目的:观察淫羊藿苷(icariin,ICA)对尿酸钠诱导的大鼠急性痛风性关节炎(gouty arthritis,GA)的影响,并探讨其相关的作用机制。方法:SD大鼠72只,雄雌各半,随机分为正常组,模型组,淫羊藿苷低、中、高剂量组(20,40,80 mg·kg-1),秋水仙碱组(1.5 mg·kg-1),每组12只,按分组预给药5 d后在大鼠右踝关节腔内注射3.0%尿酸钠溶液100 μL复制大鼠GA模型,继续给药48 h,期间测量右踝关节周径并作步态评分。造模48 h后麻醉动物,苏木素-伊红染色观察关节腔积液涂片和关节滑膜白细胞浸润情况,酶联免疫吸附测定(ELISA)测定关节囊和滑膜组织中白细胞介素-1β(IL-1β),白细胞介素-6(IL-6),肿瘤坏死因子-α(TNF-α)和前列腺素E2(PGE2)含量。蛋白质免疫印迹(Western blot)测定关节囊和滑膜组织环氧化酶-2(COX-2)蛋白含量。结果:模型组大鼠关节周围组织肿胀,步态跛行,关节积液和滑膜组织中大量白细胞浸润,炎症因子IL-1β,IL-6,TNF-α均显著高于正常组,PGE2含量明显升高(P<0.05),关节囊和滑膜组织COX-2蛋白表达升高(P<0.05);与模型组比较,ICA低、中、高剂量组可明显改善GA模型大鼠关节肿胀和步态异常,显著降低白细胞浸润和IL-1β,IL-6,TNF-α,PGE2含量,明显降低COX-2蛋白表达(P<0.05)。结论:ICA可通过抑制炎症反应改善尿酸钠诱导的大鼠急性GA症状。
Objective: To investigate the effects and potential mechanisms of icariin (ICA) on acute gouty arthritis(GA) induced by sodium urate in rats. Method: Seventy-two Sprague Dawley rats were randomly divided into normal control group

6 female and 6 male rats in each group. All rats were treated with ICA or colchicine or equal volume of normal saline by intragastric administration for 7 days according to the groups. At the 5th day
the gouty arthritis model was established by injection of 3% sodium urate (100 μL) into the right ankle joint cavity of rats. The circumference of the right ankle joint and score of gait were measured during 48 h treatment after model establishment. At 48 h after GA model establishment

and leukocyte infiltration was observed in arthroedema and synovial tissues by hematoxylin and eosin staining; the contents of interleukin (IL)-1β
IL-6
tumor necrosis factor(TNF )-α and prostaglandin E2(PGE2 ) were determined by ELISA kit; cyclooxygenase (COX-2) protein level was measured by Western blot in joint capsule and synovial tissues. Result: In model group
the tissue around joint was swelling; gait was limping; a large number of leukocytes were infiltrating in arthroedema and synovial tissues; the contents of IL-1β
IL-6
TNF-α were significantly higher than those in normal control group (P<0.05); the PGE2 and COX-2 protein levels were also significantly increased (P<0.05). As compared with model group
all of ICA low dose
middle dose and high dose treatment significantly attenuated the joint swelling and abnormal gait
and significantly decreased leukocyte infiltration and contents of IL-1β
IL-6
TNF-α and PGE2. Conclusion: ICA can attenuate the symptoms of acute GA in rats by inhibiting the inflammatory reaction.
