目的:探讨金鸡毛草水提物对糖尿病足(DF)模型大鼠足部创伤的治疗作用,并探讨其可能的作用机制。方法:80只SD大鼠,随机选取8只作为正常组,其余大鼠高脂高糖饲料联合腹腔小剂量链脲佐菌素(STZ)的方法建立糖尿病模型大鼠,再行下肢血管结扎及足部手术,建立DF大鼠模型。模型大鼠随机分为5组,分别为模型组,二甲双胍组(0.2 g·kg-1),金鸡毛草水提物高、中、低剂量(16,8,4 g·kg-1)组。分别于给药第1,4,7,14,21天对大鼠进行创面拍照观察溃疡愈合情况,治疗28 d后对缺血下肢进行免疫组织化学法观察大鼠缺血下肢肌肉组织血管新生情况,酶联免疫吸附法(ELISA)法检测细胞间黏附分子-1(ICAM-1),C反应蛋白(CRP),白细胞介素-6(IL-6),白细胞介素-1β(IL-1β),肿瘤坏死因子-α(TNF-α),内皮素-1(ET-1),血栓素A2(TXA2),硝酸还原法测定一氧化氮(NO),一氧化氮合酶(NOS);实时荧光定量-聚合酶链式反应(Real-time PCR)及蛋白免疫印迹(Western blot)法测定缺血下肢肌肉组织中血管内皮生长因子(VEGF)mRNA和蛋白的表达。结果:与正常组比较,模型组大鼠足部创面愈合延迟,金鸡毛草水提物治疗组创面愈合优于模型组;与正常组比较,模型组大鼠微血管新生情况、血清NO,NOS水平及缺血组织VEGF mRNA和蛋白表达均下降,金鸡毛草水提物高、中、低剂量组均明显升高血清NO,NOS水平及缺血组织VEGF mRNA和蛋白表达(P<0.05,P<0.01);与正常组比较,模型组ICAM-1,CRP,IL-6,IL-1β,TNF-α,ET-1,TXA2水平显著增高(P<0.01),金鸡毛草水提物高、中、低剂量组均明显降低ICAM-1,CRP,IL-6,IL-1β,TNF-α,ET-1,TXA2水平(P<0.05,P<0.01)。结论:金鸡毛草水提物可促进DF大鼠足背创面愈合,其机制可能与其抑制炎症,调节血管内皮功能,提高下肢缺血组织VEGF表达,促进血管新生改善下肢血供有关。
Objective: To investigate the effect of Gynura divaricata aqueous extract on foot injury in diabetic foot (DF) model rats
and explore its possible mechanism. Method: A total of 80 SD rats were included in the study. Among them
8 rats were randomly selected as control group
while the other rats were given with high-fat and glucose feed and intraperitoneally injected with low-dose streptozotocin (STZ) to establish the rat model of diabetes; then
the model of DF was established by ligating their lower extremity vascular and making foot surgery. The model rats (n=40) were randomly divided into model group

and NO and nitric oxide synthase(NOS) content were determined by nitric acid reduction method. Real-time PCR and Western blot were used to determine vascular endothelial growth factor(VEGF) mRNA and protein expressions in ischemic lower limb muscles. Result: Compared with the normal group

and its mechanism may be related to the inhibition of inflammation
the regulation of vascular endothelial function
the increase in the expression of VEGF
the promotion of the angiogenesis in ischemic tissues
and the improvement of the blood supply of the lower extremity.
