目的: 探讨痰热清注射液对呼吸机相关性肺炎高热患者C反应蛋白(CRP)和感染参数的影响。 方法: 选择胜利石油管理局汽修卫生院56例呼吸机相关性肺炎高热患者,按数字法随机分为对照组(n=28)和观察组(n=28),对照组采用常规治疗包括盐酸左氧氟沙星0.4 g静滴,qd,干扰素-γ,皮下注射,1次/d,12周后改隔天1次;观察组在此基础上加用痰热清注射液静滴,1个疗程用药10 d,观察两组治疗前后C反应蛋白和感染参数等指标的变化。 结果: 观察组与对照组临床有效率分别为92.86%(26/28),75.00%(21/28),具有统计学差异(P<0.05)。观察组治疗前CRP含量为(43.12±0.75) mg·L-1,NT-proBNP含量为(1 872.99±11.25)ng·L-1;治疗后CRP含量为(11.51±0.22) mg·L-1,NT-proBNP含量为(673.52±10.71)ng·L-1;对照组治疗前CRP含量为(44.82±0.92) mg·L-1,NT-proBNP含量为(1 863.02±11.93)ng·L-1;治疗后CRP含量为(19.64±0.84) mg·L-1,NT-proBNP含量为(894.12±10.73)ng·L-1;治疗后两组CRP均呈降低趋势,观察组降低优于对照组(P<0.05);观察组治疗后中性细胞数(GRAN)为(3.33±0.34)×109/L、白细胞(WBC)为(1.52±0.71)×109/L、淋巴细胞比率(LYM)为(51.61±7.64)%;对照组治疗后中性细胞数(GRAN)为(5.92±0.15)×109/L、白细胞(WBC)为(2.92±0.04)×10-9/L、淋巴细胞比率(LYM)(66.18±6.15)%;与治疗前比较,两组GRAN,WBC,LYM等指标均有明显降低(P<0.01),观察组降低程度显著优于对照组(P<0.05)。 结论: 呼吸机相关性肺炎高热患者采用痰热清静滴可以有效地抑制炎症反应,起到有效抗菌的效果。
