSHS reduced the p53-positive cells and increased the GPX4-positive cells (P<0.01). HSHS ameliorated muscle function decline after stroke by promoting the expression of GPX4, Nrf2, SLC7A11, and MyoD1 and inhibiting the expression of p53, Myostatin, MurRF1, and MAFbx to reduce ferroptosis in the muscle (P<0.01).ConclusionHSHS, prepared with reference to the method in the Synopsis of Golden Chamber, can simultaneously reduce the myolysis and increase the protein synthesis in the skeletal muscle tissue after ischemic stroke by regulating the ferroptosis pathway.

    Abstract:ObjectiveTo explore the protective effect and mechanism of Gegen Qianliantang (GQT) on intestinal mucosal barrier function in dextran sulfate sodium (DSS)-induced ulcerative colitis (UC) model mice.MethodsA UC model was established in C57BL/6 mice using a 2.5% DSS solution. Mice were randomly divided into five groups (n=8 per group): blank group, model group, mesalazine sustained-release granule group (0.52 g·kg-1), high-dose GQT group (2.23 g·kg-1), and low-dose GQT group (1.12 g·kg-1). Fecal characteristics and body weight changes were observed before and after treatment. The body weight loss and disease activity index (DAI) of UC mice were calculated to evaluate symptom severity. Hematoxylin-eosin (HE) staining and Alizarin blue-periodic acid-Schiff (AB-PAS) staining were used to detect histological changes in colon tissue. Immunohistochemistry was used to detect the expression of zonula occludens-1 (ZO-1) and mucin 2 (MUC2). Enzyme-linked immunosorbent assay (ELISA) was used to detect serum levels of pro-inflammatory cytokines interferon-γ (IFN-γ), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), IL-17A, and anti-inflammatory cytokine IL-10. Flow cytometry was used to detect the activation of helper T lymphocyte subsets (Th1, Th17), regulatory T cells (Treg), and regulatory B cells (Breg) in spleen and colon tissues. Western blot was used to detect the expression levels of T-bet, forkhead box protein P3(FoxP3), nuclear transcription factor(NF)-κB p65, phosphorylated NF-κB p65 (p-NF-κB p65), signal transducer and activator of transcription 3(STAT3), and phosphorylated STAT3 (p-STAT3).ResultsCompared with the model group, both high- and low-dose GQT groups significantly improved the body weight loss and DAI scores (P<0.05), alleviated colonic inflammation, and showed optimal efficacy in the high-dose group. AB-PAS staining showed that compared with the model group, both the high- and low-dose GQT groups significantly increased goblet cell proliferation and mucin secretion, indicating improved mucosal barrier function. GQT upregulated the expression of ZO-1 and MUC2 in colon tissue (P<0.05), suppressed IFN-γ, IL-6, and TNF-α secretion (P<0.05), elevated IL-10 secretion (P<0.05), but had no significant effect on IL-17A. At the same time, high- and low-dose GQT intervention increased the activation of CD4+ FoxP3+ Treg cells (P<0.05) and suppressed activation of CD4+ IFN-γ+ Th1 cells (P<0.05). Western blot showed that GQT downregulated T-bet, NF-κB p65, and STAT3 protein expression (P<0.05), upregulated FoxP3 (P<0.05), and also reduced phosphorylation levels of p-NF-κB p65 and p-STAT3 (P<0.05).ConclusionGQT can upregulate the activation of CD4+ FoxP3+ Treg cells, reduce the activation of CD4+ IFN-γ+ Th1 cells, inhibit the secretion of IFN-γ, IL-6, and TNF-α, and increase the secretion of IL-10. It enhances the expression of MUC2 and ZO-1 in colon tissue, thereby alleviating inflammatory damage to the intestinal mucosa and restoring mucosal barrier integrity. These effects may be related to its regulation of NF-κB p65 and STAT3 signaling pathways, ultimately regulating the activation of transcription factors T-bet and FoxP3.

    Abstract:ObjectiveTo explore the mechanism of action of Cuscutae Semen-Salviae Miltiorrhizae Radix et Rhizoma in the treatment of recurrent spontaneous abortion (RSA) through network pharmacology, molecular docking, and cell experiment verification.MethodsThe Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and UniPort databases were used to screen and organize the active ingredients and corresponding targets of Cuscutae Semen-Salviae Miltiorrhizae Radix et Rhizoma. The potential therapeutic targets of RSA were screened in Online Mendelian Inheritance in Man (OMIM), GeneCards database, DrugBank database, DisGeNET database, and Therapeutic Target Database (TTD). The potential core targets of Cuscutae Semen-Salviae miltiorrhizae Radix et Rhizoma for treating RSA were further screened by constructing a protein-protein interaction (PPI) network and topological analysis. Meanwhile, the Database for Annotation, Visualization and Integrated Discovery (DAVID) was chosen to perform enrichment analysis on intersection targets. On this basis, AutoDock software was used for molecular docking, and the data were imported into PyMOL software for visualization and composition. Finally, the cell counting kit-8 (CCK-8) experiment, Transwell cell invasion assay, and Western blot were used to detect the effects of serum containing Cuscutae Semen-Salviae miltiorrhizae Radix et Rhizoma on HTR-8/SVneo cells and observe the effects on the interleukin (IL)-6/signal transducer and activator of transcription 3(STAT3) signaling pathway and related proteins.ResultsThrough network pharmacology analysis, a total of 69 active ingredients, 73 potential therapeutic targets, and 17 core targets, including IL-6, IL-10, and STAT3, were collected. The 73 common targets were enriched in 614 Gene Ontology (GO) entries and 57 Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathways. The molecular docking results indicated that IL-6 and STAT3 had good binding ability with the main active ingredients, including matrine, cryptotanshinone, and tanshinone ⅡA. The cell experiment results showed that, compared with those of the control group, after 24 hours of treatment with the drug-containing serum, the survival and invasion rates of HTR-8/SVneo cells were significantly increased (P<0.05), and the expression of IL-6/STAT3 signaling pathway and related proteins IL-10 and c-Myc was significantly elevated (P<0.05). Moreover, the trend of action in the drug-containing serum group was consistent with that of pathway agonists.ConclusionCuscutae Semen-Salviae miltiorrhizae Radix et Rhizoma may enhance the survival rate and invasive activity of HTR-8/SVneo cells to further prevent and treat RSA by activating the IL-6/STAT3 signaling pathway and upregulating the expression of downstream factors IL-10 and c-Myc in the pathway.

  • Neurotoxicity Mechanism of Dictamni Cortex Based on Network Toxicology and Metabolomics Enhanced Publication AI Introduction

    Abstract:ObjectiveThis study aims to explore the neurotoxicity mechanism of Dictamni Cortex by integrating network toxicology and metabolomics techniques.MethodsThe neurotoxicity targets induced by Dictamni Cortex were screened by the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP), Traditional Chinese Medicine Information Database (TCM-ID), and Comparative Toxicogenomics Database (CTD). The target predictions of the components were performed by the Swiss Target Prediction tool. Neurotoxicity-related targets were collected from the Pharmacophore Mapping and Potential Target Identification Platform (PharmMapper), GeneCards Human Gene Database (GeneCards), DisGeNET Disease Gene Network (DisGeNET), and Online Mendelian Inheritance in Man (OMIM), and the intersection targets were identified. Protein-protein interaction (PPI) analysis, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis, and Gene Ontology (GO) enrichment analysis were conducted. A "drug-compound-toxicity target-pathway" network was constructed via Cytoscape software to display the core regulatory network. Based on the prediction results, the neurotoxicity mechanism of Dictamni Cortex in mice was verified by using hematoxylin-eosin (HE) staining, Nissl staining, enzyme-linked immunosorbent assay (ELISA), quantitative real-time fluorescence polymerase chain reaction (Real-time PCR), and Western blot. The effects of Dictamni Cortex on the metabolic profile of mouse brain tissue were further explored by non-targeted metabolomics.ResultsNetwork toxicology screening identified 13 compounds and 175 targets in Dictamni Cortex that were related to neurotoxicity. PPI network analysis revealed that serine/threonine-protein kinase (Akt1) and tumor protein 53 (TP53) were the core targets. Additionally, GO/KEGG enrichment analysis indicated that Dictamni Cortex may regulate the phosphatidylinositol 3-kinase (PI3K)/Akt pathway and affect oxidative stress and cell apoptosis, thereby inducing neural damage. The "Dictamni Cortex-compound-toxicity target-pathway-neural damage" network showed that dictamnine, phellodendrine, and fraxinellone may be the toxic compounds. Animal experiments showed that compared with those in the blank group, the hippocampal neurons in the brain tissue of mice treated with Dictamni Cortex were damaged. The level of superoxide dismutase (SOD) and acetylcholine (ACh) in the brain tissue was significantly reduced, while the content of malondialdehyde (MDA) was significantly increased. The level of Akt1 and p-Akt1 mRNAs and proteins in the brain tissue was significantly decreased, while the level of TP53 was significantly increased. Non-targeted metabolomics results showed that Dictamni Cortex could disrupt the level of 40 metabolites in mouse brain tissue, thereby regulating the homeostasis of 13 metabolism pathways, including phenylalanine, glycerophospholipid, and retinol. Combined analysis revealed that Akt1, p-Akt1, and TP53 were significantly correlated with phenylalanine, glycerophospholipid, and retinol metabolites. This suggested that Dictamni Cortex induced neurotoxicity in mice by regulating Akt1, p-Akt1, and TP53 and further modulating the phenylalanine, glycerophospholipid, and retinol metabolism pathways.ConclusionDictamni Cortex can induce neurotoxicity in mice, and its potential mechanism may be closely related to the activation of oxidative stress, inhibition of the PI3K/Akt signaling pathway, and regulation of phenylalanine, glycerophospholipid, and retinol metabolism pathways.

    Abstract:ObjectiveThis study aims to investigate the effect of Dictamni Cortex on intestinal barrier damage in rats and its mechanism by untargeted metabolomics and targeted metabolomics for short-chain fatty acids (SCFAs).MethodsRats were randomly divided into a control group, a high-dose group of Dictamni Cortex (8.1 g·kg-1), a medium-dose group (2.7 g·kg-1), and a low-dose group (0.9 g·kg-1). Except for the control group, the other groups were administered different doses of Dictamni Cortex by gavage for eight consecutive weeks. Hematoxylin-eosin (HE) staining was used to observe the pathological changes in the ileal tissue. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the level of cytokines, including tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-1β (IL-1β), in the ileal tissue of rats. Quantitative real-time fluorescence polymerase chain reaction (Real-time PCR) technology was used to detect the expression level of tight junction proteins, including zonula occludens-1 (ZO-1), Occludin, and Claudin-1 mRNAs, in the ileal tissue of rats to preliminarily explore the effects of Dictamni Cortex on intestinal damage. The dose with the most significant toxic phenotype was selected to further reveal the effects of Dictamni Cortex on the metabolic profile of ileal tissue in rats by non-targeted metabolomics combined with targeted metabolomics for SCFAs.ResultsCompared with the control group, all doses of Dictamni Cortex induced varying degrees of pathological damage in the ileum, increased TNF-α (P<0.01), IL-6 (P<0.01), and IL-1β (P<0.01) levels in the ileal tissue, and decreased the expression level of ZO-1 (P<0.05, P<0.01), Occludin (P<0.01), and Claudin-1 (P<0.05) in the ileal tissue, with the high-dose group showing the most significant toxic phenotypes. The damage mechanisms of the high-dose group of Dictamni Cortex on the ileal tissue were further explored by integrating non-targeted metabolomics and targeted metabolomics for SCFAs. The non-targeted metabolomics results showed that 21 differential metabolites were identified in the control group and the high-dose group. Compared with that in the control group, after Dictamni Cortex intervention, the level of 14 metabolites was significantly increased (P<0.05, P<0.01), and the level of seven metabolites was significantly decreased (P<0.05, P<0.01) in the ileal contents. These metabolites collectively acted on 10 related metabolic pathways, including glycerophospholipids and primary bile acid biosynthesis. The quantitative data of targeted metabolomics for SCFAs showed that Dictamni Cortex intervention disrupted the level of propionic acid, butyric acid, acetic acid, caproic acid, isobutyric acid, isovaleric acid, valeric acid, and isocaproic acid in the ileal contents of rats. Compared with those in the control group, the level of isobutyric acid, isovaleric acid, and valeric acid were significantly increased, while the level of propionic acid, butyric acid, and acetic acid were significantly decreased in the ileal contents of rats after Dictamni Cortex intervention (P<0.05, P<0.01).ConclusionDictamni Cortex can induce intestinal damage by regulating glycerophospholipid metabolism, primary bile acid biosynthesis, and metabolic pathways for SCFAs.

  • Mechanism of Pharmacological Liver and Kidney Injuries of Dictamni Cortex Based on UPLC-Q-TOF-MS Enhanced Publication AI Introduction

    Abstract:ObjectiveThis study aims to reveal the mechanism of liver and kidney injuries caused by Dictamni Cortex and its interrelationship by metabonomics analysis of liver and kidney via ultra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry (UPLC-Q-TOF-MS).MethodsThe content of the marker compounds of Dictamni Cortex was measured by high-performance liquid chromatography (HPLC) to carry out quality control. Sprague Dawley (SD) rats were randomly divided into a blank group (normal saline), an administration group (0.9, 2.7, 8.1 g·kg-1), and a high-dose withdrawal control group, with eight rats in each group. Continuous administration was performed once daily for 28 days. The liver and kidney injuries caused by each administration group were assessed by organ indices, pathological observations, and serum and plasma biochemical indices measured by enzyme-linked immunosorbent assay (ELISA). The potential biomarkers of liver and kidney injuries caused by Dictamni Cortex were screened, and pathway enrichment analysis and correlation analysis were performed based on UPLC-Q-TOF-MS.ResultsCompared with the blank group, both the medium- and low-dose groups showed insignificant damage to the liver and kidney of rats. The high-dose group exhibited the most serious damage, and the level of liver and kidney function indices [alanine aminotransferase (ALT), aspartate aminotransferase (AST), creatinine (Cr), and blood urea nitrogen (BUN)] and serum inflammatory indices ([interleukin 1β (IL-1β), IL-6, and tumor necrosis factor-α (TNF-α)] in the serum were significantly changed (P<0.01). The liver and kidney metabolism pathways and differential metabolites were quite different. Among them, phenylalanine metabolism, niacin and nicotinamide metabolism, and glycerophospholipid metabolism were common pathways. Correlation analysis of differential metabolites showed that there were significant correlations among disorders of 4′-Phosphopantothenoylcysteine, PC (16∶0/15∶0), phenylethylamine, arachidonic acid, and linoleic acid in liver and kidney tissue.ConclusionThe decoction of Dictamni Cortex can cause liver and kidney injuries, and its mechanism may be related to oxidative stress and lipid metabolism disorders. The correlation of differential metabolites indicates the interaction between liver and kidney injuries.

  • Metabolomics Reveals Immune System Domage of Dictamnine Enhanced Publication AI Introduction

    Abstract:ObjectiveTo explore the mechanism of the immunotoxicity induced by dictamnine (DIC) in rats and the recovery effect after drug withdrawal by ultra-performance liquid chromatography-quadrupole-time-of-flight mass spectrometry, thereby providing a theoretical basis for elucidating the toxic mechanism of DIC.MethodsSD rats were randomized into blank (normal saline), DIC (10 mg·kg-1), and DIC withdrawal (recovery period) groups (n=8). The rats were continuously treated for 7 days, once a day, and the body weight and organ weight were recorded. The levels of interleukin-1 (IL-1), IL-6, and tumor necrosis factor-α (TNF-α) in the serum and immunoglobulin A (IgA), immunoglobulin G (IgG), and immunoglobulin M (IgM) in the spleen were determined by enzyme-linked immunosorbent assay. Hematoxylin-eosin staining was used to observe the pathological changes in the spleen. ultra performance liquid chromatography quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS) was employed to screen the potential biomarkers of immune inflammation caused by DIC, and pathway enrichment analysis and correlation analysis were performed. The mRNA levels of IL-1β, TNF-α, lysophosphatidylcholine acyltransferase 2 (LPCAT2), and farnesoid X receptor (FXR) in the serum were determined by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR).ResultsCompared with the blank group, the DIC group showed elevated levels of IL-1β, IL-6, and TNF-α in the serum (P<0.01), and the DIC withdrawal group showcased lowered levels of IL-1β, IL-6, and TNF-α in the serum (P<0.01). The levels of IgA, IgG, and IgM in the spleen of rats in the DIC group were decreased (P<0.01), while those in the DIC withdrawal group were recovered (P<0.05, P<0.01). Untargeted metabolomics of the serum and spleen screened out 14 common differential metabolites and 14 common metabolic pathways. The Spearman correlation analysis between differential metabolites and inflammatory factors identified PC (32∶0), LysoPC (20∶4/0∶0), LysoPC (P-18∶0/0∶0), taurochenodeoxycholic acid, taurocholic acid, LysoPC [20∶5(5Z,8Z,11Z,14Z,17Z)/0∶0], chenodeoxycholic acid, arachidonic acid, LysoPC (18∶0/0∶0), LysoPC (15∶0/0∶0), LysoPC (16∶0/0∶0), and LysoPC (17∶0/0∶0) as the biomarkers of immunotoxicity induced by DIC in SD rats. In the process of immunotoxicity caused by DIC, lipid metabolism disorders such as glycerophospholipid metabolism, primary bile acid metabolism, and arachidonic acid metabolism were enriched, which was consistent with the DIC-induced inflammatory factors and pathological characteristics of the spleen. Compared with the blank group, the DIC group exhibited up-regulated mRNA levels of IL-1β, TNF-α, LPCAT2, and FXR (P<0.01), and the up-regulation was decreased in the withdrawal group (P<0.01).ConclusionDIC can lead to immune and inflammatory disorders. DIC withdrawal can regulate the expression of biomarkers related to serum and spleen metabolites, regulate the inflammatory metabolic pathway, reduce the inflammation level, and alleviate the metabolic disorders, thus attenuating the potential toxicity induced by DIC.

    Abstract:ObjectiveTo investigate the potential mechanisms of Zingiberis Rhizoma in treating inflammatory bowel disease (IBD) by integrating network pharmacology with in vitro and in vivo experiments.MethodsTraditional Chinese Medicine Systems Pharmacology Database And Analysis Platform (TCMSP), Traditional Chinese Medicine Integrated Database (TCMID) Database were used to obtain the active component targets of Zingiberis Rhizoma. GeneCards was used to obtain the IBD targets. DAVID was used to perform Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses on core targets. Cytoscape 3.10.2 was used to establish the "active component-disease target-signaling pathway" interaction network. Mice were randomly assigned to control, model, and Zingiberis Rhizoma (400 mg·kg-1) groups. An IBD model was induced via dextran sulfate sodium (DSS). The colonic tissue was collected post-treatment to assess histology, expression of Ly6C+ monocytes/macrophages, and mRNA levels of Toll-like receptor 4 (TLR4), and inflammatory cytokines. The effect of Zingiberis Rhizoma aqueous extract on RAW264.7 cell viability was evaluated. Furthermore, the effects of the extract at 100, 10, and 1 mg·L-1 on LPS-induced differentiation of RAW264.7 cells into Ly6Chi monocytes/macrophages, mRNA levels of TLR4 and inflammatory cytokines, and protein levels of factors in the TLR4/mitogen-activated protein kinase (MAPK) signaling pathway.ResultsA total of 241 targets were identified for Zingiberis Rhizoma and 6 787 for IBD, with 122 shared targets among Zingiberis Rhizoma, ulcerative colitis (UC), and Crohn's disease (CD). The enrichment analyses yielded 297 GO terms and 88 KEGG pathways. Associations were noted between Zingiberis Rhizoma's active component targets and IBD targets. In vivo experiments: Compared with the control group, the model group showed decreased body weight and disease activity index (DAI)(P<0.01), shortened colon length, damaged mucosal epithelium with inflammatory cell infiltration, raised pathological scores (P<0.05), increased Ly6Chi and Ly6Clo monocytes/macrophages (P<0.05), and up-regulated mRNA levels of TLR4, TNF-α, IL-1β, and IL-6 (P<0.05) and protein levels of TLR4, phosphorylated extracellular signal-regulated protein kinase 1/2 (p-ERK1/2), and phosphorylated p38 MAPK (p-p38 MAPK) (P<0.05). Zingiberis Rhizoma intervention reversed these changes and reduced Ly6Chi monocytes/macrophages (P<0.01). In vitro experiments: compared with the control, LPS increased the proportion and number of Ly6Chi monocytes/macrophages and mRNA levels of TLR4, TNF-α, IL-1β, and IL-6 (P<0.01) and enhanced the expression of TLR4, p-ERK1/2, and p-p38 MAPK (P<0.05). Zingiberis Rhizoma reduced Ly6Chi monocytes/macrophages (P<0.05), down-regulated the mRNA levels of inflammatory cytokines (P<0.05), and suppressed the TLR4/MAPK pathway (P<0.05).ConclusionZingiberis Rhizoma alleviates IBD by suppressing the TLR4/ERK/p38 MAPK signaling pathway and reducing inflammatory cytokine levels in Ly6Chi monocytes/macrophages.

    Abstract:ObjectiveTo clarify the repair effect of Mume Fructus on the intestinal mucosal epithelial barrier in the mouse model of inflammatory bowel disease (IBD) and explore the repair mechanism.MethodsThirty-six male C57BL/6 mice were randomly assigned into six groups: normal, model, low-, medium-, and high-dose (200, 400, and 800 mg·kg-1) Mume Fructus, and sulfasalazine (300 mg·kg-1). Except the normal group, the rest groups had free access to 2% dextran sulfate sodium (DSS) solution for seven days to establish the IBD model, followed by a seven-day drug intervention. The body weight change and disease activity index (DAI) were recorded. After the last administration, spleen and colon tissue samples were collected to analyze the differences in colon length and spleen index. Hematoxylin-eosin staining was used to observe the morphology of the colon tissue. The level of diamine oxidase (DAO) in the serum was measured by the DAO assay kit. Immunohistochemistry was employed to determine the expression of tight junction proteins such as Claudin-1, Occludin, and zonula occludens-1 (ZO-1) in the colon tissue. Real-time PCR was performed to measure the mRNA levels of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the colon tissue. Finally, Western blot was employed to determine the protein levels of mitogen-activated protein kinase kinase (MEK), extracellular signal-regulated kinase (ERK), phosphorylated (p)-MEK, and phosphorylated ERK in the colon tissue.ResultsCompared with the normal group, the model group exhibited decreases in body weight and colon length (P<0.01), increases in DAI, spleen index, and serum DAO level (P<0.01), damaged colonic epithelium and goblet cells, and obvious infiltration of inflammatory cells. In addition, the model group exhibited higher positive expression of Claudin-1, Occludin, and ZO-1 (P<0.01), higher mRNA levels of TNF-α and IL-1β (P<0.01), and higher protein levels of p-MEK and p-ERK (P<0.05, P<0.01) than the normal group. However, sulfasalazine and three doses of Mume Fructus markedly decreased the body weight and DAI (P<0.05), recovered the colon length and spleen index, alleviated colon tissue damage, lowered the level of DAO in the serum (P<0.01), and down-regulated the mRNA levels of TNF-α and IL-1β (P<0.01) and the protein levels of p-MEK and p-ERK (P<0.05). Sulfasalazine and low- and medium-dose Mume Fructus increased the positive expression of Occludin, Claudin-1, and ZO-1 (P<0.05, P<0.01). Furthermore, high-dose Mume Fructus elevated the protein expression of Occludin (P<0.05).ConclusionMume Fructus can restore the expression of intestinal epithelial tight junction proteins by inhibiting the phosphorylation of proteins in the MEK/ERK signaling pathway and down-regulating the levels of TNF-α and IL-1β, thus repairing the intestinal mucosal barrier in the mouse model of IBD.

    Abstract:ObjectiveTo investigate the modulating effect of modified Wumeiwan (MWMW) on the ulcerative colitis (UC)-associated intestinal helper T cell 17 (Th17)/regulatory T cell (Treg) balance and intestinal flora by using a human flora-associated model of UC patients with dampness-heat obstruction syndrome, thus providing a new idea for the UC-related research and therapeutic strategies.MethodsThe 24 male C57BL/6J mice were randomized into normal control, model, and MWMW groups (n=8). Model and MWMW groups were first treated with an antibiotic cocktail (vancomycin, 0.1 g·kg-1; neomycin sulfate, 0.2 g·kg-1; ampicillin, 0.2 g·kg-1; metronidazole, 0.2 g·kg-1) for 21 days. At the end of antibiotic treatment, the gavage of fecal microbiota suspension from UC patients with dampness-heat obstruction syndrome was started at a dose of 0.2 mL·d-1 for 19 consecutive days, by which a human flora-associated model of UC was obtained. The MWMW group was administrated daily with MWMW liquid (12.5 g·kg-1), while the normal control and model groups were administrated by gavage with an equal amount of sterile water for 7 consecutive days. The symptoms of dampness-heat obstruction were observed. The colon length and spleen index were measured and calculated, and the proportions of Th17 and Treg cells were detected by flow assay. The intestinal flora was analyzed by 16S rRNA high-throughput sequencing.ResultsCompared with the normal control group, the model group showed shortened colon (P<0.05) and increased spleen index (P<0.01). Compared with the model group, the MWMW group showed prolonged colon (P<0.01) and decreased spleen index (P<0.05). After the intervention of MWMW, the Th17 proportion and Th17/Treg ratio in the colon decreased (P<0.01), and the proportion of Treg cells increased (P<0.05). The number of species and alpha and beta diversity of intestinal flora in mice were regulated by MWMW (P<0.05). In terms of intestinal flora composition, MWMW increased the relative abundance of several phyla (Firmicutes, Proteobacteria, Fusobacteriota, Actinobacteriota, and Gemmatimonadota), the genus Bacteroides, and two species (Bacteroides thetaiotaomicron and B. fragilis) in model mice. Moreover, Spearman's correlation analysis showed that the relative abundance of B. thetaiotaomicron and B. fragilis were negatively correlated with the Th17 level (P<0.05). In addition, the above changes in intestinal flora caused the changes in microbial genes involved in 14 pathways, such as glycolysis, amino acid degradation, inorganic nutrient metabolism, biosynthesis of pyrimidine deoxyribonucleotides, antibiotic resistance, and degradation of polysaccharides.ConclusionsThe human flora-associated model successfully simulated the changes (marked by a decrease in the abundance of Bacteroides) of intestinal flora in UC patients with dampness-heat obstruction syndrome. MWMW can enrich the abundance of beneficial bacteria such as B. thetaiotaomicron and B. fragilis and promote the synergistic intestinal immune modulation with the metabolic functions centered on glycolysis, amino acid metabolism, and nucleotide synthesis through bacterial polysaccharide utilization sites to reduce the Th17/Treg ratio, thereby exerting a protective effect on UC.

  • Evolution of Medication Patterns in Traditional Chinese Medicine for Treating Inflammatory Bowel Disease Enhanced Publication AI Introduction

    Abstract:ObjectiveTo explore the evolution of medication patterns and syndrome-herb associations of traditional Chinese medicine (TCM) in treating inflammatory bowel disease (IBD), providing a theoretical foundation for precise syndrome differentiation and treatment in clinical practice.MethodsMedical case literature on TCM treatment of IBD from 1960 to 2024 was retrieved to establish a database. Frequency statistics, cluster analysis, change point detection, and association rule mining were employed to comprehensively analyze the syndrome distribution, therapeutic methods, medication patterns, and their temporal variations.ResultsA total of 685 medical cases were included. Common syndromes were dampness-heat (66.42%) and spleen deficiency (56.20%). Primary therapeutic methods included heat clearing (63.65%), spleen invigorating (47.45%), and dampness draining (36.79%). High-frequency herbs included Coptidis Rhizoma (354), Paeoniae Radix Alba (303), Aucklandiae Radix (292), Codonopsis Radix (253), and Glycyrrhizae Radix et Rhizoma (244). Initial prescription clustering revealed three core therapeutic method combinations: heat clearing and detoxifying (represented by Baitouweng Tang), spleen invigorating and Qi reinforcing (represented by Shenling Baizhusan), and cold-heat regulation (represented by Wumeiwan combined with Shaoyao tang). Temporal analysis identified 2008 as a key transition point in TCM treatment of IBD, with significantly increased usage frequency of heat-clearing and dampness-drying herbs such as Fraxini Cortex, Phellodendri Chinensis Cortex, Sophorae Flavescentis Radix, and Scutellariae Radix as well as hemostatic herbs such as carbonized Sanguisorbae Radix, Bletillae Rhizoma, Agrimoniae Herba, and Notoginseng Radix et Rhizoma. Follow-up efficacy analysis showed median improvement rates of 64.0% at the first follow-up, 76.0% at the second follow-up, and 78.7% at the third follow-up. Syndrome-drug association analysis revealed specific herb pairs with significant therapeutic advantages, including Notoginseng Radix et Rhizoma + Coicis Semen, Sanguisorbae Radix + Coptidis Rhizoma, and Codonopsis Radix + Aconii Lateralis Radix Praeparaia.ConclusionTCM medication patterns for treating IBD demonstrate distinct temporal evolution characteristics, with significantly increased usage frequency of herbs such as Fraxini Cortex, Notoginseng Radix et Rhizoma, and Agrimoniae Herba. Significant therapeutic method-herb associations and syndrome-herb association patterns exist, with the formation of specific herb pairs, providing evidence-based support for precise syndrome differentiation and treatment of IBD.

    Abstract:ObjectiveTo observe ovarian microvascular damage in rats with cold coagulation and blood stasis syndrome and to explore the mechanism by which Liangfang Wenjing decoction improves this condition in rats.MethodsFifty SPF female SD rats were randomly divided into a blank group, a model group, low-dose (8.1 g·kg-1) and high-dose groups (16.2 g·kg-1) of Liangfang Wenjing decoction, and a 4-phenylbutyric acid (0.1 g·kg-1) group, with 10 rats in each group. The ice-water bath method was employed to establish the rat model of cold coagulation and blood stasis syndrome. Concurrent with modeling, Liangfang Wenjing decoction was administered continuously for 21 days, once daily. The rats' syndrome manifestations and estrous cycles were recorded. The enzyme-linked immunosorbent assay (ELISA) was used to detect serum reproductive hormone levels and levels of endothelin-1 (ET-1), nitric oxide (NO), thrombomodulin (TM), and von Willebrand factor (vWF) in ovarian tissue. Prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), and fibrinogen (FIB) were measured. The ovarian microcirculatory blood perfusion was detected by laser speckle contrast imaging. Hematoxylin-eosin (HE) staining was performed to observe the ovarian histopathology, flow cytometry to detect ovarian apoptosis rate, and transmission electron microscopy to observe the ultrastructure of ovarian microvascular endothelial cells. Western blot was employed to detect the protein expression of endothelial nitric oxide synthase (eNOS), phosphorylated eNOS (p-eNOS), Caspase-3, B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), glucose-regulated protein 78 (GRP78), C/EBP homologous protein (CHOP), inositol-requiring enzyme1α (IRE1α), p-IRE1α, apoptosis signal-regulating kinase 1 (ASK1), p-ASK1, c-Jun N-terminal kinase (JNK), and p-JNK. Immunofluorescence was used to detect ovarian Bax and Bcl-2 expression in microvascular endothelial cells.ResultsCompared with the blank group, the model group showed signs of cold coagulation and blood stasis syndrome, prolonged estrus cycles, and reproductive hormone disorders. Histopathological results revealed a decrease in follicle counts at all stages and disorganized granulosa cell arrangement. Ovarian microcirculatory perfusion was significantly decreased (P<0.01). PT, APTT, and TT were reduced (P<0.05, P<0.01), while FIB levels were increased (P<0.05). In ovarian tissue, NO content was decreased, while ET-1, vWF, and TM levels were increased significantly (P<0.01). The apoptosis rate of ovarian cells was markedly increased (P<0.01). Furthermore, p-eNOS/eNOS and Bcl-2 were decreased (P<0.05), whereas Bax, cleaved-Caspase-3/Caspase-3, GRP78, CHOP, p-IRE1α/IRE1α, p-ASK1/ASK1, and p-JNK/JNK expression showed significant increases (P<0.05, P<0.01). Compared with the model group, Liangfang Wenjing decoction intervention alleviated the symptoms of cold coagulation and blood stasis, gradually restored the estrus cycle, and improved ovarian histopathology and endothelial cell ultrastructure. Microcirculatory blood perfusion was significantly elevated (P<0.05). NO content in ovarian tissue was elevated, while ET-1, vWF, and TM levels were significantly decreased (P<0.05, P<0.01). The p-eNOS/eNOS ratio and Bcl-2 expression were significantly elevated (P<0.05), while the expression of Bax, cleaved-Caspase-3/Caspase-3, GRP78, CHOP, p-IRE1α/IRE1α, p-ASK1/ASK1, and p-JNK/JNK was significantly decreased (P<0.05, P<0.01).ConclusionLiangfang Wenjing decoction may regulate the IRE1α/ASK1/JNK signaling pathway to inhibit endoplasmic reticulum stress, attenuate apoptosis, and improve microvascular endothelial injury in ovaries of rats with cold coagulation and blood stasis syndrome.

    Abstract:ObjectiveTo investigate the effect and molecular mechanism of exosomes derived from bone marrow mesenchymal stem cells(BMSC-exos) modified with Bushen Yisui capsule(BSYS)-containing serum on promoting the differentiation and maturation of OLN-93 oligodendrocytes by regulating miR-15b/Wnt signaling pathway.MethodsOLN-93 cells were divided into 5 groups, including the normal(NC) group, BMSC-exos group, BSYS-BMSC-exos group, BSYS-BMSC+LV-miR-15b-5p inhibitor-exos group, and BSYS-BMSC+LV-miR-15b-5p NC-exos group. DiR staining was used to observe the uptake of Exos by OLN-93 cells. The effective dosage of BSYS-BMSC-exos on OLN-93 cells was assessed by cell proliferation and activity assay(CCK-8). Stable BMSCs lentiviral transfection strains were established to inhibit miR-15b-5p expression in both BMSCs and their exos, and transfection efficiency was verified by real-time fluorescent quantitative polymerase chain reaction(Real-time PCR) detection of miR-15b-5p. The expressions of 2′,3′-cyclic nucleotide 3′-phosphodiesterase(CNPase) and myelin proteolipid protein(PLP) in OLN-93 cells were detected by immunocytochemistry(ICC) and Western blot. The mRNA expressions of miR-15b-5p and Wnt3a in OLN-93 cells were detected by Real-time PCR, and the protein expression of Wnt3a was measured by Western blot. The expression levels of key molecules in the Wnt/β-catenin signaling pathway of OLN-93 cells, including glycogen synthase kinase(GSK)-3β, β-catenin, and T-cell specific transcription factor 4/transcription factor 7-like 2(TCF4/TCF7L2), were measured by Real-time PCR and Western blot.ResultsDiR-labeled Exos were efficiently taken up by OLN-93 cells. The CCK-8 assay results indicated that 20 mg·L-1 of BSYS-BMSC-exos exhibited the most significant effect in enhancing OLN-93 cell viability(P<0.01) and this dosage was selected for subsequent experiments. Following lentiviral transfection of BMSCs, Real-time PCR results revealed that miR-15b-5p was significantly suppressed in BMSCs(P<0.01), and miR-15b-5p was also notably inhibited in BSYS-BMSC-exos(P<0.01). ICC analysis further revealed an increase in the number of differentiated, mature CNPase and PLP-positive cells following BSYS-BMSC-exos treatment(P<0.01). Western blot results demonstrated that the protein expression of CNPase and PLP was significantly enhanced with BSYS-BMSC-exos treatment(P<0.01). Additionally, BSYS-BMSC-exos also increased the expression levels of miR-15b-5p and p-β-catenin proteins in OLN-93 cells, while decreased the mRNA and protein expressions of Wnt3a, as well as the mRNA expressions of β-catenin and TCF4/TCF7L2, and the protein expression level of p-GSK-3β(Ser9) was significantly reduced(P<0.05, P<0.01). After the transfection of miR-15b-5p inhibitor into BSYS-BMSC-exos, the above effects were significantly diminished(P<0.05, P<0.01).ConclusionBSYS-BMSC-exos facilitate the differentiation and maturation of OLN-93 cells, and its mechanism is related to the upregulation of miR-15b-5p in OLN-93 cells, which inhibits the expression of Wnt3a and thereby suppresses the Wnt signaling pathway.

    Abstract:ObjectiveTo investigate the correlation between iron metabolism parameters and various syndrome types of unstable angina pectoris (UAP).MethodsA cross-sectional study was conducted from October 2021 to October 2023, encompassing 213 patients diagnosed with UAP at Xiyuan Hospital of Chinese Academy of Chinese Medical Sciences. Additionally, 30 healthy individuals were selected as control cases. Single-factor analysis was used to investigate the differences in clinical data among different Traditional Chinese Medicine (TCM) syndrome types of UAP and their correlation with iron metabolism indices. The study conducted a comparative analysis of the aforementioned clinical data among patients with and without heat-toxic and blood-stasis syndrome. Logistic regression was used to analyze the correlation between TCM syndrome types and related factors. The receiver operator characteristic (ROC) curve was employed to assess the predictive value of iron metabolism indices, along with their sensitivity and specificity.ResultsCompared to those in the control group, serum iron (SI) and serum ferritin (SF) levels were significantly increased in the UAP group (P<0.01), while transferrin (TRF) and total iron binding capacity (TIBC) levels were decreased (P<0.01). However, there was no significant difference in unsaturated iron binding capacity (UIBC). Multivariate binary Logistic regression analysis identified apolipoprotein A1 (ApoA1), homocysteine (HCY), high-sensitivity C-reactive protein (hs-CRP), and SF as independent influencing factors for the UAP patients (P<0.05, P<0.01). Additionally, statistically significant differences were observed in SI, SF, TRF, and TIBC among 213 patients with different TCM types (P<0.01). Patients with heat-toxic and blood-stasis syndrome had higher SI and SF values than those without the syndrome (P<0.01), while their TIBC and TRF values were lower (P<0.01). Multivariate binary logistic regression analysis showed that SI and LDL-C levels were closely associated with the differentiation of heat-toxic and blood-stasis syndrome.ConclusionUAP patients often experience iron metabolism disorders, and the heat-toxic and blood-stasis syndrome are significantly correlated with iron metabolism parameters.     Abstract:ObjectiveTo investigate the effects of Xinfeng capsules combined with chronic disease management of traditional Chinese medicine (TCM) on rapid disease control and short-term prognosis of patients with rheumatoid arthritis (RA).MethodsA total of 80 RA patients hospitalized in the Department of Rheumatology of The First Affiliated Hospital of Anhui University of Chinese Medicine from January 2022 to March 2024 were enrolled and randomly divided into an observation group (40 cases) and a control group (40 cases). The control group was treated with conventional methotrexate combined with standard chronic disease management, while the observation group was additionally treated with Xinfeng Capsules combined with TCM chronic disease management. The treatment course lasted 24 weeks. The outcomes were compared between two groups, including disease activity [28-joint disease activity score (DAS28), clinical disease activity index (CDAI), simplified disease activity index (SDAI)], visual analogue scale (VAS) for pain, TCM syndrome score, tender joint count (TJC), swollen joint count (SJC), morning stiffness duration, Health Assessment Questionnaire (HAQ), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS), American College of Rheumatology (ACR) 20%, 50% and 70% response rates (ACR20/50/70), erythrocyte sedimentation rate (ESR), high-sensitivity C-reactive protein (hs-CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide antibody (CCP-Ab), interleukin (IL)-6, IL-1β, tumor necrosis factor-α (TNF-α), and serum immunoglobulin G (IgG). The Chronic Disease Self-Management Scale (CDSMS) was used to evaluate patients’ self-management ability, self-care ability, and nursing satisfaction. Patients were followed up for 12 weeks to assess prognosis, and COX regression analysis was performed to determine the impact on short-term prognosis.ResultsAfter treatment, TJC, SJC, morning stiffness duration, DAS28, CDAI, SDAI, VAS, TCM syndrome score, ESR, hs-CRP, RF, CCP-Ab, IL-6, IL-1β, TNF-α, IgG, HAQ, SAS, SDS, chronic disease self-management behavior, self-efficacy, and self-care ability all improved significantly in both groups compared with baseline (P<0.05,P<0.01). Compared with the control group, the observation group showed more significant improvements in TJC, SJC, morning stiffness duration, DAS28, CDAI, SDAI, VAS, TCM syndrome score, ESR, IL-1β, IgG, HAQ, SAS, SDS, self-care ability, chronic disease self-management behavior, and self-efficacy (P<0.05 or P<0.01). The ACR70 response rate and nursing satisfaction were significantly higher in the observation group than in the control group (P<0.01). COX regression analysis showed that Xinfeng capsules combined with TCM chronic disease management reduced the risk of poor short-term prognosis in RA patients.ConclusionXinfeng capsules combined with TCM chronic disease management facilitates rapid disease control in RA patients, effectively improves short-term prognosis, and plays an important role in the treatment of the disease.

    Abstract:ObjectiveTo explore the effect of Yishen Tongluo prescription on sperm DNA fragmentation index (DFI) and sperm mitochondrial membrane potential (MMP) in patients with asymptomatic idiopathic asthenospermia infertility.MethodsA total of 128 patients with asymptomatic idiopathic asthenospermia were randomly assigned to an experimental group (64 cases) and a control group (64 cases). The experimental group received Yishen Tongluo prescription, while the control group was treated with Wuzi Yanzongwan combined with L-carnitine oral solution. One treatment course lasted 12 weeks. Spouse pregnancy rate, sperm progressive motility (PR), total sperm motility (PR+NP), sperm function (sperm tail hypotonic swelling rate, sperm acrosin activity), sperm DFI, and sperm MMP were compared between the two groups before and after treatment. Adverse reactions were observed and recorded during the study, and clinical efficacy and safety were systematically evaluated.ResultsA total of 121 patients completed the study, including 61 in the experimental group and 60 in the control group. The spouse pregnancy rate in the experimental group was 14.75% (9/61), higher than that in the control group at 6.67% (4/60), though the difference was not statistically significant. Clinical efficacy in the experimental group was superior to that in the control group (P<0.05). Compared with the results before treatment, sperm PR, PR + NP, sperm tail hypotonic swelling rate, sperm acrosin activity, sperm DFI, and sperm MMP were significantly improved in both groups after treatment (P<0.05), with greater improvements in the experimental group (P<0.05). However, there was no significant change in sperm concentration in either group after treatment. During the study, no abnormal safety indicators or significant adverse reactions occurred in either group.ConclusionThe kidney-tonifying and collateral-dredging method shows good clinical efficacy in the treatment of asymptomatic idiopathic asthenospermia infertility. Yishen Tongluo prescription can improve sperm motility, increase spouse pregnancy rate, enhance sperm function, and demonstrates good safety. Its mechanism may be related to reducing sperm DFI and increasing sperm MMP.

    Abstract:ObjectiveTo observe the clinical efficacy and safety of Dizhuo Huayu prescription combined with catgut embedding therapy in patients with nonalcoholic steatohepatitis (NASH) and dyslipidemia and explore the effect of the combined therapy on inflammatory cytokines interleukin (IL)-18 and IL-1β.MethodsA total of 82 patients with NASH and dyslipidemia from the Gastroenterology Department of the First Affiliated Hospital of Henan University of Chinese Medicine were randomly divided into a control group and a treatment group, with 41 patients in each group. The control group received Polyene Polyenylphosphatidylcholine Capsules, while the treatment group received Dizhuo Huayu prescription granules combined with catgut embedding. The treatment duration was 24 weeks for both groups. At weeks 0, 12, and 24, the traditional Chinese medicine (TCM) syndrome score, body mass index (BMI), liver fat content assessed by Fibroscan (CAP value), the level of alanine transaminase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transferase (GGT), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and free fatty acid (FFA), and the expression of inflammatory cytokines IL-18 and IL-1β in serum were observed. Adverse reactions in both groups were recorded.ResultsA comparison of the comprehensive therapeutic effects between the two groups after 24 weeks of treatment revealed that the total effective rate was 62.16% (23/37) in the control group and 85.71% (30/35) in the treatment group, with a statistically significant difference (χ2 = 5.14, P<0.05). At weeks 12 and 24 after treatment, the TCM syndrome score, BMI, CAP value, TC, TG, LDL-C, and FFA were all significantly lower in both groups compared to pre-treatment levels, while the HDL-C level significantly increased (P<0.05). The effect was better at week 24 (P<0.05) than at week 12 (P<0.05), and the treatment group showed better outcomes than the control group at weeks 12 and 24 after treatment (P<0.05). After 24 weeks of treatment, both groups exhibited significant reductions in IL-18 and IL-1β levels (P<0.05). The treatment group demonstrated superior efficacy compared to the control group after treatment (P<0.05). Both groups experienced decreases in ALT, AST, and GGT levels after treatment (P<0.05). However, there were no statistically significant differences between the 12-week and 24-week post-treatment values within each group, nor were there significant differences between the two groups. No significant adverse reactions were observed in both groups.ConclusionThe Dizhuo Huayu prescription combined with catgut embedding therapy is safe and effective in treating patients with NASH and dyslipidemia, exhibiting hepatoprotective, anti-inflammatory, lipid-regulating, and weight-reducing effects.

    Abstract:ObjectiveThis study aims to identify cuproptosis-related gene biomarkers in Alzheimer's disease(AD) using bioinformatics and machine learning methods, validate them at the clinical specimen level, and predict potential traditional Chinese medicine(TCM).MethodsDifferentially expressed genes in the AD group and normal group were obtained using the Gene Expression Omnibus (GEO) database, and intersections were taken with reported cuproptosis-related genes to obtain differentially expressed cuproptosis-related genes. Machine learning methods were applied to identify core differential genes of cuproptosis in AD. Peripheral blood's single nucleated cells from clinical AD patients were collected, and the relative gene expression was clinically verified by real-time polymerase chain reaction(Real-time PCR). Potential TCM regulating cuproptosis for AD were predicted by COREMINE database.ResultsA total of 12 cuproptosis-related genes were obtained, mainly involved in pathways of tricarboxylic acid cycle, 2-oxocarboxylic acid metabolism, and carbon metabolism. Five core cuproptosis-related genes, dihydrolipoamide dehydrogenase (DLD), glutaminase (GLS), pyruvate dehydrogenase E1 subunit beta (PDHB), full name nuclear factor (erythroid-derived 2)-related factor 2 (NFE2L2), and dihydrolipoamide branched-chain transacylase E2 (DBT) were finally screened using four machine methods. Thirty cases each of normal and AD patients were collected clinically. Compared with those in the normal group, minimum mental state examination (MMSE) and Montreal cognitive assessment (MoCA) were significantly decreased in the AD group (P<0.01), Homocysteine(Hcy), interleukin(IL)-6, C-reactive protein(CRP) , and β amyloid protein(Aβ) indexes were significantly increased (P<0.01), and malondialdehyde(MDA) indexes were decreased (P<0.05). Superoxide dismutase(SOD) levels were significantly decreased (P<0.01). The mRNA relative expressions of NFE2L2 and DBT were up-regulated (P<0.05), and those of DLD, GLS, and PDHB were significantly down-regulated (P<0.01). The TCM regulating cuproptosis-related genes for the treatment of AD were mainly based on the four Qi such as warmth, calmness, and cold, and the five flavors including bitterness, sweetness, and pungency, and it was attributed to the meridians of the liver, spleen, stomach, and kidney, with the efficacy of tonifying Qi, activating blood, eliminating phlegm, and resoling dampness.ConclusionDLD, GLS, NFE2L2, PDHB, and DBT can be used as novel diagnostic molecular markers for AD cuproptosis-related genes, and the corresponding potential therapeutic TCM can provide new ideas for the treatment of AD by TCM.

    Abstract:Wilson disease is a copper metabolism disorder caused by mutations in the ATP7B gene, which encodes a copper-transporting ATPase β, and can result in multisystem damage. The kidneys are the third most commonly affected organs after the liver and brain. In recent years, numerous diagnostic and treatment guidelines for Wilson disease have emerged. However, most of these focus primarily on hepatic and neurological manifestations and their management, with limited coverage of renal involvement. The high incidence, low awareness, and lack of clinical specificity of Wilson disease-related renal damage (WDRD) have made early detection and intervention particularly challenging in clinical practice. To further optimize the treatment of patients with WDRD, improve clinical diagnosis and management, and enhance patients' quality of life, the Neurology Committee of the Chinese Association of Integrative Medicine, in April 2024, initiated a revision of the first expert consensus on the integrated diagnosis, treatment, and management of WDRD. This effort brought together experts in hepatology, encephalopathy (neurology), and nephrology from many tertiary-level grade A hospitals and research institutions across China. Through comprehensive literature review and integration of frontline clinical experience, the expert group jointly developed Chinese Expert Consensus on Integrated Chinese and Western Medicine Management of Wilson Disease-related Renal Damage (hereinafter referred to as the "Consensus"). This article provides a detailed interpretation of the Consensus in terms of diagnostic criteria, traditional Chinese medicine (TCM) syndrome differentiation and treatment classification, and comprehensive disease management, aiming to better guide clinical application. Regarding diagnostic criteria, the Consensus integrates the latest standards in China and abroad, highlights the importance of biochemical diagnosis, and compensates for the limitations of genetic testing. In the area of TCM syndrome differentiation and treatment, the Consensus refines four major syndrome types, introduces a newly defined syndrome, i.e., phlegm, blood stasis, and heat accumulation, and elaborates on treatment principles, prescriptions, and clinical modification rules for each syndrome. For comprehensive disease management, the Consensus emphasizes multi-dimensional intervention strategies, including diet, exercise, emotional regulation, medication, and medical care, with the goal of maximally controlling the progression of renal dysfunction and helping patients achieve a better quality of life.

  • Compilation Instructions for Expert Consensus on Clinical Application of Dieda Huoxue Capsules AI Introduction

    Abstract:The Compilation Instructions for Expert Consensus on Clinical Application of Dieda Huoxue capsules systematically expound the development methods and evidence-based basis of this consensus. In view of the weak clinical application evidence and ambiguous indications of Dieda Huoxue capsules, the Institute of Basic Research in Clinical Medicine of the China Academy of Chinese Medical Sciences and Wangjing Hospital took the lead and collaborated with 33 experts from 28 medical institutions nationwide. They strictly followed the World Health Organization (WHO) guideline-making norms and the Grading of Recommendations Assessment, Development and Evaluations (GRADE) evidence-grading system and completed the compilation through multidisciplinary cooperation. The workflow included constructing clinical questions (19 items were screened by the nominal group technique), retrieving evidence (from Chinese and English databases and grey literature), assessing safety (integrating drug monitoring data and clinical investigations), and forming recommendations and consensus suggestions (3 recommendations were reached via the GRADE grid method, and 16 consensus suggestions were reached by the majority vote rule). The results indicate that the consensus clearly states that this medicine (Dieda Huoxue capsules) is applicable to conditions like traumatic injury, blood stasis-induced pain, and sudden lumbar sprains. The recommended dose is 6 capsules each time, twice a day. Combining oral administration with external application can enhance the efficacy, and elderly patients should take the medicine at intervals. Safety monitoring suggests that it should be used with caution in people with a bleeding tendency and those with an allergic constitution. The compilation process involved three rounds of reviews by internal and external experts. Literature analysis, the Delphi method, and clinical applicability tests were employed to ensure methodological rigor. The compilation instructions comprehensively present key aspects such as project approval and registration, conflict-of-interest statements, and evidence evaluation through 12 appendices, providing methodological support for the clinical translation of the consensus. In the future, it will be continuously improved through a dynamic revision mechanism.

    Abstract:ObjectiveTo compare the contents and relative molecular weight distributions of proteins and polypeptides in Naoxuekang dropping pills, Huoxue Tongmai capsules and Maixuekang capsules of Hirudo single medicinal preparations, to evaluate the in vitro anticoagulant, antiplatelet and fibrinolytic activities of the three preparations, and to investigate the effects of temperature, pH and digestive enzymes on the anticoagulant activities of the three preparations.MethodsThe contents of soluble proteins and polypeptides in the three preparations were determined by bicinchoninic acid assay(BCA) and Bradford method, and the relative molecular weight distributions of the three preparations were determined by electrophoresis combined with gel chromatography. The antithrombin activity of the three preparations was evaluated by fibrinogen-thrombin time(Fibg-TT) method, and their anticoagulant activities were further assessed by the elongations of activated partial thromboplastin time(APTT), prothrombin time(PT) and thrombin time(TT). The antiplatelet aggregation activities of the three preparations were measured by turbidimetry and the fibrinolytic activities were measured by fibrin plate method. Relative TT was used as index to investigate the effects of temperature, pH and digestive enzyme buffer on anticoagulant activities of the three preparations.ResultsAt the lowest single dosage, the contents of proteins and polypeptides were in the order of Maixuekang capsules>Huoxue Tongmai capsules>Naoxuekang dropping pills. Both Huoxue Tongmai capsules and Maixuekang capsules had 11 electrophoretic bands between 4.0 kDa and 90 kDa, the bands of Maixuekang capsules were more clear in the range of >25 kDa, and there was 1 obvious band at 14 kDa for the two capsules. Huoxue Tongmai capsules had one specific band at 9.0 kDa and Maixuekang capsules had one specific band at 48.0 kDa. Naoxuekang dropping pills only had 2 electrophoretic bands at 6.5 kDa and 8.5 kDa, primarily containing peptides below 2 kDa, most of which were oligopeptides. The anticoagulant activity concentrations of the three preparations exhibited a certain dose-dependent effect. At the lowest single dosage, The anticoagulant activity concentrations were ranked as Naoxuekang dropping pills>Huoxue Tongmai capsules>Maixuekang capsules. The prolongation effect of the three preparations on coagulation time was dose-dependent. At the same concentration, the prolongation effect of Naoxuekang dropping pills and Huoxue Tongmai capsules was APTT prolongation rate>TT prolongation rate>PT prolongation rate, whereas for Maixuekang capsules, the sequence was TT prolongation rate>APTT prolongation rate>PT lengthening rate. At the single minimum dosage, the order of APTT prolongation rate was Maixuekang capsules>Huoxue Tongmai capsules≈Naoxuekang dropping pills, the order of PT prolongation rate was Naoxuekang dropping pills≈Maixuekang capsules>Huoxue Tongmai capsules, and the order of TT prolongation rate was Maixuekang capsules>Huoxue Tongmai capsules>Naoxuekang dropping pills. The three preparations showed dose-dependent effects on platelet aggregation induced by adenosine diphosphate(ADP) and arachidonic acid(AA), and the effect induced by ADP was stronger than that induced by AA. The anti-platelet aggregation effect of Naoxuekang dropping pills was significantly stronger than that of Maixuekang capsules(P<0.01), whereas Huoxue Tongmai capsules had the effect of promoting platelet aggregation. None of the three preparations had the ability to dissolve fibrin. The anticoagulant activity of Naoxuekang dropping pills was least affected by heating, while the activities of the two capsules decreased significantly within 5 min above 80 ℃, and continued to decrease within 2 h. Compared with pure water, the anticoagulant activities of the three preparations could be increased by 1-3 times under strong acidity(pH 1-3). In the pepsin buffer, the anticoagulant activity of Naoxuekang dropping pills could be increased by 1-3 times, while the anticoagulant activities of Huoxue Tongmai capsules and Maxuekang capsules were significantly decreased, the lowest levels were about 60% and 20%, respectively. In trypsin buffer, the anticoagulant activities of Naoxuekang dropping pills, Huoxue Tongmai capsules and Maixuekang capsules decreased significantly, and the lowest levels decreased to about 41%, 41% and 35%, respectively.ConclusionThe contents of proteins and polypeptides and relative molecular weights of the preparations derived from lyophilized fresh Hirudo powder, dried Hirudo powder and reflux extract of Hirudo decrease sequentially, and the anticoagulant activity decrease gradually, but the anticoagulant pathway is different. And the anti-platelet aggregation activity of the reflux extract is significantly enhanced. The heat resistance and gastrointestinal stability of the three preparations increase successively, and the first two are suitable for enteric-soluble preparations, while the latter is suitable for routine oral administration. The above results can provide data reference for the rationality of different preparation methods, active substances, pharmacodynamics and mechanism of Hirudo preparations.

    Abstract:ObjectiveTo assess the quality consistency of black Panacis Quinquefolii Radix(bPQR) decoction pieces prepared by atmospheric and pressurized steaming processes based on chromaticity-chemical composition-vasoactive inhibition. The ultimate goal was to screen the pressurized steaming process yielding quality equivalent to atmospheric steaming, and optimize the processing technology of bPQR.MethodsThe bPQR decoction pieces were prepared using both atmospheric and pressurized steaming processes, and the chromaticity values[lightness value(L*), red/green chromaticity value(a*), yellow/blue chromaticity value(b*), total chromaticity value(E*ab)] were measured. High performance liquid chromatography(HPLC) was employed to establish fingerprint profiles for the decoction pieces, and cluster analysis was conducted on chromaticity values and the common peak areas in fingerprint profiles to elucidate the quality relationships between the decoction pieces processed by different methods. The optimal atmospheric steaming of bPQR decoction pieces was determined through zebrafish angiogenesis inhibition experiments. The contents of ginsenosides Rg1, Re, Rb1, 20(S)-Rg3, Rk1 and Rg5 in the decoction pieces were quantified, and Spearman correlation analysis was employed to investigate the relationship between saponin content, chromaticity, and angiogenesis inhibition activity during the steaming process. By integrating the consistency of chromaticity, saponin components and angiogenesis inhibition activity, pressurized steaming conditions with quality equivalent to the atmospheric pressure method were selected.ResultsCompared with the atmospheric steaming method, pressurized steaming resulted in faster color darkening and higher conversion rates of ginsenosides in bPQR decoction pieces. Moreover, the neovascularization inhibitory activity of bPQR decoction pieces continued to increase with the deepening of processing. Based on the effectiveness and safety, the optimal process for preparing bPQR decoction pieces with neovascularization inhibitory activity was determined to be atmospheric steaming for 21 h. All six ginsenosides tested exhibited strong to extremely strong correlations with both the chromaticity values of the decoction pieces and their neovascularization inhibitory activities. Among them, ginsenosides Rg1, Re and Rb1 exhibited positive correlations with chromaticity values and negative correlations with zebrafish angiogenesis inhibition activity. Conversely, ginsenosides 20(S)-Rg3, Rk1 and Rg5 showed negative correlations with chromaticity values and positive correlations with zebrafish angiogenesis inhibition activity. By integrating chromaticity values, cluster analysis results, as well as the results of activity, it was determined that the quality of bPQR decoction pieces steamed under pressurized conditions of 110 ℃(0.045 MPa) for 5 h and 115 ℃(0.07 MPa) for 3 h was highly consistent with that obtained by atmospheric steaming for 21 h.ConclusionThe preparation of bPQR decoction pieces by pressurized steaming has the advantages of short preparation time, low energy consumption, and rapid saponin conversion rate, making it a viable alternative to atmospheric steaming for preparing bPQR decoction pieces. Meanwhile, the evaluation method based on chromaticity-chemical composition-activity can provide a more scientific and effective explanation of change rules in the quality during traditional Chinese medicine processing, and offer a new model for optimizing processing technology and enhancing quality control.

    Abstract:ObjectiveTo investigate the immunomodulatory effect of polysaccharides from Astragali Radix-Angelicae Sinensis Radix herb pair(Qi-gui polysaccharides) on lipopolysaccharide(LPS)-induced RAW264.7 macrophages and to characterize the structure of the active component Qi-gui homogeneous polysaccharide(AAPS-4a), and evaluate its protective effect on ulcerative colitis(UC).MethodsThe effects of six Qi-gui polysaccharides(0.01-100 mg·L-1) on the proliferation of RAW264.7 cells were assessed by cell proliferation and activity assay(CCK-8), and enzyme-linked immunosorbent assay(ELISA) was used to investigate the effects of the six polysaccharides(3, 10 mg·L-1) on the secretion levels of tumor necrosis factor(TNF)-α, interferon(IFN)-β, and nitric oxide(NO) in LPS-induced RAW264.7 cells. After screening for active polysaccharides, high-performance size-exclusion chromatography(HPSEC) was used to determine its homogeneity and relative molecular weight, then its characteristic functional groups were identified by Fourier transform infrared spectroscopy(FT-IR), monosaccharide composition was analyzed by high performance liquid chromatography(HPLC), methylation analysis combined with gas chromatography-mass spectrometry(GC-MS) was performed to determine the types and linkage modes of sugar residues, and one- and two-dimensional nuclear magnetic resonance(NMR) were used to identify the sugar residue composition and configuration of the active polysaccharide. Finally, experimental animals were divided into the normal group, model group, AAPS-4a low-dose group(50 mg·kg-1), AAPS-4a high-dose group(100 mg·kg-1), and sulfasalazine(SASP) group (75 mg·kg-1). Except for the normal group, the acute UC mouse model was induced using 3.5% dextran sulfate sodium salt(DSS). Each treatment group was administered the corresponding dose via oral gavage for 7 days, and changes in body weight were recorded. After treatment, the spleen index and disease activity index(DAI) score were calculated, TNF-α and interleukin-6(IL-6) levels in the serum were detected by ELISA, and histopathological changes in colon tissue were observed by hematoxylin-eosin(HE) staining.ResultsAt the cellular level, AAPS-4a exhibited a dose-dependent inhibition of LPS-induced increases in TNF-α, IFN-β, and NO levels(P<0.01). Structural characterization of AAPS-4a revealed that it was a homogeneous polysaccharide with a relative molecular weight of 7.6×103 Da, consisting of mannose(Man), glucose(Glc), and galactose(Gal) in a molar ratio of 1.3∶23.9∶1.0. It was primarily composed of five sugar residues of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp, and 1,2-α-D-Galp. In vivo experiments showed that compared with the normal group, the model group demonstrated markedly increased DAI score and spleen index, significantly reduced colon length, and significantly elevated levels of TNF-α and IL-6(P<0.01). Compared with the model group, the AAPS-4a high-dose group significantly reduced the DAI score and spleen index, as well as TNF-α and IL-6 levels, and improved colonic atrophy(P<0.05, P<0.01). Pathological observations showed that AAPS-4a significantly inhibited inflammatory cell infiltration in colon tissue and alleviated pathological damage.ConclusionAAPS-4a, a neutral homogeneous polysaccharide composed of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp and 1,2-α-D-Galp, is identified as a key bioactive component contributing to the anti-UC effect of the Qi-gui herb pair. Its immunoregulatory and anti-UC properties suggest its potential as a therapeutic agent for UC.

    Abstract:Houpo Sanwutang, included in the Catalogue of Ancient Classical Prescriptions (Second Batch), was first recorded in the Synopsis of Golden Chamber written by ZHANG Zhongjing from the Eastern Han dynasty and was modified by successive generations of medical experts. A total of 37 pieces of effective data involving 37 ancient Chinese medical books were retrieved from different databases. Through literature mining, statistical analysis, and data processing, combined with modern articles, this study employed bibliometrics to investigate the historical origin, composition, decoction methods, clinical application, and other key information. The results showed that the medicinal origin of Houpo Sanwutang was clearly documented in classic books. Based on the conversion of the measurements from the Han Dynasty, it is recommended that 110.4 g Magnolia Officinalis Cortex, 55.2 g Rhei Radix et Rhizoma, and 72 g Aurantii Fructus Immaturus should be taken. Magnolia Officinalis Cortex and Aurantii Fructus Immaturus should be decocted with 2 400 mL water first, and 1 000 mL should be taken from the decocted liquid. Following this, Rhei Radix et Rhizoma should be added for further decoction, and then 600 mL should be taken from the decocted liquid. A single dose of administration is 200 mL, and the medication can be stopped when patients restore smooth bowel movement. Houpo Sanwutang has the effect of moving Qi, relieving stuffiness and fullness, removing food stagnation, and regulating bowels. It can be used in treating abdominal distending pain, guarding, constipation, and other diseases with the pathogenesis of stagnated heat and stagnated Qi in the stomach. The above results provide reference for the future development and research of Houpo Sanwutang.

    Abstract:Under the background of the Healthy China strategy, the integration of traditional Chinese medicine (TCM) into the public health emergency response system has become an important measure to enhance the capacity for coping with public health emergencies. In recent years, the role of TCM in responding to such emergencies has become increasingly prominent. Taking major emerging epidemics as an example, TCM has developed a rich theoretical system and practical experience in epidemic prevention and treatment over thousands of years, and has played a significant role in successive outbreaks with its unique advantages. Based on the concept of ''preventing disease before its onset'' and the theoretical framework of treatment based on syndrome differentiation, TCM has achieved remarkable results through early intervention and full participation in the integrated model of TCM and Western medicine, from severe acute respiratory syndrome (SARS) to corona virus disease-2019 (COVID-19), in improving clinical symptoms and outcomes, reducing adverse reactions, and promoting recovery. From the perspective of the Healthy China strategy, this paper systematically reviews the historical development of TCM in epidemic prevention and treatment, with particular attention to recent epidemics such as SARS, influenza A (H1N1), and COVID-19. It further examines the similarities and differences between TCM and Western medicine in responding to major emerging epidemics, as well as relevant policies related to TCM in epidemic prevention and control. In addition, it summarizes the existing problems in TCM's role in the prevention and treatment of major emerging epidemics, and explores measures to improve its rapid response capacity under the Healthy China strategy. This study not only provides a ''Chinese solution'' for the prevention and control of newly emerging infectious diseases worldwide, but also offers theoretical and practical references for strengthening the public health emergency response system, carrying strategic significance for promoting the modernization and internationalization of TCM.

    Abstract:As an important part of Chinese traditional culture, the processing technology of traditional Chinese medicine(TCM) carries the wisdom of TCM for thousands of years, and its process is complex and rigorous. With the popularization of modern production technology, traditional processing techiques are facing the dual pressures from technological innovation and production standardization under the perspective of intangible cultural heritage. The modernization of TCM processing technology is an inevitable trend for industrial upgrading, but it cannot be separated from the foundation of traditional skills and ignore the core concepts and cultural values it embodies. Therefore, by analyzing the core characteristics of TCM processing technology and its differences with modern production, this paper discusses the establishment of a synergistic innovation mechanism between traditional techniques and modern technologies, the promotion of joint research and development between scientific research institutes and the industry, the strengthening of standardization of processing techniques, and the enhancement of social education and industry training to improve the recognition and inheritance of processing techniques in order to achieve the goal of innovation and protection of TCM processing technologies in the context of modernization, and to promote the high-quality development of the TCM processing industry.

    Abstract:To implement the instructions and directives of General Secretary Xi Jinping on important documents related to traditional Chinese medicine (TCM), adhere to the equal emphasis on TCM and Western medicine, and promote mutual complementarity, win-win cooperation, and coordinated development between TCM and Western medicine, the China Association of Chinese Medicine has organized seminars on the clinical dominant diseases of TCM in multiple professional fields, achieving a series of results. The aim is to implement the research on dominant diseases of TCM described in the Law of the People's Republic of China on Traditional Chinese Medicine and the Opinions of the Central Committee of the Communist Party of China and the State Council on Promoting the Inheritance, Innovation and Development of Traditional Chinese Medicine, thus serving the construction of advantageous specialties, clinical talent cultivation, national scientific and technological layout, and academic innovation leadership. In the field of otolaryngology, the China Association of Chinese Medicine and many Chinese and Western medicine experts across the country have conducted multiple in-depth discussions and research. At the 10th Clinical Dominant Disease Series Youth Salon, they have elaborated and demonstrated the basic and specific suggestions and consensus on the clinical advantages of TCM and integrated TCM and Western medicine in the treatment of allergic rhinitis (AR), making great progress. However, there is still a lack of detailed research paths. Under the guidance of the China Association of Chinese Medicine and based on the TCM Dominant Disease Series Salon for AR, this study analyzed the difficulties encountered in AR diagnosis and treatment according to the occurrence and development law of AR. Based on the advantages and characteristics of AR diagnosis and treatment by TCM and integrated TCM and Western medicine, it explored the research paradigm and technological layout points of AR and put forward suggestions. The layout involved four aspects: Optimization of the AR diagnosis and treatment system, prevention and control of comorbidities, management of chronic diseases, and a platform for TCM inheritance. Additionally, suggestions for layout and research directions, expected goals and values, and priority levels for funding were proposed. The study is expected to provide a theoretical basis and development ideas for the future prevention and treatment of AR with TCM and integrated TCM and Western medicine and promote the high-quality development of TCM.

  • Anti-tumor Effect and Mechanism of Active Ingredients from Yin-nourishing Chinese Herbs: A Review AI Introduction

    Abstract:Tumor has become a major disease that seriously threatens human health and life. The incidence rate is increasing year by year, yet the underlying mechanisms remain incompletely understood. Traditional Chinese medicine (TCM), a treasure of the Chinese nation and a wealth for people worldwide, plays an important role in the treatment of tumors and has been receiving increasing attention both in China and abroad. In earlier work, based on the symptoms and metastatic characteristics of tumors, and drawing on the TCM theory of Yin and Yang in combination with modern medical research on tumors, the ''Yin deficiency-cancer correlation'' hypothesis was proposed. This hypothesis holds that ''Yin deficiency'' of the body is a major cause of malignant tumors, and that nourishing Yin to eliminate the pathogenic factor of Yin deficiency can treat cancer. By using Yin-nourishing drugs to tonify Yin deficiency, the occurrence and development of malignant tumors can be effectively prevented. Common anti-tumor Yin-nourishing drugs include Glehniae Radix, Lilii Bulbus, Ophiopogonis Radix, Liriopes Radix, Asparagi Radix, Dendrobii Caulis, Dendrobii Officinalis Caulis, Polygonati Odorati Rhizoma, Polygonati Rhizoma, Lycii Fructus, Mori Fructus, Ligustri Lucidi Fructus, Ecliptae Herba, Rehmanniae Radix, and Anemarrhenae Rhizoma. These drugs are generally sweet in flavor, cold and cool in nature, and moist in texture. They have the functions of nourishing Yin fluids, generating body fluids, and moistening dryness, and can also clear heat, being primarily indicated for Yin deficiency with depletion of body fluids. In view of the potential advantages and value of treating malignant tumors by tonifying Yin deficiency with Chinese medicine, this paper reviews recent studies on the anti-tumor effects of active components of Yin-nourishing drugs. It further summarizes their mechanisms of action in inducing apoptosis of tumor cells, arresting tumor cell proliferation, inhibiting tumor invasion, metastasis, and angiogenesis, enhancing and regulating immune function, augmenting the efficacy of chemotherapeutic drugs, and reversing tumor drug resistance. This study provides an objective overview of research progress on Yin-nourishing drugs in tumor treatment and offers new ideas for cancer therapy.

  • Coptidis Rhizoma and Its Prescriptions in Treatment of Diabetic Cognitive Impairment: A Review Enhanced Publication AI Introduction

    Abstract:Diabetic cognitive impairment (DCI) has an insidious onset and progressive and irreversible development. There is currently no first-line treatment for DCI. Early intervention of diabetes with traditional Chinese medicine (TCM) can effectively control blood sugar and improve cognitive impairment, which has significant advantages. As a representative of bitter and cold heat-clearing medicines, Coptidis Rhizoma, known for its abilities to clear heat and dampness and remove turbidity and toxins, has been widely used in the clinical prevention and treatment of diabetes, Alzheimer's disease, vascular dementia, and other cognitive impairments. This article systematically summarized relevant literature and observed that Coptidis Rhizoma has shown good potential in the prevention and treatment of DCI with its active ingredients such as berberine and quercetin, drug pairs such as Coptidis Rhizoma-Scutellariae Radix, Coptidis Rhizoma-Acorus Tatarinowii Rhizoma, Coptidis Rhizoma-Pinelliae Rhizoma, Coptidis Rhizoma-Zingiberis Rhizoma, and prescriptions such as Gegen Qinliantang, Huanglian Jiedutang, Banxia Xiexintang, Huanglian Wendantang, Jiaotai Wan, Danggui liuhuangtang, and related Chinese patent medicines. Its mechanism may be related to regulating glucose metabolism, improving insulin resistance, improving amyloid β-protein (Aβ) deposition and tau protein phosphorylation, inhibiting neuroinflammation and oxidative stress, and regulating the "microbe-gut-brain axis". The article systematically reviewed the research progress of Coptidis Rhizoma and its prescriptions in the prevention and treatment of DCI, aiming to preliminarily explain the scientific connotation of Coptidis Rhizoma and provide a basis for its clinical application in the prevention and treatment of DCI.

  • Compatibility Effect and Mechanism of Astragali Radix-Curcumae Rhizoma on Antitumor: A Review Enhanced Publication AI Introduction

    Abstract:Malignant tumor is a serious and difficult disease threatening human health, which has a high morbidity and mortality rate worldwide. Traditional Chinese medicine has unique advantages in improving the therapeutic effect of malignant tumors and alleviating adverse reactions. Traditional Chinese medicine believes that Qi deficiency and blood stasis are important pathogeneses in the development of malignant tumors, and the method of supplementing Qi and activating blood is an effective strategy for treating malignant tumors. Astragali Radix, sweet in taste and warm in nature, has effects of tonifying Qi and rising Yang, strengthening the exterior and reducing sweat, promoting fluid and nourishing blood. Curcumae Rhizoma, acrid and bitter in taste and warm in nature, has the effects of promoting Qi and breaking blood stasis, eliminating mass, and relieving pain. Astragali Radix-Curcumae Rhizoma, as the classic herb pair of invigorating Qi and activating blood, has a clear effect on inhibiting tumor growth and metastasis. Studies have shown that Astragali Radix-Curcumae Rhizoma contains astragalus polysaccharide, astragaloside, calycosin, formononetin, curcumin, β-elemene, curcumenol, curcumenone, curcumendione, gemacrone, and other anti-tumor active ingredients. It can significantly inhibit the occurrence and development of liver cancer, colorectal cancer, gastric cancer, lung cancer, ovarian cancer, cervical cancer, breast cancer, and other cancers and has the advantages of superposition effect, synergistic complementarity, and increased dissolution compared with single herb and monomer of Chinese traditional herbs and has been widely valued in the field of TCM anti-cancer. Its anti-tumor mechanism includes inhibition of tumor cell proliferation, promotion of tumor cell apoptosis and autophagy, anti-invasion and metastasis, regulation of immune function, and enhancement of anti-tumor drug sensitivity. By combining Chinese and foreign literature, the compatibility effect and anti-tumor mechanism of Astragali Radix-Curcumae Rhizoma were summarized, and then scientific compatibility of these two herbs was expounded, in order to provide a useful reference for clinical application and future research of Astragali Radix-Curcumae Rhizoma.

  • Wumeiwan in Treatment of Ulcerative Colitis: A Review Enhanced Publication AI Introduction

    Abstract:Ulcerative colitis (UC) is an idiopathic,chronic inflammatory disease of the colonic mucosa that originates in the rectum and is an important contributing factor in the development of clinical conditions such as small bowel obstruction and rectal cancer. In modern clinical treatment of UC,equal emphasis is placed on both Chinese and Western medicine,demonstrating certain advantages in inducing remission and preventing recurrence. Wumeiwan (WMW) originates from Article 338 of the Jueyin section in Zhang Zhongjing's Treatise on Cold Damage and Miscellaneous Diseases during the Han Dynasty. The original text states that "for roundworm reversal,WMW is indicated. It is also used for chronic diarrhea". It is a well-established prescription for treating prolonged diarrhea and dysentery,with functions of regulating Qi and blood,unblocking the triple energizers,and clearing heat in the upper while warming the lower body. Studies have shown that WMW has favorable effects in the prevention and treatment of UC. This review summarizes research on the indications and pharmacological basis of WMW in treating UC,as well as its clinical applications and underlying mechanisms. Findings indicate that WMW,whether used as the original formula,modified,or in combination with chemical drugs,compound prescriptions,or acupuncture and moxibustion,can exert therapeutic effects by resisting oxidative stress,reducing inflammatory factor levels,inhibiting apoptosis,enhancing immune responses,regulating the intestinal microbiota,and suppressing signaling pathways such as Toll-like receptor 4 (TLR4)/nuclear factor-kappa B (NF-κB)/myeloid differentiation factor 88 (MyD88),Janus kinase (JAK)/signal transducer and activator of transcription (STAT),and Notch. These mechanisms provide a theoretical basis for the clinical application of WMW in the treatment of UC.
