目的: 观察血府逐瘀汤对冠心病血瘀模型家兔心室重构、心功能的影响并讨论其作用相关机制。 方法: 60只健康家兔随机分为对照组、假手术组、模型组、血府逐瘀汤低、中、高剂量组6组,每组10只。模型组采用左冠状动脉放置缩窄环方法,制备冠心病血瘀模型;假手术组开胸但不放置缩窄环;在模型组基础上,术后第12周起,每天早上8点血府逐瘀汤低、中、高剂量组分别按10,20,40 g·kg-1 ig,1次/d,连续21 d。对照组、假手术组、模型组则ig等体积生理盐水。记录左心室舒张末期容积(LVEDV),左室收缩末期容积(LVESV),左心室舒张末期内径(LVDd), 收缩末期内径(LVDs), 每搏量(SV),左心室射血分数(LVEF),检测左心室质量指数(LVWI)、右心室质量指数(RVWI),RT-PCR法检测心肌细胞α-肌球蛋白(α-MHC)mRNA,β-肌球蛋白(β-MHC)mRNA表达,ELISA法检测心房利尿钠肽(ANP)、血管紧张素Ⅱ(AngⅡ)水平,细胞色素C还原试验检测还原型辅酶Ⅱ(NADPH)氧化酶(NADPH oxidase)活性。 结果: 和对照组比较,模型组AngⅡ水平显著升高(P<0.01),NADPH oxidase活性显著增高(P<0.01), α-MHC mRNA表达显著下降(P<0.01),β-MHC mRNA表达显著升高(P<0.01),同时,心功能指标LVEDV,LVESV, LVWI,RVWI均升高(P<0.05),LVDs,SV,LVEF 显著下降(P<0.01)。和模型组比较,低剂量组ANP降低(P<0.05);中剂量组NADPH oxidase活性降低(P<0.05),AngⅡ,ANP均显著降低(P<0.01),高剂量组AngⅡ,NADPH oxidase活性,ANP均显著降低(P<0.01),同时,在低剂量组各心功能指标有改善,而以中剂量组、高剂量组心功能指标LVEDV,LVESV, LVWI,RVWI,LVDs,SV,LVEF改善明显(P<0.05 或P<0.01)。 结论: 血府逐瘀汤能恢复冠心病血瘀证AngⅡ分泌水平,推测其改善心功能和心室重构功效可能部分通过AngⅡ介导调节NADPH oxidase活性,ANP,α-MHC/β-MHC水平而实现。
Objective: To observe the effects of Xuefu Zhuyu decoction on ventricular remodeling and cardiac function in the rabbits with coronary heart disease at the stage of blood stasis syndrome and discuss its therapeutic mechanisms. Method: Sixty healthy rabbits were randomly divided into control group

model group
and Xuefu Zhuyu decoction low-dose
middle-dose
high-dose groups six groups with ten rabbits in each group. Left coronary artery was implanted constrictor ring to establish the model of coronary heart disease at the stage of blood stasis syndrome. The sham-operation group was just opened the chest but not implanted the constrictor ring. On the basis of the model group from the first day of the twelfth-week after operation

right ventricular weitht index(RVWI).To examine the levels of atrial natriuretic peptide(ANP)and angiotensin Ⅱ(AngⅡ)by ELISA and the expression of α-myosin heavy chain(α-MHC)mRNA and β-myosin heavy chain(β-MHC) mRNA by RT-PCR. To test nicotinamide adenine dinucleotide phosphate oxidase(NADPH oxidase) activity by revivification test of cytochrome C. Result: Compared with the control group
in the model group the levels of AngⅡ increased significantly(P<0.01)
NADPH oxidase activity increased significantly(P<0.01)
the expression of α-MHC mRNA reduced significantly(P<0.01)while the expression of β-MHC mRNA increased significantly(P<0.01)

Conclusion: Xuefu Zhuyu decoction could recover the level of AngⅡ in the rabbits with coronary heart disease at the stage of blood stasis syndrome and regulate NADPH oxidase activity
