目的

比较百药煎发酵炮制前后对溃疡性结肠炎(UC)小鼠肠道菌群及肠黏膜中闭合蛋白(Occludin)和闭锁小带蛋白-1(ZO-1)表达的影响,探讨五倍子和百药煎治疗UC的作用机制,指导中医临床区别用药。
方法

将50只小鼠随机分为5组,每组10只,随机选取1组为空白组,其余4组采用右旋葡聚糖硫酸钠(DSS)诱导UC模型;造模完成后,空白组和模型组给予生理盐水,给药组分别给予美沙拉嗪(0.8 g·kg
-1
),五倍子水煎液(1.8 g·kg
-1
)和百药煎水煎液(2.7 g·kg
-1
)灌胃7 d;运用16S rRNA测序技术检测小鼠粪便肠道菌群的变化,苏木精-伊红(HE)染色观察结肠组织病理学变化,免疫组化对比小鼠结肠组织Occludin和ZO-1的表达。
结果

UC小鼠肠道菌群丰度、多样性较空白组均明显降低,结肠组织充血增厚,出现明显溃疡,Occludin和ZO-1的表达量显著降低(

0.01)。经五倍子和百药煎治疗后,菌群丰度及多样性均有提升,门水平主要表现为五倍子组厚壁菌门、变形菌门和放线菌门相对丰度显著增加(

0.01),拟杆菌门丰度显著减少(

0.01);百药煎组变形菌门和放线菌门相对丰度显著增加(

0.01),厚壁菌门相对丰度增加和拟杆菌门相对丰度减少,但差异均无统计学意义。属水平主要表现为五倍子组拟杆菌属、瘤胃球菌属和
Allobaculum
相对丰度显著降低(

0.01),罗斯氏菌属、普雷沃菌属、颤螺旋菌属和帕拉普氏菌属相对丰度明显升高(

0.01);百药煎组拟杆菌属和
Allobaculum
相对丰度明显降低(

0.01),普雷沃菌属、颤螺旋菌属、罗斯氏菌属和瘤胃球菌属相对丰度显著升高(

0.01)。与模型组比较,五倍子组和百药煎组结肠组织均明显恢复,充血减轻,仅见散在溃疡;Occludin和ZO-1的表达量升高,且百药煎组的表达量高于五倍子组。
结论

百药煎治疗UC效果优于五倍子,其机制可能为通过调节肠道菌群的相对丰度和多样性,改善肠道菌群紊乱状况,提高ZO-1和Occludin的表达,保护肠道黏膜屏障功能,起到减轻肠道炎症的作用。
Objective

To compare the effects of Baiyaojian before and after fermentation on intestinal flora and expression of Occludin and zonula occludens protein-1 (ZO-1) in intestinal mucosa of mice with ulcerative colitis (UC), and to explore the mechanism of Baiyaojian and Galla Chinensis in the treatment of UC.
Method

Totally 50 mice were randomly divided into 5 groups with 10 mice in each group, one group was randomly selected as blank group, and the other 4 groups were treated with dextran sodium sulfate (DSS) to induce UC model. After modeling, mice in the blank group and model group were given normal saline, and treatment groups were given Mesalazine (0.8 g·kg
-1
), Galla Chinensis decoction (1.8 g·kg
-1
) and Baiyaojian decoction (2.7 g·kg
-1
) by intragastric administration for 7 days. The 16S rRNA sequencing technology was used to detect the changes of intestinal flora in mouse feces. The histopathological changes of colon tissue were observed by hematoxylin-eosin (HE) staining, and the expression of Occludin and ZO-1 in colon tissue of mice were compared by immunohistochemistry.

Compared with the blank group, the abundance and diversity of intestinal flora in UC mice were significantly decreased, and the colonic tissue was thickened with congestion and obvious ulcers, and the expression levels of Occludin and ZO-1 were significantly decreased (

0.01). After treatment with Galla Chinensis and Baiyaojian, the abundance and diversity of flora were improved. At the phylum level, relative abundance of Firmicutes, Proteobacteria and Actinobacteria increased significantly (
P

0.01), while the relative abundance of Bacteroidetes decreased significantly (
P
<
0.01) in Galla Chinensis group. In Baiyaojian group, the relative abundance of Proteobacteria and Actinobacteria increased significantly (
P

0.01), while the relative abundance of Firmicutes increased and the relative abundance of Bacteroidetes decreased, but there was no significant difference. At the genus level, the relative abundance of
Bacteroides
,
Allobaculum
and
Ruminococcus
decreased significantly (
P

0.01), the relative abundance of
Roseburia
,
Prevotella
,
Oscillospira
and
Paraprevotella
increased significantly (
P
<
0.05,

<
0.01) in Galla Chinensis group. In Baiyaojian group, the relative abundance of
Bacteroides
and
Allobaculum
decreased significantly (

0.05,

<
0.01), and the relative abundance of
Prevotella
,
Oscillospira
,
Roseburia
and
Ruminococcus
increased significantly (
P
<
0.01). Compared with model group, colon tissue of Galla Chinensis group and Baiyaojian group was recovered obviously, congestion was alleviated, only scattered ulcers were seen. The expression of Occludin and ZO-1 increased, and the expression level of Baiyaojian group was higher than that of Galla Chinensis group.
Conclusion

The effect of Baiyaojian is better than Galla Chinensis in the treatment of UC. The mechanism may be through regulating the abundance and diversity of intestinal flora, improving the disorder of intestinal flora and increasing the expression of ZO-1 and Occludin and protecting the intestinal mucosal barrier function for alleviating intestinal inflammation.
