目的:

探讨参藤三黄汤合葛根芩连汤对糖尿病肾脏疾病(DN)的保护机制。
方法:

通过单侧肾切除术联合注射链脲佐菌素(STZ)复制DN模型大鼠,采用随机数字表法分为空白组、模型组、厄贝沙坦组(15 mg·kg
-1
)、参藤三黄汤合葛根芩连汤高、中、低剂量(5,10,20 g·kg
-1
)组。分别予以相应药物灌胃8周,检测各组大鼠体质量、血糖、血尿素氮(BUN),24 h尿量(24 h U-vol),24 h尿蛋白(24 h U-pro),血清肌酐(SCr),肾质量/体质量指数(KW/BW),取各组大鼠肾组织,匀浆取上清,试剂盒检测丙二醛(MDA),超氧化物歧化酶(SOD)及过氧化氢酶(CAT)的含量,蛋白免疫印迹法(Western blot)检测Smad泛素化调节因子2(Smurf2),基质金属蛋白酶-9(MMP-9)及转化生长因子-
β
1
(TGF-
β
1
)的表达情况。
结果:

与空白组比较,模型组大鼠体质量明显降低(
P

0.05),KW/BW增大(

0.05),各项生化指标明显异常,表现在血糖明显升高(
P
<
0.05),BUN,24 h U-vol,24 h U-pro,SCr及MDA明显升高,SOD及CAT明显降低(

<
0.05)。Western blot显示,Smurf2及TGF-
β
1
蛋白表达升高(
P

0.05),MMP-9蛋白表达降低(

0.05)。与模型组比较,厄贝沙坦组和参藤三黄汤合葛根芩连汤各组指标改善明显,体质量增加(

0.05),血糖,BUN,24 h U-vol,24 h U-pro,SCr及MDA明显降低,SOD及CAT明显升高(

结论:

参藤三黄汤合葛根芩连汤能有效改善DN模型大鼠多项生化指标,改善肾脏功能,其作用机制与降低Smurf2及TGF-
β
1
表达,增强及MMP-9的表达密切相关。
Objective:

To explore the protective mechanism of Shenteng Sanhuang decoction and Gegen Qinlian Tang on diabetic nephropathy (DN).
Method:

Rat DN models were duplicated with unilateral nephrectomy combined with streptozotocin (STZ). The rats were randomly divided into blank group
model group
irbesartan group and traditional Chinese medicine group. After 8 weeks of administration of corresponding drugs
the body weight
blood sugar
blood urea nitrogen (BUN)
24-hour urine volume (24 h U-vol)
24-hour urinary protein (24 h U-pro)
serum creatinine (SCr)
kidney weight/body weight (KW/BW) mass index of rats in each group were measured. The kidney tissues of rats in each group were homogenized
and supernatant was taken. Expressions of Smad ubiquitination regulatory factor 2 (Smurf2)
matrix metalloprotein-9 (MMP-9)
TGF-
β
1
and malondialdehyde (MDA)
superoxide dismutase (SOD) and catalase (CAT) were detected by Western blot or special test kit.
Result:

Compared with the blank group
the biochemical indicators

SCr and MDA were significantly higher or increased(

while SOD and CAT were significantly decreased in the model group(

0.05). Western blot showed that the expression of Smurf2 and TGF-

while the expression of MMP-9 was low(

0.05). Compared with the model group
the biochemical indicators of irbesartan group and traditional Chinese medicine group improved significantly(

Conclusion:

Shenteng Sanhuang decoction and Gegen Qinlian Tang can effectively improve many biochemical indexes of rat DN models
and improve renal function. Its mechanism is closely related to reducing the expressions of Smurf2 and TGF-
β
1
and enhancing the expression of MMP-9.
