目的:研究乌头汤的镇痛作用及相关机制。方法:健康SD大鼠,随机分为正常组、模型组、乌头汤高、中、低剂量组以及布洛芬组,除正常组外其余各组建立慢性炎症痛模型,造模时在SD大鼠右踝关节腔部位注入完全弗氏佐剂(CFA)。乌头汤高、中、低剂量组于造模当天分别给予乌头汤4,2,1 g·kg-1,ig,布洛芬组30 mg·kg-1,ig,每日 1 次,连续 6 d。用游标卡尺测量大鼠患足踝关节肿胀度,冷板仪测量患足冷板痛阈值,爪触觉测试仪测量机械痛阈值,热测痛仪测量热辐射痛阈值,酶联免疫法检测血浆中前列腺素E2(PGE2)和5-羟色胺(5-HT)的水平,免疫组织化学检测背根神经节中瞬时感受器电位香草酸受体1(TRPV1)、瞬时感受器电位作用受体蛋白M8(TRPM8)的水平。结果:CFA致炎后,大鼠踝关节出现明显肿胀,冷板痛阈值、机械痛阈值和热辐射痛阈值明显降低;血浆中PGE2,5-HT含量明显升高;背根神经节中TRPV1的表达量明显升高,TRPM8的表达量明显降低。与模型组相比,乌头汤高、中剂量组能降低大鼠关节肿胀度(P<0.05),乌头汤各剂量组能升高冷板痛阈值(P<0.01或P<0.05)、高、中剂量组能提高机械痛阈值(P<0.01,P<0.05),高剂量组对热辐射痛阈值也有一定的增高作用(P<0.05),同时,能降低血浆中的PGE2,5-HT和背根神经节中的TRPV1表达水平,提高TRPM8表达水平。结论:乌头汤具有镇痛作用,其机制可能与降低血浆中PGE2和5-HT水平以及调节背根神经节中TRPV1和TRPM8的表达有关。

5-HT and TRPV1 are decreased while the level of TRPM8 is increased after the treatment of Wutou decoction. Conclusion: Wutou decoction has the analgesic effect
and the mechanism may be related to inhibiting the level of PGE2 and 5-HT in plasma and regulating the expression of TRPV1 and TRPM8 in DRG.
