目的:研究苦碟子注射液(简称苦碟子)对单侧输尿管梗阻大鼠肾间质纤维化的作用及机制。方法:采用单侧输尿管结扎法制备大鼠肾间质纤维化模型,随机分为正常组、假手术组、模型组、缬沙坦(0.01 g · kg-1)组、苦碟子低、高剂量(0.15,0.30 g · kg-1)组。各组大鼠在给药后第7,14天分批次处死,采集血液,肾组织,测定血清肌酐(serum creatinine,SCr),尿素氮(blood urea nitrogen,BUN)含量;肾组织匀浆检测丙二醛(malondialdehyde,MDA)含量,超氧化物歧化酶(superoxide dismutase,SOD)活性;肾脏苏木素-伊红(HE),马松(Masson)染色观察肾脏病理变化;免疫组化法检测各组大鼠肾组织转化生长因子-β1(transforming growth factor-β1,TGF-β1),磷酸化-Smad2/3(phosphorylated Smad2/3,p-Smad2/3),骨形成蛋白-7(bone morphogenetic protein-7,BMP-7)的表达,并行半定量分析;逆转录聚合酶链反应(RT-PCR)检测肾组织TGF-β1,Smad2,Smad3,BMP-7及I型胶原(ColⅠ)mRNA表达。结果:苦碟子能降低肾间质纤维化大鼠血肌酐水平,能减轻肾间质小管损伤,减少肾间质胶原相对面积,MDA含量,下调TGF-β1,p-Smad2/3,ColⅠ的表达;增加SOD的活性以及BMP-7表达水平,与苦碟子低剂量组比较,以苦碟子高剂量(0.30 g · kg-1)组效果最为显著(P<0.05)。结论:苦碟子对大鼠肾间质纤维化的保护作用,可能与其降低大鼠肾组织氧化应激水平、抑制TGF-β1/Smad/BMP-7信号通路传导,从而抑制细胞外基质增生有关。
Abstract
Objective: To investigate the effect of Kudiezi injection on renal tubulointerstitial fibrosis and the possible mechanisms. Method: Unilateral ureteral ligation was used to establish renal interstitial fibrosis models

the rats were sacrificed for blood samples and renal tissues.Serum creatinine (SCr) and blood urea nitrogen (BUN) levels were measured for renal function. Malondialdehyde(MDA) content and superoxide dismutase(SOD) activity were measured to reflect renal oxidative stress levels.Hematoxylin eosin(HE) and Masson staining were used to visualize renal fibrosis in each group. Immunohistochemical analysis was used to detect the expression of transforming growth factor-β1(TGF-β1)
phosphorylated-Smad2/3 (p-Smad2/3)
and bone morphogenetic protein-7(BMP-7). The expression levels of TGF-β1

BMP-7 and type I collagen(ColⅠ) mRNA in renal tissue were detected by reverse trconscription polymerase chain reaction (RT-PCR). Result: Kudiezi could decrease the level of serum creatinine in renal interstitial fibrosis rats

Kudiezi high dose (0.30 g · kg-1) group showed better effect and the difference was statistically significant (P<0.05). Conclusion: The protective effect of Kudiezi on renal interstitial fibrosis in rats might be related to the reduction of oxidative stress level and inhibition of TGF-β1/Smad/BMP-7 signaling pathway
thereby inhibiting the proliferation of extracellular matrix.
