目的:通过观察补阳还五汤合八珍汤联合抗阻有氧运动对糖尿病肌萎缩的血糖、胰岛功能、神经传导功能及血清血管内皮生长因子(VEGF),神经生长因子(NGF)及碱性成纤维细胞生长因子(bFGF),炎性因子的影响,探讨其疗效及机制。方法:78例糖尿病肌萎缩患者按照随机数字表法随机分为对照组和观察组,各39例。在常规治疗基础上,对照组加用抗阻有氧运动,观察组联合补阳还五汤合八珍汤与抗阻有氧运动。疗程3个月。评价两组的临床疗效,测定并比较两组治疗前后血糖、神经电生理检查、胰岛素抵抗及血清VEGF,bFGF,NGF,炎性因子水平。结果:观察组的症状评分有效率为97.4%,体质量改善有效率为94.9%,均优于对照组的74.4%,76.9%(P<0.05)。治疗后两组的正中神经及腓总神经的运动神经传导速度(MCV)与正中神经及腓肠神经的感觉神经传导速度(SCV)均较本组治疗前增快(P<0.05),且观察组优于对照组(P<0.05)。治疗后两组早餐后2 h血糖(2 hPBG),糖化血红蛋白(HbA1c)均较本组治疗前明显下降(P<0.05)。治疗后两组胰岛素抵抗指数(HOMA-IR),白细胞介素(IL)-6,肿瘤坏死因子-α(TNF-α)水平均较本组治疗前明显降低,观察组的胰岛素敏感指数(ISI)较本组治疗前明显升高(P<0.05);且治疗后观察组的HOMA-IR,IL-6,TNF-α水平明显低于对照组(P<0.05)。治疗后两组的VEGF,NGF,bFGF水平明显升高(P<0.05),且观察组高于对照组(P<0.05)。结论:补阳还五汤合八珍汤联合抗阻有氧运动能更好的改善糖尿病肌萎缩患者的血糖水平,改善胰岛素抵抗,减轻临床症状,其机制可能与改善机体的炎症状态,提高VEGF,NGF,bFGF水平有关。
Objective:To observe the Buyang Huanwu Tang combined with Bazhentang on blood glucose
islet function
nerve conduction function and serum vascular endothelial (VEGF)
nerve growth factor (NGF) and basic fibroblast growth factor (bFGF) as well as the expression of inflammatory factors in patients with diabetic muscle atrophy
and investigate its efficacy and mechanism. To explore the therapeutic effect and mechanism. Method:The 78 patients with diabetic myatrophy were randomly divided into control group and observation group according to the random digital table method
with 39 cases in each group. On the basis of routine treatment
the control group was additionally treated with anti-obstructive aerobic exercise
and the observation group received Buyang Huanwu Tang combined with Bazhentang and anti-obstructive aerobic exercise. The course of treatment was 3 months in both groups. The clinical efficacy of the two groups was evaluated

NGF and inflammatory factors in two groups before and after treatment were measured and compared. Result:The effective rate of symptom score was 97.4% and the effective rate of weight improvement was 94.9% in the observation group
significantly better than 74.4% and 76.9% in control group (P<0.05). After treatment
the motor nerve conduction velocity (MCV) of median nerve and common peroneal nerve as well as sensory nerve conduction velocity (SCV) of median nerve and the sural nerve were faster than the conditions before treatment in both groups (P<0.05)
and the effect in observation group was better than that in control group (P<0.05). After treatment
the two hours postprandial blood glucose (2 hPBG) and Hemoglobin A1C (HbA1c) were significantly lower than those before the treatment in both groups (P<0.05). After treatment
the levels of homeostasis model assessment-insulin resistance index (HOMA-IR)
interleukin (IL) -6 and tumor necrosis factor (TNF) -α were significantly lower than those before the treatment in both groups
and the insulin sensitive index (ISI) in observation group was significantly higher than that before the treatment (P<0.05). After treatment
the levels of HOMA-IR
IL-6 and TNF-α in observation group were significantly lower than those in control group (P<0.05). The levels of VEGF
NGF and bFGF were significantly higher after treatment in both groups (P<0.05)
and the observation group was higher than that of control group (P<0.05). Conclusion:Buyang Huanwu Tang combined with Bazhentang and anti-obstructive aerobic exercise can better improve blood glucose level
improve insulin resistance and relieve clinical symptoms in patients with diabetic muscular atrophy
and its mechanism may be related to improving the inflammatory state of the body and improving the levels of VEGF
NGF and bFGF.
