摘要

目的：观察丹芪益肾方对气阴两虚夹瘀型糖尿病肾病(DN)患者的疗效及对血清凝血酶调节蛋白(TM)，血管性假性血友病因子(vWF)，内皮素(ET)及尿中胰岛素样生长因子(IGF)-1和IGF-2水平的影响。方法：筛选收治的DN患者150例，按数字表法随机分为对照组和治疗组各75例。两组患者参照《糖尿病肾病防治专家共识(2014年版)》给予常规治疗。对照组采用还原型谷胱甘肽静脉滴注，1次/d。治疗组注射西药同对照组，并加服丹芪益肾方，l剂/d，常规水煎煮2次内服。两组患者均连续治疗3个月。比较两组患者血糖和血脂水平、气阴两虚夹瘀证症状评分、肾功能及临床疗效。检测两组患者血清凝血酶调节蛋白(TM)，血管性假性血友病因子(vWF)，内皮素(ET)及尿中胰岛素样生长因子(IGF)-1和IGF-2水平。结果：治疗后，治疗组患者空腹血糖(FPG)，餐后2 h血糖(PPG)，总胆固醇(TC)，甘油三酯(TG)水平及气阴两虚夹瘀证症状评分均明显低于对照组(P<0.01)。治疗后，治疗组患者的肾功能指标血清肌酐(SCr)，血尿素氮(BUN)，24 h尿总蛋白，24 h尿清蛋白及尿β2微球蛋白(β2-MG)水平均明显低于对照组(P < 0.01)。治疗组患者临床总有效率为88％，优于对照组有效率73.33％(P<0.05)。治疗后，治疗组患者血清TM，vWF，ET水平及尿中IGF-1，IGF-2水平均明显低于对照组(P<0.01)。结论：丹芪益肾方配合还原型谷胱甘肽静脉滴注治疗糖尿病肾病，可促进气阴两虚夹瘀证症状及肾功能的改善，提高临床疗效，且可调节患者体内血清和肾功能指标水平，其机制可能是通过抑制TM，vWF，ET，IGF-1和IGF-2释放有关。

Abstract

Objective: To investigate the efficacy of Danqi Yishen prescription on diabetic nephropathy with Qi Yin deficiency and blood-stasis syndrome and the effect on serum levels of thrombin regulating protein (TM)
