摘要

目的：观察当归芍药散合薏苡附子败酱散加减治疗中、重度痤疮湿热瘀结证的临床疗效作用及对血清白细胞介素-1α（IL-1α），白细胞介素8（IL-8），肿瘤坏死因子-α（TNF-α），胰岛素样生长因子-1（IGF-1）水平的影响。方法：将符合要求的152例患者按就诊先后采用随机数字表法分为对照组75例和联合组77例。20例受试者作为健康对照组。对照组Ⅱ级患者使用他扎罗汀倍他米松乳膏，适量外涂，2次/d；Ⅲ级患者在使用外涂的同时，口服盐酸多西环素片，100 mg/次，2次/d；Ⅳ级患者，在此基础上给予阿维A胶囊，20 mg/次，2次/d，服用4周。联合组在对照组治疗的基础上加用当归芍药散和薏苡附子败酱散加减治疗。两组疗程均为8周。进行治疗前后皮损严重程度评分（GAGS）和中医证候评分，进行治疗前后痤疮患者特异性生活质量量表（Acne-QOL）评分；检测治疗前后IL-1α，IL-8，TNF-α，IGF-1水平。结果：联合组总有效率为90.91%，高于对照组的78.67%（P<0.05）；治疗后联合组GAGS评分和中医证候评分均低于对照组（P<0.01）；治疗后联合组Acne-QOL量表4个维度（自我感知、情感功能、痤疮症状、社会功能）评分和总分均高于对照组（P<0.01）；与健康对照组比较，两组患者血清IL-1α，IL-8，TNF-α和IGF-1水平均明显升高（P<0.01）；治疗后两组IL-1α，IL-8，TNF-α和IGF-1水平均较治疗前下降（P<0.01），治疗后联合组IL-1α，IL-8，TNF-α和IGF-1水平低于对照组（P<0.01），但仍高于健康对照组（P<0.01）。结论：在常规西医分级治疗的基础上加服当归芍药散合薏苡附子败酱散加减治疗中、重度痤疮患者，能减轻患者皮损程度，改善中医症状，提高疾病的临床疗效和生活质量，其作用机制可能与下调血清IL-1α，IL-8，TNF-α和IGF-1水平有关。

Abstract

Objective: To observe the clinical efficacy of modified prescription of Danggui Shaoyao San combined with Yiyi Fuzi Baijiang San in treating moderate or severe acne with damp-heat blood stasis syndrome

and its effect on levels of interleukin-1α (IL-1α)

interleukin-8 (IL-8)

itumor necrosis factor-α (TNF-α) and insulin-like growth factor-1 (IGF-1). Method: One hundred and fifty-two patients were randomly divided into control group (75 cases) and combination group (77 cases) by random number table

with the other 20 subjects in healthy control group. The grade Ⅱ patients in control group took Tazarotene and Betamethasone Dipropionate Cream

2 times/day. The grade Ⅲ patients were additionally administered with Doxycycline Hyclate tablets

100 mg/time

2 times/day. The grade Ⅳ patients were additionally given Acitretin capsules

20 mg/time

2 times/day

for 4 weeks. In addition to the therapy of control group

the patients in combination group was additionally administered with modified prescription of Danggui Shaoyao San combined with Yiyi Fuzi Baijiang San. The courses of treatment were 8 months. Before and after the treatment

Global Acne Grading System (GAGS)

traditional Chinese medicine(TCM) syndromes and Acne-quality of life (QOL) were scored. And levels of IL-1α

TNF-α and IGF-1 were detected. Result: The total clinical effective rate in combination group was 90.91%

modified prescription of Danggui Shaoyao San combined with Yiyi Fuzi Baijiang San can relieve injuries on the face

improve clinical efficacy and quality of life

and the action mechanism may be correlated with down-regulation of levels of IL-1α

IL-8

TNF-α and IGF-1.
