目的:研究健脾活血方对胃癌前病变(PLGC)大鼠胃黏膜组织中环氧合酶-2(COX-2)及基质金属蛋白酶-2(MMP-2)表达的影响。方法:60只大鼠随机分为2组:正常组10只和造模组50只。正常组正常喂养,余下大鼠每天给予150 mg·L-1的甲基硝基亚硝基胍(MNNG)溶液自由饮用,0.03% 雷尼替丁加入170 mg·L-1的MNNG溶液中按10 mL·kg-1每天灌胃1次,40%的乙醇按10 mL·kg-1 每周灌胃2次,连续12周,其中第5周开始结合饥饱失常,建立胃癌前病变大鼠模型。将造模成功的40只大鼠随机分为模型组(0.9%氯化钠溶液)、胃复春组(0.86 g·kg-1)、健脾活血方高、中、低剂量组(32,16,8 g·kg-1),每组8只,每组每天给予不同药物灌胃一次,连续10周。实验第22周末处死大鼠,给予相应处理后,快速免疫组化检测COX-2及MMP-2表达情况。结果:与正常组相比,模型组COX-2,MMP-2表达均显著升高(P<0.01);与模型组相比,健脾活血方高、中、低剂量组、胃复春组COX-2,MMP-2表达均显著降低(P<0.01或P<0.05);与胃复春组相比,健脾活血方高剂量组COX-2,MMP-2表达均显著降低(P<0.05),中、低剂量组COX-2,MMP-2表达与胃复春组相比无明显差异。结论:健脾活血方治疗胃癌前病变与降低胃黏膜中COX-2及MMP-2的表达相关。
Objective: To investigate the effects of Jianpi Huoxue decoction on cyclooxygenase-2(COX-2) and matrix metalloproteinase-2 (MMP-2) expression in rats with precancerous lesions of gastric cancer(PLGC). Method: Sixty rats were randomly divided into the normal group(10) and the modeling group(50).Except normal group
PLGC model was established in other rats for 12 weeks with following. Method: 150 mg·L-1 N-methyl-N-nitro N-nitrosoguanidine(MNNG) solusion was drunk freely
170 mg·L-1 MNNG which contained 0.03% ranitidine at dose of 10 mL·kg-1 was administered once a day
40% alcohol (10 mL·kg-1)was administered twice a week
and irregular diet was given from the 5th week.Forty rats after modeling were randomly divided into the model control group(0.9%NS)

8 rats in each group.Each group was given corresponding medicines once a day for 10 weeks.All of the rats were sacrificed by the 22th weekend.The expression of COX-2 and MMP-2 was detected by rapid immunohistochemistry. Result: Compared with the normal group
the COX-2 and MMP-2 expressions were significantly increased in the model group(P<0.01)
respectively. Compared with the model group
the COX-2 and MMP-2 expressions were significantly decreased in the high-dose
middle-dose
low-dose Jianpi Huoxue decoction groups and Weifuchun group(P<0.01 or P<0.05)
respectively.Compared with the Weifuchun group
the COX-2 and MMP-2 expressions were significantly decreased in the high-dose of Jianpi Huoxue decoction group(P<0.05)
respectively.But there was no significant difference in the the COX-2 and MMP-2 expression between the Weifuchun group and the middle-dose and low-dose Jianpi Huoxue decoction groups. Conclusion: The therapeutic effect of Jianpi Huoxue decoction is associated with the reduction of COX-2 and MMP-2 expression in gastric mucosa.
